Abrupt closure: The CAVEAT I experience
Objectives.This study sought to assess the incidence and consequences of abrupt closure in a series of patients undergoing directional coronary atherectomy versus percutaneous coronary angioplasty.Background.Abrupt closure with coronary angioplasty has been associated with adverse outcome. The results from the Coronary Angioplasty Versus Excisional Atherectomy Trial (CAVEAT) I, a randomized trial of coronary angioplasty versus directional coronary atherectomy, were analyzed.Method.This multicenter trial enrolled 1,012 patients from 1991 to 1992. All records from patients with abrupt closure, which was coded as a discrete complication, were reviewed.Results.Abrupt closure occurred in 60 patients (5.9%) and was associated with a significantly longer hospital stay (median 8 vs. 3 days). Severe proximal target vessel tortuosity was more common in patients with abrupt closure (20.3% vs. 11.6%, p = 0.046), as was preexistent coronary artery thrombus (30.5% vs. 18.3%, p = 0.02). Abrupt closure was associated with a marked increase in subsequent complications (myocardial infarction 46.7% vs. 2.1%, emergency bypass surgery 383% vs. 0.32%, death 33% vs. 0%) and occurred more frequently in the directional coronary atherectomy group (8.0% vs. 3.8%, p = 0.005). In the coronary angioplasty group, the occlusion usually occurred at the target lesion (91%), presumably related to the effects of barotrauma. In the directional coronary atherectomy group, the site of the occlusion was the target lesion in only 58% (p = 0.045). The remaining occlusions related to problems with the technique (guide catheter or nose cone trauma), reflecting the fact that directional coronary atherectomy is a more complex procedure.Conclusions.Abrupt closure remains the principal determinant of adverse outcome after percutaneous procedures for the treatment of coronary artery disease. Transcriptome profiling of ontogeny in the acridid grasshopper Chorthippus biguttulus
Acridid grasshoppers (Orthoptera:Acrididae) are widely used model organisms
for developmental, evolutionary, and neurobiological research. Although there
has been recent influx of orthopteran transcriptomic resources, many use
pooled ontogenetic stages obscuring information about changes in gene
expression during development. Here we developed a de novo transcriptome
spanning 7 stages in the life cycle of the acridid grasshopper Chorthippus
biguttulus. Samples from different stages encompassing embryonic development
through adults were used for transcriptomic profiling, revealing patterns of
differential gene expression that highlight processes in the different life
stages. These patterns were validated with semi-quantitative RT-PCR. Embryonic
development showed a strongly differentiated expression pattern compared to
all of the other stages and genes upregulated in this stage were involved in
signaling, cellular differentiation, and organ development. Our study is one
of the first to examine gene expression during post-embryonic development in a
hemimetabolous insect and we found that only the fourth and fifth instars had
clusters of genes upregulated during these stages. These genes are involved in
various processes ranging from synthesis of biogenic amines to chitin binding.
These observations indicate that post-embryonic ontogeny is not a continuous
process and that some instars are differentiated. Finally, genes upregulated
in the imago were generally involved in aging and immunity. Our study
highlights the importance of looking at ontogeny as a whole and indicates
promising directions for future research in orthopteran development
Individual differences in newborn visual attention associate with temperament and behavioral difficulties in later childhood
Recently it was shown that individual differences in attention style in infants are
associated with childhood effortful control, surgency, and hyperactivity-inattention. Here we
investigated whether effortful control, surgency and behavioral problems in childhood can be
predicted even earlier, from individual differences in newborns’ average duration of gaze to
stimuli. Eighty newborns participated in visual preference and habituation studies. Parents
completed questionnaires at follow up (mean age = 7.5 years, SD = 1.0 year). Newborns’
average dwell time was negatively associated with childhood surgency (β = -.25, R2 = .04, p =
.02) and total behavioral difficulties (β = -.28, R2 = .05, p = .04) but not with effortful control (β
= .03, R2 = .001, p = .76). Individual differences in newborn visual attention significantly
associated with individual variation in childhood surgency and behavioral problems, showing
that some of the factors responsible for this variation are present at birth
Effect of sitagliptin on cardiovascular outcomes in type 2 diabetes
BACKGROUND: Data are lacking on the long-term effect on cardiovascular events of adding sitagliptin, a dipeptidyl peptidase 4 inhibitor, to usual care in patients with type 2 diabetes and cardiovascular disease. METHODS: In this randomized, double-blind study, we assigned 14,671 patients to add either sitagliptin or placebo to their existing therapy. Open-label use of antihyperglycemic therapy was encouraged as required, aimed at reaching individually appropriate glycemic targets in all patients. To determine whether sitagliptin was noninferior to placebo, we used a relative risk of 1.3 as the marginal upper boundary. The primary cardiovascular outcome was a composite of cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for unstable angina. RESULTS: During a median follow-up of 3.0 years, there was a small difference in glycated hemoglobin levels (least-squares mean difference for sitagliptin vs. placebo, -0.29 percentage points; 95% confidence interval [CI], -0.32 to -0.27). Overall, the primary outcome occurred in 839 patients in the sitagliptin group (11.4%; 4.06 per 100 person-years) and 851 patients in the placebo group (11.6%; 4.17 per 100 person-years). Sitagliptin was noninferior to placebo for the primary composite cardiovascular outcome (hazard ratio, 0.98; 95% CI, 0.88 to 1.09; P<0.001). Rates of hospitalization for heart failure did not differ between the two groups (hazard ratio, 1.00; 95% CI, 0.83 to 1.20; P = 0.98). There were no significant between-group differences in rates of acute pancreatitis (P = 0.07) or pancreatic cancer (P = 0.32). CONCLUSIONS: Among patients with type 2 diabetes and established cardiovascular disease, adding sitagliptin to usual care did not appear to increase the risk of major adverse cardiovascular events, hospitalization for heart failure, or other adverse events
Effect of alirocumab on mortality after acute coronary syndromes. An analysis of the ODYSSEY OUTCOMES randomized clinical trial
Background: Previous trials of PCSK9 (proprotein convertase subtilisin-kexin type 9) inhibitors demonstrated reductions in major adverse cardiovascular events, but not death. We assessed the effects of alirocumab on death after index acute coronary syndrome. Methods: ODYSSEY OUTCOMES (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) was a double-blind, randomized comparison of alirocumab or placebo in 18 924 patients who had an ACS 1 to 12 months previously and elevated atherogenic lipoproteins despite intensive statin therapy. Alirocumab dose was blindly titrated to target achieved low-density lipoprotein cholesterol (LDL-C) between 25 and 50 mg/dL. We examined the effects of treatment on all-cause death and its components, cardiovascular and noncardiovascular death, with log-rank testing. Joint semiparametric models tested associations between nonfatal cardiovascular events and cardiovascular or noncardiovascular death. Results: Median follow-up was 2.8 years. Death occurred in 334 (3.5%) and 392 (4.1%) patients, respectively, in the alirocumab and placebo groups (hazard ratio [HR], 0.85; 95% CI, 0.73 to 0.98; P=0.03, nominal P value). This resulted from nonsignificantly fewer cardiovascular (240 [2.5%] vs 271 [2.9%]; HR, 0.88; 95% CI, 0.74 to 1.05; P=0.15) and noncardiovascular (94 [1.0%] vs 121 [1.3%]; HR, 0.77; 95% CI, 0.59 to 1.01; P=0.06) deaths with alirocumab. In a prespecified analysis of 8242 patients eligible for ≥3 years follow-up, alirocumab reduced death (HR, 0.78; 95% CI, 0.65 to 0.94; P=0.01). Patients with nonfatal cardiovascular events were at increased risk for cardiovascular and noncardiovascular deaths (P<0.0001 for the associations). Alirocumab reduced total nonfatal cardiovascular events (P<0.001) and thereby may have attenuated the number of cardiovascular and noncardiovascular deaths. A post hoc analysis found that, compared to patients with lower LDL-C, patients with baseline LDL-C ≥100 mg/dL (2.59 mmol/L) had a greater absolute risk of death and a larger mortality benefit from alirocumab (HR, 0.71; 95% CI, 0.56 to 0.90; Pinteraction=0.007). In the alirocumab group, all-cause death declined wit h achieved LDL-C at 4 months of treatment, to a level of approximately 30 mg/dL (adjusted P=0.017 for linear trend). Conclusions: Alirocumab added to intensive statin therapy has the potential to reduce death after acute coronary syndrome, particularly if treatment is maintained for ≥3 years, if baseline LDL-C is ≥100 mg/dL, or if achieved LDL-C is low. Elucidation of essential maps for 4Cs and informationmedia literacy
Curriculum development is a dynamic process of iterating multiliteracy which inspires the creation of knowledge and enable to prepare students to meet the challenges of the modern world. Driven by societal imperative and contextual relevance, ideally, the 21st century learning skills and information-media literacy skill shall provide the backbone of Science teaching. Thus, this Action Research aims, to elucidate the integration of 4Cs (communication, collaboration, critical thinking and creativity) as well as to trace the development of information-media literacy and skills through essential mapping technique in order to support the ongoing implementation of the recently crafted Junior High School Science curriculum of the Philippines. As a result, learning activities promoting 4Cs are limited to: performing scientific investigations; role playing; chart & poster making; improvisation & demonstration activity; story making; role playing; multi-media & visual presentation; and simulation activity. The information-media literacy can be developed by the following learning strategies: multimedia presentation; group reporting; performing scientific investigations & experimentations; debating activity; modelling, role playing & simulation activity; portfolio making; improvisation & modelmaking activity; concept making; and diagram making & illustration activity. Problems such as the platforms, the teacher\u27s socio-economic status, tedious teacher\u27s tasks, and lacking support from the community & other stakeholders affect the implementation of the curriculum framework. © 2019 Association for Computing Machinery
