    Fretting of CoCrMo and Ti6Al4V Alloys in Modular Prostheses

    Implantation of a total hip replacements (THR) is an effective intervention in the management of arthritis. Modularity at the taper junction of THR was introduced in order to improve the ease with which the surgeon could modify the length of the taper section and the overall length of the replacement. Cobalt chromium (Co–28Cr–6Mo) and titanium (Ti–6Al–4V) alloys are the most commonly used materials for the device. This study investigates the fretting behaviour of both CoCr–CoCr and CoCr–Ti couplings and analyses their damage mechanisms. A reciprocating tribometer ball on plate fretting contact was instrumented with in situ electrochemistry to characterise the damage inflicted by tribocorrosion on the two couplings. Fretting displacements amplitudes of 10, 25 and 50 mm at an initial contact pressure of 1 GPa were assessed. The results reveal larger metallic volume loss from the CoCr–CoCr alloy compared to the CoCr–Ti alloy, and the open circuit potential indicates a depassivation of the protective oxide layer at displacement amplitudes .25 mm. In conclusion, the damage mechanisms of CoCr–CoCr and CoCr–Ti fretting contacts were identified to be wear and fatigue dominated mechanisms respectively

    Sexual behaviour of in-school adolescents in Ibadan, South-West Nigeria

    This study was a crossectional survey conducted among 716 senior secondary school adolescents in Ibadan from March to August 2005. The result of 695 that was analyzed revealed that the mean age of 15±2.6years. 28.3% of the respondents had previous sexual exposure with higher proportions being male (p=0.00043). Majorities\' first sexual exposure was unplanned. Family settings and educational level do not have significant influence on the previous sexual exposure. The methods of sexual activity were mainly through vagina route while some had also practiced oral and anal sex. Most of those that are sexually exposed had more than one partner. About half of the respondents learn about sex from their friends while others through their parents and media. We conclude that in-school adolescents practiced unsafe sexual activity and they are therefore predisposed to STI/HIV and other reproductive health risks.     Randomization of two dosing regimens of vaginal misoprostol for cervical ripening and labor induction in a low resource setting

    Objectives: To compare the effectiveness of two dosing regimens of vaginal misoprostol for cervical ripening and induction of labour.Materials and Methods: Pregnant women with singleton low risk pregnancy at term scheduled for elective induction of labour were randomized to  receive either 25 µg or 50 µg of vaginal misoprostol for pre.labour  cervical ripening. All the patients received antenatal care and delivered at the University College Hospital (UCH) from January 1st to May 31st 2006. A total of 128 patients were randomized; 65 patients received 25 µg and 63 patients received 50 µg of vaginal misoprostol.Results: Significantly higher number of patients in the 50 µg group   progressed to active labour as compared with the 25 µg group (95.2% versus 84.6%, P < 0.05). The need for oxytocin augmentation of labour was higher among the 25 µg as compared with 50 µg (39.7% versus 16.4%, P = 0.007). There was higher proportion of patients in the 50 µg group delivering vaginally within 24 hours as compared with the 25 µg group (98.2% versus 90.0%, P = 0.063). However, the mean interval between the first dose of misoprostol and vaginal delivery was not  statistically different in the two groups (754 } 362 minutes and 885 } 582 minutes, P = 0.152). The incidence of caesarean section was similar in the two groups (7.7% versus 11%, P = 0.580). Labour complications, such as precipitate labour, tachysystole and abnormal fetal heart rate patterns were greater in the 50 µg group.Conclusion: Twenty.five microgram of misoprostol appears to be as  effective as 50 µg for pre.induction cervical ripening and labour induction. Though 50 µg of vaginal misoprostol resulted in relatively faster delivery and less need for oxytocin augmentation, it was associated with more  labour complications as compared with 25 µg of misoprostol.Key words: Cervical ripening, labour induction, misoprosto

    Lymphomas of the female genital tract in Ibadan

    Context: Female genital tract cancers are second only to breast malignancies. Cervical cancer is the commonest while epithelial cancers constitute the major histologic type. Lymphoid neoplasms are extremely rare, with the Burkitt's variety accounting for majority of cases seen in the African female.Objective: This is a review of the obstetric/gynaecological patients seen at the University College Hospital, Ibadan between 1976 and 2005, predating the commencement of a nationwide research into non – Hodgkin's lymphoma funded by the Aboderin Foundation.Study Design: Records of subjects were analysed for their  sociodemographic and clinicopathological characteristics, recruiting only those who satisfy the criteria for diagnosing genital tract lymphomas according to Kosari et al1. Retrievable archival paraffin blocks of subjects were also analysed using immunophenotyping.Results: Fifteen cases were identified within the study period; the highest yearly incidence rate being 3 in 1977. In eighteen of the years studied no cases were seen. Most cases (80.0%) occurred in the ovaries. Eighty percent (12) of patients were 25 years or below and nulliparas and grandmultiparas formed 46.7 and 13.4% respectively. Unilateral orbilateral oophorectomy was the commonest surgical procedure (53.3%) while 20.0% had total abdominal hysterectomy with or without bilateral salpingoophorectomy. Data were unavailable concerning use of adjuvanttherapy and outcome of treatment. Four archival paraffin blocks were available for immunophenotyping using the affordable panel of CD20, CD10 and Bcl – 2 antibodies; all were Burkitt's lymphoma.Conclusion: Female genital lymphomas are very rare, affecting the young and nulliparous.     Management of the infertile couple: an evidence-based protocol

    <p>Abstract</p> <p>Background</p> <p>Infertility is defined as inability of a couple to conceive naturally after one year of regular unprotected sexual intercourse. It remains a major clinical and social problem, affecting perhaps one couple in six. Evaluation usually starts after 12 months; however it may be indicated earlier. The most common causes of infertility are: male factor such as sperm abnormalities, female factor such as ovulation dysfunction and tubal pathology, combined male and female factors and unexplained infertility.</p> <p>Objectives</p> <p>The aim of this study is to provide the healthcare professionals an evidence-based management protocol for infertile couples away from medical information overload.</p> <p>Methods</p> <p>A comprehensive review where the literature was searched for "Management of infertility and/or infertile couples" at library website of University of Bristol (MetaLib) by using a cross-search of different medical databases besides the relevant printed medical journals and periodicals. Guidelines and recommendations were retrieved from the best evidence reviews such as that from the American College of Obstetricians and Gynaecologists (ACOG), American Society for Reproductive Medicine (ASRM), Canadian Fertility and Andrology Society (CFAS), and Royal College of Obstetricians and Gynaecologists (RCOG).</p> <p>Results</p> <p>A simple guide for the clinicians to manage the infertile couples.</p> <p>Conclusions</p> <p>The study deploys a new strategy to translate the research findings and evidence-base recommendations into a simplified focused guide to be applied on routine daily practice. It is an approach to disseminate the recommended medical care for infertile couple to the practicing clinicians.</p
