Cowden Syndrome and Concomitant Pulmonary Neuroendocrine Tumor: A Presentation of Two Cases
Cowden Syndrome is a rare autosomal dominantly inherited disorder. Patients with Cowden Syndrome are at increased risk of various benign and malignant neoplasms in breast, endometrium, thyroid, gastrointestinal tract, and genitourinary system. Neuroendocrine tumors are ubiquitous neoplasms that may occur anywhere in the human body. Bronchopulmonary neuroendocrine tumors include four different histological subtypes, among these, typical and atypical pulmonary carcinoids. No association between Cowden Syndrome and neuroendocrine tumors has previously been described. We present two cases of Cowden Syndrome that were diagnosed with pulmonary carcinoids
Mechanisms for reducing low back pain: a mediation analysis of a multifaceted intervention in workers in elderly care
Purpose A multifaceted workplace intervention consisting of participatory ergonomics, physical training, and cognitive–behavioural training (CBT) has shown effectiveness for reducing low back pain (LBP). However, the mechanisms of action underlying these intervention components are not well understood. Methods This was a mediation analysis of a cluster-randomised controlled trial of a multifaceted intervention in 420 workers in elderly care. Mediation analysis was carried out via structural equation modelling. Potential mediators investigated were: fear-avoidance beliefs, perceived muscle strength, use of assistive devices at work and perceived physical exertion at work. LBP outcomes assessed were: days with LBP, LBP intensity and days with bothersome LBP. Results There were no significant indirect effects of the intervention on LBP outcomes. There were significant effects of the intervention on both fear-avoidance measures [β = − 0.63, 95% CI (1.23, 0.03); β = − 1.03, 95% CI (− 1.70, − 0.34)] and the use of assistive devices [β = − 0.55, 95% CI (− 1.04, − 0.05)], but not on perceived muscle strength [β = − 0.18, 95% CI (− 0.50, 0.13)] or physical exertion [β = − 0.05, 95% CI (− 0.40, 0.31)]. The only potential mediator with a significant effect on LBP outcomes was physical exertion, which had a significant effect on LBP intensity [β = 0.14, 95% CI (0.04, 0.23)]. Conclusions A multifaceted intervention consisting of participatory ergonomics, physical training, and CBT was able to decrease fear-avoidance beliefs and increase use of assistive devices in the workplace. However, these changes did not explain the effect of any of the intervention components on days with LBP, LBP intensity and days with bothersome LBP
Improving adherence to medication in stroke survivors (IAMSS): a randomised controlled trial: study protocol
Background: Adherence to therapies is a primary determinant of treatment success, yet the World Health Organisation estimate that only 50% of patients who suffer from chronic diseases adhere to treatment recommendations. In a previous project, we found that 30% of stroke patients reported sub-optimal medication adherence, and this was associated with younger age, greater cognitive impairment, lower perceptions of medication benefits and higher specific concerns about medication. We now wish to pilot a brief intervention aimed at (a) helping patients establish a better medication-taking routine, and (b) eliciting and modifying any erroneous beliefs regarding their medication and their stroke. Methods/Design: Thirty patients will be allocated to a brief intervention (2 sessions) and 30 to treatment as usual. The primary outcome will be adherence measured over 3 months using Medication Event Monitoring System (MEMS) pill containers which electronically record openings. Secondary outcomes will include self reported adherence and blood pressure. Discussion: This study shall also assess uptake/attrition, feasibility, ease of understanding and acceptability of this complex intervention. Adopting a healthy lifestyle when pregnant and obese - an interview study three years after childbirth
Background: Obesity during pregnancy is increasing and is related to life-threatening and ill-health conditions in both mother and child. Initiating and maintaining a healthy lifestyle when pregnant with body mass index (BMI) ≥ 30 kg/m2 can improve health and decrease risks during pregnancy and of long-term illness for the mother and the child. To minimise gestational weight gain women with BMI ≥ 30 kg/m2 in early pregnancy were invited to a lifestyle intervention including advice and support on diet and physical activity in Gothenburg, Sweden. The aim of this study was to explore the experiences of women with BMI ≥ 30 kg/m2 regarding minimising their gestational weight gain, and to assess how health professionals' care approaches are reflected in the women's narratives.
Methods: Semi-structured interviews were conducted with 17 women who had participated in a lifestyle intervention for women with BMI ≥ 30 kg/m2 during pregnancy 3 years earlier. The interviews were digitally recorded and transcribed in full. Thematic analysis was used.
Results: The meaning of changing lifestyle for minimising weight gain and of the professional's care approaches is described in four themes: the child as the main motivation for making healthy changes; a need to be seen and supported on own terms to establish healthy routines; being able to manage healthy activities and own weight; and need for additional support to maintain a healthy lifestyle.
Conclusions: To support women with BMI ≥ 30 kg/m2 to make healthy lifestyle changes and limit weight gain during pregnancy antenatal health care providers should 1) address women's weight in a non-judgmental way using BMI, and provide accurate and appropriate information about the benefits of limited gestational weight gain; 2) support the woman on her own terms in a collaborative relationship with the midwife; 3) work in partnership to give the woman the tools to self-manage healthy activities and 4) give continued personal support and monitoring to maintain healthy eating and regular physical activity habits after childbirth involving also the partner and family
Successful Reach and Adoption of a workplace health promotion RCT targeting a group of high-risk workers
<p>Abstract</p> <p>Background</p> <p>Cleaners are rarely introduced to workplace health promotion programs. The study's objective was to evaluate the reach and adoption of a workplace randomized controlled trial (RCT) among cleaners in Denmark.</p> <p>Methods</p> <p>Cleaning businesses with at least 30 employees, that could offer a weekly 1-hour intervention during working hours, were invited to participate. Employees working at least 20 hours/week were invited to answer a screening questionnaire and consent to participate. Analyses determined the differences in health variables between responders and non-responders, consenters and non-consenters, participants and non-participants and between participants of the RCT's three groups: physical coordination training, cognitive-behavioural theory-based training and reference group.</p> <p>Results</p> <p>From 16 eligible workplaces, a representative sample of 50% adopted the trial. Of 758 eligible employees, 78% responded to the screening questionnaire and 49% consented to participate. Consenters and participants differed from non-consenters and non-participants by having higher BMI, more chronic diseases and poorer musculoskeletal health.</p> <p>Conclusions</p> <p>This study indicates that workplace health promotion programs directed at health risk factors among cleaners enable significant adoption and reach to a high-risk subgroup of the Danish workforce.</p> <p>Trial registration</p> <p>Trial registration ISRCTN96241850</p
Four aspects of self-image close to death at home
Living close to death means an inevitable confrontation with one's own existential limitation. In this article, we argue that everyday life close to death embodies an identity work in progress. We used a narrative approach and a holistic-content reading to analyze 12 interviews conducted with three persons close to death. By illuminating the unique stories and identifying patterns among the participants’ narratives, we found four themes exemplifying important aspects of the identity work related to everyday life close to death. Two of the themes, named “Inside and outside of me” and “Searching for togetherness,” represented the core of the self-image and were framed by the other themes, “My place in space” and “My death and my time.” Our findings elucidate the way the individual stories moved between the past, the present, and the future. This study challenges the idea that everyday life close to impending death primarily means limitations. The findings show that the search for meaning, new knowledge, and community can form a part of a conscious and ongoing identity work close to death
COMAP Early Science: VII. Prospects for CO Intensity Mapping at Reionization
We introduce COMAP-EoR, the next generation of the Carbon Monoxide Mapping
Array Project aimed at extending CO intensity mapping to the Epoch of
Reionization. COMAP-EoR supplements the existing 30 GHz COMAP Pathfinder with
two additional 30 GHz instruments and a new 16 GHz receiver. This combination
of frequencies will be able to simultaneously map CO(1--0) and CO(2--1) at
reionization redshifts () in addition to providing a significant
boost to the sensitivity of the Pathfinder. We examine a set of
existing models of the EoR CO signal, and find power spectra spanning several
orders of magnitude, highlighting our extreme ignorance about this period of
cosmic history and the value of the COMAP-EoR measurement. We carry out the
most detailed forecast to date of an intensity mapping cross-correlation, and
find that five out of the six models we consider yield signal to noise ratios
(S/N) for COMAP-EoR, with the brightest reaching a S/N above 400.
We show that, for these models, COMAP-EoR can make a detailed measurement of
the cosmic molecular gas history from , as well as probe the
population of faint, star-forming galaxies predicted by these models to be
undetectable by traditional surveys. We show that, for the single model that
does not predict numerous faint emitters, a COMAP-EoR-type measurement is
required to rule out their existence. We briefly explore prospects for a
third-generation Expanded Reionization Array (COMAP-ERA) capable of detecting

COMAP Early Science: III. CO Data Processing
We describe the first season COMAP analysis pipeline that converts raw
detector readouts to calibrated sky maps. This pipeline implements four main
steps: gain calibration, filtering, data selection, and map-making. Absolute
gain calibration relies on a combination of instrumental and astrophysical
sources, while relative gain calibration exploits real-time total-power
variations. High efficiency filtering is achieved through spectroscopic
common-mode rejection within and across receivers, resulting in nearly
uncorrelated white noise within single-frequency channels. Consequently,
near-optimal but biased maps are produced by binning the filtered time stream
into pixelized maps; the corresponding signal bias transfer function is
estimated through simulations. Data selection is performed automatically
through a series of goodness-of-fit statistics, including and
multi-scale correlation tests. Applying this pipeline to the first-season COMAP
data, we produce a dataset with very low levels of correlated noise. We find
that one of our two scanning strategies (the Lissajous type) is sensitive to
residual instrumental systematics. As a result, we no longer use this type of
scan and exclude data taken this way from our Season 1 power spectrum
estimates. We perform a careful analysis of our data processing and observing
efficiencies and take account of planned improvements to estimate our future
performance. Power spectrum results derived from the first-season COMAP maps

COMAP Early Science: VI. A First Look at the COMAP Galactic Plane Survey
We present early results from the COMAP Galactic Plane Survey conducted
between June 2019 and April 2021, spanning in Galactic
longitude and |b|<1.\!\!^{\circ}5 in Galactic latitude with an angular
resolution of . The full survey will span -
and will be the first large-scale radio continuum survey at
GHz with sub-degree resolution. We present initial results from the first part
of the survey, including diffuse emission and spectral energy distributions
(SEDs) of HII regions and supernova remnants. Using low and high frequency
surveys to constrain free-free and thermal dust emission contributions, we find
evidence of excess flux density at GHz in six regions that we interpret
as anomalous microwave emission. Furthermore we model UCHII contributions using
data from the GHz CORNISH catalogue and reject this as the cause of the
GHz excess. Six known supernova remnants (SNR) are detected at GHz,
and we measure spectral indices consistent with the literature or show evidence
of steepening. The flux density of the SNR W44 at GHz is consistent with
a power-law extrapolation from lower frequencies with no indication of spectral
steepening in contrast with recent results from the Sardinia Radio Telescope.
We also extract five hydrogen radio recombination lines to map the warm ionized
gas, which can be used to estimate electron temperatures or to constrain
continuum free-free emission. The full COMAP Galactic plane survey, to be
released in 2023/2024, will be an invaluable resource for Galactic
