Extracorporeal Membrane Oxygenation for Severe Acute Respiratory Distress Syndrome associated with COVID-19: An Emulated Target Trial Analysis.
RATIONALE: Whether COVID patients may benefit from extracorporeal membrane oxygenation (ECMO) compared with conventional invasive mechanical ventilation (IMV) remains unknown. OBJECTIVES: To estimate the effect of ECMO on 90-Day mortality vs IMV only Methods: Among 4,244 critically ill adult patients with COVID-19 included in a multicenter cohort study, we emulated a target trial comparing the treatment strategies of initiating ECMO vs. no ECMO within 7 days of IMV in patients with severe acute respiratory distress syndrome (PaO2/FiO2 <80 or PaCO2 â„60 mmHg). We controlled for confounding using a multivariable Cox model based on predefined variables. MAIN RESULTS: 1,235 patients met the full eligibility criteria for the emulated trial, among whom 164 patients initiated ECMO. The ECMO strategy had a higher survival probability at Day-7 from the onset of eligibility criteria (87% vs 83%, risk difference: 4%, 95% CI 0;9%) which decreased during follow-up (survival at Day-90: 63% vs 65%, risk difference: -2%, 95% CI -10;5%). However, ECMO was associated with higher survival when performed in high-volume ECMO centers or in regions where a specific ECMO network organization was set up to handle high demand, and when initiated within the first 4 days of MV and in profoundly hypoxemic patients. CONCLUSIONS: In an emulated trial based on a nationwide COVID-19 cohort, we found differential survival over time of an ECMO compared with a no-ECMO strategy. However, ECMO was consistently associated with better outcomes when performed in high-volume centers and in regions with ECMO capacities specifically organized to handle high demand. Une Ă©tude du genre Erythroxylum P.Browne dans lâarchipel des Comores rĂ©vĂšle deux nouvelles espĂšces, Erythroxylum choungiense E.Bidault, Traclet & M.Pignal, sp. nov. de Mayotte et E. labatii E.Bidault & M.Pignal, sp. nov. de MohĂ©li et Grande Comore, qui sont dĂ©crites et illustrĂ©es. En consĂ©quence, la distribution de Erythroxylum elegans Baill. est mise Ă jour, cette espĂšce Ă©tant dĂ©sormais uniquement connue avec certitude de Mayotte et dâAnjouan, et possiblement de Grande Comore. Les caractĂšres morphologiques permettant de distinguer les deux nouvelles espĂšces sont discutĂ©s, et des statuts de conservation selon les catĂ©gories et critĂšres de la Liste Rouge de lâUICN sont proposĂ©s. Une clĂ© de dĂ©termination des espĂšces du genre Erythroxylum Ă Mayotte et aux Comores est proposĂ©e, en français et en anglais. Erythroxylum choungiense sp. nov. est endĂ©mique du Mont Choungi, et est Ă©valuĂ©e comme En Danger Critique. Le Mont Choungi prĂ©sente un fourrĂ© hygrophile submontagnard, un type de vĂ©gĂ©tation unique Ă Mayotte, dont au moins cinq espĂšces sont dĂ©sormais connues pour y ĂȘtre endĂ©miques : Cassipourea ovata Tul., Eugenia choungiensis Byng & N.Snow, Psiadia pascalii Labat & Beentje, Syzygium labatii Byng & N.Snow, et Erythroxylum choungiense, sp. nov. Erythroxylum labatii sp. nov. est uniquement connue des forĂȘts submontagnardes de crĂȘtes et de pentes fortes, entre 600 et 1300 m dâaltitude, et est Ă©valuĂ©e comme En Danger.
Dataset GBIFA study of the genus Erythroxylum P.Browne from the Comoros archipelago reveals two new species, E. choungiense E.Bidault, Traclet & M.Pignal, sp. nov. from Mayotte and E. labatii E.Bidault & M.Pignal, sp. nov. from Mwali and Ngazidja, both of which are described and illustrated. The distribution of a third species, E. elegans Baill., is now only known with certainty to include Mayotte and Nzwani, and possibly Ngazidja, following the exclusion of material now assigned to the new species. A risk of extinction assessment using the IUCN Red List criteria is provided for the new taxa and the morphological features that distinguish them from their apparently close relative are summarized. A key to the species of Erythroxylum in the Comoros is provided in both English and French. Erythroxylum choungiense sp. nov. is endemic to Mount Choungi, and its conservation status is assessed as Critically Endangered. Mount Choungi harbors the unique patch of hygrophilous submontane shrubland in Mayotte, to which at least five species are now known to be strictly endemic: Cassipourea ovata Tul., Eugenia choungiensis Byng & N.Snow, Psiadia pascalii Labat & Beentje, Syzygium labatii Byng & N.Snow, and Erythroxylum choungiense sp. nov.. Erythroxylum labatii sp. nov. is only known from the submontane forests on ridges and steep slopes on Moheli and Ngazidja, at an elevation of 600 to 1300 m a.s.l., and is assessed as Endangered.

Dans cet article, nous dĂ©crivons Mollinedia arianeae LĂrio & M.Pignal, sp. nov., une nouvelle espĂšce microendĂ©mique du Parc national dâItatiaia, dans lâĂ©tat de Rio de Janeiro, au BrĂ©sil. La nouvelle espĂšce est trĂšs similaire Ă M. jorgearum Peixoto et M. leucantha Molz & D.Silveira et se diffĂ©rencie dâelles principalement par le port, la forme et la couleur des feuilles une fois sĂ©chĂ©es, le type et lâindument de lâinflorescence, la forme du rĂ©ceptacle et le contour des tĂ©pales des fleurs staminĂ©es. Nous fournissons une description morphologique, une comparaison avec les espĂšces apparentĂ©es, la distribution gĂ©ographique, des commentaires sur lâĂ©cologie et une Ă©valuation prĂ©liminaire du risque dâextinction. Au-delĂ du lectorat acadĂ©mique, nous pensons quâil est indispensable de promouvoir la science, la technologie et lâĂ©ducation au BrĂ©sil. Nous proposons donc aussi un rĂ©sumĂ© en portugais destinĂ© aux botanistes amateurs et aux Ă©tudiants qui pourraient ĂȘtre intĂ©ressĂ©s par ce travail.In this paper we describe Mollinedia arianeae LĂrio & M.Pignal, sp. nov., a new microendemic species from the Itatiaia National Park, at the Rio de Janeiro state, Brazil. The new species is most similar to M. jorgearum Peixoto and M. leucantha Molz & D.Silveira, being differentiated of them mainly by the habit, shape and colour of leaves when dried, type and indumentum of inflorescence, shape of the receptacle and margin of tepals shape in staminate flowers. We provide a morphological description, a comparison with related species, geographic distribution, comments on ecology and a preliminary extinction risk assessment. Beyond the academic readership, we believe that it is essential to promote science, technology and education in Brazil. We therefore also propose a summary in Portuguese intended for amateur botanists and students who may be interested in this work.</p
Morphological and anatomical investigation of New Caledonian graminoid Dendrobium (Orchidaceae) with the description of two new species
International audienceMorphological characters support the description of two new species of Orchidaceae from New Caledonia: Dendrobium butinii M.Pignal & Munzinger sp. nov. and Dendrobium letocartiorum Munzinger & M.Pignal sp. nov. The lectotypes of Dendrobium camaridiorum Rchb.f. and D. crassifolium Schltr. are designated here. A leaf anatomical study was conducted and provided useful characters to distinguish the five New Caledonian graminoid Dendrobiineae species. Two identification tools are provided: an anatomical key for sterile material of grass-like Dendrobiineae and a morphological key for the New Caledonian graminoid Dendrobium. Une rĂ©vision taxonomique du genre Ivodea Capuron est prĂ©sentĂ©e. Trente espĂšces sont reconnues dont 27 endĂ©miques de Madagascar, deux endĂ©miques des Comores et une commune aux deux archipels. Une clĂ© dâidentification des espĂšces est prĂ©sentĂ©e en français et en anglais, avec des cartes de distribution, une Ă©valuation prĂ©liminaire du statut de conservation de chaque espĂšce et des illustrations des espĂšces nouvellement dĂ©crites.A taxonomic revision of genus Ivodea Capuron is presented in which 30 species are recognized, 27 endemic to Madagascar, 2 restricted to the Comoro Islands, and 1 occurring in both archipelagos. Deux nouvelles espĂšces endĂ©miques de Nouvelle-CalĂ©donie proches de D. finetianum Schltr. sont dĂ©crites et illustrĂ©es. VĂ©gĂ©tativement trĂšs proches, elles forment un «âgroupe D. finetianumâ». Les diffĂ©rences entre les trois espĂšces concernent la taille et la couleur des fleurs, et surtout la forme du labelle. Le nom D. kanakorum KrĂ€nzl est placĂ© en synonymie avec D. finetianum, pour lequel un lectotype est dĂ©signĂ©. Une nouvelle espĂšce, Garcinia urceolata Munzinger, Bruy & M.Pignal, sp. nov. (Clusiaceae), est dĂ©crite pour la Nouvelle-CalĂ©donie. Elle est restreinte Ă la Province Nord sur lâĂźle principale de la Nouvelle-CalĂ©donie (Grande-Terre), connue de la vallĂ©e de la Tchamba au sud de la province Ă MandjĂ©lia au nord, en forĂȘt dense humide sur des substrats non ultramafiques. Ce petit arbre avait Ă©tĂ© rĂ©coltĂ© il y a plus de cinquante ans, mais il avait Ă©tĂ© confondu avec G. virgata Vieill. ex Guillaumin car les deux espĂšces ont de petites feuilles. Garcinia urceolata, sp. nov. se distingue de G. virgata par un certain nombre de caractĂ©ristiques des feuilles, des fleurs et des fruits. Il pousse Ă©galement plus en altitude et dans des conditions plus humides. Les deux espĂšces sont supposĂ©es avoir des fruits dĂ©hiscents, comme plusieurs espĂšces de la Nouvelle-CalĂ©donie, un caractĂšre distinctif qui a Ă©tĂ© utilisĂ© il y a longtemps pour justifier la description du genre Septogarcinia Kosterm., mais que nous nâavons pas retenu pour distinguer ce genre de Garcinia L. Des dessins au trait et des photos couleur sont fournis pour la nouvelle espĂšce et pour G. virgata Ă titre de comparaison, ainsi quâune Ă©valuation prĂ©liminaire du risque dâextinction, qui indique que Garcinia urceolata, sp. nov. est VulnĂ©rable (VU), tandis que G. virgata a pour statut Near Threatened (NT), «âQuasi menacĂ©â».A new species, Garcinia urceolata Munzinger, Bruy & M.Pignal, sp. nov. (Clusiaceae), is described from New Caledonia. It is restricted to North Province, from the Tchamba valley in the south to MandjĂ©lia in the north of the main island of Grande-Terre, occurring in dense humid rainforests on non-ultramafic substrate. Material of this small tree species was first collected more than fifty years ago but was confused with G. virgata Vieill. ex Guillaumin as both taxa have small leaves. Garcinia urceolata, sp. nov. differs from G. virgata in a number of features of the leaves, flowers and fruits. Garcinia urceolata, sp. nov. also grows at higher elevation and in wetter conditions than G. virgata. Both species are purported to have dehiscent fruit, along with other New Caledonian members of the genus, a distinctive character that was used to justify the description of the genus Septogarcinia Kosterm., which is no longer regarded as distinct from Garcinia L. Line drawings and colour photos are provided for both the new species and for G. virgata for comparison, along with a preliminary risk of extinction assessment for each of them, which indicates that G. urceolata, sp. nov. is Vulnerable (VU) while G. virgata is Near Threatened (NT).</p
