La Reforma Laboral en clave de contratación
[spa] La excesiva temporalidad constituye una problemática de inmensa envergadura para la sanidad de cualquier mercado de trabajo ya que, junto con otros factores como podrían ser una elevada tasa de desempleo o la existencia de remuneraciones insuficientes para afrontar los gastos inherentes a la propia subsistencia, genera una precariedad laboral generalizada entre las personas trabajadoras con todas las implicaciones socioeconómicas que ello conlleva.
Una de las formas más eficaces de hacer frente a esta situación, dejando a un lado las medidas económicas que cada Estado pudiera adoptar, es la de modificar la legislación vigente para tratar de adaptar la estructura de este mercado a la realidad de la situación. Por ello, en un momento en que el mercado de trabajo español presentaba elevadas tasas de temporalidad y desempleo, agravado además por la explosión de una crisis económico-sanitaria a causa de la pandemia de la COVID-19, el Gobierno sacó adelante el Real Decreto-ley 32/2021, de 28 de diciembre, con el objetivo principal de aportar algo de estabilidad a la situación.
Las medidas adoptadas con este fin van dirigidas principalmente a la potenciación de la contratación indefinida y a dificultar la contratación temporal mediante la eliminación de tipos contractuales de esta última modalidad, la reducción de los límites temporales para la transformación de un contrato temporal a indefinido y el fortalecimiento de la presunción de que un contrato es indefinido salvo que lo contrario se acredite debidamente.
Un año después de la entrada en vigor del citado texto normativo, los pocos datos disponibles sobre los efectos derivados del mismo arrojan una primera perspectiva positiva ya que muestran una importante reducción de la tasa de temporalidad y una estimulación sin precedentes de la contratación indefinida.
No obstante, dada la brevedad de la vigencia de la norma, es pronto todavía para afirmar sin lugar a dudas que se trata de una norma acertada en su integridad siendo necesario un análisis periódico de la evolución de los datos disponibles en cada momento para poder hacerlo.[eng] Excessive temporary employment constitutes a huge problem for the health of any labour market since, together with other factors such as a high unemployment rate or the existence of insufficient remuneration to face the expenses inherent to one’s subsistence, it generates a widespread job insecurity among workers with all the socioeconomic implications that this entails.
One of the most effective ways to deal with this situation, leaving the economic measures that each State could adopt aside, is to modify the current legislation with the objective of adapting the structure of this market to the reality of the situation. Thus, at a time when the Spanish labour market showed high rates of temporary employment and unemployment, furthermore aggravated by the explosion of an economic-sanitary crisis due to the COVID-19 pandemic, the Government got the Royal Decree-Law 32/2021 of December 28th off the ground with the main objective of providing some stability to the situation.
The measures taken for this purpose are mainly directed to promoting indefinite contracts and making temporary contracts more difficult to sign by eliminating contractual modalities of this type, reducing the time limits for the transformation of a temporary contract into indefinite one and strengthening of the presumption that a contract is indefinite unless the contrary is duly accredited.
One year after the entry into force of the aforementioned regulatory text, the few data available on the effects derived from it show a first positive perspective, since they show a significant reduction in the temporary employment rate and an unprecedented stimulation of indefinite contracts.
However, given the brevity of the regulation’s validity, it is still early to affirm without a doubt that it is a correct regulation in its entirety. In order to do so, it would be necessary to carry out a periodic analysis of the evolution of the data available at all times

A Silent Corticotroph Pituitary Carcinoma: Lessons From an Exceptional Case Report
Nowadays, neither imaging nor pathology evaluation can accurately predict the aggressiveness or treatment resistance of pituitary tumors at diagnosis. However, histological examination can provide useful information that might alert clinicians about the nature of pituitary tumors. Here, we describe our experience with a silent corticothoph tumor with unusual pathology, aggressive local invasion and metastatic dissemination during follow-up. We present a 61-year-old man with third cranial nerve palsy at presentation due to invasive pituitary tumor. Subtotal surgical approach was performed with a diagnosis of silent corticotroph tumor but with unusual histological features (nuclear atypia, frequent multinucleation and mitotic figures, and Ki-67 labeling index up to 70%). After a rapid regrowth, a second surgical intervention achieved successful debulking. Temozolomide treatment followed by stereotactic fractionated radiotherapy associated with temozolomide successfully managed the primary tumor. However, sacral metastasis showed up 6 months after radiotherapy treatment. Due to aggressive distant behavior, a carboplatine-etoposide scheme was decided but the patient died of urinary sepsis 31 months after the first symptoms. Our case report shows how the presentation of a pituitary tumor with aggressive features should raise a suspicion of malignancy and the need of follow up by multidisciplinary team with experience in its management. Dietary α‐Linolenic Acid, Marine ω‐3 Fatty Acids, and Mortality in a Population With High Fish Consumption: Findings From the PREvención con DIeta MEDiterránea (PREDIMED) Study
Background: Epidemiological evidence suggests a cardioprotective role of α‐linolenic acid (ALA), a plant‐derived ω‐3 fatty acid. It is unclear whether ALA is beneficial in a background of high marine ω‐3 fatty acids (long‐chain n‐3 polyunsaturated fatty acids) intake. In persons at high cardiovascular risk from Spain, a country in which fish consumption is customarily high, we investigated whether meeting the International Society for the Study of Fatty Acids and Lipids recommendation for dietary ALA (0.7% of total energy) at baseline was related to all‐cause and cardiovascular disease mortality. We also examined the effect of meeting the society's recommendation for long‐chain n‐3 polyunsaturated fatty acids (≥500 mg/day). Methods and Results: We longitudinally evaluated 7202 participants in the PREvención con DIeta MEDiterránea (PREDIMED) trial. Multivariable‐adjusted Cox regression models were fitted to estimate hazard ratios. ALA intake correlated to walnut consumption (r=0.94). During a 5.9‐y follow‐up, 431 deaths occurred (104 cardiovascular disease, 55 coronary heart disease, 32 sudden cardiac death, 25 stroke). The hazard ratios for meeting ALA recommendation (n=1615, 22.4%) were 0.72 (95% CI 0.56–0.92) for all‐cause mortality and 0.95 (95% CI 0.58–1.57) for fatal cardiovascular disease. The hazard ratios for meeting the recommendation for long‐chain n‐3 polyunsaturated fatty acids (n=5452, 75.7%) were 0.84 (95% CI 0.67–1.05) for all‐cause mortality, 0.61 (95% CI 0.39–0.96) for fatal cardiovascular disease, 0.54 (95% CI 0.29–0.99) for fatal coronary heart disease, and 0.49 (95% CI 0.22–1.01) for sudden cardiac death. The highest reduction in all‐cause mortality occurred in participants meeting both recommendations (hazard ratio 0.63 [95% CI 0.45–0.87]). Conclusions: In participants without prior cardiovascular disease and high fish consumption, dietary ALA, supplied mainly by walnuts and olive oil, relates inversely to all‐cause mortality, whereas protection from cardiac mortality is limited to fish‐derived long‐chain n‐3 polyunsaturated fatty acids.
