    Extended-spectrum beta-lactamaseproducing bacteria in a Tertiary Care Hospital in Madrid: Epidemiology, risk factors and antimicrobial susceptibility patterns

    Introduction: Extended-spectrum beta-lactamase (ESBL) producing bacteria have been increasingly reported as causal agents of nosocomial infection worldwide. Resistance patterns vary internationally, and even locally, from one institution to the other. We investigated the clinical isolates positive for ESBL-producing bacteria in our institution, a tertiary care hospital in Madrid (Spain), during a 2-year period (2007 2008). Methods: Clinical and microbiological data were retrospectively reviewed. Two hundred and nineteen patients were included in the study. Results: Advanced age, diabetes, use of catheters, previous hospitalization and previous antibiotic treatment were some of the risk factors found among patients. Escherichia coli was the most frequent isolate, and urinary tract the most common site of isolation. Internal Medicine, Intensive Care Unit (ICU) and General Surgery presented the highest number of isolates. There were no outbreaks during the study period. Antibiotic patterns showed high resistance rates to quinolones in all isolates. There was 100% sensitivity to carbapenems. Conclusion: Carbapenems continue to be the treatment of choice for ESBL-producing bacteria. Infection control measures are of great importance to avoid the spread of these nosocomial infection

    Evaluation of optical tracking and augmented reality for needle navigation in sacral nerve stimulation

    Background and objective: Sacral nerve stimulation (SNS) is a minimally invasive procedure where an electrode lead is implanted through the sacral foramina to stimulate the nerve modulating colonic and urinary functions. One of the most crucial steps in SNS procedures is the placement of the tined lead close to the sacral nerve. However, needle insertion is very challenging for surgeons. Several x-ray projections are required to interpret the needle position correctly. In many cases, multiple punctures are needed, causing an increase in surgical time and patient's discomfort and pain. In this work we propose and evaluate two different navigation systems to guide electrode placement in SNS surgeries designed to reduce surgical time, minimize patient discomfort and improve surgical outcomes. Methods: We developed, for the first alternative, an open-source navigation software to guide electrode placement by real-time needle tracking with an optical tracking system (OTS). In the second method, we present a smartphone-based AR application that displays virtual guidance elements directly on the affected area, using a 3D printed reference marker placed on the patient. This guidance facilitates needle insertion with a predefined trajectory. Both techniques were evaluated to determine which one obtained better results than the current surgical procedure. To compare the proposals with the clinical method, we developed an x-ray software tool that calculates a digitally reconstructed radiograph, simulating the fluoroscopy acquisitions during the procedure. Twelve physicians (inexperienced and experienced users) performed needle insertions through several specific targets to evaluate the alternative SNS guidance methods on a realistic patient-based phantom. Results: With each navigation solution, we observed that users took less average time to complete each insertion (36.83 s and 44.43 s for the OTS and AR methods, respectively) and needed fewer average punctures to reach the target (1.23 and 1.96 for the OTS and AR methods respectively) than following the standard clinical method (189.28 s and 3.65 punctures). Conclusions: To conclude, we have shown two navigation alternatives that could improve surgical outcome by significantly reducing needle insertions, surgical time and patient's pain in SNS procedures.     Mini instructivo técnico comunitario para la producción de plátanos y bananos en patios y parcelas (Original)

    In Cuba, bananas and bananas are a matter of high priority within the national food program, due to their ability to produce every month of the year, their high productive potential, deep-rooted consumption habit and diversity of uses. The work was carried out in the community of the Cay of the Baire town council of the Contramaestre municipality in the Santiago de Cuba, as of November 2016, comprising 201 homes in the semi-urban perimeter with a total of 844 inhabitants, in the that most of the inhabitants do not have the knowledge to obtain acceptable production results, presenting as problematic that the application of science and technology in the production of bananas in this area is insufficient, which limits the production of larger volumes of the product . Considering the objective of developing a mini community technical instruction manual for the production of bananas and bananas in patios and plots, which served as a basis for a better understanding of the management of this crop in the aforementioned context, drawing intervention actions in the training of the community in function of achieving best agroecological practices. Considering the number of inhabitants, the bunches to be produced in the year, with an average weight of 8 kg, the needs of food at 14% are satisfied considering the FAO standard of 150 g of food per day per inhabitant. It also manages to ensure the application of technical requirements, to produce organic plantain locally, encouraging activity among the residents of the community.En Cuba, los bananos y plátanos constituyen una cuestión de elevada prioridad dentro del programa alimentario nacional, debido a su capacidad de producir todos los meses del año, su elevado potencial productivo, arraigado hábito de consumo y diversidad de usos. El trabajo se desarrolló en la comunidad el Cayo del consejo de poblado Baire del municipio Contramaestre en Santiago de Cuba, a partir del mes de noviembre de 2016 ,  compuesta por 201 viviendas en el perímetro semiurbano  con un total de 844 habitantes, en los que la mayoría de los habitantes no poseen los conocimientos para obtener resultados de producción aceptables, presentándose como problemática que es insuficiente la aplicación de la ciencia y la técnica en la producción del plátano en esta zona, lo que limita la producción de mayores volúmenes del producto . Considerando como objetivo elaborar un mini instructivo técnico comunitario para la producción de plátanos y bananos en patios y parcelas, lo cual sirvió de base para conocer mejor el manejo de este cultivo en el citado contexto, trazándose acciones de intervención en la capacitación de la comunidad en función de lograr mejores prácticas agroecológicas. Considerando la cantidad de habitantes, los racimos a producir en el año, con un peso promedio de 8 kg se satisfacen las necesidades de viandas a un 14% considerando la norma FAO de 150 g de viandas diario por  habitante. Se logra además asegurar la aplicación de las exigencias técnicas, para de forma local producir plátano ecológico, incentivando la actividad entre los pobladores de la comunidad

    Recurrent anal fistulae: Limited surgery supported by stem cells

    AIM: To study the results of stem-cell therapy under a Compassionate-use Program for patients with recurrent anal fistulae. METHODS: Under controlled circumstances, and approved by European and Spanish laws, a Compassionate-use Program allows the use of stem-cell therapy for patients with very complex anal fistulae. Candidates had previously undergone multiple surgical interventions that had failed to resolve the fistulae, and presented symptomatic recurrence. The intervention consisted of limited surgery (with closure of the internal opening), followed by local implant of stem cells in the fistula- tract wall. Autologous expanded adipose-derived stem cells were the main cell type selected for implant. The first evaluation was performed on the 8th postoperative week; outcome was classified as response or partial response. Evaluation one year after the intervention confirmed if complete healing of the fistula was achieved. RESULTS: Ten patients (8 male) with highly recurrent and complex fistulae were treated (mean age: 49 years, range: 28-76 years). Seven cases were non- Crohn’s fistulae, and three were Crohn’s-associated fistulae. Previous surgical attempts ranged from 3 to 12. Two patients presented with preoperative incontinence (Wexner scores of 12 and 13 points). After the intervention, six patients showed clinical response on the 8th postoperative week, with a complete cessation of suppuration from the fistula. Three patients presented a partial response, with an evident decrease in suppuration. A year later, six patients (60%) remained healed, with complete reepithelization of the external opening. Postoperative Wexner Scores were 0 in six cases. The two patients with previous incontinence improved their scores from 12 to 8 points and from 13 to 5 points. No adverse reactions or complications related to stem-cell therapy were reported during the study period. CONCLUSION: Stem cells are safe and useful for treating anal fistulae.     [ES] La profilaxis antibiótica en cirugía es una de las medidas más eficaces para la prevención de la infección de localización quirúrgica, aunque su uso es con frecuencia inadecuado, pudiendo incrementar el riesgo de infección, toxicidades y resistencias bacterianas. Debido al avance en las técnicas quirúrgicas y la emergencia de microorganismos multirresistentes las actuales pautas de profilaxis precisan ser revisadas. La Sociedad Española de Enfermedades Infecciosas (SEIMC), conjuntamente con la Asociación Española de Cirujanos (AEC) ha revisado y actualizado las recomendaciones de profilaxis antimicrobiana para adaptarlas a cada tipo de intervención quirúrgica y a la epidemiología actual. En este documento se recogen las recomendaciones de los antimicrobianos utilizados en profilaxis en los diferentes procedimientos, las dosis, la duración, la profilaxis en huéspedes especiales, y en situación epidemiológica de multirresistencia, de tal forma que permitan un manejo estandarizado, un uso racional, seguro y efectivo de los mismos en la cirugía electiva.[EN] Antibiotic prophylaxis in surgery is one of the most effective measures for preventing surgical site infection, although its use is frequently inadequate and may even increase the risk of infection, toxicities and antimicrobial resistance. As a result of advances in surgical techniques and the emergence of multidrug-resistant organisms, the current guidelines for prophylaxis need to be revised. The Sociedad Española de Enfermedades Infecciosas (Spanish Society of Infectious Diseases and Clinical Microbiology) (SEIMC) together with the Asociación Española de Cirujanos (Spanish Association of Surgeons) (AEC) have revised and updated the recommendations for antibiotic prophylaxis in surgery to adapt them to any type of surgical intervention and to current epidemiology. This document gathers together the recommendations on antimicrobial prophylaxis in the various procedures, with doses, duration, prophylaxis in special patient groups, and in epidemiological settings of multidrug resistance to facilitate standardized management and the safe, effective and rational use of antibiotics in elective surgery

    Asymmetries of the Muscle Mechanical Properties of the Pelvic Floor in Nulliparous and Multiparous Women, and Men: A Cross-Sectional Study

    This study aimed to identify if the muscle mechanical properties (MMPs) of both sides of pelvic floor muscles (PFMs) are symmetrical in different populations of both sexes. Between-sides comparisons of MMPs of PFMs, assessed with manual myotonometry, were performed in three groups, with 31 subjects each, composed of healthy nulliparous women (without any type of delivery or pregnancy), multiparous women (with at least two vaginal deliveries), and healthy adult men. Intra-group correlations between MMPs and age, body mass index (BMI), or clinical state of pelvic floor were also obtained. The nulliparous women and the men showed no between-sides differences in any MMP of PFMs. However, the multiparous women showed that the right side displayed less frequency (−0.65 Hz, 95% CI = −1.01, −0.20) and decrement (0.5, 95% CI = 0.11, 0.01), and more relaxation (1.00 ms, 95% CI = 0.47, 1.54) and creep (0.07 De, 95% CI = 0.03, 0.11), than the left side. Further, MMPs were related to age, sex, and BMI, also depending on the population, with the multiparous women being the only group with some between-sides asymmetries, which in this case were positive and of fair intensity for the left side of the PFMs, between BMI, and frequency and stiffness (rho Spearman coefficient: 0.365 and 0.366, respectively). The symmetry of MMPs of the PFMs could depend on the subject’s condition. Multiparous women show a higher tendency to asymmetries than nulliparous women and men, which should be considered in research and clinical settings

    La Asignación Universal por Hijo: integralidad, territorialidad y organización

    Mediante este trabajo, pretendemos problematizar críticamente el rol de la universidad pública como institución no sólo como generadora y productora de conocimiento, sino también que esté al servicio de la comunidad y se comprometa con los modelos nacionales y populares latinoamericanos. Vamos a compartir como práctica extensionista nuestro proyecto llamado “Hacia una política social integral a la niñez en la provincia de San Luis, El derecho a la Asignación Universal por Hijo (AUH)”, que tiene el propósito fundamental de ayudar a que los sujetos (niños, niñas, adolescentes, mujeres, familias) accedan a algunas políticas sociales integrales como la mencionada. Nos proponemos por un lado, analizar el impacto que tienen estas políticas públicas en territorios con situaciones de vulnerabilidad de la ciudad de San Luis y de Villa Mercedes, que sirvan como muestra de cómo se implementan tales políticas. Por otro lado, pretendemos articular esta información con una práctica que implique brindar algunas herramientas de organización que faciliten el acceso a otros derechos, como por ejemplo el derecho al trabajo, el derecho a la participación comunitaria, el derecho a la no violencia hacia las mujeres y niños, y el derecho tanto a la educación como a la salud pública.  En el marco de las políticas sociales nacionales del bicentenario, se plantea que desde el abordaje integral se intenta producir una ruptura con los modelos neoliberales anteriores instaurados en la Argentina hasta el 2003, que favoreció la fragmentación social. El concepto de integralidad de la política social implica la articulación y la gestión compartida de recursos administrativos a nivel provincial, regional y nacional. La integralidad supone dos perspectivas: integralidad en la mirada y en la gestión. Por todo lo expuesto anteriormente, los objetivos de nuestro proyecto son ayudar a  que los sujetos accedan a políticas sociales integrales y finalmente, realizar un abordaje territorial de la implementación de la Asignación Universal por Hijo (AUH).

    Dendritic Cell-Mediated Cross-Priming by a Bispecific Neutralizing Antibody Boosts Cytotoxic T Cell Responses and Protects Mice against SARS-CoV-2

    17 p.-4 fig.Administration of neutralizing antibodies (nAbs) has proved to be effective by providing immediate protection against SARS-CoV-2. However, dual strategies combining virus neutralization and immune response stimulation to enhance specific cytotoxic T cell responses, such as dendritic cell (DC) cross-priming, represent a promising field but have not yet been explored. Here, a broadly nAb, TNT, are first generated by grafting an anti-RBD biparatopic tandem nanobody onto a trimerbody scaffold. Cryo-EM data show that the TNT structure allows simultaneous binding to all six RBD epitopes, demonstrating a high-avidity neutralizing interaction. Then, by C-terminal fusion of an anti-DNGR-1 scFv to TNT, the bispecific trimerbody TNTDNGR-1 is generated to target neutralized virions to type 1 conventional DCs (cDC1s) and promote T cell cross-priming. Therapeutic administration of TNTDNGR-1, but not TNT, protects K18-hACE2 mice from a lethal SARS-CoV-2 infection, boosting virus-specific humoral responses and CD8+ T cell responses. These results further strengthen the central role of interactions with immune cells in the virus-neutralizing antibody activity and demonstrate the therapeutic potential of the Fc-free strategy that can be used advantageously to provide both immediate and long-term protection against SARS-CoV-2 and other viral infections.SARS-CoV-2 B.1.351 and B.1.167.2 viruses used in this study were obtained through the European Virus Archive Global (EVA-GLOBAL) project that has received funding from the European Union's Horizon 2020 research and innovation programme under grant agreement No 653316.
