    Analysis of the Health Prevention study program in the Medicine career

    Introducción: el continuo perfeccionamiento en el sector salud, sustentado en un nuevo modelo económico, traza dentro de sus objetivos: el fortalecimiento de las estrategias de formación, capacitación e investigación. Como resultado de este proceso hubo un perfeccionamiento en la carrera de Medicina y, con ello, en la asignatura.Objetivo: evaluar el programa de la asignatura Prevención en Salud, a partir de lo regulado en el reglamento vigente para el trabajo docente-metodológico.Métodos: se realizó un análisis crítico, valorativo y reflexivo, que conllevó a una revisión exhaustiva del contenido, del basamento legal que norma el trabajo docente-metodológico para la enseñanza superior, así como artículos originales y de revisión relacionados con la temática. También se analizaron las orientaciones metodológicas para los cursos académicos 2017-2018, 2018-2019 y 2019-2020, y el tipo de profesional que demanda la sociedad.  Resultados: se redujeron 16 horas en el tema III y se incrementaron al tema II, de manera que permitiera al estudiante trabajar no solo lo relacionado con familia y comunicación, sino también profundizar en la identificación de los problemas de salud de las familias, e integrar el conocimiento de algunos de los programas priorizados abordados en el tema I, así como en los contenidos de las asignaturas precedentes.Conclusiones: las deficiencias detectadas en el programa de la asignatura Prevención en Salud, como resultado del análisis crítico, valorativo y reflexivo, encontraron respuestas en las propuestas de cambio que realizaron los autores.Introduction: the continuous improvement in the health sector, supported by a new economic model, outlines within its objectives: the strengthening of education, training and research strategies. As a result of this process, there was an improvement in the Medicine degree and, with it, in the subject.Objective: evaluate the program of the Health Prevention subject, based on what is regulated in the current regulations for teaching-methodological work.Methods: a critical, evaluative and reflective analysis was carried out, which led to an exhaustive review of the content, the legal basis that regulates the teaching-methodological work for higher education, as well as original and review articles related to the topic. The methodological guidelines for the 2017-2018, 2018-2019 and 2019-2020 academic years were also analyzed, and the type of professional that society demands.Results: 16 hours were reduced in topic III and increased to topic II, in a way that allowed the student to work not only on what is related to family and communication, but also to deepen the identification of family health problems, and integrate knowledge of some of the prioritized programs addressed in topic I, as well as the contents of the preceding subjects.Conclusions: the deficiencies detected in the program of the subject Prevention in Health, as a result of the critical, evaluative and reflective analysis, found answers in the proposals for change made by the authors

    Switching monopolar mode for RF-assisted resection and superficial ablation of biological tissue: computational modeling and ex vivo experiments

    [EN] Radiofrequency (RF)-based monopolar (MM) and bipolar mode (BM) applicators are used to thermally create coagulation zones (CZs) in biological tissues with the aim of destroying surface tumors and minimizing blood losses in surgical resection. Both modes have disadvantages as regards safely and in obtaining a sufficiently deep coagulation zone (CZ). In this study, we compared both modes versus a switching monopolar mode (SMM) in which the role of the active electrode changes intermittently between the two electrodes of the applicator. In terms of clinical impact, the three modes can easily be selected by the surgeon according to the surgical maneuver. We used computational and experimental models to study the feasibility of working in MM, BM, and SMM and to compare their CZ characteristics. We focused exclusively on BM and SMM, since MM only creates small coagulation zones in the area between the electrodes. The results showed that SMM produces the deepest CZ between both electrodes (33% more than BM) and SMM did not stop the generator when an electrode lost contact with the tissue, as occurred in BM.     UNA MIRADA ACERCA DEL COMPORTAMIENTO DEL RENDIMIENTO ACADÉMICO

    El alumno, protagonista de su propio aprendizaje, es un elemento principal en el proceso docente educativo. El conocimiento de sus características, al menos en relación con su aprendizaje, es de decisiva importancia. Se trata, en definitiva, de disponer de los distintos elementos de dicho proceso, de tal modo que favorezcan al máximo el aprendizaje que cada alumno sea capaz de adquirir en función de sus condiciones personológicas, la individualización de la enseñanza, en combinación con la anterior, serán eficaces en la medida en que estas condiciones o recursos lo permitan. El presente artículo se enmarca en un estudio teórico sobre el rendimiento académico en los jóvenes. Se aplicaron una serie de técnicas e instrumentos como: análisis de documentos, entrevistas cualitativas, cuestionario de autovaloración, técnicas de diez deseos, técnica de distribución de tiempo, análisis de productos y grupos de discusión. Todos estos instrumentos se analizan de manera complementaria e integradora, lo que permite conocer las concepciones que tienen los estudiantes acerca del rendimiento académico; abriendo las posibilidades para el establecimiento de contextos educativos dirigidos a la formación y el desarrollo individual de los estudiantes.     Single Nucleotide Polymorphisms in the Vitamin D Metabolic Pathway as Survival Biomarkers in Colorectal Cancer

    Several studies have suggested that single nucleotide polymorphisms (SNPs) related to vitamin D metabolism may affect CRC carcinogenesis and survival. The aim of this study was to evaluate the influence of 13 SNPs involved in the vitamin D metabolic pathway on CRC survival. We conducted an observational retrospective cohort study, which included 127 Caucasian CRC patient from the south of Spain. SNPs in VDR, CYP27B1, CYP2R1, CYP24A1, and GC genes were analyzed by real-time polymerase chain reaction. Progression-free survival (PFS) and overall survival (OS) were assessed. Cox regression analysis adjusted for metastasis, age of diagnosis, stage (IIIB, IV or IVB), ECOG score (2–4), lymph node involvement, adjuvant chemotherapy, and no family history of CRC showed that the VDR ApaI (p = 0.036), CYP24A1 rs6068816 (p < 0.001), and GC rs7041 (p = 0.006) were associated with OS in patients diagnosed with CRC, and CYP24A1 rs6068816 (p < 0.001) was associated with PFS adjusted for metastasis, age of diagnosis, stage (IIIB, IV or IVB), ECOG score (2–4), lymph node involvement, adjuvant chemotherapy, and no primary tumor resection. The rest of the SNPs showed no association with CRC survival.     Inhaled Methoxyflurane Provides Greater Analgesia and Faster Onset of Action Versus Standard Analgesia in Patients With Trauma Pain: InMEDIATE: A Randomized Controlled Trial in Emergency Departments

    Study objective: The objective of the InMEDIATE study was to evaluate the change in intensity of traumatic pain over the first 20 min in adult patients treated with methoxyflurane versus standard analgesic treatment in Spain. This the first randomized, active-controlled, multicenter trial of methoxyflurane in the emergency setting in Europe. Methods: This was a randomized, controlled study that enrolled adult patients with acute moderate to severe (score >= 4 on the 11-point Numeric Rating Scale) trauma-associated pain in 14 Spanish emergency departments. Patients were randomized 1:1 to methoxyflurane (up to 2x3 mL) or standard analgesic treatment. Coprimary endpoints were the change from baseline in Numeric Rating Scale pain intensity score during the first 20 minutes of treatment and time to first pain relief. Results: Three hundred five patients were randomized (methoxyflurane 156; standard analgesic treatment 149). Most patients in the standard analgesic treatment group (70%) received intravenous first-step analgesics and 9.4% of patients were treated with opioids. Mean decrease from baseline in Numeric Rating Scale pain intensity score was greater for methoxyflurane than standard analgesic treatment at all points, with a significant treatment difference overall up to 20 minutes (repeated-measures model 2.47 versus 1.39; treatment difference 1.00; 95% confidence interval 0.84 to 1.32). Median time to first pain relief was significantly shorter for methoxyflurane than standard analgesic treatment (3 versus 10 minutes). Methoxyflurane achieved better patient and clinician ratings for pain control and comfort of treatment than standard analgesic treatment and exceeded patient and clinician expectations of treatment in, respectively, 77% and 72% of cases compared with 38% and 19% for standard analgesic treatment. Conclusion: These results support consideration of methoxyflurane as a nonnarcotic, easy-to-administer, rapid-acting, first-line alternative to currently available analgesic treatments for trauma pain

    Autoimmune Diseases and COVID-19 as Risk Factors for Poor Outcomes: Data on 13,940 Hospitalized Patients from the Spanish Nationwide SEMI-COVID-19 Registry

    (1) Objectives: To describe the clinical characteristics and clinical course of hospitalized patients with COVID-19 and autoimmune diseases (ADs) compared to the general population. (2) Methods: We used information available in the nationwide Spanish SEMI-COVID-19 Registry, which retrospectively compiles data from the first admission of adult patients with COVID-19. We selected all patients with ADs included in the registry and compared them to the remaining patients. The primary outcome was all-cause mortality during admission, readmission, and subsequent admissions, and secondary outcomes were a composite outcome including the need for intensive care unit (ICU) admission, invasive and non-invasive mechanical ventilation (MV), or death, as well as in-hospital complications. (3) Results: A total of 13,940 patients diagnosed with COVID-19 were included, of which 362 (2.6%) had an AD. Patients with ADs were older, more likely to be female, and had greater comorbidity. On the multivariate logistic regression analysis, which involved the inverse propensity score weighting method, AD as a whole was not associated with an increased risk of any of the outcome variables. Habitual treatment with corticosteroids (CSs), age, Barthel Index score, and comorbidity were associated with poor outcomes. Biological disease-modifying anti-rheumatic drugs (bDMARDs) were associated with a decrease in mortality in patients with AD. (4) Conclusions: The analysis of the SEMI-COVID-19 Registry shows that ADs do not lead to a different prognosis, measured by mortality, complications, or the composite outcome. Considered individually, it seems that some diseases entail a different prognosis than that of the general population. Immunosuppressive/immunoregulatory treatments (IST) prior to admission had variable effects

    Retos del PATEC ante la comparativa de otras experiencias de acción tutorial universitaria

    Tras más de una década de la puesta en marcha del Programa de Acción Tutorial en la Facultad de Económicas siguen persistiendo debilidades que tratan de resolverse año a año. En este sentido, el Programa se ha tratado de mejorar desde la creación de la Red de Tutores del PATEC en el curso 2013-2014. Del trabajo de la Red ha surgido el Manual del tutor, entendido como un recurso que recopila la información básica que va a necesitar el tutor para orientar a su alumnado; la idea de implantar la figura del alumno-tutor en la Facultad para acercar el Programa al conjunto del alumnado; y en el presente curso se ha planteado estudiar la acción tutorial en otras universidades españolas con el objetivo de recopilar buenas prácticas y analizar la posibilidad de implementarlas en la Facultad. Esta comunicación recoge el trabajo realizado por la Red y se estructura de la siguiente manera: en primer lugar, se identifican las prácticas de acción tutorial de diversas universidades españolas; en segundo lugar, se comparan con las actividades realizadas en el marco del PATEC; y, finalmente, se proponen acciones que contribuyan a mejorar el funcionamiento del PATEC

    Red de tutores del Programa de Acción Tutorial de la Facultad de Económicas (PATEC)

    El Plan de Acción Tutorial (PAT, PATEC en la Facultad de Ciencias Económicas y Empresariales) se viene desarrollando en la Universidad de Alicante (UA) desde el curso 2005-2006. Tras más de una década de su puesta en marcha en la Facultad de Ciencias Económicas y Empresariales, el PATEC se ha ido consolidando año a año y, aunque son muchas sus fortalezas, existen aún ciertas debilidades que persisten a las que hay que dar respuesta. Para ello, en el curso 2013-2014 surge la Red de Tutores del PATEC como un punto de encuentro en el que reflexionar sobre el funcionamiento del Programa. En el curso 2015-2016 su objetivo es doble. Por un lado, y continuando la labor que comenzó en el curso anterior referida a analizar experiencias de acción tutorial en otras universidades españolas, extraer las buenas prácticas que supongan un nuevo impulso para el PATEC. Por otro, conocer la experiencia de la primera promoción de alumnos-tutores de la Facultad
