    La ventilación asistida es una técnica aplicada a pacientes graves con problemas respiratorios debido a múltiples factores patológicos empleada en salas de cuidados intensivos y quirófanos. Los sistemas actuales de respiración asistida (ventiladores inteligentes), suelen incorporar sondas invasivas complejas con múltiples sensores, los cuales son colocados vía esofágica lo que presupone elevados daños para el paciente durante su colocación y uso. Además, suelen ser empleados como conductos para garantizar la alimentación de los sujetos durante largos períodos de recuperación clínica, lo cual impone la necesidad de su reposición periódica ( 95%) existente entre los registros torácicos y la medición nasal de la frecuencia respiratoria, lo cual permite simplificar el proceso (medición no invasiva) de seguimiento de la mecánica respiratoria en individuos con ventilación asistida

    This work shows the study and characterization of whole-body electric impedance measurement with a channel of development KIT (AD5933) and an arrangement of electrodes placed in the superior and inferior extremities in voluntary healthy peoples and with renal critical problems. The characteristics of measurement are analyzed with the study of a phantom developed at laboratory level, where is demonstrated the correspondence among the variability of the impedance with regard to the loss of the volume of the liquid stored, similar as dialysis process. We are carried out measurements in 17 voluntary people (men and women between 20 and 85 years), being analyzed the variability of the whole-body electric impedance in the interval from 5 up to 55 kHz. We observed the relationship among the differential average of whole-body electrical impedance measurement variability and their relation with the Dry Weight variable obtained before and later to hemodialysis process in different studied peoples.El trabajo aborda el estudio y caracterización de la medición de impedancia eléctrica corporal con ayuda de un canal de medición de un KIT de desarrollo (AD5933) y un arreglo de electrodos colocados en las extremidades superiores e inferiores en sujetos voluntarios, sanos y con problemas renales crónicos. Se analizan las características de las mediciones realizadas a un phantom desarrollado a nivel de laboratorio, donde se demuestra la correspondencia entre la variabilidad de la impedancia con respecto a la pérdida del volumen de líquido almacenado en el mismo, similar a lo que ocurre durante el proceso de hemodiálisis. Se realizan mediciones a una muestra de 17 sujetos voluntarios (hombres y mujeres comprendidas entre 20 y 85 años), analizándose la variabilidad de la impedancia eléctrica corporal en el intervalo desde 5 kHz hasta 55 kHz. Se observó la relación entre el promedio de la variabilidad diferencial de la impedancia corporal medida y su posible relación con la variable Peso Seco obtenida antes y posterior al proceso de hemodiálisis en los sujetos estudiados

    Detection of pathogenic microorganisms in urine samples with BluBAC system

    The main purpose of this research was to evaluate the performance of BluBAC- an innovative system for the detection of pathogenic microorganisms in urine samples (uroculture). BluBAC system is based on the combination of photostimulation and turbidimetry. The urine samples were analyzed by the following three methods: direct count in Petri dish (reference method), DIRAMIC system and BluBAC system. Based on the conventional protocol for the obtention of samples for the analysis of uroculture, 350 samples from inpatients and outpatients. Results show a 100 % coincidence with the reference method and a detection time lower than 20 minutes in the 90 samples classified as positive. Although the obtained results must be amplified and corroborated with other studies, the sensitivity shown by BluBAC system in the detection of various enterobacteria is a new alternative to the diagnosis of urinary tract infections if consideration is given to the high costs of the systems for the rapid microbiological diagnosis available in the market and their negative impact on the economy of small and medium-sized laboratories, clinics and hospitals

    Specific association of HLA-DRB1*03 with anti-carbamylated protein antibodies in patients with rheumatoid arthritis

    [Objective]: Recognition of a new type of rheumatoid arthritis (RA)-specific autoantibody, the anti-carbamylated protein antibodies (anti-CarP), has provided an opportunity to improve the management and understanding of RA. The current study was undertaken to assess the relationship between anti-CarP antibodies and HLA-DRB1 alleles in RA. [Methods]: Serum samples were obtained from 3 different collections, comprising a total of 1,126 RA patients. Serum reactivity against in vitro carbamylated fetal calf serum proteins was determined by enzyme-linked immunosorbent assay. HLA-DRB1 alleles were determined using either hybridization techniques or imputation from HLA-dense genotypes. Results of these analyses were combined in a meta-analysis with data from 3 previously reported cohorts. The carrier frequencies of the common HLA-DRB1 alleles were compared between the antibody-positive RA subgroups and the double-negative subgroup of RA patients stratified by anti-citrullinated protein antibody (ACPA)/anti-CarP antibody status, and also between the 4 RA patient strata and healthy controls. [Results]: Meta-analysis was conducted with 3,709 RA patients and 2,305 healthy control subjects. Results revealed a significant increase in frequency of HLA-DRB1*03 carriers in the ACPA-/anti-CarP+ subgroup as compared to ACPA-/anti-CarP- RA patients and healthy controls; this was consistently found across the 6 sample collections. This association of HLA-DRB1*03 with ACPA-/anti-CarP+ RA was independent of the presence of the shared allele (SE) and any other confounders analyzed. No other allele was specifically associated with the ACPA-/anti-CarP+ RA patient subgroup. In contrast, frequency of the SE was significantly increased in the ACPA+/anti-CarP- and ACPA+/anti-CarP+ RA patient subgroups, without a significant distinction between them. Furthermore, some alleles (including HLA-DRB1*03) were associated with protection from ACPA+ RA.     La anterior selección, motivados por la aseveración de Manuel Castells -hace casi 20 años ya- que la innovación y la difusión de la tecnología parecía ser la herramienta apropiada para el desarrollo en la era de la información. Este 2020, sin embargo, ante la situación disruptiva que aquejó y aqueja a la sociedad red como una estructura social emergente de la Era de la Información basada en redes de producción, energizadas por el poder y la experiencia; falló y debe reencontrar su rumbo. Es así que los problemas acuciantes, ahora, fueron: la atención sanitaria y la superación de la epidemia de Sars Cov 2; tomó forma la, hasta entonces, visión irrealista de Castells que … no podemos avanzar con nuestros modelos de desarrollo actual, destruyendo nuestro entorno y excluyendo a la mayor parte de la humanidad de los beneficios de la revolución tecnológica más extraordinaria de la historia, sin sufrir una devastadora reacción por parte de la sociedad y la naturaleza. Fue así que el Cuarto Mundo, específicamente, donde la suficiencia de recurso humano, de capital, trabajo, información y mercado -vinculados todos a través de la tecnología- supuso que atendería eficazmente a través de la población que podía por su capacidad hacer uso racional y profesional del conocimiento, las necesidades de la mayoritaria población vulnerable y vulnerada. Por lo anterior, poner en el centro a las personas, en entornos de tarea y trabajo globales hiperconectados combinando espacios físicos, corrientes de información con canales de conexión expeditos, y formando profesionales del conocimiento que asuman y afronten los retos derivados de la transformación digital de empresas, universidades, y organizaciones, pero en condiciones de equidad y sujetos de prosperidad, será el desafío en los escenarios presentes y futuros inmediatos.The previous selection, motivated by the assertion of Manuel Castells -almost 20 years ago- that innovation and diffusion of technology seemed to be the appropriate tool for development in the information age. This 2020, however, in the face of the disruptive situation that afflicted and continues to afflict the network society as an emerging social structure of the Information Age based on production networks, energized by power and experience; He failed and must find his way again.     Contemporary use of cefazolin for MSSA infective endocarditis: analysis of a national prospective cohort

    Objectives: This study aimed to assess the real use of cefazolin for methicillin-susceptible Staphylococcus aureus (MSSA) infective endocarditis (IE) in the Spanish National Endocarditis Database (GAMES) and to compare it with antistaphylococcal penicillin (ASP). Methods: Prospective cohort study with retrospective analysis of a cohort of MSSA IE treated with cloxacillin and/or cefazolin. Outcomes assessed were relapse; intra-hospital, overall, and endocarditis-related mortality; and adverse events. Risk of renal toxicity with each treatment was evaluated separately. Results: We included 631 IE episodes caused by MSSA treated with cloxacillin and/or cefazolin. Antibiotic treatment was cloxacillin, cefazolin, or both in 537 (85%), 57 (9%), and 37 (6%) episodes, respectively. Patients treated with cefazolin had significantly higher rates of comorbidities (median Charlson Index 7, P <0.01) and previous renal failure (57.9%, P <0.01). Patients treated with cloxacillin presented higher rates of septic shock (25%, P = 0.033) and new-onset or worsening renal failure (47.3%, P = 0.024) with significantly higher rates of in-hospital mortality (38.5%, P = 0.017). One-year IE-related mortality and rate of relapses were similar between treatment groups. None of the treatments were identified as risk or protective factors. Conclusion: Our results suggest that cefazolin is a valuable option for the treatment of MSSA IE, without differences in 1-year mortality or relapses compared with cloxacillin, and might be considered equally effective
