    EXPERIENCES OF UAV SURVEYS APPLIED TO ENVIRONMENTAL RISK MANAGEMENT

    In this paper the results of some surveys carried out in an area of Apulian territory affected by serious environmental hazard are presented. Unmanned Aerial Vehicles (UAV) are emerging as a key engineering tool for future environmental survey tasks. UAVs are increasingly seen as an attractive low-cost alternative or supplement to aerial and terrestrial photogrammetry due to their low cost, flexibility, availability and readiness for duty. In addition, UAVs can be operated in hazardous or temporarily inaccessible locations, that makes them very suitable for the assessment and management of environmental risk conditions. In order to verify the reliability of these technologies an UAV survey and A LIDAR survey have been carried outalong about 1 km of coast in the Salento peninsula, near the towns of San Foca, Torre dellOrso and SantAndrea( Lecce, Southern Italy). This area is affected by serious environmental risks due to the presence of dangerous rocky cliffs named falesie. The UAV platform was equipped with a photogrammetric measurement system that allowed us to obtain a mobile mapping of the fractured fronts of dangerous rocky cliffs. UAV-images data have been processed using dedicated software (AgisoftPhotoscan). The point clouds obtained from both the UAV and LIDAR surveys have been processed using Cloud Compare software, with the aim of testing the UAV results with respect to the LIDAR ones. The total error obtained was of centimeter-order that is a very satisfactory result. The environmental information has been arranged in an ArcGIS platform in order to assess the risk levels. The possibility to repeat the survey at time intervals more or less close together depending on the measured levels of risk and to compare the output allows following the trend of the dangerous phenomena. In conclusion, for inaccessible locations of dangerous rocky bodies the UAV survey coupled with GIS methodology proved to be a key engineering tool for the management of environmental risks

    A case of Brucella endocarditis in association with subclavian artery thrombosis

    Brucellosis is a common zoonosis, endemic in Mediterranean countries, and caused by bacteria of Brucella genus. Brucellosis is a systemic infection and the clinical presentation varies widely from asymptomatic and mild to severe disease. Cardiovascular complications are extremely rare. We present a case of arterial thrombosis in a previously healthy young patient with Brucella endocarditis. Careful attention must be paid to any sign or symptom of thrombosis in patients affected by brucellosis, regardless of the presence of endocarditis and cardiovascular risk factors.     Disseminated tuberculosis in a patient treated with a JAK2 selective inhibitor: a case report

    Background Primary myelofibrosis is a myeloproliferative disorder characterized by bone marrow fibrosis, abnormal cytokine expression, splenomegaly and anemia. The activation of JAK2 and the increased levels of circulating proinflammatory cytokines seem to play an important role in the pathogenesis of myelofibrosis. Novel therapeutic agents targeting JAKs have been developed for the treatment of myeloproliferative disorders. Ruxolitinib (INCB018424) is the most recent among them. Case presentation To our knowledge, there is no evidence from clinical trials of an increased risk of tuberculosis during treatment with JAK inhibitors. Here we describe the first case of tuberculosis in a patient treated with Ruxolitinib, a male with a 12-year history of chronic idiopathic myelofibrosis admitted to our Institute because of fever, night sweats, weight loss and an enlarging mass in the left inguinal area for two months. Conclusion Treatment with Ruxolitinib may have triggered the reactivation of latent tuberculosis because of an inhibition of Th1 response.     Molecular biogeography of Mediterranean and southern African disjunctions as exemplified by pollen beetles of the Meligethes planiusculus species-group and related taxa (Coleoptera: Nitidulidae; Meligethinae)

    We investigated the apparent disjunction expressed in two related assemblages of species of the genus Meligethes, i.e. M. fruticola and its allies in the Cape region of South Africa and the M. planiusculus group in the Mediterranean region. We also inferred possible dynamics in the radiation of the Meligethes planiusculus complex within Macaronesia utilizing morphological, molecular and bionomical data, exploring potential historical and palaeoecological scenarios regulated by a molecular clock dating system. We reconstructed phylogenetic relationships of the M. planiusculus complex and of related Mediterranean (M. tristis), tropical (M. scotti) and southern African (M. chevrolati, M. conformis, and M. fruticola) species, using COI mitochondrial gene sequences. Phylogenetic reconstructions support an unambiguous distinction of two major clades grouping European-Mediterranean M. canariensis, M. isoplexidis andM. planiusculus specimens in one clade and the South-African specimens related to M. fruticola in another. Molecular markers suggested that the European-Mediterranean taxon M. tristis is unambiguously more distantly related to the partly sympatric M. canariensis, M. isoplexidis and M. planiusculus, than to the geographically isolated Southern African taxon, M. fruticola. However, morphological data revealed that M. tristis is more closely related to M. planiusculus and its allies while occupying a position internal to the M. planiusculus species group, but external to the M. planiusculus complex. Results of divergence estimation analyses suggest a splitting between ancestors of the European-Mediterranean species of the M. planiusculus complex and that of the African species M. fruticola at ~21-23 MYA. Molecular results also demonstrated that the remaining Afrotropical species are more related to the M. planiusculus and M. fruticola complexes than to M. tristis. This evidence clearly indicates that the Holarctic M. planiusculus group represents a paraphyletic assemblage with heterochronic Afrotropical origin. The estimated times of divergence supports evidence from other researchers of an ‘Arid Corridor’, or of a ‘Central or Eastern High Africa Corridor’, which connected several times in the last twenty MY the European-Mediterranean and eastern/southern African areas, and facilitated species migration northwards and southwards. The dynamics of the Meligethes planiusculus complex radiation in Macaronesia apparently followed a contradictory biogeographical scenario than the sequence of events recently hypothesized for their host-plants (Echium, Boraginaceae)

    Rituximab Unveils Hypogammaglobulinemia and Immunodeficiency in Children with Autoimmune Cytopenia

    BACKGROUND: Rituximab (RTX; anti-CD20 mAb) is a treatment option in children with refractory immune thrombocytopenia, autoimmune hemolytic anemia (AHA), and Evans syndrome (ES). Prevalence and clinical course of RTX-induced hypogammaglobulinemia in these patients are poorly known. OBJECTIVE: To evaluate the prevalence and risk factors for persistent hypogammaglobulinemia (PH) after RTX use. METHODS: Clinical and immunologic data from children treated with RTX for immune thrombocytopenia, AHA, and ES were collected from 16 Italian centers and 1 UK center at pre-RTX time point (0), +6 months, and yearly, up to 4 years post-RTX. Patients with previously diagnosed malignancy or primary immune deficiency (PID) were excluded. RESULTS: We analyzed 53 children treated with RTX for immune thrombocytopenia (n = 36), AHA (n = 13), and ES (n = 4). Median follow-up was 30 months (range, 12-48). Thirty-two percent of patients (17 of 53) experienced PH, defined as IgG levels less than 2 SD for age at last follow-up (>12 months after RTX). Significantly delayed B-cell recovery was observed in children experiencing PH (hazard ratio, 0.55; P < .05), and 6 of 17 (35%) patients had unresolved B-cell lymphopenia at last follow-up. PH was associated with IgA and IgM deficiency, younger age at RTX use (51 vs 116 months; P < .01), a diagnosis of AHA/ES, and better response to RTX. Nine patients with PH (9 of 17 [53%]) were eventually diagnosed with a PID. CONCLUSIONS: Post-RTX PH is a frequent condition in children with autoimmune cytopenia; a sizable proportion of patients with post-RTX PH were eventually diagnosed with a PID. In-depth investigation for PID is therefore recommended in these patients

    SWI/SNF catalytic subunits’ switch drives resistance to EZH2 inhibitors in ARID1A-mutated cells

    © 2018, The Author(s). Inactivation of the subunits of SWI/SNF complex such as ARID1A is synthetically lethal with inhibition of EZH2 activity. However, mechanisms of de novo resistance to EZH2 inhibitors in cancers with inactivating SWI/SNF mutations are unknown. Here we show that the switch of the SWI/SNF catalytic subunits from SMARCA4 to SMARCA2 drives resistance to EZH2 inhibitors in ARID1A-mutated cells. SMARCA4 loss upregulates anti-apoptotic genes in the EZH2 inhibitor-resistant cells. EZH2 inhibitor-resistant ARID1A-mutated cells are hypersensitive to BCL2 inhibitors such as ABT263. ABT263 is sufficient to overcome resistance to an EZH2 inhibitor. In addition, ABT263 synergizes with an EZH2 inhibitor in vivo in ARID1A-inactivated ovarian tumor mouse models. Together, these data establish that the switch of the SWI/SNF catalytic subunits from SMARCA4 to SMARCA2 underlies the acquired resistance to EZH2 inhibitors. They suggest BCL2 inhibition alone or in combination with EZH2 inhibition represents urgently needed therapeutic strategy for ARID1A-mutated cancers

    The Italian Registry for Primary Immunodeficiencies (Italian Primary Immunodeficiency Network; IPINet): Twenty Years of Experience (1999&#8211;2019)

    Primary immunodeficiencies (PIDs) are heterogeneous disorders, characterized by variable clinical and immunological features. National PID registries offer useful insights on the epidemiology, diagnosis, and natural history of these disorders. In 1999, the Italian network for primary immunodeficiencies (IPINet) was established. We report on data collected from the IPINet registry after 20\ua0years of activity. A total of 3352 pediatric and adult patients affected with PIDs are registered in the database. In Italy, a regional distribution trend of PID diagnosis was observed. Based on the updated IUIS classification of 2019, PID distribution in Italy showed that predominantly antibody deficiencies account for the majority of cases (63%), followed by combined immunodeficiencies with associated or syndromic features (22.5%). The overall age at diagnosis was younger for male patients. The minimal prevalence of PIDs in Italy resulted in 5.1 per 100.000 habitants. Mortality was similar to other European registries (4.2%). Immunoglobulin replacement treatment was prescribed to less than one third of the patient cohort. Collectively, this is the first comprehensive description of the PID epidemiology in Italy

    A European monitoring protocol for the stag beetle, a saproxylic flagship species

    Developing protocols for threatened invertebrates is often challenging, because they are not only rare but also elusive. This is the case with the stag beetle (Lucanus cervus), a protected and flagship species for the saproxylic beetle fauna in Europe. We applied a standard transect walk at a European scale (8 countries, 29 transects) to test its practicability and reliability as survey design. A total of 533 sightings were recorded throughout the sampling period, but detection probability changed as the season progressed. Considering the observed activity pattern, occupancy models showed that a short period of three consecutive weeks, between the middle of June and the first week of July, resulted in a high probability of detection (P > 0.7). As time of the peak of activity varies from year to year and between sites, we propose to extend the sampling period to five weekly surveys. Detailed information on the transect characteristics and the optimal time for surveying were analysed. The data indicate that a weekly transect at dusk provides a reliable method for monitoring this species throughout its distributional range. No correlation was found between latitude, longitude and phenology of sightings, however. However, a standard method such as the one presented, allows broadening the scale of monitoring studies, provinding data to evaluate the efficacy of conservation measures. © 2016 The Royal Entomological Societ

    Positioning of darunavir/cobicistat-containing antiretroviral regimens in real life: Results from a large multicentre observational prospective cohort (SCOLTA)

    Background: Study aim was to evaluate the safety and durability of darunavir/cobicistat (DRV/c) in a real life setting. Methods: Multicentre prospective cohort study performed in the context of SCOLTA (Surveillance Cohort Long-Term Toxicity Antiretrovirals). Patients were evaluated at baseline, week 24 and 48. Changes were evaluated using the paired t test or signed rank test. The multivariable analysis was performed using a general linear model, after ranking of not normally distributed variables. Results: A total of 249 patients were included, 72 (29%) were in DRV/c-based dual therapies (DT). Hypercholesterolemia, HC, (total cholesterol (TC) ≥ 200 mg/dL or low density-C (LDL-C) ≥ 130 or statin use) was present in 121 (48.6%) and hypertriglyceridemia, (triglycerides (TG) ≥ 200 mg/dl or fibrate use) in 41 (16.5%) patients. Blood lipid profile did not change significantly in either the global population or patients with HC. After a median observation of 17 months (IQR 13-20), 59 (25.3%) patients discontinued DRV/c, of which 13 were in DT. The durability DT resulted higher than that of triple therapy (log-rank test p = 0.01). Main reasons for stopping DRV/c were simplification (15 patients), adverse events (13 patients), planned discontinuation for treatment initiation with DAA (4 patients), treatment failure (2 patients); death (2 patients), other causes (10 patients). Twenty-six were lost to follow-up. Conclusions: DRV/c was safe and well tolerated. Dual therapies showed a better profile of tolerability and a longer durability compared to triple therapies
