    Lions, whales, and the web : transforming moment inertia into conservation action

    When Farley Mowat wrote A Whale for the Killing in 1972, the titular fin whale, stranded and intentionally wounded in a Newfoundland pond, was long dead, yet the story of Moby Joe and the spectacle surrounding her death would become a cornerstone of the emerging anti-whaling movement (see below). The media frenzy that descended on the small town of Burgeo as the whale struggled to survive, and the subsequent publication of Mowat's book, are among the first examples of efforts to turn spontaneous outpourings of outrage, curiosity, or empathy into conservation action by actively focusing media attention, a phenomenon that we have dubbed moment inertia. We use “moment” because this phenomenon arises from focus of attention around a single, clarifying event, or moment, and “inertia” because that attention propagates, undirected, through media unless acted upon by outside forces, much like physical inertia. Almost half a century later, the events leading to the publication of A Whale for the Killing stand among the most effective uses of moment inertia in the conservation movement

    Prevalence of substandard amoxicillin oral dosage forms in the National Capital District of Papua New Guinea

    Antibiotics are commonly reported as being substandard or falsified in low- to middle-income countries, having potential to contribute to the development of antimicrobial resistance and drug-resistant infections. Amoxicillin, used to treat a number of infections and listed by the WHO as an essential medicine, presented as a good drug candidate for this study. We aimed to measure the prevalence of substandard and falsified amoxicillin oral products (tablets, capsules, and suspensions) in the National Capital District of Papua New Guinea (PNG). These oral products were surveyed in 2018 and 2019 from retail pharmacies, private and public health facilities, and the Area Medical Store, representing more than 90% of licensed medicine outlets. The product packaging was visually inspected, and the samples were analyzed for amoxicillin content using a validated high-performance liquid chromatography method. Although no falsified products were identified, 15% of the 190 products analyzed contained substandard amounts of amoxicillin. Quality varied with the dosage form (P = 0.002), with capsules exhibiting the lowest incidence of substandard content (4% in 2019) and tablets collected in 2018 experiencing the highest failure rate (50%). Suspension (40%) quality was compromised by failure to achieve homogeneity on reconstitution. A higher incidence of substandard content (P = 0.002) was associated with one major retail group. Routine testing of medicines by resource-poor countries is often unachievable, leading to the circulation of poor quality drugs, which is a global public health concern. Our study highlighted that substandard amoxicillin oral products are indeed prevalent in the NCD of PNG

    Longitudinal evaluation of cognition after stroke - A systematic scoping review

    BACKGROUND: Cognitive impairment affects up to 80 percent of the stroke population, however, both the available evidence about post-stroke cognition and the measures used to evaluate it longitudinally have not been well described. The aims of this systematic scoping review were: to identify and characterize studies evaluating cognition longitudinally after stroke; to summarize the cognitive instruments used and the domains they target; and to organize cognitive domains assessed using the International Classification of Functioning, Disability and Health (ICF). METHODS: We used a systematic scoping approach to search for peer-reviewed articles involving adults with stroke that evaluated cognition longitudinally. Screening of titles, abstracts, and full reports was completed independently by two reviewers, across six electronic databases (PubMed, PsycInfo, Medline, Cinahl Plus, Embase, and Web of Science). Cognitive domains were mapped to an ICF function independently by the same two reviewers, using a previously tested, standardized approach. RESULTS: A total of 5,540 records were found; 257 were included, representing a total pooled sample of 120,860 stroke survivors. Of these studies, 200 (78%) provided specific cognitive outcomes from the longitudinal evaluations, 57 (22%) reported model predictions, and 77 (30%) included interventions. Cognition was evaluated with 356 unique instruments, targeting 95 distinct cognitive domains, and 17 mental functions from the ICF. The Mini-Mental State Examination was the most frequently used instrument (117 reports, 46%). Other tools used longitudinally were the Trail Making Test (17% of reports), tests of verbal fluency (14%), the Functional Independence Measure (14%), the Montreal Cognitive Assessment (13%), the Digit Span (11%), and the Stroop test (10%). Global cognition was evaluated in 170 reports (66%), followed by higher-level cognitive functioning (29%), memory (28%), language (21%), attention (21%), and perceptual skills (14%). Studies using functional (or performance-based) cognitive assessments over time were scarce (\u3c 1%). CONCLUSION: Our findings indicate that whilst there is a substantial number of studies available that report longitudinal evaluations of cognition after stroke, there is large variability in the measures used and the cognitive domains they target. Nonetheless, the available data for evaluation of cognition over time after stroke can be organized and described systematically

    Incorporating Universal Design into Tsunami Modeling Results for Cascadia Subduction Zone Faults to Create an Inundation Map and Universally Designed Evacuation Map for Port Angeles, WA

    Current tsunami hazard inundation and evacuation maps in the Puget Sound are based primarily on Cascadia and Seattle fault tsunamis. The standard evaluation process for tsunami impacts focuses on elevation and hypothetical fault rupture of known and predicted earthquakes. However, there are several known tsunami deposits in the Puget Sound that are not from Cascadia or Seattle fault tsunamis, potentially from other faults within the region, that could affect tsunami mitigation. Work to understand newly discovered crustal deformation and faults in Puget Sound is ongoing, therefore evacuation and inundation maps need to be updated to include these new faults and integrate universal design more broadly. Methods involved using GeoClaw software to map tsunamis from the Cascadia Subduction Zone (CSZ), Leech River fault (LRF), and Utsalady Point fault (UPF). Modeled tsunamis determined the overall inundation of Port Angeles, Washington through a wide range of earthquake inputs of magnitude, proximity, and recurrence. The output simulations were evaluated with key components of universal design to create a new tsunami hazard map. Comparison between the universal design-based map to current the tsunami hazard map allowed for an evaluation of the current evacuation map. This evaluation can improve the assessment of bridges and other evacuation mechanisms. This research can contribute to future tsunami hazard map revisions saving lives, can help with emergency management planning, and spur reevaluating evacuation plans within the tsunami impact area

    Prevalence of Exclusive Breastfeeding in Bangladesh and Its Association with Diarrhoea and Acute Respiratory Infection: Results of the Multiple Indicator Cluster Survey 2003

    The objective of this study was to investigate the association between the prevalence of exclusive breastfeeding and morbidity (diarrhoeal diseases and acute respiratory infection) in infants aged 0-3 month(s) using the Multiple Indicator Cluster Survey (MICS) 2003 data from Bangladesh. The study population included 1,633 infants aged 0-3 month(s). The prevalence of diarrhoea and acute respiratory infection was compared using the chi-square tests between infants aged 0-3 month(s) who were exclusively breastfed and infants who were not exclusively breastfed. Logistic regression was used to adjust for confounders and for calculating adjusted odds ratios. To adjust for cluster sampling and reduced variability, the adjusted chi-square value was divided by the design effect, and a re-estimated p value was calculated. The prevalence of diarrhoea and acute respiratory infection in this sample of 0-3-month old infants in Bangladesh was 14.3% and 31.2% respectively. The prevalence of both illnesses was significantly associated with lack of exclusive breastfeeding. The adjusted odds ratio for diarrhoea was 0.69 (95% confidence interval [CI] 0.49-0.98, p=0.039), and the adjusted odds ratio for acute respiratory infection was also 0.69 (95% CI 0.54-0.88, p=0.003). Only 192 infants (11.7% of total sample) were exclusively breastfed at the time of interview, and 823 infants (50.3%) were never exclusively breastfed. The prevalence of prelacteal feeding was 66.6%. The results confirmed a protective effect of exclusive breastfeeding against infectious diseases-related morbidity in infancy and showed that frequently-collected cross-sectional datasets could be used for estimating effects. The low prevalence of exclusive breastfeeding in Bangladesh needs to be improved to decrease child morbidity
