    Patients With Kidney Cancer

    To develop a preoperative prognostic model in order to predict recurrence-free survival in patients with nonmetastatic kidney cancer.A multi-institutional data base of 1889 patients who underwent surgical resection between 1987 and 2007 for kidney cancer was retrospectively analyzed. Preoperative variables were defined as age, gender, presentation, size, presence of radiological lymph nodes and clinical stage. Univariate and multivariate analyses of the variables were performed using the Cox proportional hazards regression model. A model was developed with preoperative variables as predictors of recurrence after nephrectomy. Internal validation was performed by Harrells concordance index.The median follow-up was 23.6 months (1222 months). During the follow-up, 258 patients (13.7) developed cancer recurrence. The median follow-up for patients who did not develop recurrence was 25 months. The median time from surgery to recurrence was 13 months. The 5-year freedom from recurrence probability was 78.6. All variables except age were associated with freedom from recurrence in multivariate analyses (P 0.05). Age was marginally significant in the univariate analysis. All variables were included in the predictive model. The calculated c-index was 0.747.This preoperative model utilizes easy to obtain clinical variables and predicts the likelihood of development of recurrent disease in patients with kidney tumors

    Molekularna dijagnostika i serološka istraživanja pestivirusa u ovaca.

    In this research, the virological and serological presence of pestiviruses, such as border disease virus (BDV) was investigated in a sheep in the Central and Eastern Black Sea Regions of Turkey. The study material consisted of 40 organ materials collected from 13 aborted lambs, which were suspected to have pestiviruses, BDV and Bovine Viral Diarrhoea Virus (BVDV). Viral nucleic acids were investigated by using the reverse transcription polymerase chain reaction (RT-PCR). Nine of 13 aborted lambs (69.2%) and 24 of 40 organ samples (60%) obtained from those sheep were BDV RNA positive, whilst all the animals and organ samples were BVDV-RNA negative. Serum samples collected from 401 randomly selected sheep were investigated for pestivirus antibodies using competitive ELISA (cELISA) and the serum microneutralisation test (SNT) using BVDV-NADL strain. Seropositivity was found in between 7.22 and 74.38% with cELISA and 4.81 and 67.76% with SNT. Seropositivity rates in Amasya and Tokat provinces were higher than in Samsun and Giresun. The obtained data indicated that pestivirus infection in sheep is widespread in the Central and Eastern Black Sea Region.U radu su prikazana virološka i serološka istraživanja pestivirusa odnosno borderske bolesti u ovaca na središnjem i istočnom području Crnoga mora u Turskoj. Ukupno je bilo pretraženo 40 uzoraka tkiva različitih organa uzetih od 13 pobačenih plodova pod sumnjom da je uzročnik bio virus borderske bolesti ili virus virusnog proljeva goveda. Uzorci su bili pretraženi na prisutnost virusne nukleinske kiseline lančanom reakcijom polimerazom uz prethodnu reverznu transkripciju (RT-PCR). Devet od 13 pobačenih plodova (69,2%) i 24 od 40 uzoraka tkiva organa (60%) sadržavalo je RNA virusa borderske bolesti. Svi pretraženi uzorci bili su negativni na virus virusnoga proljeva. Uzorci seruma nasumce prikupljeni od 401 ovce bili su pretraženi na prisutnost specifičnih protutijela uporabom kompetitivnoga imunoenzimnoga testa (cELISA) i serum neutralizacijskoga testa (SNT) sa sojem NADL virusa virusnoga proljeva goveda. Ustanovljeno je da sepostotak serološki pozitivnih cELISA-om kretao od 7,22 do 74,38%, a SNT-om od 4,81 do 67,76%. Postotak serološki pozitivnih životinja u provincijama Amasya i Tokat bio je veći nego u provincijama Samsun i Giresun. Rezultati upućuju na zaključak da su pestivirusne zaraze rasprostranjene u ovaca na središnjem i istočnom području Crnoga mora

    Dealing with fuzziness in active mobile database systems

    Current needs of industry required the development of advanced database models like active mobile database systems. An active mobile database system can be designed by incorporation of triggering rules into a mobile computing environment in which the users are able to access a collection of database services using mobile and non-mobile computers at any location. Fuzzy concepts are adapted to the field of databases in order to deal with ambiguous, uncertain data. Fuzziness comes into picture in active mobile databases especially with spatial queries on moving objects. Incorporating fuzziness into rules would also improve the effectiveness of active mobile databases as it provides much flexibility in defining rules for the supported application. In this paper we present some methods to adapt the concepts developed for fuzzy systems to active mobile databases

    Rule-based inference and decomposition for distributed in-network processing in wireless sensor networks

    Wireless sensor networks are application specific and necessitate the development of specific network and information processing architectures that can meet the requirements of the applications involved. A common type of application for wireless sensor networks is the event-driven reactive application, which requires reactive actions to be taken in response to events. In such applications, the interest is in the higher-level information described by complex event patterns, not in the raw sensory data of individual nodes. Although the central processing of information produces the most accurate results, it is not an energy-efficient method because it requires a continuous flow of raw sensor readings over the network. As communication operations are the most expensive in terms of energy usage, the distributed processing of information is indispensable for viable deployments of applications in wireless sensor networks. This method not only helps in reducing the total amount of packets transmitted in the network and the total energy consumed by the sensor nodes, but also produces scalable and fault-tolerant networks. For this purpose, we present two schemes that distribute information processing to appropriate nodes in the network. These schemes use reactive rules, which express relations between event patterns and actions, in order to capture reactive behavior. We also share the results of the performance of our algorithms and the simulations based on our approach that show the success of our methods in decreasing network traffic while still realizing the desired functionality. © 2016, Springer-Verlag London

    Investigation of temporal bone asymmetry in cases with unilateral tinnitus: morphometric and multicentric clinical study

    The aim of this multicentric study was to compare the anatomical structures of temporal bone in patients with unilateral tinnitus with their healthy ears. We also aimed at evaluating whether age and gender-related asymmetrical changes occur in temporal bones or not. Fifty two ears of 26 patients who had unilateral tinnitus were included into the retrospective study. The patients who had subjective nonpulsatile tinnitus and who previously had temporal computed tomography according to their file records were accepted to study. Temporal CT scans and audiometric results of patients were examined retrospectively. Middle ear volume, diameter of internal acoustic meats and diameter of jugular bulb were evaluated by both anatomist and radiologist, interobserverly. Internal acoustic meats and jugular bulb were found larger in the ears that had tinnitus than healthy ears; however, there was no statistically significance. The stereological morphometrical study of temporal bone asymmetry in humans correlate with sex is of importance for both otolarygologs and anatomists. These results will contribute to data on middle ear volume, internal acustic meats and jugular bulb sizes

    Extra-hepatic fascioliasis with peritoneal malignancy tumor feature

    Fascioliasis is a zoonose parasitic disease caused by Fasciola hepatica and Fasciola gigantica and is widespread in most regions of the world. Ectopic fascioliasis usually caused by juvenile Fasciola spp., but in recent years a few cases of tissue-embedded ova have been reported from different endemic areas. A 79-year-old Iranian man resident in Eird-e-Mousa village from Ardabil Province, north-west of Iran, complained with abdominal pain, nausea, and intestinal obstruction symptoms referred to Ardabil Fatemi hospital. In laparotomy multiple intestinal masses with peritoneal seeding resembling of a malignant lesion were seen. After appendectomy and peritoneal mass biopsy with numerous intraperitoneal adenopathy, paraffin embedded blocks were prepared from each tissues. A blood sample was taken from the patient 5 months later for serological diagnosis. Histopathological examination of sections showed fibrofatty stroma with dense mixed inflammatory cells infiltration and fibrosis in peritoneal masses. Large numbers of ova of Fasciola spp. were noted with typical circumscribed granulomas. Despite of anti-fasciola treatment, IHA test for detecting anti F. hepatica antibodies was positive 5 months after surgery with a titer of 1/128. Due to multiple clinical manifestation of extra-hepatic fascioliasis, its differential diagnosis from intraperitoneal tumors or other similar diseases should be considered

    Mass Dependence of M3Y-Type Interactions and the Effects of Tensor Correlations

    The mass dependence of the M3Y-type effective interactions and the effects of tensor correlations are examined. Two-body nuclear matrix elements are obtained by the lowest order constrained variational (LOCV) technique with and without tensor correlations. We have found that the tensor correlations are important especially in the triplet-even (TE) and tensor-even (TNE) channels in order to reproduce the G-matrix elements obtained previously. Then M3Y-type potentials for inelastic scattering are obtained by fitting our two-body matrix elements to those of a sum of Yukawa functions for the mass numbers A=24, A=40 and A=90.Comment: 13 pages, 6 table

    Unusual metastases of lung cancer: bulbus oculi and maxillary sinus

    Lung adenocarcinoma often makes metastasis to the brain, liver, kidneys, bone, bone marrow and adrenal glands. It can also make metastasis to other parts of the body rarely for example eye, nose, parotid gland and paranasal sinus. We did not encounter with combined ocular bulbus and the maxillary sinus metastases of lung cancer in the accessible literature. In this case report, a patient who was combined ocular bulbus and the maxillary sinus metastases of lung adenocarcinoma will be discussed.     Accessory spleen is a congenital form of an ectopic splenic tissue. In this report, we present a case of a patient who was followed with the diagnosis of rectal and sigmoid colon cancer and an accessory spleen hypertrophy, which was thought to be colon cancer metastasis in the left hypochondriac region. After colectomy and splenectomy, accessory spleen that mimics cancer metastasis was diffrentially diagnosed using scintigraphy
