    Techniques of intestinal transplantation in rat

    Two surgical models of intestinal transplantation in the rat are described. One is the implantation of fetal and newborn intestine as free grafts into the omentum of adult recipients, the other the adult intestine transplantation as an accessory graft using vascular anastomoses. A hundred and sixteen small-bowel transplantations were done; 36 of which were fetal intestine (group I), 40 of newborn intestine (group II), and 40 of adult intestine (group III). In the fetal and newborn intestinal transplantation, we emphasize the practices that allowed us to avoid ischemic and traumatic injury to the graft. In the adult intestine transplantation with vascular anastomoses, we heighten the modifications in the surgical technique that made the operation easier and the strategies used to prevent hypothermia and hypovolemic shock. Once experienced with the two chosen surgical techniques, transplantation using an avascular segment became much easier and quicker than transplantation with vascular anastomoses. © 1998 Wiley-Liss, Inc.     Anatomic Aspects of Inguinal Lymph Nodes Applied to Lymphadenectomy in Penile Cancer

    Objectives. To provide a better understanding of the distribution of inguinal nodes in order to prevent the complications of unnecessary and extended dissections in penile cancer. Methods. The bilateral inguinal regions of 19 male cadavers were dissected. Nodal distribution was noted and quantified based on anatomical location. The superficial nodes were subdivided into quarters as follows: superomedial, superolateral, inferomedial, and inferolateral. Statistical analysis was performed comparing node distribution between quarters using one-way analysis of variance (ANOVA), and the unpaired T-test was used between superficial and deep nodes. Results. Superficial nodes were found in all inguinal regions studied (mean = 13.60), and their distribution was more prominent in the superomedial quarter (mean = 3.94) and less in the inferolateral quarter (mean = 2.73). There was statistical significance between quarters when comparing the upper group with the lower one (P = 0.02). Nodes were widely distributed in the superficial region compared with deep lymph nodes (mean = 13.60 versus 1.71, P < 0.001). Conclusions. A great number of inguinal lymph nodes are distributed near the classical anatomical landmarks for inguinal lymphadenectomy, more prominent in upper quadrants

    Geometrically-controlled twist transitions in nematic cells

    We study geometrically-controlled twist transitions of a nematic confined between a sinusoidal grating and a flat substrate. In these cells the transition to the twisted state is driven by surface effects. We have identified the mechanisms responsible for the transition analytically and used exact numerical calculations to study the range of surface parameters where the twist instability occurs. Close to these values the cell operates under minimal external fields or temperature variations

    Green turtles highlight connectivity across a regional marine protected area network in west Africa

    Networks of marine protected areas (MPAs) are invaluable for the protection of species with high dispersal capacity, yet connectivity within networks is poorly understood. We demonstrate the connectivity within the regional MPA network in West Africa (RAMPAO), mediated by the largest green turtle population in the eastern Atlantic Ocean. We equipped with satellite tags 45 female green turtles nesting in the Bijagó s Archipelago, Guinea-Bissau, and tracked them during internesting, migration, and foraging to quantify the degree of coverage the RAMPAO network provides during each of these critical periods. During the internesting period, turtles were largely concentrated around the nesting islands, with a mean of 94.8% (SD 0.1%, range: 46% - 100%, n = 40 turtles) of tracking positions falling within MPA limits. Among the 35 turtles successfully tracked into the foraging period, we identified variable migratory strategies, with 12 turtles remaining near-resident at distances of 40-90 km from breeding sites, 10 turtles migrating 300-400 km to The Gambia and Senegal, and 13 turtles traveling >1000 km to northern Mauritania. Of the 35 foraging turtles, 26 used MPAs, with a mean of 78.0% (SD 34.8%, range: 3.7% - 100%) of their tracking positions falling within the limits of RAMPAO MPAs, across Guinea-Bissau, Senegal and Mauritania. Migration corridors with high concentrations of passing turtles were mostly located nearshore, and 21% of these high passage areas fell within the MPA network. Overall, we found that this population connects five RAMPAO MPAs, yet some foraging sites (e.g., in the Bijagó s) and important migration areas (e.g., Cap-Vert peninsula) described here are currently unprotected. These results are relevant to any considerations of MPA extension or establishment within the regional network, which would contribute towards meeting the Convention on Biological Diversity targets for national marine protected area estate coverage.     Annotated plant pathology databases for image-based detection and recognition of diseases.

    Over the last few years, considerable effort has been spent by Embrapa in the construction of a plant disease database representative enough for the development of effective methods for automatic plant disease detection and recognition. In October of 2016, this database, called PDDB, had 2326 images of 171 diseases and other disorders affecting 21 plant species. PDDB size, although considerable, is not enough to allow the use of powerful techniques such as deep learning. In order to increase its size, each image was subdivided according to certain criteria, increasing the number of images to 46,513. Both the original (PDDB) and subdivided (XDB)databases are now being made freely available for academic research purposes, thus supporting new studies and contributing to speed up the advances in the area.     Multi-factor service design: identification and consideration of multiple factors of the service in its design process

    Service design is a multidisciplinary area that helps innovate services by bringing new ideas to customers through a design-thinking approach. Services are affected by multiple factors, which should be considered in designing services. In this paper, we propose the multi-factor service design (MFSD) method, which helps consider the multi-factor nature of service in the service design process. The MFSD method has been developed through and used in five service design studies with industry and government. The method addresses the multi-factor nature of service for systematic service design by providing the following guidelines: (1) identify key factors that affect the customer value creation of the service in question (in short, value creation factors), (2) define the design space of the service based on the value creation factors, and (3) design services and represent them based on the factors. We provide real stories and examples from the five service design studies to illustrate the MFSD method and demonstrate its utility. This study will contribute to the design of modern complex services that are affected by varied factors

    A utilização de espĂ©cies vegetais, como Chenopodium ambrosioides L., para o tratamento da leishmaniose na terapĂȘutica tradicional tem despertado interesse na busca de novos compostos mais eficazes e menos tĂłxicos. Nosso grupo demonstrou as atividades imunoestimuladora e anti-Leishmania in vivo do extrato bruto hidroalcoĂłlico (EBH) de C. ambrosioides e efeito anti-promastigota in vitro do EBH e das suas fraçÔes. Neste trabalho, avaliou-se a atividade anti-Leishmania do EBH e suas fraçÔes acetato de etila (FAc) e clorofĂłrmica (FCHCl3) em macrĂłfagos infectados in vitro por Leishmania amazonensis. Foram realizados dois modelos: “proflĂĄtico” e “terapĂȘutico”. No primeiro, macrĂłfagos peritoneais de camundongos Swiss  foram tratados com EBH, FAc ou FCHCl3 nas concentraçÔes de 62,5”g/mL, 125”g/mL e 250”g/mL e, apĂłs 4 horas, infectados com formas promastigotas do parasito na razĂŁo de 1:10 por 24 horas. No segundo, os macrĂłfagos foram infectados com promastigotas (1:10) e, apĂłs 4 horas, tratados com EBH, FAc ou FCHCl3 por 24 horas. Foram entĂŁo realizados a quantifcação das amastigotas fagocitadas e o cĂĄlculo das taxas de infecção. No modelo “proflĂĄtico”, apenas os macrĂłfagos expostos ao EBH nas maiores concentraçÔes apresentaram  taxas de  infecção  inferiores ao controle negativo. Entretanto, no modelo “terapĂȘutico”, as  trĂȘs concentraçÔes de EBH e tambĂ©m da FAc reduziram a infecção de macrĂłfagos em relação ao controle negativo, sendo a maior concentração do EBH mais efetiva inclusive que o controle positivo.  Em conclusĂŁo, o EBH de folhas de C. ambrosioides e a sua FAc possuem efeito terapĂȘutico anti-Leishmania na infecção in vitro de macrĂłfagos.Descritores: Leishmaniose; Leishmania amazonensis; Chenopodium ambrosioides.AbstractInhibit  of  in  vitro  macrophage  infection  by  Leishmania  amazonensis  by  extract  and  fractions  from Chenopodium ambrosioides L. The use of plant species such as Chenopodium ambrosioides L. for the treatment of leishmaniasis in traditional medicine has aroused interest in fnding new, more effective and less toxic compounds. Our group demonstrated the immunostimulatory and in vivo anti-Leishmania activities of the crude hydroalcoholic extract (HCE) from C. ambrosioides L. and the in vitro anti-promastigote effect of the HCE and its fractions. In this study, we evaluated the anti-Leishmania activity of the HCE and its fractions ethyl acetate (FAc) and chloroform (FCHCl3) in macrophages infected in vitro with Leishmania amazonensis. Two models, “prophylactic” and “therapeutic”, were performed. In the frst, Swiss mice peritoneal macrophages were treated with CHE, FAc or FCHCl3 in concentrations of 62,5ÎŒg/mL, 125ÎŒg/mL and 250ÎŒg/mL and, after 4 hours, infected with promastigote forms in the ratio of 1:10 for 24 hours. In the second model, the macrophages were infected with promastigotes (1:10)  and,  after 4 hours,  treated with HCE, FAc or FCHCl3 for 24 hours. Quantifcation of phagocytosed amastigotes and calculation  of  infection  rates were  then  perfomed.  In  the  “prophylactic” model,  only macrophages  exposed  to  the  highest concentrations of HCE presented  infection  rates  lower  than  the negative  control. However,  in  the  “therapeutic” model,  the three concentrations of both the HCE and FAc reduced the infection of macrophages compared to the negative control, with the highest concentration of HCE being even more effective than the positive control. In conclusion, the HCE from leaves of Chenopodium ambrosioides and its FAc have an anti-Leishmania therapeutic effect on the in vitro macrophages infection.Descriptors: Leishmaniasis.     Estresse ocupacional e satisfação dos usuĂĄrios com os cuidados de saĂșde primĂĄrios em Portugal

    The Portuguese primary healthcare sector has suffered changes due to a reform on the lines of the conceptual framework referred to by some authors as "New Public Management." These changes may be generating higher levels of occupational stress with a negative impact at individual and organizational levels. This study examines the experience of stress in 305 health professionals (physicians, nurses and clinical secretaries) and satisfaction with the services provided by them from 392 users. The population under scrutiny is taken from 10 type A and 10 type B Family Health Units (FHU). The results show that 84.2% of professionals report moderate to high levels of occupational stress with the nurses being those with higher levels. Users reported good levels of satisfaction, especially with the nursing services. There were no differences in stress level between type A and type B FHU, though there were at the level of user satisfaction of type B FHU users who show higher levels of satisfaction.     Guarana (Paullinia cupana var. sorbilis) is a plant native to the central Amazon basin. Roasted seed extracts have been used as medicinal beverages since pre-Colombian times, due to their reputation as stimulants, aphrodisiacs, tonics, as well as protectors of the gastrointestinal tract. Guarana plants are commercially cultivated exclusively in Brazil to supply the national carbonated soft-drink industry and natural product stores around the world. In this report, we describe and discuss the annotation of 15,387 ESTs from guarana seeded-fruits, highlighting sequences from the flavonoid and purine alkaloid pathways, and those related to biotic stress avoidance. This is the largest set of sequences registered for the Sapindaceae family

    Complex speech-language therapy interventions for stroke-related aphasia: The RELEASE study incorporating a systematic review and individual participant data network meta-analysis

    Background: People with language problems following stroke (aphasia) benefit from speech and language therapy. Optimising speech and language therapy for aphasia recovery is a research priority. Objectives: The objectives were to explore patterns and predictors of language and communication recovery, optimum speech and language therapy intervention provision, and whether or not effectiveness varies by participant subgroup or language domain. Design: This research comprised a systematic review, a meta-analysis and a network meta-analysis of individual participant data. Setting: Participant data were collected in research and clinical settings. Interventions: The intervention under investigation was speech and language therapy for aphasia after stroke. Main outcome measures: The main outcome measures were absolute changes in language scores from baseline on overall language ability, auditory comprehension, spoken language, reading comprehension, writing and functional communication. Data sources and participants: Electronic databases were systematically searched, including MEDLINE, EMBASE, Cumulative Index to Nursing and Allied Health Literature, Linguistic and Language Behavior Abstracts and SpeechBITE (searched from inception to 2015). The results were screened for eligibility, and published and unpublished data sets (randomised controlled trials, non-randomised controlled trials, cohort studies, case series, registries) with at least 10 individual participant data reporting aphasia duration and severity were identified. Existing collaborators and primary researchers named in identified records were invited to contribute electronic data sets. Individual participant data in the public domain were extracted. Review methods: Data on demographics, speech and language therapy interventions, outcomes and quality criteria were independently extracted by two reviewers, or available as individual participant data data sets. Meta-analysis and network meta-analysis were used to generate hypotheses. Results: We retrieved 5928 individual participant data from 174 data sets across 28 countries, comprising 75 electronic (3940 individual participant data), 47 randomised controlled trial (1778 individual participant data) and 91 speech and language therapy intervention (2746 individual participant data) data sets. The median participant age was 63 years (interquartile range 53-72 years). We identified 53 unavailable, but potentially eligible, randomised controlled trials (46 of these appeared to include speech and language therapy). Relevant individual participant data were filtered into each analysis. Statistically significant predictors of recovery included age (functional communication, individual participant data: 532, n = 14 randomised controlled trials) and sex (overall language ability, individual participant data: 482, n = 11 randomised controlled trials; functional communication, individual participant data: 532, n = 14 randomised controlled trials). Older age and being a longer time since aphasia onset predicted poorer recovery. A negative relationship between baseline severity score and change from baseline (p < 0.0001) may reflect the reduced improvement possible from high baseline scores. The frequency, duration, intensity and dosage of speech and language therapy were variously associated with auditory comprehension, naming and functional communication recovery. There were insufficient data to examine spontaneous recovery. The greatest overall gains in language ability [14.95 points (95% confidence interval 8.7 to 21.2 points) on the Western Aphasia Battery-Aphasia Quotient] and functional communication [0.78 points (95% confidence interval 0.48 to 1.1 points) on the Aachen Aphasia Test-Spontaneous Communication] were associated with receiving speech and language therapy 4 to 5 days weekly; for auditory comprehension [5.86 points (95% confidence interval 1.6 to 10.0 points) on the Aachen Aphasia Test-Token Test], the greatest gains were associated with receiving speech and language therapy 3 to 4 days weekly. The greatest overall gains in language ability [15.9 points (95% confidence interval 8.0 to 23.6 points) on the Western Aphasia Battery-Aphasia Quotient] and functional communication [0.77 points (95% confidence interval 0.36 to 1.2 points) on the Aachen Aphasia Test-Spontaneous Communication] were associated with speech and language therapy participation from 2 to 4 (and more than 9) hours weekly, whereas the highest auditory comprehension gains [7.3 points (95% confidence interval 4.1 to 10.5 points) on the Aachen Aphasia Test-Token Test] were associated with speech and language therapy participation in excess of 9 hours weekly (with similar gains notes for 4 hours weekly). While clinically similar gains were made alongside different speech and language therapy intensities, the greatest overall gains in language ability [18.37 points (95% confidence interval 10.58 to 26.16 points) on the Western Aphasia Battery-Aphasia Quotient] and auditory comprehension [5.23 points (95% confidence interval 1.51 to 8.95 points) on the Aachen Aphasia Test-Token Test] were associated with 20-50 hours of speech and language therapy. Network meta-analyses on naming and the duration of speech and language therapy interventions across language outcomes were unstable. Relative variance was acceptable (< 30%). Subgroups may benefit from specific interventions. Limitations: Data sets were graded as being at a low risk of bias but were predominantly based on highly selected research participants, assessments and interventions, thereby limiting generalisability. Conclusions: Frequency, intensity and dosage were associated with language gains from baseline, but varied by domain and subgroup
