Körperliches Training bei mitochondrialen Erkrankungen
Zusammenfassung: Körperliches Training gilt bei mitochondrialen Myopathien als einer der vielversprechendsten therapeutischen Ansätze. Effektivität und Sicherheit sind bewiesen. Ausdauer- und Krafttraining haben unterschiedliche Wirkungen auf die Muskulatur von Patienten mit mitochondrialer Myopathie: Als therapeutischer Mechanismus des Krafttrainings gilt das so genannte "gene shifting", die trainingsinduzierte Verschiebung des Anteils mutierter mitochondrialer DNS (mtDNS) zugunsten von Wildtyp-mtDNS durch Induktion muskulärer Satellitenzellen. Ausdauertraining regt die mitochondriale Biogenese an und hilft somit, den Circulus vitiosus aus verringertem Mitochondriengehalt, verringerter Kapazität der oxidativen Phosphorylierung, Belastungsintoleranz und daraus resultierender fortschreitender muskulärer Dekonditionierung zu durchbrechen. Die Effektivität und die Sicherheit medikamentöser Induktoren der mitochondrialen Biogenese - möglicherweise in Kombination mit Training - könnten Gegenstand künftiger Untersuchungen sei
Trends in sexually transmitted infections in general practice 1990-2000: population based study using data from the UK general practice research database
Objective: To describe the contribution of primary care to the
diagnosis and management of sexually transmitted infections in
the United Kingdom, 1990-2000, in the context of increasing
incidence of infections in genitourinary medicine clinics.
Design: Population based study.
Setting: UK primary care.
Participants: Patients registered in the UK general practice
research database.
Main outcome measures: Incidence of diagnosed sexually
transmitted infections in primary care and estimation of the
proportion of major such infections diagnosed in primary care.
Results: An estimated 23.0% of chlamydia cases in women but
only 5.3% in men were diagnosed and treated in primary care
during 1998-2000, along with 49.2% cases of non-specific
urethritis and urethral discharge in men and 5.7% cases of
gonorrhoea in women and 2.9% in men. Rates of diagnosis in
primary care rose substantially in the late 1990s.
Conclusions: A substantial and increasing number of sexually
transmitted infections are diagnosed and treated in primary
care in the United Kingdom, with sex ratios differing from
those in genitourinary medicine clinics. Large numbers of men
are treated in primary care for presumptive sexually
transmitted infections
Dysferlinopathy in Switzerland: clinical phenotypes and potential founder effects.
BACKGROUND: Dysferlin is reduced in patients with limb girdle muscular dystrophy type 2B, Miyoshi myopathy, distal anterior compartment myopathy, and in certain Ethnic clusters.
METHODS: We evaluated clinical and genetic patient data from three different Swiss Neuromuscular Centers.
RESULTS: Thirteen patients from 6 non-related families were included. Age of onset was 18.8 ± 4.3 years. In all patients, diallelic disease-causing mutations were identified in the DYSF gene. Nine patients from 3 non-related families from Central Switzerland carried the identical homozygous mutation, c.3031 + 2 T>C. A possible founder effect was confirmed by haplotype analysis. Three patients from two different families carried the heterozygous mutation, c.1064_1065delAA. Two novel mutations were identified (c.2869 C>T (p.Gln957Stop), c.5928 G>A (p.Trp1976Stop)).
CONCLUSIONS: Our study confirms the phenotypic heterogeneity associated with DYSF mutations. Two mutations (c.3031 + 2 T>C, c.1064_1065delAA) appear common in Switzerland. Haplotype analysis performed on one case (c. 3031 + 2 T>C) suggested a possible founder effect
Restoration of critically endangered elkhorn coral (Acropora palmata) populations using larvae reared from wild-caught gametes
AbstractElkhorn coral (Acropora palmata) populations provide important ecological functions on shallow Caribbean reefs, many of which were lost when a disease reduced their abundance by more than 95% beginning in the mid-1970s. Since then, a lack of significant recovery has prompted rehabilitation initiatives throughout the Caribbean. Here, we report the first successful outplanting and long-term survival of A. palmata settlers reared from gametes collected in the field. A. palmata larvae were settled on clay substrates (substrate units) and either outplanted on the reef two weeks after settlement or kept in a land-based nursery. After 2.5 years, the survival rate of A. palmata settlers outplanted two weeks after settlement was 6.8 times higher (3.4%) than that of settlers kept in a land-based nursery (0.5%). Furthermore, 32% of the substrate units on the reef still harbored one or more well-developed recruit compared to 3% for substrate units kept in the nursery. In addition to increasing survival, outplanting A. palmata settlers shortly after settlement reduced the costs to produce at least one 2.5-year-old A. palmata individual from 13 USD. Thus, this study not only highlights the first successful long-term rearing of this critically endangered coral species, but also shows that early outplanting of sexually reared coral settlers can be more cost-effective than the traditional approach of nursery rearing for restoration efforts aimed at rehabilitating coral populations
Moduli-Space Dynamics of Noncommutative Abelian Sigma-Model Solitons
In the noncommutative (Moyal) plane, we relate exact U(1) sigma-model
solitons to generic scalar-field solitons for an infinitely stiff potential.
The static k-lump moduli space C^k/S_k features a natural K"ahler metric
induced from an embedding Grassmannian. The moduli-space dynamics is blind
against adding a WZW-like term to the sigma-model action and thus also applies
to the integrable U(1) Ward model. For the latter's two-soliton motion we
compare the exact field configurations with their supposed moduli-space
approximations. Surprisingly, the two do not match, which questions the

Religious positions on climate change and climate policy in the United States
February 2006, a group of 86 evangelical leaders, under the auspices of the Evangelical Climate Initiative, challenged the Bush administration on global warming. Other religious groups and leaders in the USA, and other countries, have taken positions as well. As the US evangelical community seems to have a considerable influence on the views and policy of (Republican) national leaders, these developments are relevant for assessing US and international climate policy. Using argumentative discourse analysis, this paper analyzes the religious positions on climate change and climate policy in the United States, as evident in their communication in the media, opinion documents, and websites. Religious positions show a wide range of views, images, and discourses that deal with fundamental moral and ethical questions concerning climate change, stewardship and social justice. Our main conclusion is that both proponents and opponents of strict climate policy strongly value these concepts, but that they interpret them in different ways. A robust policy strategy (regarding support in the religious community) should pay careful attention to the effects of both climate change and climate policy on the poor in both developing nations and the USA itself.Keywords: environmental justice, equity, ethics, religion and environment, climate policy, United State

Musicoterapia en una Unidad de Cuidados Intensivos Neonatales: experiencia benéfica para el binomio
ResumenIntroducciónDesde hace varios años se está utilizando la música como herramienta terapéutica y preventiva en la neonatología, se han documentado en un gran número de artículos de investigación, sin embargo, en México esta técnica es poco utilizada.ObjetivoMejorar las condiciones de los neonatos atendidos en una Unidad de Cuidados Intensivos (UCIN) y sus familiares, mediante el uso de musicoterapia, para minimizar el estrés y los daños colaterales.Materiales y métodosUn grupo de 2 neonatólogos, 3 residentes de pediatría, 10 enfermeras y un concertista profesional de violín, llevaron a cabo 6 sesiones musicales semanales en la Unidad de Cuidados Intensivos Neonatales Externos del Hospital Civil de Guadalajara. Durante el concierto se valoró individualmente a cada neonato, registrando los signos vitales por monitorización electrónica, además se registró el estado anímico de los familiares.ResultadosDurante las sesiones de música los neonatales no presentaron cambios significativos en sus constantes vitales, en el personal de la UCIN y en los padres de recién nacidos hospitalizados se vieron efectos benéficos, refiriendo disminución del estrés y altos índices de satisfacción.ConclusionesLa música puede tener un efecto positivo como terapia coadyuvante en neonatos de alto riesgo, principalmente en prematuros. Se recomienda difundir y aplicar esta metodología en las UCIN del país.AbstractIntroductionSince several years ago, music has been used as a therapeutic and preventive tool in neonatology. A great number of research papers have been written on the subject; however, this technique is rarely used in Mexico.ObjectiveTo improve the condition of neonates at Neonatal Intensive Care Units (NICU) and their relatives, in order to minimize any stress and collateral damage.Material and methods: A group of two neonatologists, 3 residents, 10 nurses and a professional concert violinist, conducted six weekly music sessions in the Outpatient Neonatal Intensive Care Unit (ONICU) of our institution. During the concert, we individually assessed each newborn, recording vital signs through electronic monitoring; additionally, we recorded the mood of any other family members present.ResultsDuring the music sessions, the neonatal patients showed no significant changes in vital signs; yet, ONICU staff and the parents/relatives of hospitalized newborns showed beneficial effects, reporting decreased stress and high levels of satisfaction.ConclusionsMusic can have a positive effect as adjunctive therapy in high-risk newborns, especially in premature babies. We therefore recommend applying this methodology in the ONICU
Clostridioides difficile-associated diarrhea in surgical service patients in Mexico
Introduction: Clostridium difficile is the first cause of healthcare-associated diarrhea in developed
countries. In recent years the incidence of C. difficile infection (CDI) has increased
worldwide. There is not much information on the topic in Mexico, and little is known about
the risk factors for the infection in patients that are hospitalized in surgical services.
Materials and methods: A case-control study was conducted that compared the epidemiologic
findings and risk factors between surgical patients with PCR-confirmed CDI, surgical patients
with diarrhea and a negative PCR test, and surgical patients with no diarrhea. The statistical
analysis was carried out using the SPSS version 22.0 program.
Results: The majority of the surgical patients with CDI belonged to the areas of neurosurgery,
cardiac surgery, orthopedics, and general surgery. A total of 53% of the CDI cases were associated
with the hypervirulent CD NAP1/027 strain. The presence of mucus in stools (OR: 1.5, p = 0.001),
fever (OR: 1.4, p = 0.011), leukocytes in stools (OR: 3.2, p < 0.001), hospitalization within the
past twelve weeks (OR: 2.0, p < 0.001), antibiotic use (OR: 1.3, p = 0.023), and ceftriaxone use
(OR: 1.4, p = 0.01) were independent risk factors for the development of CDI
