    Fatores associados ao tempo de permanência hospitalar de mulheres submetidas à cesariana

    OBJECTIVE: To evaluate whether age group, complications or comorbidities are associated with the length of hospitalization of women undergoing cesarean section. METHODS: A cross-sectional study was carried out between June 2012 and July 2017, with 64,437 women undergoing cesarean section and who did not acquire conditions during their hospital stay. Hospital discharge data were collected from national health institutions, using the Diagnosis-Related Groups system (DRG Brasil). The DRG referring to cesarean section with additional complications or comorbidities (DRG 765) and cesarean section without complications or associated comorbidities (DRG 766) were included in the initial diagnosis. The influence of age group and comorbidities or complications present at admission on the length of hospital stay was assessed based on the means of the analysis of variance. The size of the effect was verified by Cohen’s D, which allows evaluating clinical relevance. The criticality levels were identified using the Duncan test. RESULTS: The longest length of hospital stay was observed in the age group from 15 to 17 years old and among those aged 45 years old or more. The hospital stay of women with complications or comorbidities at the time of admission was also longer. Moreover, it was noted that the increase in criticality level was associated with an increase in the mean length of hospital stay. CONCLUSIONS: The length of hospital stay of women is higher among those belonging to the age group ranging from 15 to 17 years old and for those aged 45 years old or more. The presence of associated comorbidities, such as eclampsia, pre-existing hypertensive disorder with superimposed proteinuria and gestational hypertension (induced by pregnancy) with significant proteinuria increase the length of hospital stay. This study enabled the construction of distinct criticality level profiles based on the combination of age groups and the main comorbidities, which were directly related to the length of hospital stay.OBJETIVO: Avaliar se grupo etário, complicações ou comorbidades estão associados ao tempo de internação de mulheres submetidas à cesariana. MÉTODOS: Estudo transversal realizado entre junho de 2012 e julho de 2017, com 64.437 mulheres submetidas à cesariana e que não apresentaram condições adquiridas durante o tempo de permanência hospitalar. Os dados foram coletados a partir da alta hospitalar nas instituições nacionais de saúde, utilizando o sistema Diagnosis-Related Groups [Grupos de Diagnósticos Relacionados] (DRG Brasil). Foram incluídos os DRG referentes à cesariana com complicações ou comorbidades adicionais ao diagnóstico inicial (DRG 765) e cesariana sem complicações ou comorbidades associadas (DRG 766). A influência do grupo etário e comorbidades ou complicações presentes na admissão sobre o tempo de permanência hospitalar foi avaliada por meio da análise de variância. O tamanho do efeito foi verificado pelo d de Cohen, que permite avaliar a relevância clínica. Os níveis de criticidade foram identificados utilizando o teste de Duncan. RESULTADOS: O maior tempo de permanência hospitalar foi observado nos grupos etários de 15 a 17 anos e 45 anos ou mais. Mulheres que apresentaram complicações ou comorbidades presentes à admissão também apresentaram maior tempo de permanência hospitalar. Quantos aos níveis de criticidade, notou-se que seu aumento estava associado ao aumento na média do tempo de permanência hospitalar. CONCLUSÕES: O tempo de permanência hospitalar de mulheres é maior entre aquelas pertencentes aos grupos etários de 15 a 17 anos e 45 anos ou mais. A presença de comorbidades associadas, como a eclâmpsia, o distúrbio hipertensivo pré-existente com proteinúria superposta e a hipertensão gestacional (induzida pela gravidez) com proteinúria significativa aumentam o tempo de permanência hospitalar. Este estudo possibilitou a construção de perfis distintos de níveis de criticidade a partir da combinação de grupos etários e das principais comorbidades, os quais se apresentaram diretamente relacionados ao tempo de permanência hospitalar

    OBJETIVO: comparar os resultados neonatais dos partos vaginais espontâneos ou assistidos com fórcipe de Simpson-Braun em nulíparas. MÉTODO: em estudo de corte retrospectivo foram avaliados dois grupos de primíparas atendidas no Centro Obstétrico do CAISM/UNICAMP, que tiveram parto vaginal sob analgesia epidural. O grupo fórcipe foi formado por 119 pacientes que tiveram parto a fórcipe de Simpson-Braun, e o grupo normal por 114 casos de parto vaginal espontâneo. Foram estudadas as variáveis neonatais imediatas como o estado do líquido amniótico e os índices de Apgar, assim como a evolução neonatal nos primeiros dias de vida. Para análise estatística foram utilizados os testes c², exato de Fisher e t de Student para comparação de médias e considerada significativa a diferença correspondente a p<0,05. RESULTADOS: as indicações para uso do fórcipe Simpson-Braun foram: alívio materno-fetal (90 casos) e abreviação de período expulsivo (29 casos). Nas pacientes atendidas com fórcipe Simpson-Braun houve 8 casos de laceração de canal de parto (6,7%), valor semelhante ao observado no grupo de parto vaginal espontâneo. As médias de dias de internação das puérperas e neonatos foram idênticas (2,4). Os dois grupos foram semelhantes quanto aos índices de Apgar menores que 7 no primeiro (7,5 e 4,3%) e quinto minuto (1,6 e 1,7%) e quanto ao peso dos recém-nascidos (3.146 e 3.016 g). A evolução neonatal foi semelhante nos dois grupos. CONCLUSÃO: o uso do fórcipe de Simpson-Braun mostrou-se seguro, quando comparado ao parto vaginal espontâneo.<br>PURPOSE: to compare maternal and neonatal outcomes between spontaneous vaginal and Simpson-Braun forceps deliveries in nulliparous women. METHOD: a retrospective study including two groups of nulliparous women, who had vaginal delivery under peridural anesthesia in the obstetric unit of the CAISM-UNICAMP: the forceps group included 119 women who had Simpson-Braun forceps delivery, and the normal group included 114 women who delivered spontaneously. Neonatal outcomes, such as Apgar score and the evolution in the first days of life, were studied. Data were compared in both groups and, for statistical analysis, c² test, Fisher exact, and Student t tests were used. The differences were considered significant when p<0.05. RESULTS: the indications for Simpson-Braun forceps delivery were maternal-fetal relief (90 cases) and abbreviation of the expulsive period (29 cases). In the forceps group there were 8 cases (6.7%) of vaginal injuries; a similar result was observed in the normal group. The number of hospitalization days for the parturient and the newborns was identical, 2.4 days. The newborns in the two groups had similar Apgar scores inferior to 7 at the first minute (7.5 vs 4.3%) and at the fifth minute (1.6 vs1.7%). The weight in the two groups had similar results (3,146 and 3,016 g). The first days of life did not differ between the groups. CONCLUSIONS: the use of Simpson-Braun forceps was safe, when compared to spontaneous vaginal delivery

    While monoethylene glycol (MEG) is an efficient alternative to prevent the generation of hydrates into the natural gas (NG) processing pipes, this specie also generates undesirable effects such as pipe corrosion, catalyst poisoning, quality loss of the fuel, and environment contamination. Thus, MEG is removed from the system in final stages of the NG processing and is regenerated for reuse, making mandatory its monitoring in both regenerated samples and fuels such as the NG condensate (NGC). Herein, we address a simple and fast method to determine MEG traces in NGC which was based on two stages: microfluidic liquid–liquid extraction (LLE) and electrochemical detection. High throughput (residence time of 0.05 s) and efficient LLEs were obtained in a single run by pumping the immiscible phases at harsh flow rates (up to 40 mL min–1) into a bulky chip (without interface) composed of bisphenol A (BSA)-based epoxy resin, which was prototyped using a clean-room-free and bondless approach. This unprecedented substrate in microfluidics showed resistance to elastic deformation and swelling in different organic media. The extraction was essential to allow the electrochemical determination of MEG, in which the aqueous acceptor phase from LLE was used as an electrolytic sample. Nickel disks modified with Ni(OH)2 nanoparticles provided a sensitive quantification of MEG, because of the high electrode surface area and catalytic activity of Ni(OH)2 for the irreversible oxidation of MEG. This electrode further requires a simple surface modification.     The Neotropical region hosts 4225 freshwater fish species, ranking first among the world's most diverse regions for freshwater fishes. Our NEOTROPICAL FRESHWATER FISHES data set is the first to produce a large-scale Neotropical freshwater fish inventory, covering the entire Neotropical region from Mexico and the Caribbean in the north to the southern limits in Argentina, Paraguay, Chile, and Uruguay. We compiled 185,787 distribution records, with unique georeferenced coordinates, for the 4225 species, represented by occurrence and abundance data. The number of species for the most numerous orders are as follows: Characiformes (1289), Siluriformes (1384), Cichliformes (354), Cyprinodontiformes (245), and Gymnotiformes (135). The most recorded species was the characid Astyanax fasciatus (4696 records). We registered 116,802 distribution records for native species, compared to 1802 distribution records for nonnative species. The main aim of the NEOTROPICAL FRESHWATER FISHES data set was to make these occurrence and abundance data accessible for international researchers to develop ecological and macroecological studies, from local to regional scales, with focal fish species, families, or orders. We anticipate that the NEOTROPICAL FRESHWATER FISHES data set will be valuable for studies on a wide range of ecological processes, such as trophic cascades, fishery pressure, the effects of habitat loss and fragmentation, and the impacts of species invasion and climate change.     Brazilian Flora 2020: Leveraging the power of a collaborative scientific network

    International audienceThe shortage of reliable primary taxonomic data limits the description of biological taxa and the understanding of biodiversity patterns and processes, complicating biogeographical, ecological, and evolutionary studies. This deficit creates a significant taxonomic impediment to biodiversity research and conservation planning. The taxonomic impediment and the biodiversity crisis are widely recognized, highlighting the urgent need for reliable taxonomic data. Over the past decade, numerous countries worldwide have devoted considerable effort to Target 1 of the Global Strategy for Plant Conservation (GSPC), which called for the preparation of a working list of all known plant species by 2010 and an online world Flora by 2020. Brazil is a megadiverse country, home to more of the world's known plant species than any other country. Despite that, Flora Brasiliensis, concluded in 1906, was the last comprehensive treatment of the Brazilian flora. The lack of accurate estimates of the number of species of algae, fungi, and plants occurring in Brazil contributes to the prevailing taxonomic impediment and delays progress towards the GSPC targets. Over the past 12 years, a legion of taxonomists motivated to meet Target 1 of the GSPC, worked together to gather and integrate knowledge on the algal, plant, and fungal diversity of Brazil. Overall, a team of about 980 taxonomists joined efforts in a highly collaborative project that used cybertaxonomy to prepare an updated Flora of Brazil, showing the power of scientific collaboration to reach ambitious goals. This paper presents an overview of the Brazilian Flora 2020 and provides taxonomic and spatial updates on the algae, fungi, and plants found in one of the world's most biodiverse countries. We further identify collection gaps and summarize future goals that extend beyond 2020. Our results show that Brazil is home to 46,975 native species of algae, fungi, and plants, of which 19,669 are endemic to the country. The data compiled to date suggests that the Atlantic Rainforest might be the most diverse Brazilian domain for all plant groups except gymnosperms, which are most diverse in the Amazon. However, scientific knowledge of Brazilian diversity is still unequally distributed, with the Atlantic Rainforest and the Cerrado being the most intensively sampled and studied biomes in the country. In times of “scientific reductionism”, with botanical and mycological sciences suffering pervasive depreciation in recent decades, the first online Flora of Brazil 2020 significantly enhanced the quality and quantity of taxonomic data available for algae, fungi, and plants from Brazil.
