Tratamiento de la Litiasis Biliar por mini-incisiones adaptadasy por videolaparoscopia. Estudio prospectivo y comparativo

Desde la década pasada, se considera a la colecistectomía por video-laparoscópica como el gold-standard. Objetivos: El objetivo de este estudio es evaluar los resultados obtenidos con la mini- incisiones adaptadas y con la video-laparoscopia en el tratamiento de la colecistolitiasis, ampliando en esta experiencia el mismo concepto, pero aplicándolo al tratamiento de la litiasis de la vía biliar principal. Diseño: Prospectivo y comparativo protocolizado.Lugar de aplicación: Servicio de Cirugía General “Pablo Luis Mirizzi” del Hospital Nacional de Clínicas y en nuestra practica profesional privada Clínica Caraffa. Material y Métodos: En el presente trabajo se analiza dos grupos de pacientes en el tratamiento de la litiasis biliar electiva y sus complicaciones entre enero de 2000 y diciembre del 2006. En el primero (Grupo 1), se operaron 370 pacientes que se realizo la colecistectomía por video-laparoscopía. En el segundo (Grupo 2), se operaron 424 pacientes por una colecistectomía por mini-incisión adaptadas. Los pacientes portadores de litiaisis de la vía biliar principal fueron analizados en un tercer grupo (Grupo 3) y abordados quirúrgicamente por mini-incisiones adaptadas. Resultados: No hubo mortalidad en el grupo 1 y 2. En el tercero fallecio un paciente lo que representa el 1,18%. En relación a la morbilidad, en el primer grupo de colecistectomía por video laparoscopía hubo 63 pacientes con complicaciones (17,22%). Dentro de ellas, dos pacientes con complicaciones médicas (0,58%). Hubo 54 pacientes (14,58%) con complicaciones intra-operatorias. En las complicaciones postoperatorias hubo 7 pacientes (2,05%). RESUMEN:

En 7 casos (1,83%) por complicaciones médicas. No hubo complicaciones intraoperatorias. 29 pacientes (7,59%) tuvieron complicaciones post-operatorias. Debemos destacar que en tamaño de las incisiones vario de 3, 5 a 7 cm, lo cual se reduce entre 3,5 a 4,5 en los pacientes con colecistolitiasis no complicada. En el grupo 3, en 11 pacientes (20,37%) tuvieron morbilidad. De ellos 4 casos (7,40%) complicaciones médicas. No hubo complicaciones intraoperatorias. Siete pacientes (12,96 %) tuvieron complicaciones postoperatorias. El tamaño de las incisiones en este grupo fue entre 5 a 7 cm. En los pacientes controlados no se ha observado eventracion.
Conclusiones: En la colecistectomía laparoscópica las indicaciones son similares a las de la cirugía convencional. Sin embargo, creemos que la sospecha de un cáncer de vesícula y el síndrome de Mirizzi tipo II sigue siendo resorte de la cirugía a cielo abierto. El porcentaje de colangiografía intraoperatoria o Mirizzigrafía fue significativamente superior en el grupo de pacientes tratados con mini-incisiones adaptadas. La incidencia de litiaisis colédociana en este estudio fue para el primer grupo de 3,24 % y del 9,90 % para el grupo 2. Con respecto al tiempo de hospitalización en el grupo 1 fue de 1,5 días, mientras que para el grupo 2 fue de 2 días y en el grupo 3 fue de 4,5 días. De acuerdo a nuestros resultados la colecistolitiasis puede ser abordada perfectamente con mini-incisiones adaptadas, que nos brinda resultados superponibles a los de la video-laparoscopia en términos de días de hospitalización y rehabilitación, pero con significativa ventaja en relación a las complicaciones intraoperatorias, por lo general siempre graves y de pronóstico incierto en los pacientes abordados por video-laparoscopia.Fil: Lada, Paul Eduardo. Matrix metalloproteinase-9 predicts pulmonary status declines in α1-antitrypsin deficiency
<p>Abstract</p> <p>Background</p> <p>Matrix metalloproteinase-9 (MMP-9) may be important in the progression of emphysema, but there have been few longitudinal clinical studies of MMP-9 including pulmonary status and COPD exacerbation outcomes.</p> <p>Methods</p> <p>We utilized data from the placebo arm (n = 126) of a clinical trial of patients with alpha<sub>1</sub>-antitrypsin deficiency (AATD) and emphysema to examine the links between plasma MMP-9 levels, pulmonary status, and COPD exacerbations over a one year observation period. Pulmonary function, computed tomography lung density, incremental shuttle walk test (ISWT), and COPD exacerbations were assessed at regular intervals over 12 months. Prospective analyses used generalized estimating equations to incorporate repeated longitudinal measurements of MMP-9 and all endpoints, controlling for age, gender, race-ethnicity, leukocyte count, and tobacco history. A secondary analysis also incorporated highly-sensitive C-reactive protein levels in predictive models.</p> <p>Results</p> <p>At baseline, higher plasma MMP-9 levels were cross-sectionally associated with lower FEV<sub>1 </sub>(p = 0.03), FVC (p < 0.001), carbon monoxide transfer factor (p = 0.03), resting oxygen saturation (p = 0.02), and ISWT distance walked (p = 0.02) but were not associated with radiographic lung density or total lung capacity (TLC). In longitudinal analyses, MMP-9 predicted a further decline in transfer factor (p = 0.04) and oxygen saturation (p < 0.001). MMP-9 also predicted worsening lung density (p = 0.003), increasing TLC (p = 0.02), and more frequent COPD exacerbations over follow-up (p = 0.003). Controlling additionally for hs-CRP levels did not substantively change the longitudinal associations between MMP-9 and these outcomes.</p> <p>Conclusions</p> <p>Increased plasma MMP-9 levels generally predicted pulmonary status declines, including worsening transfer factor and lung density as well as greater COPD exacerbations in AATD-associated emphysema.</p
The efficiency of star formation in clustered and distributed regions
We investigate the formation of both clustered and distributed populations of
young stars in a single molecular cloud. We present a numerical simulation of a
10,000 solar mass elongated, turbulent, molecular cloud and the formation of
over 2500 stars. The stars form both in stellar clusters and in a distributed
mode which is determined by the local gravitational binding of the cloud. A
density gradient along the major axis of the cloud produces bound regions that
form stellar clusters and unbound regions that form a more distributed
population. The initial mass function also depends on the local gravitational
binding of the cloud with bound regions forming full IMFs whereas in the
unbound, distributed regions the stellar masses cluster around the local Jeans
mass and lack both the high-mass and the low-mass stars. The overall efficiency
of star formation is ~ 15 % in the cloud when the calculation is terminated,
but varies from less than 1 % in the the regions of distributed star formation
to ~ 40 % in regions containing large stellar clusters. Considering that large
scale surveys are likely to catch clouds at all evolutionary stages, estimates
of the (time-averaged) star formation efficiency for the giant molecular cloud
reported here is only ~ 4 %. This would lead to the erroneous conclusion of

Evidence for Dynamical Changes in a Transitional Protoplanetary Disk with Mid-infrared Variability
We present multi-epoch Spitzer Space Telescope observations of the
transitional disk LRLL 31 in the 2-3 Myr-old star forming region IC 348. Our
measurements show remarkable mid-infrared variability on timescales as short as
one week. The infrared continuum emission exhibits systematic
wavelength-dependent changes that suggest corresponding dynamical changes in
the inner disk structure and variable shadowing of outer disk material. We
propose several possible sources for the structural changes, including a
variable accretion rate or a stellar or planetary companion embedded in the
disk. Our results indicate that variability studies in the infrared can provide

Star formation in metal-poor gas clouds
Observations of molecular clouds in metal-poor environments typically find
that they have much higher star formation rates than one would expect based on
their observed CO luminosities and the molecular gas masses that are inferred
from them. This finding can be understood if one assumes that the conversion
factor between CO luminosity and H2 mass is much larger in these low
metallicity systems than in nearby molecular clouds. However, it is unclear
whether this is the only factor at work, or whether the star formation rate of
the clouds is directly sensitive to the metallicity of the gas.
To investigate this, we have performed numerical simulations of the coupled
dynamical, chemical and thermal evolution of model clouds with metallicities
ranging from 0.01 Z_solar to Z_solar. We find that the star formation rate in
our model clouds has little sensitivity to the metallicity. Reducing the
metallicity of the gas by two orders of magnitude delays the onset of star
formation in the clouds by no more than a cloud free-fall time and reduces the
time-averaged star formation rate by at most a factor of two. On the other
hand, the chemical state of the clouds is highly sensitive to the metallicity,
and at the lowest metallicities, the clouds are completely dominated by atomic
gas. Our results confirm that the CO-to-H2 conversion factor in these systems
depends strongly on the metallicity, but also show that the precise value is
highly time-dependent, as the integrated CO luminosity of the most metal-poor
clouds is dominated by emission from short-lived gravitationally collapsing
regions. Finally, we find evidence that the star formation rate per unit H2
mass increases with decreasing metallicity, owing to the much smaller H2

El Neurofibroma es una neoplasia benigna que se origina en la vaina de los nervios periféricos, la cual es más frecuente de localizarse en los tejidos superficiales, sobre todo en las formas aisladas. Los neurofibromas pueden ser de dos tipos, localizados o difusos, estos últimos íntimamente relacionados con la enfermedad de Von Recklinghausen o NF-1. En un paciente masculino de 37 años oligo-sintomático tanto la tomografía computada, como la resonancia magnética nuclear se puede observar el tumor en vecindad con el riñón derecho, el músculo psoas, la columna vertebral y la compresión de la vena cava, más no pueden determinar con exactitud la invasión de dichas estructuras. The Spitzer Survey of Interstellar Clouds in the Gould Belt. III. A Multi-Wavelength View of Corona Australis
We present Spitzer Space Telescope IRAC and MIPS observations of a 0.85 deg^2
field including the Corona Australis (CrA) star-forming region. At a distance
of 130 pc, CrA is one of the closest regions known to be actively forming
stars, particularly within its embedded association, the Coronet. Using the
Spitzer data, we identify 51 young stellar objects (YSOs) in CrA which include
sources in the well-studied Coronet cluster as well as distributed throughout
the molecular cloud. Twelve of the YSOs discussed are new candidates, one of
which is located in the Coronet. Known YSOs retrieved from the literature are
also added to the list, and a total of 116 candidate YSOs in CrA are compiled.
Based on these YSO candidates, the star formation rate is computed to be 12 M_o
Myr^-1, similar to that of the Lupus clouds. A clustering analysis was also
performed, finding that the main cluster core, consisting of 68 members, is
elongated (having an aspect ratio of 2.36), with a circular radius of 0.59 pc
and mean surface density of 150 pc^-2.
In addition, we analyze outflows and jets in CrA by means of new CO and H_2
data. We present 1.3 mm interferometric continuum observations made with the
Submillimeter Array (SMA) covering R CrA, IRS 5, IRS 7, and IRAS 18595-3712
(IRAS 32). We also present multi-epoch H_2 maps and detect jets and outflows,
study their proper motions, and identify exciting sources. The Spitzer and
ISAAC/VLT observations of IRAS 32 show a bipolar precessing jet, which drives a
CO (2-1) outflow detected in the SMA observations. There is also clear evidence
for a parsec-scale precessing outflow, E-W oriented, and originating in the SMA
2 region, likely driven by SMA 2 or IRS 7A.Comment: Accepted for publication in ApJS. 112 pages, 42 figures (quality
