    Generation of non-diffractive Bessel beams by inward cylindrical traveling wave aperture distributions

    International audienceThe focusing capabilities of an inward cylindrical traveling wave aperture distribution and the non-diffractive behaviour of its radiated field are analyzed. The wave dynamics of the infinite aperture radiated field is clearly unveiled by means of closed form expressions, based on incomplete Hankel functions, and their ray interpretation. The non-diffractive behaviour is also confirmed for finite apertures up to a defined limited range. A radial waveguide made by metallic gratings over a ground plane and fed by a coaxial feed is used to validate numerically the analytical results. The proposed system and accurate analysis of non-diffractive Bessel beams launched by inward waves opens new opportunities for planar, low profile beam generators at microwaves, Terahertz and optics

    When to start and when to stop antifibrotic therapies

    Idiopathic pulmonary fibrosis (IPF) is characterised by progressive changes of the lung architecture causing cough and dyspnoea and ultimately leading to lung failure and death. Today, for the first time, two drugs that may reduce the inexorable progression of the disease are available, suggesting that treatment with specific drugs for IPF should be started as soon as diagnosis is made. This applies to any disease and particularly to IPF, which is marked by a 5-year survival comparable or even worse than many cancers. However, despite common sense and even worse, in spite of scientific data coming from clinical trials, post hoc analysis, long-term safety studies and real-life experiences, the question of when to start and when to stop treatment with antifibrotics is still debated. In IPF, particularly when the disease is diagnosed at an early stage, "wait and watch" behaviour is not rare to observe. This is largely due to the lack of awareness of both patients and clinicians regarding the progression of the disease and its prognosis. Another important issue is when treatment should be stopped. In general, there are two main reasons to stop a therapy: unbearable side-effects and/or lack of efficacy. According to current (although preliminary) evidence, antifibrotic drugs should not be discontinued except for safety issues

    On the Co-Design of AV-Enabled Mobility Systems

    The design of autonomous vehicles (AVs) and the design of AV-enabled mobility systems are closely coupled. Indeed, knowledge about the intended service of AVs would impact their design and deployment process, whilst insights about their technological development could significantly affect transportation management decisions. This calls for tools to study such a coupling and co-design AVs and AV-enabled mobility systems in terms of different objectives. In this paper, we instantiate a framework to address such co-design problems. In particular, we leverage the recently developed theory of co-design to frame and solve the problem of designing and deploying an intermodal Autonomous Mobility-on-Demand system, whereby AVs service travel demands jointly with public transit, in terms of fleet sizing, vehicle autonomy, and public transit service frequency. Our framework is modular and compositional, allowing one to describe the design problem as the interconnection of its individual components and to tackle it from a system-level perspective. To showcase our methodology, we present a real-world case study for Washington D.C., USA. Our work suggests that it is possible to create user-friendly optimization tools to systematically assess costs and benefits of interventions, and that such analytical techniques might gain a momentous role in policy-making in the future.Comment: 8 pages, 4 figures.     A Systematic Review of Pharmacologic and Rehabilitative Treatment of Small Fiber Neuropathies

    The aim of this systematic review is to guide the physician in defining the pharmacologicand rehabilitative therapeutic approaches for adopting the best strategies described in the currentliterature. The search was conducted in PubMed, EMBASE, Cochrane Library and Web of Scienceto identify the treatment of small fiber neuropathies. Two reviewers independently reviewed andcame to a consensus on which articles met inclusion/exclusion criteria. The authors excluded theduplicates, animal studies and included the English articles in which the treatment of patients withsmall fiber neuropathies was described. The search identified a total of 975 articles with the keywords“small fiber neuropathy” AND “rehabilitation” OR “therapy” OR “treatment”. Seventy-eight selectedfull-text were analyzed by the reviewers. Forty-two publications met the inclusion criteria and wereincluded in the systematic review to describe the rehabilitative and pharmacologic treatment of smallfiber neuropathies. Despite the range of different protocols of treatment for small fiber neuropathy,other robust trials are needed. In addition, always different therapeutic approaches are used; a uniqueprotocol could be important for the clinicians. More research is needed to build evidence for the beststrategy and to delineate a definitive therapeutic protocol

    On the Near-Field Shaping and Focusing Capability of a Radial Line Slot Array

    International audienceWe describe the design of a radial line slot array antenna with a shaped and focused near field. The antenna is designed in such a way to control the side lobe level and beamwidth of the normal component of the electric field with respect to the radiating aperture. The design procedure consists of two steps. In the first step, the requirements on the near-field pattern are provided over a focusing plane at a given distance from the radiating aperture. A set theoretic approach is then used to derive the aperture field distribution fitting the requirements over the near field. In the second step, the aperture field distribution is synthesized by accurately placing and sizing the slots of the antenna. The spillover efficiency is maximized during the design process. The antenna is centrally fed by a simple coaxial probe. The antenna design is validated by a prototype and measurements at 12.5 GHz

    Therapeutic approaches to pediatric pseudotumor cerebri: new insights from literature data

    The pseudotumor cerebri syndrome (PTCS), also known as idiopathic intracranial hypertension (IIH), is characterized by signs and symptoms of increased intracranial pressure such as headache and cranial nerve palsies, in the absence of any space-occupying mass. This condition commonly affects overweight women of childbearing age, even if it is also frequent in men and children. Children with PTCS may present with atypical signs and symptoms, with a different prognosis compared to adults. However, the treatment is the same for both children and adults, even if there are no strict treatment guidelines in regards. All treatment strategies in children are based on retrospective studies and none has been evaluated in prospective or randomized controlled trial studies.     Advanced multimodal laser imaging tool for urothelial carcinoma diagnosis (AMPLITUDE)

    Bladder cancer (BC) is the eleventh most diagnosed cancer worldwide. The age-standardized incidence rate (per 100 000 person/years) is 9.0 for men and 2.2 for women [1]. Urothelial carcinoma (UC) represents about 90% of all bladder tumors, thus carrying an enormous social and economic burden [2]. UCs are classified in different stages and grades, depending on their invasiveness and on their degree of cytological abnormalities. The key aspect for a positive prognosis is the early and accurate diagnosis of the lesion stage, in order to identify the most aggressive disease forms and treat them promptly. It is well known that tissue metabolism constitutes a basic mechanism, which is at the base of many pathologies, especially BC. Being able to detect and characterize tissue metabolism and molecular fingerprints at the cellular level could be a key aspect in characterizing the pathology and enabling both early detection and therapy monitoring. The new European Union Horizon 2020 project called AMPLITUDE, the ‘Advanced Multimodal Photonics Laser Imaging Tool for Urothelial Diagnosis in Endoscopy’, starting in January 2020, proposes the development of an advanced multi-modal imaging tool exploiting new laser technologies in an approach combining confocal and non-linear imaging to fulfil unmet clinical needs in terms of the specificity and accuracy of urothelial cancer diagnosis and therapy monitoring. The project is coordinated by Tampere University (Finland) and carried out in cooperation with leading European research organizations including Aston Insitute of Photonic Technologies—AIPT (UK), Consiglio Nazionale delle Ricerche—CNR (Italy), Institute of Photonic Sciences—ICFO (Spain), University of Milan-Bicocca, Modus Research and Innovation Ltd. (UK) and University of Florence (Italy), as well as industrial partners: Ampliconyx Oy (Finland), Femtonics Ltd. (Hungary), HC Photonics (Taiwan), and LEONI Fiber Optics GmbH (Germany)
