    Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

    Partiendo de clichés y estereotipos vinculados a lo masculino y lo femenino, esta investigación delimita en qué medida estos van a verse reflejados en los procederes de las artistas y los artistas que emplean el objeto como material de trabajo. Teniendo también los objetos connotaciones de género, nuestro análisis comparativo nos permite vislumbrar una respuesta a estas cuestiones ¿Los procesos que emplean los artistas hombres son más agresivos, duros o asépticos con el objeto que los emplea-dos por sus compañeras que podrían describirse como suaves, blandos o basados más en el cuidado que en la destrucción? Recorriendo esta tipología de obra desde las vanguardias, que inauguraron el uso de los objetos como material de trabajo, hasta las producciones más actuales, asumimos otra de las cuestiones importantes a tratar en esta tesis que es el estudio del momento de análisis o lectura de las obras y si es en este el espacio donde se vuelcan las concepciones sociales vinculadas a los géneros, más que existir estas diferencias en el propio proceder. Teniendo el objetivo de la investigación definido nos acercamos a otras cuestiones que nos permiten acotar la construcción de estos arquetipos, desde las características en que se basa la idea de genio, hasta las observaciones que se han hecho del juego infantil en niñas y niños. Los diversos casos de estudio, que ponen su foco en las producciones objetuales más recientes, en la diferencia entre objeto femenino y objeto feminista o en el análisis del uso del juguete, van conformando un cuerpo de trabajo del que vamos obteniendo conclusiones y argumentaciones que validan o refutan el conjunto de preguntas que planteamos

A Arte na mole dos tempos é então uma forma agarrada a uma perspetiva, uma imagem na retina semantizada. O contexto é de perigo, na emergência ambiental, expondo uma arte que pode ser doente, com o mal reificado do capital.     Resumen basado en el de la publicaciónLa noción de equipos docentes en Educación Superior define una práctica de coordinación docente, bien para el logro de determinados propósitos transversales, bien en un sentido mucho más estricto, en el despliegue coordinado de una Titulación. Se aborda la pregunta de por qué y sobre qué ha de coordinarse el profesorado universitario, como reto fundamental a la cultura organizativa docente, basada en el ejercicio individual del profesorado y en la fragmentación de tareas, de responsabilidades y del conocimiento. Los cambios apuntados en las diversas contribuciones obedecen a las transformaciones que se están dando en la universidad española como consecuencia de nuevas demandas socio-formativas, las cuales generan nuevas necesidades, funciones y estructuras. Ello da lugar a nuevas identidades académicas.     Epigenetic footprint enables molecular risk stratification of hepatoblastoma with clinical implications

    Background & aims: Hepatoblastoma (HB) is a rare disease. Nevertheless, it is the predominant pediatric liver cancer, with limited therapeutic options for patients with aggressive tumors. Herein, we aimed to uncover the mechanisms of HB pathobiology and to identify new biomarkers and therapeutic targets in a move towards precision medicine for patients with advanced HB. Methods: We performed a comprehensive genomic, transcriptomic and epigenomic characterization of 159 clinically annotated samples from 113 patients with HB, using high-throughput technologies. Results: We discovered a widespread epigenetic footprint of HB that includes hyperediting of the tumor suppressor BLCAP concomitant with a genome-wide dysregulation of RNA editing and the overexpression of mainly non-coding genes of the oncogenic 14q32 DLK1-DIO3 locus. By unsupervised analysis, we identified 2 epigenomic clusters (Epi-CA, Epi-CB) with distinct degrees of DNA hypomethylation and CpG island hypermethylation that are associated with the C1/C2/C2B transcriptomic subtypes. Based on these findings, we defined the first molecular risk stratification of HB (MRS-HB), which encompasses 3 main prognostic categories and improves the current clinical risk stratification approach. The MRS-3 category (28%), defined by strong 14q32 locus expression and Epi-CB methylation features, was characterized by CTNNB1 and NFE2L2 mutations, a progenitor-like phenotype and clinical aggressiveness. Finally, we identified choline kinase alpha as a promising therapeutic target for intermediate and high-risk HBs, as its inhibition in HB cell lines and patient-derived xenografts strongly abrogated tumor growth. Conclusions: These findings provide a detailed insight into the molecular features of HB and could be used to improve current clinical stratification approaches and to develop treatments for patients with HB. Lay summary: Hepatoblastoma is a rare childhood liver cancer that has been understudied. We have used cutting-edge technologies to expand our molecular knowledge of this cancer. Our biological findings can be used to improve clinical management and pave the way for the development of novel therapies for this cancer
