    Picadas de escorpião na Turquia: aspectos epidemiológicos e clínicos entre os anos de 1995 e 2004

    The most important health-threatening scorpions found in Turkey are; Androctonus crassicauda, Leiurus quinquestriatus, Mesobuthus gibbosus and M. eupeus species, all of which belong to the Buthidae family. The epidemiological and clinical findings of scorpion stings in Turkey were evaluated between the years 1995 and 2004 based on data recorded in the National Poison Information Center (NPIC). A total of 930 cases were recorded. The cases mostly occurred in the month of July. The gender distribution was 50.22% female and 45.48% male. It was shown that the 20-29 age group presented more scorpion stings. Most of the stings occurred in Central Anatolia and Marmara regions of Turkey. Patients at the hospital showed signs of localized (pain, hyperemia, edema and numbness) and systemic effects (hyperthermia, nausea and vomiting, tachycardia, shivering and lethargy) but no lethality was notified. According to records, 33% of the poisoned patients were treated with antivenin in healthcare facilities.Os mais importantes escorpiões que comprometem a saúde na Turquia são: Androctonus crassicauda, Leiurus quinquestriatus, Mesobuthus gibbosus e M. eupeus, todos eles pertencentes à família Buthidae. Os achados clínicos e epidemiológicos da picada de escorpiões na Turquia foram avaliados no período entre os anos de 1995 a 2004 baseados em dados do Centro Nacional de Informações de Envenenamentos (NPIC). Foram registrados 930 casos que ocorreram principalmente no mês de julho. A distribuição por sexo foi 50,22% femininos e 45,48% masculinos. Demonstrou-se que o grupo etário de 20 a 29 anos apresentou maior número de picadas de escorpião. A maior parte das picadas ocorreu na Anatólia Central e na região de Marmara, na Turquia. Os pacientes hospitalizados mostraram sinais localizados (dor, hiperemia, edema e confusão mental) e sistêmicos (hipertermia, náusea e vômitos, taquicardia, arrepios de frio e letargia), mas não houve notificação de letalidade. De acordo com os prontuários 33% dos pacientes foram tratados com antiveneno nas unidades de saúde

    A new approximate evaluation method for two-echelon inventory systems with emergency shipments

    We consider the control of repairable spare parts in a network consisting of a central warehouse, a central repair facility, and multiple local warehouses. Demands for spare parts occur at the local warehouses. If a local warehouse is out of stock, then an arriving demand is satisfied by an emergency delivery from the central warehouse or the central repair facility. Such emergency shipments are common practice for networks that support technical systems with high downtime costs, and it is important to take them into account when the inventory is optimized. Our main contribution consists of the development of a new approximate evaluation method. This method gives accurate approximations for the key performance measures, as we show via numerical analysis. The method is also fast and thus can easily be incorporated in existing (greedy) heuristic optimization methods. Our method outperforms the approximate evaluation method of Muckstadt and Thomas (1980), as we also show via the numerical analysis. Finally, we show that the performance of the system is rather insensitive to the leadtime distribution of the repairs at the central repair facility, which implies that our method works well for generally distributed repair leadtimes

    This article revisits British assessments on the Dardanelles during and after the Taba crisis in 1906. It is known that the assessments produced throughout 1906 were pessimistic. What is missing from the current historiography is the intelligence dimension. This paper fills this gap by arguing that the assessments were pessimistic because military and political assessors knew that the Dardanelles defences were strong. They knew this fact because British intelligence departments had compiled detailed and accurate information demonstrating that the Dardanelles defence system had been modernised and strengthened by the Ottomans over the preceding three decades

    Tree Growth Yield and Fruit Quality of Different Apple Cultivars Trained as Super Spindle

    This study was carried out on Topaz, Cooper 39 and Muscat apple cultivars grafted on M27 rootstocks under Tokat ecological conditions during 2008 - 2010. The trees planted in 2.0 m x 0.5 m row spacing (10,000 trees ha-1) and trained to super spindle (SS) training system. The vegetative development, yield and fruit quality performances of the trees supported by wire – pole combination were observed for three years. At the end of the experiment, it was determined that Cooper 39 had a higher trunk cross sectional area (TCA) than Topaz and Muscat. While cumulative yield (CY) per tree and cumulative yield efficiency (CYE) were determined to be the highest in Cooper 39, these values were found to be the lowest in Topaz. CY per hectare over the first three crop year was found to be the highest in Cooper 39 and the yield reached to 142.9 t.ha1. The lowest CY per hectare (69.6 ton ha-1) was determined in Topa

    An improved method of supercharged transposed latissimus dorsi flap with the skin paddle for the management of a complicated lumbosacral defect

    OBJECTIVE: Treatment of nonhealing wounds of lower back often poses a powerful challenge. We present one of the first report of treatment of a lumbosacral defect with a supercharged latissimus dorsi flap with the skin paddle. CASE REPORT: We report a case of a 59 yearold man with myeloma of the sacral spine who underwent radiotherapy and chemotherapy and subsequently, laminectomies and placement of hardware for ongoing paresis and spine instability. Then, he developed an open wound and osteomyelitis of the spine with culture positive tuberculous granulomas. After multiple surgical debridement, he presented to our service and was treated with a single stage debridement followed by the performance of a latissimus dorsi musculocutaneous flap based on paraspinal perforators and supercharged. RESULTS: This solution, allowed for augmentation of blood flow to the muscle with the inferior gluteal artery, provided coverage of the defect resistant to the pressure, and simplified post-operative management of the patient. CONCLUSIONS: Alternative treatment options, including free tissue transfer, posed difficulties in finding suitable recipient vessels near the defect, in inserting the flap so as to restore its original length without compromising blood flow, and in postoperative care of the patient. Treatment of a lumbosacral defect with a supercharged latissimus dorsi flap with the skin paddle may represent a milestone procedure for complicated lower spine wounds

    The effect of polyethylene glycol (PEG 8000) supplementation on in vitro gas production kinetics of leaves from tannin containing trees

    The objective of this study was to determine the effect of inclusion of polyethylene glycol (PEG 8000) during in vitro incubation on gas production kinetics, organic matter digestibility (OMD) and the metabolisable energy (ME) content of foliage from the tannin containing tree species, Pistica lentiscus, Arbutus andrachne and Juniperus communis. The amount of gas produced when the foliage was incubated with buffered rumen fluid, was determined after 0, 3, 6, 12, 24, 48, 72 and 96 h of incubation in the presence of PEG at inclusions rates of 15, 30, 60 and 90 mg and in the absence of PEG. Their kinetics were described using the equation p = a + b (1-e-ct). Addition of PEG resulted in an increased gas production at almost all incubation times in all tree species. However species showed variable responses. After 3 h of incubation the PEG addition showed no significant effect on gas production when the foliage from A. andrachne was incubated, but had a significant effect on gas production as duration of incubation extended. The increase in gas production in response to increased levels of PEG inclusion was linear for P. lentiscus and J. communis. However, when the PEG inclusion rates exceeded 60 mg there was no significant increase in gas production when A. andrachne was incubated. The estimated parameters such as gas production rate(c) and gas production (a) from the immediately soluble fraction were not affected by the level PEG treatment, except that PEG addition at 90 mg had a significant effect on the gas production (a) from immediately soluble fraction of leaves of J. communis. Gas production (b) from the insoluble fraction (mL) and potential gas production (a+b), OMD and ME of tree leaves increased significantly with increasing levels of PEG addition. However, when PEG inclusion exceeded 60 mg these parameters showed no significant increase when leaves from A. andrachne were incubated. Although the mean increase in OMD per mg PEG supplementation was 0.131 digestibility units, the increase in ME per mg PEG supplementation was 0.0201 ME units. The elevated levels of gas produced, and increased OMD and ME estimates with the inclusion of PEG demonstrated the negative effect of tannins in foliage on digestibility.     Effect of cultivar and formaldehyde treatment of barley grain on rumen fermentation characteristics using in vitro gas production

    The aim of this study was to determine the effects of cultivar and formaldehyde treatment of barley grains on rumen fermentation characteristics using the in vitro gas production technique. Amount of gas produced (mL/g organic matter (OM)) during fermentation was determined after 0, 3, 6, 12, 24, 48, 72 and 96 h of incubation in buffered rumen fluid. The gas production kinetics were described using the equation: y = A {1 – exp [- b (t-T) – c (√t - √T)]} where b and c are the initial gas production rate constant (h-1) and later gas production rate constant (h-1/2), respectively. Cultivar and formaldehyde treatment had significant effects on gas production kinetics. Total gas production (A) ranged from 389.9 to 410.8 (mL/g OM) with the cultivar, Esterel, producing the largest volume of gas of the cultivars. Due to low gas production rates at 3, 6 and 12 h of incubation the cultivars, Viva and Cecilla, took the longest to produce 50% of their total volume of gas. Formaldehyde treatment reduced the rate (μ) of gas production at 3, 6 and 12 h of incubation, and the total volume of gas (A), but increased the time (h) to produce 50% of A and reduced the time (h) to produce 95% of A. The reduction in gas production ranged from 33.3 to 51 mL/g OM with 6 h incubation showing the highest decrease in gas production. It is concluded that formaldehyde treatment may provide an opportunity to manipulate the site of digestion of barley grain in the digestive tract of ruminants. Through the selection of suitable cultivars and through formaldehyde treatment the nutritional and health problems associated with the fermentation of barley grain in the rumen could be reduced.     Snakebites in Turkey: epidemiological and clinical aspects between the years 1995 and 2004

    The venom of poisonous snakes comprises a complex mixture of several proteins with other less significant constituents, resulting in principles capable of changing viable tissues. The hemotoxic factor is the main responsible for necrosis and tissue sloughing. Envenomations are common in rural areas of Turkey caused by snake species that present hepatotoxic venom, which causes local swelling, ecchymosis and alterations in blood profile. The epidemiological and clinical findings of snake envenomations in Turkey were evaluated based on data recorded by the National Poison Information Center (NPIC) between 1995 and 2004, in a total of 550 snakebite cases. The month of peak incidence was June (24.3%) while most incidents occurred in Marmara, Central Anatolia and Black Sea regions of Turkey. The victims were mainly adults (54.1%). Hospitalized patients displayed clinical signs of local (75.2%) and systemic effects (24.7%). Local clinical symptoms comprised edema, pain, hyperemia, numbness and ecchymosis, while systemic clinical symptoms included nausea, vomiting, hypotension, tachycardia, dyspnea, dry mouth, paresthesia, generalized edema, cyanosis and compartment syndrome. Occasionally, convulsions, confusion, loss of consciousness, hyperthermia, hepatic and circulation failure, hematoma, drowsiness, epistaxis, chest and abdominal pain, venous spasm, thrombocytopenia and bradycardia were recorded. Approximately one third (34.2%) of the patients were treated symptomatically, while 10.5% required antivenom therapy along with symptomatic treatments and 26.3% of all patients were exclusively treated with antivenom. Although a significant number of incidents were reported, no deaths occurred. These findings emphasize the presence of multiple medically important snake species in Turkey and that public awareness and therapeutic approaches appear sufficient to manage snakebite incidents

    Fourier transform infrared spectroscopy as a novel approach for analyzing the biochemical effects of anionic surfactants on a surfactant-degrading acrobacter butzleri strain

The A. butzleri isolate displayed efficient surfactant-biodegrading capacity for sodium dodecyl sulfate (SDS) at concentrations of up to 100 mg/L in 6 days, corresponding to 99.0% removal efficiency. Fourier transform infrared spectroscopy was applied to observe the effects of varying concentrations of SDS on the biochemistry of bacterial cells.
