    Protocol for the effective feedback to improve primary care prescribing safety (EFIPPS) study : a cluster randomised controlled trial using ePrescribing data

    High-risk prescribing in primary care is common and causes considerable harm. Feedback interventions to improve care are attractive because they are relatively cheap to widely implement. There is good evidence that feedback has small to moderate effects, but the most recent Cochrane review called for more high-quality, large trials that explicitly test different forms of feedback. The study is a three-arm cluster-randomised trial with general practices being randomised and outcomes measured at patient level. 262 practices in three Scottish Health Board areas have been randomised (94% of all possible practices). The two active arms receive different forms of prescribing safety data feedback, with rates of high-risk prescribing compared with a ‘usual care’ arm. Sample size estimation used baseline data from participating practices. With 85 practices randomised to each arm, then there is 93% power to detect a 25% difference in the percentage of high-risk prescribing (from 6.1% to 4.5%) between the usual care arm and each intervention arm. The primary outcome is a composite of six high-risk prescribing measures (antipsychotic prescribing to people aged ≥75 years; non-steroidal anti-inflammatory drug (NSAID) prescribing to people aged ≥75 without gastroprotection; NSAID prescribing to people prescribed aspirin/clopidogrel without gastroprotection; NSAID prescribing to people prescribed an ACE inhibitor/angiotensin receptor blocker and a diuretic; NSAID prescription to people prescribed an oral anticoagulant without gastroprotection; aspirin/clopidogrel prescription to people prescribed an oral anticoagulant without gastroprotection). The primary analysis will use multilevel modelling to account for repeated measurement of outcomes in patients clustered within practices. The study was reviewed and approved by the NHS Tayside Committee on Medical Research Ethics B (11/ES/0001). The study will be disseminated via a final report to the funder with a publicly available research summary, and peer reviewed publications

Unfortunately, the epidemiology of Pyrenopeziza brassicae, the agent causing this disease, remains poorly understood under Irish growing conditions and fungicides are relied upon to provide control. To investigate if the cropping strategies currently used are the best for the control of disease, we screened three populations of P. brassicae isolates from different regions of Ireland for alterations in the genes targeted by azole and Methyl benzamidazole carbamate fungicides. As molecular mechanisms associated with a decrease in fungicide sensitivity were observed in the populations, the results will be correlated with data from fungicide sensitivity tests using these classes of fungicides.Non peer reviewe

    Viewers base estimates of face matching accuracy on their own familiarity: Explaining the photo-ID paradox

    Matching two different images of a face is a very easy task for familiar viewers, but much harder for unfamiliar viewers. Despite this, use of photo-ID is widespread, and people appear not to know how unreliable it is. We present a series of experiments investigating bias both when performing a matching task and when predicting other people’s performance. Participants saw pairs of faces and were asked to make a same/different judgement, after which they were asked to predict how well other people, unfamiliar with these faces, would perform. In four experiments we show different groups of participants familiar and unfamiliar faces, manipulating this in different ways: celebrities in experiments 1 to 3 and personally familiar faces in experiment 4. The results consistently show that people match images of familiar faces more accurately than unfamiliar faces. However, people also reliably predict that the faces they themselves know will be more accurately matched by different viewers. This bias is discussed in the context of current theoretical debates about face recognition, and we suggest that it may underlie the continued use of photo-ID, despite the availability of evidence about its unreliability

All rights reserved.Light leaf spot is caused by the fungal pathogen Pyrenopeziza brassicae and is the mosteconomically damaging disease of oilseed rape (Brassica napus) in the UK. Current controlrelies on repeated fungicide applications; however, pathogen fungicide-insensitivitydevelopment highlights the need for non-chemical controls like host resistance. A study wasdone to assess light leaf spot disease phenotype on the susceptible B. napus cultivar Charger indifferent treatment conditions; factors studied included plant age and inoculum concentration.Results showed that older plants grown in a controlled-environment cabinet produced the mostvisible symptoms. Plants that received a greater inoculum concentration (105spores/ml) weresignificantly shorter by 5 cm than those inoculated with a smaller inoculum concentration (104spores/ml), suggesting possible correlations between fungal inoculum concentration and plantgrowth. Additionally, > 20 P. brassicae field isolates were collected from leaf samples acrossEngland through single-spore isolation and will be screened for virulence

    Hereditary Angioedema (HAE) is a rare disease and for this reason proper diagnosis and appropriate therapy are often unknown or not available for physicians and other health care providers. For this reason we convened a group of specialists that focus upon HAE from around the world to develop not only a consensus on diagnosis and management of HAE, but to also provide evidence based grades, strength of evidence and classification for the consensus. Since both consensus and evidence grading were adhered to the document meets criteria as a guideline. The outcome of the guideline is to improve diagnosis and management of patients with HAE throughout the world and to help initiate uniform care and availability of therapies to all with the diagnosis no matter where the residence of the individual with HAE exists

    Reflexivity Through a Yoga Class Experience: Preparing for My Health Promotion Without Borders Excursion to Mongolia

    In this paper, I (S. L. Deibert) share my story of discovering the relationship between reflexivity, autoethnography, and yoga through a meaningful experience. Yoga has been an important influence on my physical and mental well-being for over a decade, but I did not consider its implications in my academic life until I was asked to write a reflexive assignment for a course. The task was exploring who I am in connection to my master’s thesis project; the challenge was finding a starting point for my reflexive journey of self-discovery. Frustrated by the latter, I turned to yoga for refuge; instead of escaping the assignment, I found that my quest for self-exploration was intertwined with my yoga practice. The purpose of this paper is to delve further into my experience with yoga as a medium for developing reflexivity. Using autoethnography, I share my journey of developing critical thinking through a narrative related to my yoga class experience. Linking my research to my yoga practice allowed me to better understand myself as a person and researcher, become mindful of how my own views shape my experiences, and develop a deeper level of critical reflection. Overall, this work demonstrates the experience of a connection between yoga, reflexivity, and autoethnography, and adds to the sparse literature exploring the intersection of these three

    Prion infectivity in the spleen of a <em>PRNP</em> heterozygous individual with subclinical variant Creutzfeldt-Jakob disease

    Blood transfusion has been identified as a source of human-to-human transmission of variant Creutzfeldt–Jakob disease. Three cases of variant Creutzfeldt–Jakob disease have been identified following red cell transfusions from donors who subsequently developed variant Creutzfeldt–Jakob disease and an asymptomatic red cell transfusion recipient, who did not die of variant Creutzfeldt–Jakob disease, has been identified with prion protein deposition in the spleen and a lymph node, but not the brain. This individual was heterozygous (MV) at codon 129 of the prion protein gene (PRNP), whereas all previous definite and probable cases of variant Creutzfeldt–Jakob disease have been methionine homozygotes (MM). A critical question for public health is whether the prion protein deposition reported in peripheral tissues from this MV individual correlates with infectivity. Additionally it is important to establish whether the PRNP codon 129 genotype has influenced the transmission characteristics of the infectious agent. Brain and spleen from the MV blood recipient were inoculated into murine strains that have consistently demonstrated transmission of the variant Creutzfeldt–Jakob disease agent. Mice were assessed for clinical and pathological signs of disease and transmission data were compared with other transmission studies in variant Creutzfeldt–Jakob disease, including those on the spleen and brain of the donor to the index case. Transmission of variant Creutzfeldt–Jakob disease was observed from the MV blood recipient spleen, but not from the brain, whereas there was transmission from both spleen and brain tissues from the red blood cell donor. Longer incubation times were observed for the blood donor spleen inoculum compared with the blood donor brain inoculum, suggesting lower titres of infectivity in the spleen. The distribution of vacuolar pathology and abnormal prion protein in infected mice were similar following inoculation with both donor and recipient spleen homogenates, providing initial evidence of similar transmission properties after propagation in PRNP codon 129 MV and MM individuals. These studies demonstrate that spleen tissue from a PRNP MV genotype individual can propagate the variant Creutzfeldt–Jakob disease agent and that the infectious agent can be present in the spleen without CNS involvement

    Berotralstat in hereditary angioedema due to C1 inhibitor deficiency: first real-world evidence from a Canadian center

    BackgroundHereditary angioedema due to C1 inhibitor deficiency is a rare genetic condition that causes recurrent swelling with consequent functional impairment and decreased quality of life. Long-term prophylaxis (LTP) to prevent angioedema episodes is a key component of disease management. Berotralstat, an oral, once-daily plasma kallikrein inhibitor, was approved for LTP by Health Canada in 2022.MethodsWe conducted a retrospective, real-world study investigating the effectiveness and adverse effects of berotralstat. Data on angioedema frequency, disease control, and adverse events were tabulated. Patient satisfaction with treatment was scored on a 5-point Likert scale, with 1 representing very unsatisfied and 5 representing very satisfied with therapy.ResultsFrom June, 2022 and May, 2023, 8 patients with HAE type 1 or type 2 received berotralstat. Effectiveness data were available for 7 patients who continued the drug for at least 3 months, 4 of whom switched to berotralstat from plasma-derived C1 inhibitor LTP. In these 7 patients, the average number of attacks per month decreased from 3.3 to 1.6 (p&lt;0.05), representing a ~52% reduction in attack frequency. Median angioedema control test score numerically improved from 8 to 13 (p=0.0781). Of the 8 patients who received berotralstat, 3 reported no adverse effects and 5 experienced gastrointestinal side effects, which were mild and transient in 3 and led to discontinuation in 1. Average treatment satisfaction was between satisfied and very satisfied at 4.3.ConclusionBerotralstat is an effective agent for long-term prophylaxis in HAE. Most patients experienced no adverse effects or mild, transient gastrointestinal symptoms
