    Circular Cylinder In Axial Flow

    The boundary layer formed on the outer surface of a semi-infinite circular cylinder in steady axial incompressible flow is studied in this thesis. Governing equations are solved using local similarity techniques and a nonsimilar numerical approach.;Two obvious similarity transformations can be used to obtain solutions for this problem, but they do not yield the same results, since the flow is essentially nonsimilar. In the extreme case that the radius of the cylinder is much larger than the boundary layer thickness, only one of the transformation leads to the correct solution, i.e., the Blasius solution. The other transformation yields an axial velocity profile which is deceptively close to the Blasius. This is also strongly suggested by comparing the series expansions of axial velocity profiles from each transformation. Solutions obtained by using either transformation merge at downstream locations.;Since the use of a single similarity variable does not solve the problem in its full range, an overall numerical solution is obtained by applying Keller\u27s Box method with primitive variables and similarity coordinates. Similarity coordinates scale the axial and radial coordinates such that boundary layer growth does not appear explicitly as we move downstream. Thus, the numerical mesh does not need to be enlarged and this leads to increased efficiency in computation. Results are obtained in the range that start with the Blasius solution and proceed far downstream.;A local similarity method, which is very efficient, using primitive variables with similarity coordinates is also applied to obtain solutions that are valid over a wide range of the flow.;Results obtained by these methods compare well with previously obtained analytical and numerical solutions, but they extend considerably the range of solution

    An unusual arteriovenous malformation involving the cervical vessels treated with endovascular repair

    We present an unusual and complex arteriovenous malformation involving the vertebral artery, subclavian artery, and internal jugular vein in a 31-year-old man with no history of trauma or catheterization. The repair was done using endovascular techniques to minimize complications from nerve or vascular injury. The massively dilated jugular vein has remained diminished in size and the patient has remained asymptomatic at 8 months. We discuss the occurrence of this rare malformation as well as treatment options along with their risks and benefits

    Toward Enriched Conceptions of Work Learning: Participation, Expansion, and Translation Among Individuals With/In Activity

    Despite the long recognition in HRD theory that learning is socially and materially situated in activity and relations, HRD literature indicates a continuing strong emphasis on individualistic theories representing learning as knowledge acquisition or individual development. It is argued here that understandings of work learning within HRD theory can be fruitfully enriched by more fully incorporating practice-based perspectives. Three contemporary theories that analyse learning as a relation of individuals with/in activity have been selected for discussion here: the participational perspective of situated cognition, the notion of expansion from cultural-historical activity theory, and the constructs of translation and mobilization presented by actor-network theory. While these are not particularly new to HRD, the contribution of this discussion is to bring together these theories, along with published empirical workplace research based on them, to highlight selected dynamics that may be useful tools for HRD theory development. One element in particular is read across the three theories: the dialectic of ‘flying’ and ‘grounding’, or lines of discontinuity and continuity characterising work learning. The argument is theory-driven, drawing from HRD literature of work learning and practice-based theories of social activity and knowledge production

    Alzheimer's early detection in post-acute COVID-19 syndrome: a systematic review and expert consensus on preclinical assessments.

    The risk of developing Alzheimer's disease (AD) in older adults increasingly is being discussed in the literature on Post-Acute COVID-19 Syndrome (PACS). Remote digital Assessments for Preclinical AD (RAPAs) are becoming more important in screening for early AD, and should always be available for PACS patients, especially for patients at risk of AD. This systematic review examines the potential for using RAPA to identify impairments in PACS patients, scrutinizes the supporting evidence, and describes the recommendations of experts regarding their use. We conducted a thorough search using the PubMed and Embase databases. Systematic reviews (with or without meta-analysis), narrative reviews, and observational studies that assessed patients with PACS on specific RAPAs were included. The RAPAs that were identified looked for impairments in olfactory, eye-tracking, graphical, speech and language, central auditory, or spatial navigation abilities. The recommendations' final grades were determined by evaluating the strength of the evidence and by having a consensus discussion about the results of the Delphi rounds among an international Delphi consensus panel called IMPACT, sponsored by the French National Research Agency. The consensus panel included 11 international experts from France, Switzerland, and Canada. Based on the available evidence, olfaction is the most long-lasting impairment found in PACS patients. However, while olfaction is the most prevalent impairment, expert consensus statements recommend that AD olfactory screening should not be used on patients with a history of PACS at this point in time. Experts recommend that olfactory screenings can only be recommended once those under study have reported full recovery. This is particularly important for the deployment of the olfactory identification subdimension. The expert assessment that more long-term studies are needed after a period of full recovery, suggests that this consensus statement requires an update in a few years. Based on available evidence, olfaction could be long-lasting in PACS patients. However, according to expert consensus statements, AD olfactory screening is not recommended for patients with a history of PACS until complete recovery has been confirmed in the literature, particularly for the identification sub-dimension.     Virus-Induced Cancers of the Skin and Mucosa:Are We Dealing with "Smoking Guns" or "Smoke and Mirrors" in the Operating Theatre?

    Introduction: Human papillomavirus (HPV) alone is thought to cause ~610,000 cases of cancer per year, and is the dominant aetiological agent for ano-genital (esp. cervical) and head and neck cancers (esp. oropharyngeal). Merkel cell polyomavirus (MCV) is a more recently discovered virus which causes Merkel cell carcinoma, a rare but highly aggressive skin malignancy. Methods: We explored the available published evidence to see if transmission of live HPV or MCV virus in smoke generated by laser or diathermy was feasible, and would pose an infection risk. Long-term infection with such carcinogenic viruses would then pose an increased risk for the development of virus-induced cancers in medical personnel. Results: The morphological structures of both HPV and MCV are very similar, and the size, external capsids and genomic structures show striking similarity. Both viruses have a non-enveloped external protein capsid consisting of 72 capsomeres, and a double-stranded DNA core. Sizes of both viruses range from 50 to 60 nm. There are now recent data demonstrating live and infectious HPV in smoke, and that these viruses can be used to infect cells in vitro. Further, anecdotal reports of virus transmission leading to disease causation in the production of respiratory airway viral warts (benign disease), and, finally, reports of HPV-induced oropharyngeal carcinoma (malignant disease) in two gynaecological surgeons as an occupational health hazard have been published recently. Conclusion: There is now sufficient evidence to support the hypotheses that live infectious carcinogenic viruses can be transmitted via smoke generated from surgical procedures, and, in rare instances, actually cause significant disease. Protective measures such as smoke extraction and airway protection should be instituted for all healthcare personnel, particularly those with multiple repeated exposures such as gynaecological surgeons

    Quantitative predictions on auxin-induced polar distribution of PIN proteins during vein formation in leaves

    The dynamic patterning of the plant hormone auxin and its efflux facilitator the PIN protein are the key regulator for the spatial and temporal organization of plant development. In particular auxin induces the polar localization of its own efflux facilitator. Due to this positive feedback auxin flow is directed and patterns of auxin and PIN arise. During the earliest stage of vein initiation in leaves auxin accumulates in a single cell in a rim of epidermal cells from which it flows into the ground meristem tissue of the leaf blade. There the localized auxin supply yields the successive polarization of PIN distribution along a strand of cells. We model the auxin and PIN dynamics within cells with a minimal canalization model. Solving the model analytically we uncover an excitable polarization front that triggers a polar distribution of PIN proteins in cells. As polarization fronts may extend to opposing directions from their initiation site we suggest a possible resolution to the puzzling occurrence of bipolar cells, such we offer an explanation for the development of closed, looped veins. Employing non-linear analysis we identify the role of the contributing microscopic processes during polarization. Furthermore, we deduce quantitative predictions on polarization fronts establishing a route to determine the up to now largely unknown kinetic rates of auxin and PIN dynamics.Comment: 9 pages, 4 figures, supplemental information included, accepted for publication in Eur.     Inlet and Outlet Boundary Conditions and Uncertainty Quantification in Volumetric Lattice Boltzmann Method for Image-Based Computational Hemodynamics

    Inlet and outlet boundary conditions (BCs) play an important role in newly emerged image-based computational hemodynamics for blood flows in human arteries anatomically extracted from medical images. We developed physiological inlet and outlet BCs based on patients’ medical data and integrated them into the volumetric lattice Boltzmann method. The inlet BC is a pulsatile paraboloidal velocity profile, which fits the real arterial shape, constructed from the Doppler velocity waveform. The BC of each outlet is a pulsatile pressure calculated from the three-element Windkessel model, in which three physiological parameters are tuned by the corresponding Doppler velocity waveform. Both velocity and pressure BCs are introduced into the lattice Boltzmann equations through Guo’s non-equilibrium extrapolation scheme. Meanwhile, we performed uncertainty quantification for the impact of uncertainties on the computation results. An application study was conducted for six human aortorenal arterial systems. The computed pressure waveforms have good agreement with the medical measurement data. A systematic uncertainty quantification analysis demonstrates the reliability of the computed pressure with associated uncertainties in the Windkessel model. With the developed physiological BCs, the image-based computation hemodynamics is expected to provide a computation potential for the noninvasive evaluation of hemodynamic abnormalities in diseased human vessels

    Tutor and teacher timescapes : lessons from a home-school partnership

    A partnership project was developed in which parents volunteered to support teachersin training years 1-3 children in computer skills at a primary school in a small, lowsocio-economic community. This article identifies the ways teachers and the &lsquo;tutors&rsquo;(as the volunteers were called) understood the value of the project. &lsquo;Being a teacher&rsquo;and &lsquo;being a volunteer&rsquo; were structured by different forms of social engagement,which in turn influenced the ways individuals were able to work with each other incollaborative processes. We argue that the discursive practices encoded in homeschool-community partnership rhetoric represent ruling-class ways of organising andnetworking that may be incompatible with those of people from low socio-economicbackgrounds. When such volunteers work in schools their attendance may be sporadicand short-term whereas teachers would like &lsquo;reliable&rsquo; ongoing commitment. Thismismatch wrought of teachers&rsquo; and volunteers&rsquo; differing everyday realities needs to beunderstood before useful models for partnerships in disadvantaged communities maybe realised.<br /

    Systematic review of antiepileptic drugs’ safety and effectiveness in feline epilepsy

    Understanding the efficacy and safety profile of antiepileptic drugs (AEDs) in feline epilepsy is a crucial consideration for managing this important brain disease. However, there is a lack of information about the treatment of feline epilepsy and therefore a systematic review was constructed to assess current evidence for the AEDs’ efficacy and tolerability in cats. The methods and materials of our former systematic reviews in canine epilepsy were mostly mirrored for the current systematic review in cats. Databases of PubMed, CAB Direct and Google scholar were searched to detect peer-reviewed studies reporting efficacy and/or adverse effects of AEDs in cats. The studies were assessed with regards to their quality of evidence, i.e. study design, study population, diagnostic criteria and overall risk of bias and the outcome measures reported, i.e. prevalence and 95% confidence interval of the successful and affected population in each study and in total
