    A brief account on enzyme mining using metagenomic approach

    Metagenomics is an approach for directly analyzing the genomes of microbial communities in the environment. The use of metagenomics to investigate novel enzymes is critical because it allows researchers to acquire data on microbial diversity, with a 99% success rate, and different kinds of genes encode an enzyme that has yet to be found. Basic metagenomic approaches have been created and are widely used in numerous studies. To promote the success of the advance research, researchers, particularly young researchers, must have a fundamental understanding of metagenomics. As a result, this review was conducted to provide a thorough insight grasp of metagenomics. It also covers the application and fundamental methods of metagenomics in the discovery of novel enzymes, focusing on recent studies. Moreover, the significance of novel biocatalysts anticipated from varied microbial metagenomes and their relevance to future research for novel industrial applications, the ramifications of Next-Generation Sequencing (NGS), sophisticated bio-informatic techniques, and the prospects of the metagenomic approaches are discussed.     Cost Effectiveness Analysis Evaluating Real-Time Characterization of Diminutive Colorectal Polyp Histology using Narrow Band Imaging (NBI)

    Background: Endoscopists and new computer-aided programs can achieve performance benchmarks for real-time diagnosis of colorectal polyps using Narrow-Band Imaging (NBI), though do not perform as well as endoscopists with expertise in advanced imaging. Previous cost-effectiveness studies on optical diagnosis have focused on expert performance, potentially over-estimating its benefits. Aim: Determine cost-effectiveness of an NBI ‘characterize, resect and discard (CRD)’ strategy using updated assumptions based on non-expert performance. Methods: Markov model was constructed to compare cost-effectiveness of the CRD strategy, where diminutive polyps characterized as non-adenomas with high confidence are not resected and adenomas are resected and discarded, versus standard of care (SOC) in which all polyps are resected with histologic analysis. Rates related to NBI performance, missed polyps, polyp progression, malignancy, and complications, as well as quality-adjusted life years (QALYs) were derived from the literature. Costs were age and insurer-specific. Mean QALYs and costs were calculated using first order Monte Carlo simulation. Deterministic and probabilistic sensitivity analyses were conducted. Results: The mean QALY estimates were similar for the CRD (8.563, 95% CI: 8.557-8.571) and SOC strategy (8.563, 8.557-8.571), but costs were reduced (2,693.06vs.2,693.06 vs. 2,800.27, mean incremental cost savings: 107.21/person).Accountingforcolonoscopyrates,theCRDstrategywouldsave107.21/person). Accounting for colonoscopy rates, the CRD strategy would save 708 million to $1.06 billion annually. The model was sensitive to the incidence of tubular adenomas; the results were otherwise robust in all other one-way and probabilistic analyses. Conclusions: An NBI CRD strategy is cost-effective when compared to the SOC, even when employed by non-experts. The appreciated benefit is primarily due to cost savings of the CRD strategy

    Markers of Maternal and Infant Metabolism are Associated with Ventricular Dysfunction in Infants of Obese Women with Type 2 Diabetes

    BACKGROUND To test the hypothesis that infants born to obese women with pregestational type 2 diabetes mellitus (IBDM) have ventricular dysfunction at one month that is associated with markers of maternal lipid and glucose metabolism. METHODS In a prospective observational study of IBDM (OB+DM, n=25), echocardiography measures of septal, left (LV) and right ventricular (RV) function and structure were compared at one month of age to infants born to OB mothers without DM (OB, n=24), and non-OB without DM (Lean, n=23). Basal maternal lipid and glucose kinetics and maternal plasma and infant (cord) plasma were collected for hormone and cytokine analyses. RESULTS RV, LV, and septal strain measures were lower in the OB+DM infants vs. other groups, without evidence of septal hypertrophy. Maternal hepatic insulin sensitivity, maternal plasma free fatty acid concentration, and cord plasma insulin and leptin most strongly predicted decreased septal strain in the OB+DM infants. CONCLUSION IBDM’s have reduced septal function at one month in the absence of septal hypertrophy, which is associated with altered maternal and infant lipid and glucose metabolism. These findings suggest that maternal obesity and DM may have a prolonged impact on the cardiovascular health of their offspring, despite resolution of cardiac hypertrophy

    Virtual Gastroenterology Fellowship Recruitment During COVID-19 and Its Implications for the Future

These permissions are granted for the duration of the World Health Organization (WHO) declaration of COVID-19 as a global pandemic.Background and Aims Amid the COVID-19 pandemic, medical education organizations endorsed a virtual recruitment format, representing a stark change from traditional in-person interviews. We aimed to identify the attitudes and perceptions of Gastroenterology Fellowship Program Directors (PDs) and applicants regarding the virtual interview experience and the role of virtual interviews (VI) in the future. Methods We designed separate surveys targeting PDs and applicants using the Qualtrics software. At the end of the interview season, we e-mailed both survey links to all PDs and requested that they forward the applicant survey to their interviewed candidates. Surveys were voluntary and anonymous. Descriptive statistics were used to analyze the data with results presented as percentages. Results A total of 29.7% of PDs completed the survey. Compared to traditional interviews, VI were viewed by 46.5% of PDs to be very suboptimal or suboptimal. Yet, 69.1% envisioned a role for VI in the future. A total of 14.2% of applicants completed the survey. Compared to traditional interviews, VI were viewed by 42.3% of applicants to be very suboptimal or suboptimal. However, 61.8% saw a future role for VI. While both applicants and PDs reported that establishing an interpersonal connection was a disadvantage with VI, applicants placed more emphasis on this need for connection (p = 0.001). Conclusion Overall, PDs and applicants report mixed views with regard to VI but anticipate that it may continue to have a future role. VI may augment future recruitment cycles with care taken to not disadvantage applicants, who rely heavily on the interview process to create personal connections with programs

    Delphi Initiative for Early-Onset Colorectal Cancer (DIRECt) International Management Guidelines

    Background & aims: Patients with early-onset colorectal cancer (eoCRC) are managed according to guidelines that are not age-specific. A multidisciplinary international group (DIRECt), composed of 69 experts, was convened to develop the first evidence-based consensus recommendations for eoCRC. Methods: After reviewing the published literature, a Delphi methodology was used to draft and respond to clinically relevant questions. Each statement underwent 3 rounds of voting and reached a consensus level of agreement of ≄80%. Results: The DIRECt group produced 31 statements in 7 areas of interest: diagnosis, risk factors, genetics, pathology-oncology, endoscopy, therapy, and supportive care. There was strong consensus that all individuals younger than 50 should undergo CRC risk stratification and prompt symptom assessment. All newly diagnosed eoCRC patients should receive germline genetic testing, ideally before surgery. On the basis of current evidence, endoscopic, surgical, and oncologic treatment of eoCRC should not differ from later-onset CRC, except for individuals with pathogenic or likely pathogenic germline variants. The evidence on chemotherapy is not sufficient to recommend changes to established therapeutic protocols. Fertility preservation and sexual health are important to address in eoCRC survivors. The DIRECt group highlighted areas with knowledge gaps that should be prioritized in future research efforts, including age at first screening for the general population, use of fecal immunochemical tests, chemotherapy, endoscopic therapy, and post-treatment surveillance for eoCRC patients. Conclusions: The DIRECt group produced the first consensus recommendations on eoCRC. All statements should be considered together with the accompanying comments and literature reviews. We highlighted areas where research should be prioritized. These guidelines represent a useful tool for clinicians caring for patients with eoCRC

    Post-Operative Functional Outcomes in Early Age Onset Rectal Cancer

    Background: Impairment of bowel, urogenital and fertility-related function in patients treated for rectal cancer is common. While the rate of rectal cancer in the young (<50 years) is rising, there is little data on functional outcomes in this group. Methods: The REACCT international collaborative database was reviewed and data on eligible patients analysed. Inclusion criteria comprised patients with a histologically confirmed rectal cancer, <50 years of age at time of diagnosis and with documented follow-up including functional outcomes. Results: A total of 1428 (n=1428) patients met the eligibility criteria and were included in the final analysis. Metastatic disease was present at diagnosis in 13%. Of these, 40% received neoadjuvant therapy and 50% adjuvant chemotherapy. The incidence of post-operative major morbidity was 10%. A defunctioning stoma was placed for 621 patients (43%); 534 of these proceeded to elective restoration of bowel continuity. The median follow-up time was 42 months. Of this cohort, a total of 415 (29%) reported persistent impairment of functional outcomes, the most frequent of which was bowel dysfunction (16%), followed by bladder dysfunction (7%), sexual dysfunction (4.5%) and infertility (1%). Conclusion: A substantial proportion of patients with early-onset rectal cancer who undergo surgery report persistent impairment of functional status. Patients should be involved in the discussion regarding their treatment options and potential impact on quality of life. Functional outcomes should be routinely recorded as part of follow up alongside oncological parameters

    Bi-allelic Loss-of-Function CACNA1B Mutations in Progressive Epilepsy-Dyskinesia.

    The occurrence of non-epileptic hyperkinetic movements in the context of developmental epileptic encephalopathies is an increasingly recognized phenomenon. Identification of causative mutations provides an important insight into common pathogenic mechanisms that cause both seizures and abnormal motor control. We report bi-allelic loss-of-function CACNA1B variants in six children from three unrelated families whose affected members present with a complex and progressive neurological syndrome. All affected individuals presented with epileptic encephalopathy, severe neurodevelopmental delay (often with regression), and a hyperkinetic movement disorder. Additional neurological features included postnatal microcephaly and hypotonia. Five children died in childhood or adolescence (mean age of death: 9 years), mainly as a result of secondary respiratory complications. CACNA1B encodes the pore-forming subunit of the pre-synaptic neuronal voltage-gated calcium channel Cav2.2/N-type, crucial for SNARE-mediated neurotransmission, particularly in the early postnatal period. Bi-allelic loss-of-function variants in CACNA1B are predicted to cause disruption of Ca2+ influx, leading to impaired synaptic neurotransmission. The resultant effect on neuronal function is likely to be important in the development of involuntary movements and epilepsy.     A tutorial on the Bayesian statistical approach to inverse problems

    Inverse problems are ubiquitous in science and engineering. Two categories of inverse problems concerning a physical system are (1) estimate parameters in a model of the system from observed input–output pairs and (2) given a model of the system, reconstruct the input to it that caused some observed output. Applied inverse problems are challenging because a solution may (i) not exist, (ii) not be unique, or (iii) be sensitive to measurement noise contaminating the data. Bayesian statistical inversion (BSI) is an approach to tackle ill-posed and/or ill-conditioned inverse problems. Advantageously, BSI provides a “solution” that (i) quantifies uncertainty by assigning a probability to each possible value of the unknown parameter/input and (ii) incorporates prior information and beliefs about the parameter/input. Herein, we provide a tutorial of BSI for inverse problems by way of illustrative examples dealing with heat transfer from ambient air to a cold lime fruit. First, we use BSI to infer a parameter in a dynamic model of the lime temperature from measurements of the lime temperature over time. Second, we use BSI to reconstruct the initial condition of the lime from a measurement of its temperature later in time. We demonstrate the incorporation of prior information, visualize the posterior distributions of the parameter/initial condition, and show posterior samples of lime temperature trajectories from the model.     Effect of Solvent Composition on Solubility, Thermodynamics, Metastable Zone Width (MSZW) and Crystal Habit of L-Ascorbic Acid

    This work investigates the effect of different solvent systems on solubility, thermodynamics, metastable zone width (MSZW), and crystal habit of ascorbic acid, in order to help optimize its crystallization process. The solubility curves and metastable zone (MSZ) limits were determined experimentally using the polythermal method in pure solvents: water and alcohols (methanol/ethanol/isopropanol), as well as water-alcohol binary solvent systems. The solubility decreases with increasing alcohol composition for all solvent systems. The solubility data were well correlated using the Jouyban–Acree model as a function of two variables: temperature and solvent composition. The dissolution enthalpy (ΔHdiss), dissolution entropy (ΔSdiss), and Gibbs free energy (ΔGdiss) were determined using Van’t Hoff and Jouyban–Acree models. The thermodynamic properties increase with increasing alcohol composition. The lowest and highest values of enthalpy were obtained for water (20.52 kJ mol −1) and isopropanol (35.33 kJ mol−1), respectively. Pure alcohols as solvents widen the metastable zone width, indicating high supersaturation required for the nucleation. Crystal images captured during cooling crystallization in water confirm the cubic crystal habit formation, whereas increasing alcohol composition in the solvent system promotes preferential growth along one crystallographic axis, leading to elongated prism-shaped crystals in methanol and ethanol and needle-shaped crystals in isopropanol
