    Encontros no centro de saúde: a medicina de família e comunidade (MFC) e o sofrimento social

Programa de Pós-Graduação Interdisciplinar em Ciências HumanasA tese trata, com um enfoque interdisciplinar, do tema do sofrimento social na Atenção Primária à Saúde brasileira (APS). As duas principais perguntas da pesquisa voltaram-se para: uma caracterização das situações de sofrimento levadas pelas pessoas aos Centros de Saúde (C.S.); o papel dos médicos de família e comunidade (MFC) nesses encontros. As informações mais importantes para a análise provieram de uma observação participante em 4 C.S. de uma capital do sul do Brasil, especialmente do seguimento de consultas entre MFC e pacientes. Também foram utilizadas outras fontes, conforme a exploração realizada, como: artigos científicos, livros e documentos institucionais da medicina de família e comunidade (MFC), além de entrevistas com MFC; textos voltados para o tema da saúde mental; e, do mesmo modo, referências bibliográficas da psiquiatria. Ainda que haja um tema geral, que poderia ser definido como a (des)medicalização do sofrimento na APS, cada um dos capítulos traz um tema mais específico, com uma abordagem e considerações próprias. No que trata das situações de sofrimento que chegam aos C.S. percebe-se como estes representam um recurso para os mais variados lamentos e narrativas, identifica-se a dificuldade de delimitar ou classificar essa demanda e a sua tênue e perigosa relação com os diagnósticos biomédicos. O capítulo seguinte volta-se para a MFC e sua relação pendular com a biomedicina. Vemos como a MFC que cresce a partir da segunda metade do século passado aproxima-se e distancia-se dos ditames biomédicos conforme a situação. Em relação ao tema do sofrimento, algumas características peculiares do trabalho do MFC promovem uma relativização dos diagnósticos psiquiátricos. Esses conflitos entre os profissionais da APS e seus cânones, aqueles que pertencem ao que chamamos de Nova Psiquiatria e, por fim, os reformadores, que pensam e propõem as redes de saúde mental, são discutidos em um capítulo à parte. O que se percebe é que há visões distintas em relação à essa demanda que chega aos C.S. E, em outra parte, ainda, aborda-se o encontro propriamente dito entre MFC e pacientes, em que se apresenta como essa relação está atravessada por múltiplos aspectos e como ela é importante para definir o papel do MFC e suas condutas diante das situações observadas. Ressalta-se na conclusão a dupla função que pode cumprir o MFC em um tempo em que os C.S. são muito usados como recurso para a narrativa de sofrimentos cotidianos, a de expandir ou não a medicalização

    Fatigue and stress responses in athletes performing functional-fitness workout and its association with well-being

    We monitored fatigue and stress using heart rate variability and session rating perceived exertion in trained athletes performing a single bout of functional-fitness training workout. Also, we verified the association between heart rate variability and session rating perceived exertion with well-being. In the first week of tapering, eleven national athletes (age: 25.7 ± 3.3y; body mass index: 27.7 ± 2.8 kg·m-2; training history: > 4y) participated in this study. Heart rate variability was analyzed basal, before and after the experimental protocol. Session rating perceived exertion was analyzed after the experimental protocol, and after the assessments, the association between them and well-being was performed. Repeated measures of ANOVA were performed to compare condition x time, and Pearson correlation was used to analyze the associations. Heart rate variability decreased its values after the training workout (ηp2=11.5, p<0.001), and session rating perceived exertion was high (25.8 ± 6.9 a.u.). We did not find associations between heart rate variability or session rating perceived exertion and well-being (r between -0.34 and 0.35, p>0.05). This study did not support the idea of a significant relationship between objective/subjective, physiological assessments and well-being in one bout of training workout. Functional-fitness coaches and athletes should know the limited evidence about objective/subjective assessments and well-being

    Evaluation of contamination for extraneous materials in “sun meat” sold in the “houses of the north” in the municipality of Diadema (SP, Brazil)

    The “sun meat” is a handmade product, combining surface techniques of salting and dehydration, commonly used by people from the North and Northeast regions of Brazil. The sun meat cooking process lacks in sophisticated technology and official standards of procedure and quality. Thus, production, sale and distribution under unsatisfactory sanitary conditions are risky to the consumers’ health. This paper is aimed at evaluating the sanitary conditions of “sun meat” sold at the “houses of the north” for the presence of extraneous materials. Analysis of 44 samples of “sun meat” from 22 “houses of the north” located in “Diadema District” (SP, Brazil) had been carried out. The product conditions of exposure and sale in the “houses of the north” were evaluated and provided additional information to assess the degree of food safety of this product. In 44 samples were found various types of extraneous materials such as whole insects and debris, larvae, exuvia, mites, rodent hairs, bird feather pieces, fungi, and sharp  objects. Mechanical vectors were observed at 11 locations of the sale area. The results indicated that 90.9% of the “sun meat” has unsatisfactory sanitary conditions, caused by the presence of physical hazards of contamination. These results, associated with conditions found in the “houses of the north”, indicated that these products can put the health of consumers at risk.     Prevalence of peripheral arterial occlusive disease in patients referred to a tertiary care hospital in Salvador, Bahia, Brazil, for coronary angiography

    The presence of peripheral arterial occlusive disease increases the morbidity and mortality of patients with coronary artery disease. The objective of the present study was to calculate the prevalence of peripheral arterial occlusive disease in patients referred for coronary angiography. This prevalence study was carried out at the Hemodynamics Unit of Hospital Santa Isabel, Salvador, Brazil, from December 2004 to April 2005. After approval by the Ethics Committee of the hospital, 397 patients with angiographic signs of coronary artery disease were enrolled. Diagnosis of peripheral arterial occlusive disease was made using the ankle-brachial blood pressure index (£0.90). Statistical analyses were performed using the z test and a level of significance of a = 5%, 95%CI, the chi-square test and t-test, and multiple logistic regression analysis. The prevalence of peripheral arterial occlusive disease was 34.3% (95%CI: 29.4-38.9). Mean age was 65.7 ± 9.4 years for patients with peripheral arterial occlusive disease, and 60.3 ± 9.8 years for patients without peripheral arterial occlusive disease (P = 0.0000003). The prevalence of peripheral arterial occlusive disease was 1.57 times greater in patients with hypertension (P = 0.007) and 2.91 times greater in patients with coronary stenosis ³50% (P = 0.002). Illiterate patients and those with little education had a 44% higher chance of presenting peripheral arterial occlusive disease probably as a result of public health prevention policies of limited effectiveness.
