    Sistema de evaluación y diagnóstico en psiquiatría forense

    Se presenta un trabajo de investigación destinado a aplicar la tecnología informática a la Psiquiatría forense. Se diseñaron programas de simulación de diálogos inteligentes para el diagnóstico psiquiátrico y el dictamen pericial psiquiátrico, en varias áreas del Derecho: Derecho Penal (Imputabilidad), Derecho Civil (capacidad), Derecho Canónico (consentimiento matrimonial). Se elaboraron árboles de decisión, glosarios médico-psiquiátrico y jurídico, archivos de jurisprudencia, doctrina y bibliografía sobre el tema. Fue éste un trabajo interdisciplinario, dirigido por un médico psiquiatra profesor de Psiquiatría Clínica y Psiquiatría Forense, en el que colaboraron un Ingeniero de Sistemas y tres estudiantes: uno de derecho y dos de ingeniería de sistemas; el equipo de trabajo pertenecía a dos Universidades: La Autónoma (Facultad de Derecho), la industrial de Santander (Facultad de Medicina y Facultad de ingeniería de Sistemas)

    Naturally presented HLA class I–restricted epitopes from the neurotrophic factor S100-β are targets of the autoimmune response in type 1 diabetes

    Type 1 diabetes (T1D) results from the destruction of pancreatic β-cells by the immune system, and CD8+ T lymphocytes are critical actors in this autoimmune response. Pancreatic islets are surrounded by a mesh of nervous cells, the peri-insular Schwann cells, which are also targeted by autoreactive T lymphocytes and express specific antigens, such as the neurotrophic factor S100-β. Previous work has shown increased proliferative responses to whole S100-β in both human T1D patients and the nonobese diabetic (NOD) mouse model. We describe for the first time naturally processed and presented epitopes (NPPEs) presented by class I human leukocyte antigen–A*02:01 (A2.1) molecules derived from S100-β. These NPPEs triggered IFN-γ responses more frequently in both newly diagnosed and long-term T1D patients compared with healthy donors. Furthermore, the same NPPEs are recognized during the autoimmune response leading to diabetes in A2.1-transgenic NOD mice as early as 4 wk of age. Interestingly, when these NPPEs are used to prevent diabetes in this animal model, an acceleration of the disease is observed together with an exacerbation in insulitis and an increase in S100-β–specific cytotoxicity in vaccinated animals.     Four different wreckfish (Polyprion americanus) broodstock batches were maintained in research facilities under different photo and thermo-period conditions, one in Greece, the Helenic Center for Marine Research (HCMR, n = 3) and three in Spain: Instituto Español de Oceanografía (IEO, n = 13) in Vigo, Aquarium Finisterrae (MC2, n = 21) in A Coruña and Consellería do Mar (CMRM, n = 11). The CMRM includes two centers that work together: Instituto Galego de Formación en Acuicultura (IGAFA) and Centro de Investigacións Mariñas (CIMA), both in Pontevedra. During the five years of the project DIVERSIFY (Exploring the biological and socio-economic potential of new-emerging candidate fish species for the expansion of the European aquaculture industry, 2013–2018) works focused on the reproductive biology of the species, broodstock, and larvae nutrition and development of incubation and larval rearing protocols have been carried out. In terms of reproduction, catch methods of new wild animals, the reproductive cycle, sperm characteristics evaluation, and spontaneous and induced spawning methods have been described for wreckfish. Regarding nutrition, the positive effect of two types of enrichment on the fatty acid profiles of Artemia and rotifer has been verified. The relationship between the fatty acid profile of the diets supplied to the broodstock and the fatty acid profile obtained in the oocytes and eggs of the females fed with different diets, has also been demonstrated. Finally, early larval ontogeny has been described and incubation and larval rearing protocols have been proposed based on the results obtained in the different experiments of temperature, growth, survival, and larval feeding that were carried out.Versión del edito

    Everolimus plus minimized tacrolimus on kidney function in liver transplantation: REDUCE, a prospective, randomized controlled study

    Background and aim: reduction in calcineurin inhibitor levels is considered crucial to decrease the incidence of kidney dysfunction in liver transplant (LT) recipients. The aim of this study was to evaluate the safety and impact of everolimus plus reduced tacrolimus (EVR + rTAC) vs. mycophenolate mofetil plus tacrolimus (MMF + TAC) on kidney function in LT recipients from Spain. Methods: the REDUCE study was a 52-week, multicenter, randomized, controlled, open-label, phase 3b study in de novo LT recipients. Eligible patients were randomized (1:1) 28 days post-transplantation to receive EVR + rTAC (TAC levels <_ 5 ng/mL) or to continue with MMF + TAC (TAC levels = 6-10 ng/mL). Mean estimated glomerular filtration rate (eGFR), clinical benefit in renal function, and safety were evaluated. Results: in the EVR + rTAC group (n = 105), eGFR increased from randomization to week 52 (82.2 [28.5] mL/min/1.73 m2 to 86.1 [27.9] mL/min/1.73 m2) whereas it decreased in the MMF + TAC (n = 106) group (88.4 [34.3] mL/min/1.73 m2 to 83.2 [25.2] mL/min/1.73 m2), with significant (p < 0.05) differences in eGFR throughout the study. However, both groups had a similar clinical benefit regarding renal function (improvement in 18.6 % vs. 19.1 %, and stabilization in 81.4 % vs. 80.9 % of patients in the EVR + rTAC vs. MMF + TAC groups, respectively). There were no significant differences in the incidence of acute rejection (5.7 % vs. 3.8 %), deaths (5.7 % vs. 2.8 %), and serious adverse events (51.9 % vs. 44.0 %) between the 2 groups. Conclusion: EVR + rTAC allows a safe reduction in tacrolimus exposure in de novo liver transplant recipients, with a significant improvement in eGFR but without significant differences in renal clinical benefit 1 year after liver transplantation

    Red de coordinación de la implantación del primer curso del grado en Tecnologías de la Información para la Salud

    En este trabajo se ha intentado recoger las acciones que se han llevado a cabo en la coordinación de la implantación del primer curso del nuevo grado de Tecnologías de la Información para la Salud, grado con un marcado carácter multidisciplinar que condiciona la enseñanza de los contenidos. Entre estas acciones se han revisado las guías de cada asignatura participante en la red y se han realizado reuniones de seguimiento de la implantación de las asignaturas para detectar fortalezas y debilidades en el proceso docente. También se ha contado con la opinión de alumnos bien a través de encuestas bien directamente en conversaciones con los representantes del alumnado. Pretendemos que las conclusiones obtenidas tras el análisis de resultados se puedan aplicar el curso que viene y, de esta forma, avanzar en la implantación de la titulación con calidad y, especialmente, con el objetivo de formar egresados que satisfagan las necesidades de la sociedad
