    Diseño de un tiovivo a pedales

    En este trabajo de final de grado se quiere conseguir diseñar un tiovivo a pedales lo más desmontable posible para facilitar su movilidad y almacenaje. Para ello se tendrá en cuenta la normativa UNE-EN 13814 maquinaria y estructuras para parques y ferias de atracciones. Para comprobar que el diseño es correcto se realizan diferentes cálculos como, por ejemplo, la resistencia de los ejes a fatiga. Para comprobar que estos cálculos son correctos se realiza una simulación mediante SolidWorks de los elementos críticos. También se realizan los planos explosionados de los conjuntos y los de las piezas diseñadas. Para finalizar se calcula el presupuesto con el precio que costaría fabricar un prototipo

    Benchmarking Deep Learning Architectures for Predicting Readmission to the ICU and Describing Patients-at-Risk

    Objective: To compare different deep learning architectures for predicting the risk of readmission within 30 days of discharge from the intensive care unit (ICU). The interpretability of attention-based models is leveraged to describe patients-at-risk. Methods: Several deep learning architectures making use of attention mechanisms, recurrent layers, neural ordinary differential equations (ODEs), and medical concept embeddings with time-aware attention were trained using publicly available electronic medical record data (MIMIC-III) associated with 45,298 ICU stays for 33,150 patients. Bayesian inference was used to compute the posterior over weights of an attention-based model. Odds ratios associated with an increased risk of readmission were computed for static variables. Diagnoses, procedures, medications, and vital signs were ranked according to the associated risk of readmission. Results: A recurrent neural network, with time dynamics of code embeddings computed by neural ODEs, achieved the highest average precision of 0.331 (AUROC: 0.739, F1-Score: 0.372). Predictive accuracy was comparable across neural network architectures. Groups of patients at risk included those suffering from infectious complications, with chronic or progressive conditions, and for whom standard medical care was not suitable. Conclusions: Attention-based networks may be preferable to recurrent networks if an interpretable model is required, at only marginal cost in predictive accuracy

    Introduction: A limitation with randomized controlled trials is that, while they provide unbiased evidence of the efficacy of interventions, they do so under unreal conditions and in a very limited and highly selected patient population. Our aim was to provide data about the effectiveness of liraglutide treatment in a real-world and clinical practice setting. Methods: In a retrospective and observational study, data from 753 patients with type 2 diabetes were recorded through an online tool (eDiabetes-Monitor). Results: Mean baseline glycated hemoglobin (HbA1c) was 8.4 ± 1.4% and mean body mass index (BMI) was 38.6 ± 5.4 kg/m2. After 3–6 months of treatment with liraglutide, we observed a change in HbA1c of -1.1 ± 1.2%, -4.6 ± 5.3 kg in weight and -1.7 ± 2.0 kg/m

    Clinical and Laboratory Development of Echinocandin Resistance in Candida glabrata: Molecular Characterization

    The pathogenic yeast Candida glabrata has become a public health issue due to the increasing number of echinocandin resistant clinical strains reported. In this study, acquisition and development of resistance to this antifungal class were studied in serial C. glabrata isolates from five patients admitted in two Spanish hospitals with a resistant profile against echinocandins associated with different mutations in hot-spot 1 of FKS2 gene. For two of these patients susceptible FKS wild-type isolates obtained prior to resistant ones were also investigated. Isolates were genotyped using multilocus sequence typing and microsatellite length polymorphism techniques, which yielded comparable results. Susceptible and resistant isolates from the same patient had the same genotype, being sequence type (ST) 3 the most prevalent among them. Isolates with different FKS mutations but the same ST were present in the same patient. MSH2 gene alterations were also studied to investigate their correlation with antifungal resistance acquisition but no association was found with antifungal resistance nor with specific genotypes. In vitro exposure to increasing concentrations of micafungin to susceptible isolates developed colonies carrying FKS mutations in agar plates containing a minimum concentration of 0.06 mg/L of micafungin after less than 48 h of exposure. We investigated the correlation between development of resistance and genotype in a set of susceptible strains after being in vitro exposed to micafungin and anidulafungin but no correlation was found. Mutant prevention concentration values and spontaneous growth frequencies after selection with both echinocandins were statistically similar, although FKS mutant colonies were more abundant after micafungin exposure (p < 0.001).     La marcha humana es uno de los aspectos que comprometen directamente el nivel de bienestar del ser humano, además deimpactar de manera emocional y física, también en el cotidiano vivir. Para este estudio nos encontramos inmersos en el reto de poder mejoraralgunas condiciones de la marcha en un paciente que haya sufrido pérdida de miembros inferiores, específicamente a nivel transtibial o transfemoral.Dado que nuestro propósito fue el análisis, diseño y manufactura de un componente protésico de miembro inferior que supla las necesidades propiasy de funcionalidad, es necesario construir un pie con todos los estándares de calidad que constituya todos y cada uno de los movimientos requeridospara formar el complejo patrón fundamental de la marcha; además que pueda soportar el peso fácilmente y el uso cotidiano además de lascaracterísticas físicas de nuestro paciente objeto de estudio. Al realizar la validación física y en la marcha humana, se observa una respuestaadecuada en términos mecánicos, de material de construcción y el dinamismo del componente, evidenciando la adecuada construcción y ensambledel componente. Por otro lado, podemos evidenciar que el diseño y la verificación del componente nos muestra un elemento competitivo, comparadocon los elementos existentes en el mercado; Haciendo necesario la verificación ante Instituto Nacional de Vigilancia de Medicamentos y AlimentosINVIMA, y la puesta en marcha del dispositivo replicador de uso, para la verificación del componente frente a la norma ISO 10328.Walking is one of the aspects directly compromising human wellbeing, as it has a physical and emotional impact in daily life.For this study, we delve into the challenge of improving some walking conditions in a patient suffering lower limb loss, specifically at transtibialor transfemoral levels. Given that our purpose was the analysis, design and manufacture of a lower-limb prosthetic component, which fills the needsfor functionality, it became necessary to build a foot with all the quality standards associated to each and all movements required to form thecomplex fundamental pattern of walking. Besides, this foot should also easily endure weight, daily use and physical characteristics of the patientobject of this study. When performing physical validation and during human walk, a proper response is observed in terms of mechanics, materialsand dynamics of the component, thus making evident proper construction and assembly. On the other hand, it is feasible that design and verificationof the component provided a competitive element, as compared to existing elements currently in the market. The previous situation generated theneed for verification from the National Institute for Medications and Food (INVIMA), as well as the revision of the use replying device, forcomponent verification, in accordance with ISO 10328

    El objetivo general del plan de negocios es evaluar la viabilidad económica de la empresa de zapatos loafers de biocuero para damas con accesorios personalizados mediante e-commerce en Lima moderna. Los objetivos específicos incluyen mantener un flujo de caja positivo para respaldar el crecimiento sostenible y la expansión del negocio, y asegurar la disponibilidad adecuada de capital de trabajo para respaldar la operación diaria de la empresa. Para lograr estos objetivos, se realizó un análisis cuantitativo y descriptivo de la data recopilada en una encuesta, donde se examinó la información obtenida para cada pregunta formulada en el cuestionario. Los resultados indicaron que el 87.66% de las mujeres encuestadas pertenecen al rango de edad de 20 a 40 años, el ingreso promedio del 80% de las encuestadas es superior a los S/2500 y el 70% de mujeres encuestadas tiene de 11 a 20 pares de zapatos. En conclusión, este plan de negocios presenta una oportunidad de negocio prometedora para atender a las mujeres peruanas que buscan estar a la moda con exclusividad y sentirse cómodas.     A systematic cost-benefit analysis of 29 road safety measures

    Economic evaluations of road safety measures are only rarely published in the scholarly literature. We collected and (re-)analyzed evidence in order to conduct cost-benefit analyses (CBAs) for 29 road safety measures. The information on crash costs was based on data from a survey in European countries. We applied a systematic procedure including corrections for inflation and Purchasing Power Parity in order to express all the monetary information in the same units (EUR, 2015). Cost-benefit analyses were done for measures with favorable estimated effects on road safety and for which relevant information on costs could be found. Results were assessed in terms of benefit-to-cost ratios and net present value. In order to account for some uncertainties, we carried out sensitivity analyses based on varying assumptions for costs of measures and measure effectiveness. Moreover we defined some combinations used as best case and worst case scenarios. In the best estimate scenario, 25 measures turn out to be cost-effective. 4 measures (road lighting, automatic barriers installation, area wide traffic calming and mandatory eyesight tests) are not cost-effective according to this scenario. In total, 14 measures remain cost-effective throughout all scenarios, whereas 10 other measures switch from cost-effective in the best case scenario to not cost-effective in the worst case scenario. For three measures insufficient information is available to calculate all scenarios. Two measures (automatic barriers installation and area wide traffic calming) even in the best case do not become cost-effective. Inherent uncertainties tend to be present in the underlying data on costs of measures, effects and target groups.     Method for Independent Estimation of the False Localization Rate for Phosphoproteomics

    Phosphoproteomic methods are commonly employed to identify and quantify phosphorylation sites on proteins. In recent years, various tools have been developed, incorporating scores or statistics related to whether a given phosphosite has been correctly identified or to estimate the global false localization rate (FLR) within a given data set for all sites reported. These scores have generally been calibrated using synthetic datasets, and their statistical reliability on real datasets is largely unknown, potentially leading to studies reporting incorrectly localized phosphosites, due to inadequate statistical control. In this work, we develop the concept of scoring modifications on a decoy amino acid, that is, one that cannot be modified, to allow for independent estimation of global FLR. We test a variety of amino acids, on both synthetic and real data sets, demonstrating that the selection can make a substantial difference to the estimated global FLR. We conclude that while several different amino acids might be appropriate, the most reliable FLR results were achieved using alanine and leucine as decoys. We propose the use of a decoy amino acid to control false reporting in the literature and in public databases that re-distribute the data.
