Patient- and surgeon-adjusted control charts for monitoring performance
Objectives: To determine whether an innovative graphical tool for accurate measurement of individual surgeon performance metrics, adjusted for both surgeon-specific and patient-specific factors, significantly alters interpretation of performance data. Design: Retrospective analysis of all total knee replacements (TKRs) conducted at the host institution between 1996 and 2009. The database was randomly divided into training and testing datasets. Using multivariate generalised estimating equation regression models, the training dataset enabled generation of patient-risk and surgeon-experience adjustment factors. To simulate prospective monitoring of individual surgeon outcomes, the testing dataset was mapped on control charts. Weighted κ statistics were calculated to measure the agreement between patient-risk adjusted and fully adjusted control charts. Setting: Tertiary care academic hospital. Participants: All patients undergoing TKR at the host institution 1996–2009. Main outcome measure Operative efficiency. Results: 5313 procedures were analysed. Adjusted control charts were generated using a training dataset comprising 3756 procedures performed by 13 surgeons. The operative time gradually declined by 121 min with 25 years of experience (p<0.0001). Charts were tested by monitoring four other surgeons, performing an average of 389 procedures each. Adjustment for surgeon experience significantly altered the interpretation of operative efficiency (κ=0.29 (95% CI 0.11 to 0.47)), and enhanced assessment of a surgeon's improvement or diminishment in efficiency over time. Specifically, experience adjustment inverted the interpretation of surgeon efficiency from above average to below average, or from improving to declining performance. Conclusions: Adjustment for surgeon experience is necessary for accurate interpretation of metrics over the course of a surgeon's career. Participants' experiences of care during a randomized controlled trial comparing a lay-facilitated angina management programme with usual care: A qualitative study using focus groups
Aim: This paper is a report of a qualitative study conducted as part of a randomized controlled trial comparing a lay-facilitated angina management programme with usual care. Its aim was to explore participants' beliefs, experiences, and attitudes to the care they had received during the trial, particularly those who had received the angina management intervention. Background: Angina affects over 50 million people worldwide. Over half of these people have symptoms that restrict their daily life and would benefit from knowing how to manage their condition. Design: A nested qualitative study within a randomized controlled trial of lay-facilitated angina management. Method: We conducted four participant focus groups during 2008; three were with people randomized to the intervention and one with those randomized to control. We recruited a total of 14 participants to the focus groups, 10 intervention, and 4 control. Findings: Although recruitment to the focus groups was relatively low by comparison to conventional standards, each generated lively discussions and a rich data set. Data analysis demonstrated both similarities and differences between control and intervention groups. Similarities included low levels of prior knowledge about angina, whereas differences included a perception among intervention participants that lifestyle changes were more easily facilitated with the help and support of a lay-worker. Conclusion: Lay facilitation with the Angina Plan is perceived by the participants to be beneficial in supporting self-management. However, clinical expertise is still required to meet the more complex information and care needs of people with stable angina. Postural assessment of patients with non-conventional knee endoprosthesis
Objective:To investigate the correlation between the sagittal and frontal alignment and possible postural asymmetries found in patients submitted to total knee stent placement for osteosarcoma.Methods:Twenty two individuals were divided into two groups according to tumor location: femur group (13 patients) and tibia group (nine patients), who were evaluated through postural analysis software (SAPO).Results:No statistically significant difference was found between groups, supporting previous result showing that both groups present the same postural asymmetries.Conclusion:We conclude that both groups have the same postural imbalances, especially the knee of the affected limb that presents hyperextension and center of gravity shifted anteriorly and laterally to the non-affected limb, indicating changes in weight bearing and influencing the gait pattern and balance. O18O and C18O observations of rho Oph A
Observations of the (N_J=1_1-1_0) ground state transition of O_2 with the
Odin satellite resulted in a about 5 sigma detection toward the dense core rho
Oph A. At the frequency of the line, 119 GHz, the Odin telescope has a beam
width of 10', larger than the size of the dense core, so that the precise
nature of the emitting source and its exact location and extent are unknown.
The current investigation is intended to remedy this. Telluric absorption makes
ground based O_2 observations essentially impossible and observations had to be
done from space. mm-wave telescopes on space platforms were necessarily small,
which resulted in large, several arcminutes wide, beam patterns. Although the
Earth's atmosphere is entirely opaque to low-lying O_2 transitions, it allows
ground based observations of the much rarer O18O in favourable conditions and
at much higher angular resolution with larger telescopes. In addition, rho Oph
A exhibits both multiple radial velocity systems and considerable velocity
gradients. Extensive mapping of the region in the proxy C18O (J=3-2) line can
be expected to help identify the O_2 source on the basis of its line shape and
Doppler velocity. Line opacities were determined from observations of optically
thin 13C18O (J=3-2) at selected positions. During several observing periods,
two C18O intensity maxima in rho Oph A were searched for in the 16O18O
(2_1-0_1) line at 234 GHz with the 12m APEX telescope. Our observations
resulted in an upper limit on the integrated O18O intensity of < 0.01 K km/s (3
sigma) into the 26.5" beam. We conclude that the source of observed O_2
emission is most likely confined to the central regions of the rho Oph A cloud.
In this limited area, implied O_2 abundances could thus be higher than
