Does virtual consultation between primary and specialised care improve healthcare quality? A scoping review of healthcare quality domains assessment

Virtual consultation has been proposed as a promising tool to improve the coordination and quality of healthcare between primary and specialised care. However, despite its potential facilitators, the evidence on the usefulness of virtual consultation for improving healthcare quality domains is fragmented and unclear. This scoping review aims to assess the impact of virtual consultation on different healthcare quality domains.Material and methodsWe conducted a scoping review with a rigorous search strategy on PubMed, EMBASE and Cochrane Library databases. The inclusion criteria were original articles, reviews, meta-analyses or letters to the editor, published between 1 January 2017 and 24 June 2022, and available in English, Spanish or French. For each of the articles selected, we identified the addressed healthcare quality domains, their facilitators and barriers, areas of improvement and data gaps. We have adhered to Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Review reporting standards.Results1284 manuscripts were retrieved. Finally, 235 papers were included in this review, most of which were original, descriptive studies. The most evaluated quality domain was effectiveness (223 articles). Safety and patient-centred care were the least evaluated. Simultaneous assessment of more than one domain was observed in 117 papers, being effectiveness and timeliness the most frequent combination. Our analysis revealed that virtual consultation is in development and underused. This tool has the potential to improve access to specialised care and enhance coordination between professionals.ConclusionsVirtual consultation has the potential to provide effective, efficient, equitable and timely attention. However, its contribution to safety and patient-centered care needs further evaluation. Our review emphasises the need for more rigorous research and standardised quality assessment criteria to obtain robust evidence on the usefulness of virtual consultation for improving healthcare quality domains

Triangulations, subdivisions, and secondary polytope

RESUMEN: En este trabajo se pretende estudiar las triangulaciones y subdivisiones desde su punto de vista combinatorio, es decir, trabajar con matroides orientadas de politopos y ver la relación con triangulaciones en general y con las llamadas triangulaciones regulares en particular. El concepto de triangulación regular motiva el estudio del abanico secundario, que es el espacio de configuración de todas las subdivisiones regulares y permite entender su estructura. Es de destacar también el Teorema de Gelfand, Kapranov y Zelevinsky, que demuestra la relación entre el abanico secundario de un politopo y el abanico normal de su politopo secundario. En el último capítulo de la memoria se introducen los zonotopos y sus subdivisiones zonotopales, describiendo la construcción de un abanico secundario y un zonotopo secundario con propiedades muy parecidas a las del caso de una configuración de puntos.ABSTRACT: In this memory we intend to study triangulations and subdivisions from their combinatorial point of view, that is to say, to work with oriented matroids of polytopes and the relation with triangulations in general and with the so-called regular triangulations in particular. The concept of regular triangulation motivates the study of the secondary fan, which is the configuration space of all regular subdivisions and allows us to understand their structure. It is also worth mentioning the Gelfand, Kapranov, and Zelevinsky Theorem, which proves the relation between the secondary fan of a polytope and the normal fan of its secondary polytope. The last chapter of the memory introduces zonotopes and their zonotopal subdivisions, describing the construction of a secondary fan and a secondary zonotope with very similar properties to those of a point configuration.Grado en Matemática

Treinta años de Alma-Ata: Interpretaciones y consecuencias para el bienestar psicosocial y la salud comunitaria

En este artículo se discute la experiencia de treinta años, desde que se presentó, en 1978, en la Conferencia Internacional de Alma-Ata, la nueva estrategia de la OMS de Atención Primaria de Salud. En esta, la OMS apostaba por un sistema integral basado en la equidad y en la participación comunitaria. Esta estrategia fue tachada de utópica, desarrollándose distintas interpretaciones de un mismo concepto, entre ellas, la Atención Primaria de Salud (APS) y la Atención Primaria Selectiva de Salud (APSS). Ambos enfoques de la Atención Primaria han funcionado paralelamente pero de forma irreconciliable. Sin embargo, se sostiene que la efectividad del modelo APS ha quedado demostrada en la experiencia de su aplicación en varios países, donde se ha reducido la mortandad infantil y promovido la prevención

Rol del psicólogo en cuidados paliativos y aspectos emocionales que inciden en su labor: un estudio de caso

This research, called the role of the psychologist in palliative care and emotional aspects that affect their work: a case study, sought to examine from a humanist perspective, the relationship between the psychologist who works in palliative care, the patient and his family, and from there outline the emotions that are implicit in the professional when intervening in the palliative field. Method: Cross-sectional study, with a qualitative approach, of descriptive scope, in which the in-depth interview applied to a health professional specialized in the field of palliative care was used as an instrument for data collection. General objective: to describe the role of the psychologist in palliative care and the emotional aspects that affect his work. Specific objectives: Explain the role of the psychologist in the field of palliative care, then investigate the relationship psychologist-patient and family, finally analyze the incidence of emotional aspects that mediate the work of the psychologist. Results: The most significant results show the functions performed by the psychologist in the palliative field in the accompaniment of the patient and the family in a chronic, terminal and degenerative condition and the emotions that arise in it when intervening. Discussion: Highlight the sensitivity Vs. Objectivity that transcend in the palliative field, recognize which are the clinico-aesthetic resources with which the psychologist works and how they mediate in supplying the needs of the patient and the family. Conclusions: The most relevant aspects of the study were: understanding those emotional aspects that are mobilized when intervening and identifying the work overload of the professional and the various activities that he does to deal with the emotions that allow him to reconnect with his work.La presente investigación denominada, el rol del psicólogo en cuidados paliativos y aspectos emocionales que inciden en su labor: un estudio de caso pretendió examinar desde una perspectiva humanista, la relación existente entre el psicólogo que labora en cuidados paliativos, el paciente y su familia, y de allí esbozar las emociones que se ven implícitas en el profesional al intervenir en el ámbito paliativo. Método: Estudio transversal, con enfoque cualitativo, de alcance descriptivo, en el que se empleó como instrumento para la recolección de datos la entrevista a profundidad aplicada a un profesional de la salud especialista en el ámbito de cuidados paliativos. Objetivo general: describir el rol del psicólogo en cuidados paliativos y los aspectos emocionales que inciden en su labor. Objetivos específicos: Identificar el rol que tiene el psicólogo en el ámbito de cuidados paliativos, posteriormente indagar la relación psicólogo-paciente y familia, finalmente analizar la incidencia de los aspectos emocionales que median en la labor del psicólogo. Resultados: Los resultados más significativos muestran las funciones que ejerce la psicóloga en el ámbito paliativo dentro del acompañamiento al paciente y la familia en condición crónica, terminal y degenerativa y las emociones que suscitan en ella al momento de intervenir. Discusión: Resaltar la sensibilidad Vs. Objetividad que trascienden en el ámbito paliativo, reconocer cuales son los recursos clínicos con los cuales trabaja la psicóloga y como estos median en suplir las necesidades del paciente y la familia. Conclusiones: Comprender el rol y las funciones que desempeña un psicólogo en esta ocupación: entender los aspectos emocionales tales como; el miedo, la sorpresa, aversión, ira, la alegría y tristeza que se movilizan al intervenir e identificar la sobrecarga laboral del profesional y las diversas actividades que hace para lidiar con las emociones que le permiten reconectarse frente a su trabajo

Estrés de minoría y bienestar psicológico en mujeres peruanas pertenecientes a minorías sexuales

La presente investigación busca describir, a través del modelo de estrés de minorías de Meyer (2003), las consecuencias relativas al bienestar psicológico en mujeres pertenecientes a minorías sexuales peruanas. Se distingue en este modelo entre estresores distales (discriminación, violencia) y proximales (ocultación de la orientación sexual, homofobia internalizada, expectativas de rechazo), los cuales tendrían efectos específicos sobre la salud mental al añadir factores de estrés a la vida cotidiana. Se utilizan la Escala de Bienestar Psicológico (Ryff, 1989) y la Sexual Minority Stress Scale (Iniewicz et al., 2017), siendo esta última una exploración de as experiencias de estrés de minoría adaptada a la población de la presente investigación. Como resultado general se obtuvo que, a mayor presencia de componentes del estrés de minorías, se reportaron índices menores de bienestar psicológico. Asimismo, se encontraron diferencias significativas en estas medidas según el grupo etario, situación de trabajo actual, actividad sexual, religión y pertenencia a colectivos LGB. Finalmente, se recomienda a la comunidad profesional y académica peruana la necesidad de investigar con enfoque clínico las influencias de estos estresores sobre el bienestar psicológico de las minorías sexuales, en específico sobre la población femenin

Oportunidades para la transformación digital de la cadena de suministro del sector bananero basado en software con inteligencia artificial

Artificial intelligence offers great opportunities for the supply chain, making it a competitive advantage for today's changing market. This paper aims to identify the impacts and opportunities that artificial intelligence software can offer to supply chain in the Colombian banana sector to facilitate the operation and improve the performance. The searching method consists of six steps getting 72 investigations finally. The sources of information were four databases. The main conclusion is the supply chain of the banana sector has everything for implementation of solutions based on intelligent software in order to achieve adaptation, flexibility and context awarenes and execution domain.La inteligencia artificial ofrece grandes oportunidades para la cadena de suministro, siendo esto una ventaja competitiva para el mercado cambiante de hoy en día. Este artículo tiene como objetivo identificar los impactos y oportunidades que puede ofrecer el software con inteligencia artificial para facilitar la operación y mejorar el desempeño de la cadena de suministro en el sector bananero de Colombia. La metodología de trabajo consta de seis pasos en donde se obtuvo un total de 72 investigaciones. Las fuentes de información fueron cuatro bases de datos. Diseño de un blog para promover el rendimiento académico en tiempos de pandemia Covid-19 en los estudiantes del grado 4° C de básica primaria en el área de Ciencias Sociales en la Institución Educativa Los Palmitos Sucre.

El diseño de este proyecto de investigación está fundamentado en las carencias e inconvenientes que han venido presentando los estudiantes del grado 4° C de la Institución Educativa Los Palmitos Sucre en el área de Ciencias Sociales, es muy evidente la apatía y la desmotivación hacia el aprendizaje de ésta asignatura básica y significativa, que sumada a la actual situación mundial Covid-19, conllevó a pasos agigantados a que muchos de ellos perdieran la perspectiva y el interés por su estudio, éstos son estudiantes que oscilan entre los nueve y once años de edad. El uso de las TIC en la enseñanza y en el aprendizaje, añadido a la implementación de un blog se consideran en la actualidad herramientas esenciales y muy relevantes, dado que están asociadas al estilo que hoy por hoy manejan las nuevas generaciones; por consiguiente facilitan los procesos académicos y les permiten ser más sugestivos, enriquecedores e innovadores no sólo para los estudiantes, sino para toda la comunidad educativa en general, la cual espera el mejor desempeño académico durante el desarrollo de las clases. Por otra parte y no menos importante, se cree que con la implementación y el uso de los blogs en otros grados, permitirá que muchos estudiantes fortalezcan su desempeño académico y desarrollen su facilidad para enfrentarse a una sociedad cambiante y exigente.The design of this research project is based on the shortcomings and inconveniences that students in grade 4 of the Los Palmitos Sucre Educational Institution have been presenting in the area of Social Sciences, apathy and lack of motivation towards learning is very evident of this basic and significant subject, which added to the current Covid-19 world situation, led to leaps and bounds that many of them lost perspective and interest in their study, these are students ranging between nine and 11 years of age . The use of ICT in the teaching-learning process and the implementation of a blog are currently considered essential and very relevant tools, since they are associated with the style that the new generations handle today; consequently, they facilitate academic processes and allow them to be more suggestive, enriching and innovative not only for students, but for the entire educational community in general, which expects the best academic performance during the development of classes. On the other hand and not least, it is believed that with the implementation and use of blogs in other grades, it will allow many students to strengthen their academic performance and develop their ability to face a changing and demanding society
