Mechanism For Copper(II)-Mediated Disaggregation Of A Porphyrin J-Aggregate
J-aggregates of anionic meso-tetrakis(4-sulfonatophenyl)porphyrin form at intermediate pH (2.3–3.1) in the presence of NiSO₄ or ZnSO₄ (ionic strength, I.S. = 3.2 M). These aggregates convert to monomeric porphyrin units via metallation with copper(II) ions. The kinetics for the disassembly process, as monitored by UV/vis spectroscopy, exhibits zeroth-order behavior. The observed zeroth-order rate constants show a two-term dependence on copper(II) ion concentrations: linear and second order. Also observed is an inverse dependence on hydrogen ion concentration. Activation parameters have been determined for the disassembly process leading to ΔH^≠ = (+163 ± 15) kJ·mol⁻¹ and ΔS^≠ = (+136 ± 11) J·K⁻¹. A mechanism is proposed in which copper(II) cation is in pre-equilibrium with a reactive site at the rim of the J-aggregate. An intermediate copper species is thus formed that eventually leads to the final metallated porphyrin either through an assisted attack of a second metal ion or through a direct insertion of the metal cation into the macrocycle core
Krooninen väsymysoireyhtymä : Etiologia, diagnostiikka, hoito sekä kuntoutusinterventiot
Tämän kroonisen väsymysoireyhtymän (KVO) diagnostiikkaa, etiologiaa ja hoitokeinojen vaikuttavuutta koskevan selvityksen menetelminä ovat systemaattinen kirjallisuuskatsaus ja kysely. Aiheen laajuuden vuoksi liikkeelle lähdettiin julkaistuista systemaattisista kirjallisuuskatsauksista, ja näistä saatavaa tietoa täydennettiin uusilla alkuperäistutkimuksilla. Katsausten ja hoitotutkimusten laatu arvioitiin ja näytönaste arvioitiin lopputulosmuuttujittain. Suomen hoitokäytännöistä haettiin lisätietoa KVO-potilaille sekä heitä hoitaville lääkäreille suunnatulla kyselyllä. Työ on tehty elokuun 2015 ja marraskuun 2016 välisenä aikana. Kroonisen väsymysoireyhtymän riskitekijöitä ovat traumaattiset kokemukset, aiempi masennus, yliaktiivinen elämäntapa ja täydellisyyden tavoittelu mutta toisaalta myös passiivisuus tai liikunnan välttely. Immuunijärjestelmä, neuroendokriininen järjestelmä ja autonominen hermosto ovat osallisia oireyhtymän patofysiologiassa. Pitkäkestoisella stressillä näyttää olevan tärkeä välittävä rooli. Kroonisen väsymysoireyhtymän diagnostiikassa oleellista on muiden väsymystä aiheuttavien, mahdollisesti henkeä uhkaavien, sairauksien poissulku. Erotusdiagnostiikkaa tehdään myös muiden väsymyksenä ilmenevien oireyhtymien tunnistamiseksi. Taudinmääritykseen on käytettävissä useita erilaisia kansainvälisiä diagnostisia kriteeristöjä. Kansallisia hoitosuosituksia tarvittaisiin yhdenmukaistamaan KVO-potilaiden nykyisellään kirjavaa diagnostiikkaa ja hoitoa Suomessa. Porrastettu fyysinen harjoittelu ja kognitiivis-behavioraalinen terapia ovat pitkään olleet ainoita hoitoja, joiden vaikuttavuudesta on kohtalaista näyttöä. Käytössä on myös lääkkeitä ja terapioita, joita on tutkittu vain hyvin vähän tai ei ollenkaan. Rintatolimodi ja rituksimabi ovat laskimonsisäisesti annosteltavia lääkeaineita, joilla näyttö KVO-potilaiden fyysisen toimintakyvyn parantajina on heikko. A search strategy for occupational health intervention studies
Background: As a result of low numbers and diversity in study type, occupational health intervention studies are not easy to locate in electronic literature databases. Aim: To develop a search strategy that facilitates finding occupational health intervention studies in Medline, both for researchers and practitioners. Methods: A gold standard of articles was created by going through two whole volumes of 19 biomedical journals, both occupational health specialty and non-occupational health journals. Criteria for occupational health intervention studies were: evaluating an intervention with an occupational health outcome and a study design with a control group. Each journal was searched independently by two of the authors. Search terms were developed by asking specialists and counting word frequencies in gold standard articles. Results: Out of 11 022 articles published we found 149 occupational health intervention studies. The most sensitive single terms were work*[tw] (sensitivity 71%, specificity 88%) and effect*[tw] (sensitivity 75%, specificity 63%). The most sensitive string was (effect*[tw] OR control*[tw] OR evaluation*[tw] OR program*[tw]) AND (work*[tw] OR occupation*[tw] OR prevention*[tw] OR protect*[tw]) (sensitivity 89%, specificity 78%). The most specific single terms were "occupational health"[tw] (sensitivity 22%, specificity 98%) and effectiveness[tw] (sensitivity 22%, specificity 98%). The most specific string was (program[tw] OR "prevention and control"[sh]) AND (occupational[tw] OR worker*[tw]) (sensitivity 47%, specificity 98%). Conclusion: No single search terms are available that can locate occupational health intervention studies sufficiently. The authors' search strings have acceptable sensitivity and specificity to be used by researchers and practitioners respectively. Redefinition and elaboration of keywords in Medline could greatly facilitate the location of occupational health intervention studies
Miten tunnistaa varhaiskuntoutuksen tarve työelämässä? : Kirjallisuuskatsaus työkyvyn heikkenemisen ennusmerkeistä ja varhaisen tunnistamisen työkaluista
Ammatillisesti syvennetyn lääketieteellisen varhaiskuntoutuksen vaikuttavuutta on kyseenalaistettu ja keskeinen kritiikki liittyy kuntoutuksen kohdentumiseen. Tässä katsauksessa haetaan ja pohditaan niitä riski- ja ennustekijöitä, joiden avulla kohdennusta voisi kehittää. Raportti on osa Kelan työhönkuntoutuksen laajempaa kehitystyötä. Työkyvyn heikkenemistä ennakoivat korkeampi ikä, naissukupuoli, vähäinen koulutus ja pienet tulot, sairaudet ja oireilu sekä tunnetut elämäntapariskit. Työn riskitekijöistä esiin nousivat muun muassa vuorotyö, ruumiillinen työ, osa-aikaisuus ja aikaisemmat työttömyysjaksot. Puuttuvat työn hallinnan ja oikeudenmukaisuuden kokemukset sekä aikaisemmat sairauspoissaolot ennakoivat nekin työkyvyttömyyttä, samoin kuormittavat elämäntilanteet, tyytymättömyys ja vihamielisyys. Itä- ja Pohjois-Suomi ovat maantieteellisesti suuren riskin alueita. Kuntoutusinterventioista näyttivät parhaiten hyötyvän henkilöt, joilla oli joku perussairaus ja mielialaoireita ja jotka tiedostivat terveysongelmansa. Ikä, sukupuoli tai elintavat eivät olleet määrääviä tekijöitä. Kuntoutusinterventioihin osallistuivat useammin naiset ja tupakoimattomat sekä ne, joilla oli vähemmän osallistumisen esteitä. Katsauksessa esitellään lyhyesti työkyvyn heikkenemisen riskin tunnistamiseen käytettyjä kyselyjä tai testejä. Työn fyysisen kuormittavuuden sekä käyttäytymisen ja persoonallisuustekijöiden mittareita on selvästi vähemmän kuin työn psykososiaalisen kuormittavuuden, työtyytyväisyyden sekä kestävyys- ja lihaskunnon mittareita. Raportin lopussa pohditaan, tulisiko ammatillisen varhaiskuntoutuksen osallistujavalinnassa priorisoida riskialueita ja -toimialoja tai työttömiä ja epätyypillisen työn tekijöitä. Mahdolliset kuntoutujat ryhmitellään riskitason, motivaation ja kuntoutuksen esteiden perusteella. Ihannekuntoutujien lisäksi tunnistetaan muita tärkeitä ryhmiä, joiden saaminen kuntoutukseen edellyttäisi henkilökohtaista tukea tai perusteellisia muutoksia kuntoutuksen valinnassa
Recurrence relation for relativistic atomic matrix elements
Recurrence formulae for arbitrary hydrogenic radial matrix elements are
obtained in the Dirac form of relativistic quantum mechanics. Our approach is
inspired on the relativistic extension of the second hypervirial method that
has been succesfully employed to deduce an analogous relationship in non
relativistic quantum mechanics. We obtain first the relativistic extension of
the second hypervirial and then the relativistic recurrence relation.
Furthermore, we use such relation to deduce relativistic versions of the
Pasternack-Sternheimer rule and of the virial theorem.Comment: 10 pages, no figure
Relativistically extended Blanchard recurrence relation for hydrogenic matrix elements
General recurrence relations for arbitrary non-diagonal, radial hydrogenic
matrix elements are derived in Dirac relativistic quantum mechanics. Our
approach is based on a generalization of the second hypervirial method
previously employed in the non-relativistic Schr\"odinger case. A relativistic
version of the Pasternack-Sternheimer relation is thence obtained in the
diagonal (i.e. total angular momentum and parity the same) case, from such
relation an expression for the relativistic virial theorem is deduced. To
contribute to the utility of the relations, explicit expressions for the radial
matrix elements of functions of the form and

IMMUNOPATHOLOGICAL STUDIES OF ORTHOTOPIC HUMAN LIVER ALLOGRAFTS
Twenty-six specimens obtained from twenty human orthotopic liver allografts 10-968 days after transplantation were studied by light microscopy, electron microscopy, and immunofluorescence. The main lesions consisted of mononuclear-cell infiltration around the portal tracts, centrilobular cholestasis, liver-cell atrophy and reticulin collapse, obliterative intimal thickening of hepatic arteries, and fibrosis. Moderate amounts of IgG and/or IgM and complement (β1C/β1A globulin or C'lq) were observed in four of the liver samples and smaller deposits were present in another five. A further three specimens contained IgG without complement. IgA was detected in only one of the samples. The immunoglobulins were found in the walls of the portal and central veins and of the sinusoids in all thirteen positive liver samples, in the walls of branches of the hepatic artery in three, and in the cytoplasm of some of the mononuclear cells infiltrating the portal tracts in nine of the specimens. Fibrinogen was seen in eight of the samples, usually in the spaces of Disse. Accumulations of immunoglobulins and complement were less frequent in liver than in kidney and heart allografts. These findings suggest that in the failure of human liver allografts cell-mediated immunity and non-immunological factors may be more important than humoral antibody. © 1972
The HTA Core Model: A novel method for producing and reporting health technology assessments
Objectives: The aim of this study was to develop and test a generic framework to enable international collaboration for producing and sharing results of health technology assessments (HTAs). Methods: Ten international teams constructed the HTA Core Model, dividing information contained in a comprehensive HTA into standardized pieces, the assessment elements. Each element contains a generic issue that is translated into practical research questions while performing an assessment. Elements were described in detail in element cards. Two pilot assessments, designated as Core HTAs were also produced. The Model and Core HTAs were both validated. Guidance on the use of the HTA Core Model was compiled into a Handbook. Results: The HTA Core Model considers health technologies through nine domains. Two applications of the Model were developed, one for medical and surgical interventions and another for diagnostic technologies. Two Core HTAs were produced in parallel with developing the model, providing the first real-life testing of the Model and input for further development. The results of formal validation and public feedback were primarily positive. Development needs were also identified and considered. An online Handbook is available. Conclusions: The HTA Core Model is a novel approach to HTA. It enables effective international production and sharing of HTA results in a structured format. The face validity of the Model was confirmed during the project, but further testing and refining are needed to ensure optimal usefulness and user-friendliness.
