    Can I help you? Information sharing in online discussion forums by people living with a long-term condition

    Background Peer-to-peer healthcare is increasing, especially amongst people living with a long-term condition. How information is shared is, however, sometimes of concern to healthcare professionals. Objective This study explored what information is being shared on health-related discussion boards and identified the approaches people used to signpost their peers to information. Methods This study was conducted using a qualitative content analysis methodology to explore information shared on discussion boards for people living with diabetes. Whilst there is debate about the best ethical lens to view research carried out on data posted on online discussion boards, the researchers chose to adopt the stance of treating this type of information as “personal health text”, a specific type of research data in its own right. Results Qualitative content analysis and basic descriptive statistics were used to analyse the selected posts. Two major themes were identified: ‘Information Sharing from Experience’ and ‘Signposting Other Sources of Information’. Conclusions People were actively engaging in information sharing in online discussion forums, mainly through direct signposting. The quality of the information shared was important, with reasons for recommendations being given. Much of the information sharing was based on experience, which also brought in information from external sources such as healthcare professionals and other acknowledged experts in the field. With the rise in peer-to-peer support networks, the nature of health knowledge and expertise needs to be redefined. People online are combining external information with their own personal experiences and sharing that for others to take and develop as they wish

    Moderate ischemic mitral regurgitation (IMR) and metabolic syndrome: where are we now and where are we going?

    BACKGROUND: The metabolic syndrome appears to affect 10% to 25% of adult population worldwide. Several studies have described the association between metabolic syndrome and ischaemic heart disease, however, none linked metabolic syndrome to ischemic mitral regurgitation, a serious clinical problem facing both the cardiologists and cardiac surgeons. Ischemic mitral regurgitation is mitral insufficiency caused by myocardial infarction. The myocardial ischemia can result in altered ventricular geometry, leading to mitral insufficiency. Interestingly metabolic syndrome showed more pronounced alteration of left ventricular geometry and function especially in obese subjects. PRESENTATION OF THE HYPOTHESIS: We have recently proposed that there is link between metabolic syndrome and ischemic mitral regurgitation and associated complications. Operative strategy for moderate ischaemic mitral regurgitation continues to be debated between revascularisation alone and concomitant valve repair at the time of coronary artery bypass surgery. Each of the above group has published studies, with results supporting each argument. TESTING THE HYPOTHESIS: Generally speaking the treatments available for metabolic syndrome are based in both life style modification (dietary advice and advice to increase physical activity) and medical treatment to enhance insulin sensitivity. Randomised controlled trials may show whether the current available treatment of metabolic syndrome may have an impact on moderate ischemic mitral regurgitation. IMPLICATIONS OF THE HYPOTHESIS: Metabolic syndrome was shown to alter left ventricular geometry and therefore it is possible to postulate that the variation in the response of different patients with moderate ischemic mitral regurgitation to current management may be attributed to the absence and presence of metabolic syndrome. Research testing of this hypothesis in the future may reveal whether concomitant treatment of metabolic syndrome will play part in the management of moderate ischemic mitral regurgitation

    Centrifugal pumps are widely used in many industrial applications. Defective pumps cause higher rate of energy loss with related overall performance degradation. Mechanical components fail due to decrease in pump’s strength or an increase in the force acting on it. Rolling bearings are one of the major components of the centrifugal pumps. Bearings are critical mechanical components in industrial machines. Bearing failure is often attributed to be one of the major causes of breakdown. Early detection of undesired conditions during the operation of the centrifugal pump has become very important in order to avoid consequential damages. Early detection is also vital for increasing availability and reliability. Automated health monitoring can reduce the outage time and repair costs. Using vibration analysis technique to monitor and evaluate the health of the machines has become an important technique in this field. In this research, we studied the vibration signatures of healthy and faulty conditions of the bearings of a pumping system. The pumping system consists of centrifugal pump and electrical motor. The vibration signatures analyze the bearing status with respect to the normal operation. The paper shows how to identify if there is a problem in the outer race of the Bearing Pass Frequency Outer Race (BPFO)

    Gear pumps are critical devices in many industrial applications. An unexpected failure of the gear pump may cause significant effect on its performance. Consequently, there will be economic losses. As a result of that, fault diagnosis in gears has been the subject of intensive research. Vibration analysis has been used as an effective tool in machines diagnosis and in machinery maintenance decisions. As a rule, an increased vibration level is a warning form before failure or breakdown. By measuring and analyzing the gear pump vibration, it is possible to determine both the nature and severity of the defect, and hence predict the machine’s failure. The vibration signal of a gear pump carries the signature of the fault in the gears, and early fault detection of the gear pump is possible by analyzing the vibration signal using different signal processing techniques. This paper presents, experimentally, the external gear pump signature for normal and faulty gear pumps at different rotational speeds (1080, 1200, and 1439rpm). The considered faults herein are two different notches on one of the pump teeth— small notch and large notch. The paper concludes that features of the vibration are different with the notch shape and the rotational speed. The amplitude of vibration increases by increasing both rotational speed and notch size

    Synthesis and characterization of some new ruthenium (II) complexes as photosensitizers in dye-sensitized solar cells

    New ruthenium (II) complexes, [Ru(DHZ)2(bpy)], [Ru(SCN)2(bpy)(DMSO)2], [Ru(SCN)2(dmbpy)(DMSO)2] and [RuCl2(salen)]-2, where bpy = 2,2'- bipyridine, DHZ = 1,5-diphenylthiocarbazone, dmbpy = 4,4'-dimethyl-2,2' bipyridine and salen = 2,2'- ethylenebis(nitrilomethylidene)diphenol were synthesized and characterized by elemental analysis, FTIR, UV-Vis spectroscopy and thermal analysis. From data of these investigations the structural formula and the mode of bonding were obtained. These complexes were successfully applied to sensitization of nano-crystalline TiO2 based solar cells (DSSCs). The photovoltaic efficiencies of the studied DSSCs increase in the following order [Ru(DHZ)2(bpy)]< [Ru(SCN)2(bpy)(DMSO)2]< [Ru(SCN)2(dmbpy)(DMSO)2]< [RuCl2(salen)]-2. This increase is in agreement with the light harvesting of these complexes as indicated from their absorption spectra. Ferrioxalate complex enhanced the performance of some investigated cells. Therefore, a mechanism of this improvement has been postulated. Polyaniline as well as iodine doped polyaniline modified FTO electrode has been tested as promising counter electrodes. The efficiencies of the cells using iodine doped polyaniline is higher than that of polyaniline, which is assignable to the high conductivity of iodine

    Effects of kenaf bast fibres on hydration behaviour of cement

    The compatibility between cement and kenaf bast fibre and its improvement with various types of accelerators were investigated by observation and analysis on hydration behaviour in terms of hydration characteristics, namely, maximum hydration temperature and required time to reach maximum temperature. Five extraction methods (crude, water retting, decortication, NaOH retting and benzoate retting), four accelerators (CaCl2, AlCl3, Na2SO4, CaO), three concentrations (2, 4 and 6%) and three particle sizes (0.5, 0.8 and 4.0 mm) were used. The hydration behaviour of mixtures demonstrated that NaOH and benzoate were unsuitable with cement. Meanwhile, CaCl2 and CaO were found to be effective accelerators for restraining inhibitory influences. In addition, 2% accelerator was available and acceptable for quick-curing cement. Particle sizes of 0.5 and 0.8 mm required addition of accelerators to reach maximum cement setting
