    The Open Data Movement: Young Activists between Data Disclosure and Digital Reputation

    Young citizens show an increasing interest for direct democracy tools and for the building of a new relationship with public administration through the use of digital platforms. The Open Data issue is part of this transformation. The paper analyzes the Open Data issue from the perspective of a spontaneous and informal group of digital activists with the aim of promoting data disclosure. The study is focused mainly on the case of a specific local movement, named Open Data Sicilia (ODS), combining traditional ethnographic observation with an ethnographic approach. The aim of the study is to detect the social pro-file of the Open Data movement activists, understanding how is it organized their network, what are the common purposes and solidarity models embodied by this type of movement, what are the resources mo-bilized and their strategies between on-line and off-line. The ODS case appears interesting for its evolu-tion, its strategy and organizational structure: an elitist and technocratic movement that aspires to a broad constituency. It is an expressive or a reformist movement, rather than an anti-system actor, with features that are similar to a lobby. The case study also shows all the typical characteristics of digital activism, with its fluid boundaries between ethical inspiration of civic engagement and individual interest

    Prospective double-blind randomised controlled trial protocol comparing bone marrow aspirate concentrate intra-articular injection combined with subchondral injection versus intra-articular injection alone for the treatment of symptomatic knee osteoarthritis

    Introduction: Subchondral and intra-articular injections of bone marrow aspirate concentrate (BMAC) showed promising results for knee osteoarthritis (OA) patients. To date, there is no evidence to demonstrate whether the combination of these treatments provides higher benefits than the intra-articular injection alone. Methods and analysis: Eighty-six patients with symptomatic knee OA (aged between 40 and 70 years) are randomised to BMAC intra-articular injection combined with subchondral BMAC injection or BMAC intra-articular injection alone in a ratio of 1:1. The primary outcome is the total Western Ontario and McMaster Universities Osteoarthritis Index, the secondary outcomes are the International Knee Documentation Committee Subjective and Objective Knee Evaluation Form, the Tegner activity scale, the EuroQol-Visual Analogue Scale, and the health questionnaire European Quality of Life Five Dimension score. Additional CT and MRI evaluations are performed at the baseline assessment and at the final 12-month follow-up. The hypothesis is that the combined injections provide higher knee pain and function improvement compared with BMAC intra-articular injection alone. The primary analysis follows an intention to treat principle. Ethics and dissemination: The study protocol has been approved by the Emilia Wide Area Ethical Committee of the Emilia-Romagna Region (CE-AVEC), Bologna, Italy. Written informed consent is obtained from all the participants. Findings of this study will be disseminated through peer-reviewed publications and conference presentations.     Persistent and transient olfactory deficits in COVID-19 are associated to inflammation and zinc homeostasis

    IntroductionThe Coronavirus Disease 2019 (COVID-19) is mainly a respiratory syndrome that can affect multiple organ systems, causing a variety of symptoms. Among the most common and characteristic symptoms are deficits in smell and taste perception, which may last for weeks/months after COVID-19 diagnosis owing to mechanisms that are not fully elucidated. MethodsIn order to identify the determinants of olfactory symptom persistence, we obtained olfactory mucosa (OM) from 21 subjects, grouped according to clinical criteria: i) with persistent olfactory symptoms; ii) with transient olfactory symptoms; iii) without olfactory symptoms; and iv) non-COVID-19 controls. Cells from the olfactory mucosa were harvested for transcriptome analyses. Results and discussionRNA-Seq assays showed that gene expression levels are altered for a long time after infection. The expression profile of micro RNAs appeared significantly altered after infection, but no relationship with olfactory symptoms was found. On the other hand, patients with persistent olfactory deficits displayed increased levels of expression of genes involved in the inflammatory response and zinc homeostasis, suggesting an association with persistent or transient olfactory deficits in individuals who experienced SARS-CoV-2 infection

    Frequency and type of domestic injuries among children during COVID-19 lockdown: what changes from the past? An Italian multicentre cohort study

    : Accidents are the main cause of injury in children, more than half events happen at home. Aims of this study were to assess if SARS-CoV-2 lockdown influence emergency department (ED) visits due to children domestic accident (DAs) and to identify factors associated with hospitalization. This was a multicentre, observational, and retrospective cohort study involving 16 EDs in Italy and enrolling children (3-13 years) receiving a visit in ED during March-June 2019 and March-June 2020. Risk factors for hospitalization were identified by logistic regression models. In total, 8860 ED visits due to domestic accidents in children occurred before (4380) and during (4480) lockdown, with a mean incidence of DA of 5.6% in 2019 and 17.9% in 2020 (p < 0.001) (IRR: 3.16; p < 0.001). The risk of hospitalization was influenced by the type of occurred accident, with fourfold higher for poisoning and twofold lower risk for stab-wound ones. In addition, a higher risk was reported for lockdown period vs 2019 (OR: 1.9; p < 0.001), males (OR: 1.4; p < 0.001), and it increased with age (OR: 1.1; p < 0.001).    Conclusions: The main limitation of this study is the retrospective collection of data, available only for patients who presented at the hospital. This does highlight possible differences in the total number of incidents that truly occurred. In any case, the COVID-19 lockdown had a high impact on the frequency of DAs and on hospitalization. A public health campaign aimed at caregivers would be necessary to minimize possible risks at home. What is Known: • In Italy, domestic accidents are the second leading cause of paediatric mortality after cancer. • During the first SARS-CoV-2 lockdown in 2020, a sharp decrease in the total number of Emergency Departments visits for all causes was observed, both in children and in adults. What is New: • During the first SARS-CoV-2 lockdown in 2020, domestic accidents involving children increased threefold from the previous year. • Higher risk of hospitalization was showed in minors accessing during 2020 vs 2019, in males than in females and it increased with advancing age. Considering the type of injury, a significant higher risk of hospitalization for poisoning was observed

    Extreme floods of Venice: characteristics, dynamics, past and future evolution (review article)

    Floods in the Venice city centre result from the superposition of several factors: astronomical tides; seiches; and atmospherically forced fluctuations, which include storm surges, meteotsunamis, and surges caused by atmospheric planetary waves. All these factors can contribute to positive water height anomalies individually and can increase the probability of extreme events when they act constructively. The largest extreme water heights are mostly caused by the storm surges produced by the sirocco winds, leading to a characteristic seasonal cycle, with the largest and most frequent events occurring from November to March. Storm surges can be produced by cyclones whose centres are located either north or south of the Alps. Historically, the most intense events have been produced by cyclogenesis in the western Mediterranean, to the west of the main cyclogenetic area of the Mediterranean region in the Gulf of Genoa. Only a small fraction of the inter-annual variability in extreme water heights is described by fluctuations in the dominant patterns of atmospheric circulation variability over the Euro-Atlantic sector. Therefore, decadal fluctuations in water height extremes remain largely unexplained. In particular, the effect of the 11-year solar cycle does not appear to be steadily present if more than 100 years of observations are considered. The historic increase in the frequency of floods since the mid-19th century is explained by relative mean sea level rise. Analogously, future regional relative mean sea level rise will be the most important driver of increasing duration and intensity of Venice floods through this century, overcompensating for the small projected decrease in marine storminess. The future increase in extreme water heights covers a wide range, largely reflecting the highly uncertain mass contributions to future mean sea level rise from the melting of Antarctica and Greenland ice sheets, especially towards the end of the century. For a high-emission scenario (RCP8.5), the magnitude of 1-in-100-year water height values at the northern Adriatic coast is projected to increase by 26–35 cm by 2050 and by 53–171 cm by 2100 with respect to the present value and is subject to continued increase thereafter. For a moderate-emission scenario (RCP4.5), these values are 12–17 cm by 2050 and 24–56 cm by 2100. Local subsidence (which is not included in these estimates) will further contribute to the future increase in extreme water heights. This analysis shows the need for adaptive long-term planning of coastal defences using flexible solutions that are appropriate across the large range of plausible future water height extremes

    Comparing T Cell Subsets in Broncho-Alveolar Lavage (BAL) and Peripheral Blood in Patients with Advanced Lung Cancer

    Background: Lung cancer (LC) tissue for immunological characterization is often scarce. We explored and compared T cell characteristics between broncho-alveolar lavage from tumor affected (t-BAL) and contralateral lung (cl-BAL), with matched peripheral blood (PB). Methods: BAL and PB were collected during bronchoscopy for diagnostic and/or therapeutic purposes in patients with monolateral primary lesion. Results: Of 33 patients undergoing BAL and PB sampling, 21 had histologically-confirmed LC. Most cases were locally-advanced or metastatic non-small cell LC. T cell characteristics were not significantly different in t-BAL vs. cl-BAL. Compared to PB, CD8 T cells in BAL presented features of immune activation and exhaustion (high PD-1, low IFN-g production). Accordingly, regulatory CD4 T cells were also higher in BAL vs. PB. When dichotomizing T cell density in t-BAL in high and low, we found that PD-L1 expression in LC was associated with T cell density in t-BAL. T-BAL with high T cell density had higher %IFN-g+CD8 T cells and lower %T-regs. Conclusion: In BAL from advanced LC patients, T cells present features of exhaustion. T cells in t-BAL could be the best surrogate of tumor-infiltrating T cell, and future studies should evaluate T cell phenotype and density as potential biomarkers for cancer immunotherapy outcome
