    Evolving UK policy on diversity in the armed services: multiculturalism and its discontents

    Reflecting a generally multiculturalist rhetoric, UK policy in this area has hitherto focussed on enhancing the degree to which the armed services represent or reflect the ethnic makeup of the UK population. Ambitious targets have been set and some progress made in moving towards them. However, the dynamics of population change, together with the diverse preferences of ethno-religious minorities, have meant that the goal of representativeness has remained out of reach. At the same time, the armed services have continued to struggle with an ongoing recruitment problem while the volume of operational commitments has shown little sign of reducing

    Equity in health care financing: The case of Malaysia

    Background: Equitable financing is a key objective of health care systems. Its importance is evidenced in policy documents, policy statements, the work of health economists and policy analysts. The conventional categorisations of finance sources for health care are taxation, social health insurance, private health insurance and out-of-pocket payments. There are nonetheless increasing variations in the finance sources used to fund health care. An understanding of the equity implications would help policy makers in achieving equitable financing. Objective: The primary purpose of this paper was to comprehensively assess the equity of health care financing in Malaysia, which represents a new country context for the quantitative techniques used. The paper evaluated each of the five financing sources (direct taxes, indirect taxes, contributions to Employee Provident Fund and Social Security Organization, private insurance and out-of-pocket payments) independently, and subsequently by combined the financing sources to evaluate the whole financing system. Methods: Cross-sectional analyses were performed on the Household Expenditure Survey Malaysia 1998/99, using Stata statistical software package. In order to assess inequality, progressivity of each finance sources and the whole financing system was measured by Kakwani's progressivity index. Results: Results showed that Malaysia's predominantly tax-financed system was slightly progressive with a Kakwani's progressivity index of 0.186. The net progressive effect was produced by four progressive finance sources (in the decreasing order of direct taxes, private insurance premiums, out-of-pocket payments, contributions to EPF and SOCSO) and a regressive finance source (indirect taxes). Conclusion: Malaysia's two tier health system, of a heavily subsidised public sector and a user charged private sector, has produced a progressive health financing system. The case of Malaysia exemplifies that policy makers can gain an in depth understanding of the equity impact, in order to help shape health financing strategies for the nation

    The psychological-type profile of clergywomen in ordained local ministry in the Church of England : pioneers or custodians?

    This study employs psychological-type theory to compare the psychological profile of 144 clergywomen serving in ordained local ministry in the Church of England alongside the established profile of 237 professional mobile clergywomen serving in the Church of England published by Francis, Craig, Whinney, Tilley, and Slater. The data found no significant differences between these two groups of clergywomen in terms of orientations (introversion and extraversion) or in terms of the judging process (thinking and feeling). In terms of the perceiving process, there was a significantly higher proportion of sensing types among those serving in ordained local ministry (70% compared with 35%). In terms of the attitudes, there was a significantly higher proportion of judging types among those serving in ordained local ministry (83% compared with 65%). The combined sensing judging (SJ) temperament accounted for 65% of the clergywomen serving in ordained local ministry, compared with 29% of the clergywomen serving in professional mobile ministry in the earlier study.     Rehabilitating antisocial personalities: treatment through self-governance strategies

    Offenders with antisocial personality disorder (ASPD) are widely assumed to reject psychotherapeutic intervention. Some commentators, therefore, argue that those with the disorder are better managed in the criminal justice system, where, following the introduction of indeterminate sentences, engagement with psychological treatment is coercively linked to the achievement of parole. By comparison, National Institute of Clinical Excellence guidelines on the management and treatment of ASPD recommend that those who are treatment seeking should be considered for admission to specialist psychiatric hospitals. The rationale is that prison-based interventions are underresourced, and the treatment of ASPD is underprioritised. The justification is that offenders with ASPD can be rehabilitated, if they are motivated. One problem, however, is that little is known about why offenders with ASPD seek treatment or what effect subsequent treatment has on their self-understanding. The aim of this paper is to address these unresolved issues. It draws on the findings of Economic and Social Research Council (ESRC) funded qualitative study examining the experiences of sentenced male offenders admitted to a specialist personality disorder ward within the medium secure estate and the medical practitioners who treat them. The data are analysed with reference to Michel Foucault’s work on governmentality and strategy in power relations. Two arguments are advanced: first, offenders with ASPD are motivated by legal coercive pressures to implement a variety of Foucauldian-type strategies to give the false impression of treatment progress. Second, and related, treatment does not result in changes in self-understanding in the resistive client with ASPD. This presupposes that, in respect of this group at least, Foucault was mistaken in his claim that resistive behaviours merely mask the effectiveness of treatment norms over time. Nevertheless, the paper concludes that specialist treatment in the hospital setting can effect changes in the resistive offender’s self-understanding, but not if the completion of treatment results, as is commonplace, in his prison readmission

    Age group, location or pedagogue: factors affecting parental choice of kindergartens in Hungary

    Hungary has experienced significant political, economic, demographic and social changes since the end of Soviet domination in the 1990s. The gradual move towards liberal-democracy has been accompanied by growing emphasis on individualism, choice and diversity. Universal kindergarten provision for 5-6 year olds is a long established feature of the Hungarian education system, but little is known about parental choice (Török, 2004). A case study (Yin, 2004) of factors influencing parental choice and satisfaction was undertaken in one Hungarian town. This was based on a survey of 251 parents of children attending both mixed-age and same-age groups across 12 kindergartens. Parents suggested that the most important influences were geographical location and the individual pedagogue(s). Given that traditionally each pedagogue follows ‘their’ cohort from kindergarten entry to primary school, their influence appears heightened. Although generally satisfied with their chosen arrangement, parents from same-age groups expressed significantly more confidence and satisfaction, particularly in relation to cognitive development and preparation for school. Parents appear less convinced about the trend towards mixed-age groups and questions are raised about sufficiency of evidence of their benefits in a Hungarian context and the driving factors behind change

    Home Defence and the Sandys Defence White Paper, 1957

    Long understood as the key document in Britain's Cold War history, the Duncan Sandys Defence White Paper of 1957 nevertheless has a largely forgotten context: home defence. This article argues that understanding this context allows important new conclusions to be drawn concerning the drafting, presentation and the reception of the document and the deterrent strategy it expounded. It argues that the Paper failed to establish a new doctrine for civil defence which reconciled the policy with the wider deterrent strategy. In doing this, the Paper presented a muddled policy to the public: one which failed to justify the reductions in civil defence provision but which stressed the destructive power of thermonuclear weapons. This had the effect of encouraging the critics of the government's nuclear strategy to flag up the absence of adequate civil defence measures and highlight the 'admission' that there was no defence against the hydrogen bomb

    Enforcement and compliance: critical practices for community rehabilitation companies and the new NPS?

    Efforts to secure compliance have always been a core element of probation practice,although compliance has been constructed in diverse ways and promoted through different means throughout its history. This article takes a brief historical perspective and reviews recent research on enforcement practices and developing understandings of compliance. These guide a critical discussion of the practices that might develop as responsibilities for enforcement are divided between the new National Probation Service (NPS) and Community Rehabilitation Companies (CRCs) under the Transforming Rehabilitation agenda, highlighting inevitable tensions and challenges, and anticipating how inter-agency practices might shape the ongoing construction of compliance. Charging more than one agency with responsibilities in relation to enforcement is tricky and creates risks in terms of legitimacy, credibility and justice. On the whole, future prospects seem difficult, but not hopeless and, in particular, there are examples of positive practices in probation and youth justice for the NPS and CRCs to draw upon as they develop their inter-agency structures and processes.     Despite playing a pivotal role in thousands of defendants’ experiences of criminal justice every year, the role of probation workers in the English and Welsh Magistrates’ courts has been neglected by researchers for several decades. This article presents the findings of an ethnographic study of the work of probation staff in two such courts. The study suggests that probation work in this context is being squeezed into an operating model which bears all the hallmarks of a process described by Ritzer as ‘McDonaldization’. It is argued that the proximate causes of McDonaldization in this sub-field of probation work lie at the intersection of parallel Government-led reform programmes – Transforming Rehabilitation and Transforming Justice – which have respectively focused on creating a market for probation services and enhancing the administrative efficiency of criminal proceedings. Until now, almost no attention has been paid, either by researchers or policy-makers, to the intersection of these programmes of reform in the probation suites at the Magistrates’ courts

    Why Give Birth in Health Facility? Users' and Providers' Accounts of Poor Quality of Birth Care in Tanzania.

    In Tanzania, half of all pregnant women access a health facility for delivery. The proportion receiving skilled care at birth is even lower. In order to reduce maternal mortality and morbidity, the government has set out to increase health facility deliveries by skilled care. The aim of this study was to describe the weaknesses in the provision of acceptable and adequate quality care through the accounts of women who have suffered obstetric fistula, nurse-midwives at both BEmOC and CEmOC health facilities and local community members. Semi-structured interviews involving 16 women affected by obstetric fistula and five nurse-midwives at maternity wards at both BEmOC and CEmOC health facilities, and Focus Group Discussions with husbands and community members were conducted between October 2008 and February 2010 at Comprehensive Community Based Rehabilitation in Tanzania and Temeke hospitals in Dar es Salaam, and Mpwapwa district in Dodoma region. Health care users and health providers experienced poor quality caring and working environments in the health facilities. Women in labour lacked support, experienced neglect, as well as physical and verbal abuse. Nurse-midwives lacked supportive supervision, supplies and also seemed to lack motivation. There was a consensus among women who have suffered serious birth injuries and nurse midwives staffing both BEmOC and CEmOC maternity wards that the quality of care offered to women in birth was inadequate. While the birth accounts of women pointed to failure of care, the nurses described a situation of disempowerment. The bad birth care experiences of women undermine the reputation of the health care system, lower community expectations of facility birth, and sustain high rates of home deliveries. The only way to increase the rate of skilled attendance at birth in the current Tanzanian context is to make facility birth a safer alternative than home birth. The findings from this study indicate that there is a long way to go
