Magnetic resonance imaging and neuropsychological testing in the spectrum of normal aging

Get PDF
OBJECTIVE: To understand the relationships between brain structures and function (behavior and cognition) in healthy aging. METHOD: The study group was composed of 56 healthy elderly subjects who underwent neuropsychological assessment and quantitative magnetic resonance imaging. Cluster analysis classified the cohort into two groups, one (cluster 1) in which the magnetic resonance imaging metrics were more preserved (mean age: 66.4 years) and another (cluster 2) with less preserved markers of healthy brain tissue (mean age: 75.4 years). RESULTS: The subjects in cluster 2 (older group) had worse indices of interference in the Stroop test compared with the subjects in cluster 1 (younger group). Therefore, a simple test such as the Stroop test could differentiate groups of younger and older subjects based on magnetic resonance imaging metrics. CONCLUSION: These results are in agreement with the inhibitory control hypotheses regarding cognitive aging and may also be important in the interpretation of studies with other clinical groups, such as patients with dementia and mild cognitive impairment

Implementing an Electrocardiography Telediagnostic Service in Pernambuco, Brazil

In Brazil, cardiovascular diseases account for more than 10% of hospital admissions, and are considered a significant contributor to healthcare costs. One of the most frequently ordered diagnostic tests in the investigation of cardiovascular diseases is the 12-lead electrocardiogram (ECG). Cardiologists are not widely available to provide reports in small municipalities in Brazil, and eHealth services can be a feasible alternative to low resource environments. The use of tele-electrocardiography (Tele-ECG) can improve the management of patients and support decision making regarding referral to advanced medical centres. This study describes the implementation of a telediagnostic service for Tele-ECG in the state of Pernambuco, Brazil. The service was provided as a campaign in each community and was conducted by a team composed of a nurse and a technician from the university telehealth unit using a telehealth platform HealthNet, a laptop and a mobile digital electrocardiograph. In 2016, 34 visits were held to municipalities in Pernambuco, with 6,138 ECGs performed on patients from waiting lists, 63.5% of whom had never had an ECG, with 39.6%of the tests showing evidences of abnormal clinical situations and 0.1%) showing artefacts. The field interventions highlighted the contribution of telehealth as a feasible strategy to enhance access to care and reducing time to ECG reports, the need for patient transportation and costs for the Brazilian healthcare system, the Unified Health System. Considering the restricted budgets of the municipalities to acquire digital devices to perform ECGs and the shortage of specialised medical staff, the success of this field intervention increased the interest of public managers for telediagnostic services

Normal aging is associated with morphological alte-rations in brain. Ventricular system is located deep inside brain and reflect the overall process of parenchymal atrophy. Once neurode-generative disorders course with more prominent dilatation of brain ventricles, to establish normative volumetric parameters from Brazi-lian healthy old individuals is necessary, and it may be an additional tool on differentiation from the normal to pathological. Objective: To investigate brain ventricular volume changes in Brazilian healthy el-derly people. Methods: Transversal study using magnetic resonance imaging (1.5T) of the brain from 21 elderly healthy volunteers (67À6 years old). Data were assessed with manual segmentation techni-que. Regions of interest were the brain ventricles and intracranial volumes. Old (60-69 years old, 15 women) and Older (69 years old) groups were created for analysis. Results: Volume of all ventricular compartments significantly increased (p.001) with age, with excep-tion of the fourth ventricle. The third and lateral ventricles increased between groups 2.1- and 2.8-fold, respectively. Mean total ventricular volume was 1.2À.4% of intracranial volume in Old and 3.2À1.8% in Ol-der group (p.001), which represents 15À6ml and 40À24ml (p=.001), respectively. We observed a moderate to strong positive correlation between ventricular volume and age, with the best correlation in the third ventricle (r=.710). Total intracranial volume diminished with age, but without statistical significance. Conclusions: Brain ventricles vo-lume increased significantly with age in healthy old individuals, with exception of the fourth ventricle

Introduction:Depression has emerged as the most prevalent mental disorder in patients with fibromyalgia. Stress, whose stages are alarm, resistance, near-exhaustion and exhaustion, constitutes a physical reaction to a threatening situation. Objective:To investigate the levels of stress, anxiety and depression in women with fibromyalgia, comparing them with those of healthy women.Patients and methods:Participants were 50 women, 25 with a diagnosis of fibromyalgia according to the criteria of the American College of Rheumatology, and 25 without this diagnosis, matched for age. Instruments used: Lipp Inventory of Stress Symptoms for Adults (LISS), State-Trait Anxiety Inventory (STAI) and Beck Depression Inventory (BDI).Results:The mean age was 49.36 years for the group with fibromyalgia (FM) and 49.20 years for the group without fibromyalgia (non-FM). FM showed a higher incidence of stress (96%) compared with non-FM (5%). The resistance phase was predominant in both groups, FM (42%) and non-FM (100%). In FM there was distribution of the four stages (alarm, resistance, near-exhaustion and exhaustion). The differences between phases in the analyzed groups were significant (p < 0.001). FM showed predominance of psychological symptoms (54%); non-FM did show the same frequency of psychological and physical/psychological (40%) symptoms. Symptoms of state and trait anxiety and of depression in FM were significantly higher, when compared with non-FM (p < 0.01).Conclusion:Stress index (96%), trait anxiety (over 50) and clinically relevant depression (greater than 20) in FM were relevant. The understanding of the emotional variables involved in fibromyalgia is important to define the therapeutic strategy.

Bloodstream infection (BSI) by species of Candida has been identified as an important cause of death in patients with neutropenia who undergo chemotherapy for the treatment of hematologic malignancies This study aimed to verify the occurrence of bloodstream infections by Candida species in patients admitted to the haematology-oncology service of a public hospital specialized in the treatment of cancer in Northeast Brazil. A total of 105 clinical samples from 62 patients with haematological malignancies were analyzed at the Laboratory of Medical Mycology at the Federal University of Pernambuco. Only 7 of 105 individuals were in the ICU environment. The mycological diagnosis was performed through automation (BACTEC 9120 / PHOENIX™), proteomic identification (MALDI-TOF MS) and molecular analysis (PCR). The antifungal susceptibility test followed the bloodstream infection recommendations of Clinical & Laboratory Standards Institute. Among the samples studied, nine strains (8,57%) were of the genus Candida, being six C. tropicalis and three C. albicans. The isolates were completely susceptible to the antifungal agents tested. Deaths occurred in 66,6% of the cases. Patients with hematologic malignancies hospitalized in intensive care and the state of septic shock present a higher risk of occurrence of BSI by Candida and death by this opportunistic pathogen
