    Soluble FAS and transforming growth factor beta 2 in infections with viral and bacterial zoonoses

    Određivali smo razine sFas (signalna receptor molekula koja biva otpuštena tijekom apoptoze u cirkulirajućem obliku) i TGF-β2 (multifunkcionalni citokin) u bolesnika s različitim zoonozama. Serumi su dobiveni od bolesnika hospitaliziranih u Klinici za infektivne bolesti »Dr. Fran Mihaljević«, Zagreb, Hrvatska, s dijagnozama: hemoragijska vrućica s bubrežnim sindromom – HVBS (n=20), leptospiroza (n=9), borelioza (n=7) ili krpeljni meningoencefalitis – KME (n=7). Za mjerenje razina sFas i TGF-β2 u serumima koristili smo ELISA kitove prema uputama proizvođača. Značajno povišena razina sFas je nađena u svih bolesnika. Ipak, bolesnici s KME ili boreliozom imali su značajno niže razine nego bolesnici s leptospirozom. Nasuprot tome, bolesnici s KME ili boreliozom su imali značajno više razine TGF-β2 nego zdrave kontrole. Naši preliminarni podaci ukazuju da bi povišene razine sFas i TGF-β2 mogle imati ulogu u imunopatogenezi ispitivanih virusnih i bakterijskih zoonoza.We determined levels of sFas (signal receptor molecule released during apoptosis in a soluble circulating form) and TGF-β2 (multifunctional cytokine) in patients with different zoonoses. Serum samples were obtained from patients hospitalized at the University Hospital for Infectious Diseases, Zagreb, Croatia with diagnoses of hemorrhagic fever with renal syndrome – HFRS, (n=20), leptospirosis (n=9), Lyme borreliosis (n=7), or tick-borne encephalitis – TBE (n=7). In all sera samples the levels of sFAS or TGF-β2 levels were measured using ELISAkits according to the manufacturer\u27s protocol. Significant increase in the levels of sFas was found in all patients. However, the patients with TBE or Lyme borreliosis had significantly lower levels than patients with leptospirosis. In contrary, patients with TBE or Lyme borreliosis had significantly higher levels of TGF-β2 than healthy controls. Our preliminary data indicate that an increase of sFas and TGF-β2 levels may have a role in the immunopathogenesis of tested viral and bacterial zoonoses

    Interopservacijske razlike u dijagnosticiranju azbestoze prema klasifikaciji ILO

    Inhalation of asbestos fibres leads to asbestosis of the pleura and the lung, with possible progression to lung cancer and malignant pleural or peritoneal mesothelioma. Asbestosis remains difficult to diagnose, especially in its early stages. The most important role in its diagnosis is that of chest radiographs. The aim of this cross-sectional study was to address interobserver variations in interpreting chest radiographs in asbestos workers, which remain to be an issue, despite improvements in the International Labour Office (ILO) classification system. In our ten-year study, we investigated 318 workers occupationally exposed to asbestos, and in 210 workers with diagnosed asbestos-related changes we compared interpretations of chest radiographs according to ILO by two independent radiologists. The apparent degree of interobserver variation in classifying lung fibrosis was 26.66 % for the diameter of changes and 42.2 % for the profusion of the changes. In cases with diffuse pleural thickening, the interobserver variation using ILO procedures was 34.93 %. This investigation raises the issue of standardisation and objectivity of interpretation of asbestosis according to the ILO classification system. This study has revealed a significant disagreement in the estimated degree of pleural and parenchymal asbestos pulmonary disease. This is why we believe highresolution computed tomography (HRCT) should also be used as a part of international classification.Azbestoza je bolest koja nastaje kao posljedica inhalacije azbestnih vlakana. Zahvaća pluća i pleuru, a ponekad može dovesti do karcinoma pluća, pleure i peritoneuma. Azbestozu je teško dijagnosticirati, osobito u početnom stadiju bolesti. U dijagnostici azbestoze presudna je radiološka obrada. Cilj ovog rada je utvrditi interopservacijske razlike u analizi radiograma prema kriterijima klasifikacije ILO kod profesjonalne azbestoze. U istraživanje je bilo uključeno 318 ispitanika profesionalno izloženih azbestnim česticama. U 210 ispitanika kojima je postavljena dijagnoza azbestoze usporedili smo radiološke nalaze analizirane od dvaju neovisnih radiologa eksperata u skladu s važećom klasifikacijom ILO. Uspoređujući rezultate analize malih zasjenjenja parenhima pluća prema njihovoj veličini, našli smo da se rezultati razlikuju u 26,66 % slučajeva, dok je prema prožetosti intersticija neslaganje bilo još učestalije, tj. zabilježeno je u 42,22 % slučajeva. U ispitanika s pleuralnim promjenama usporedili smo rezultate analize difuznih pleuralnih zadebljanja i našli neslaganje u debljini i/ili širini pleuralnih zadebljanja u 34,93 % slučajeva. Uočili smo značajno neslaganje u procjeni stupnja azbestne bolesti na pleuri i na parenhimu pluća. Zbog toga smatramo da je u kvantificiranju promjena izazvanih azbestom i u procjeni stupnja tjelesnog oštećenja nužno rabiti kompjutoriziranu tomografiju visoke rezolucije kao znatno osjetljiviju i pouzdaniju slikovnu tehniku

    Internet of Things for Water Sustainability

    The water is a finite resource. The issue of sustainable withdrawal of freshwater is a vital concern being faced by the community. There is a strong connection between the energy, food, and water which is referred to as water-food-energy nexus. The agriculture industry and municipalities are struggling to meet the demand of water supply. This situation is particularly exacerbated in the developing countries. The projected increase in world population requires more fresh water resources. New technologies are being developed to reduce water usage in the field of agriculture (e.g., sensor guided autonomous irrigation management systems). Agricultural water withdrawal is also impacting ground and surface water resources. Although the importance of reduction in water usage cannot be overemphasized, major efforts for sustainable water are directed towards the novel technology development for cleaning and recycling. Moreover, currently, energy technologies require abundant water for energy production. Therefore, energy sustainability is inextricably linked to water sustainability. The water sustainability IoT has a strong potential to solve many challenges in water-food-energy nexus. In this chapter, the architecture of IoT for water sustainability is presented.     Intraperitoneal drain placement and outcomes after elective colorectal surgery: international matched, prospective, cohort study

    Despite current guidelines, intraperitoneal drain placement after elective colorectal surgery remains widespread. Drains were not associated with earlier detection of intraperitoneal collections, but were associated with prolonged hospital stay and increased risk of surgical-site infections.Background Many surgeons routinely place intraperitoneal drains after elective colorectal surgery. However, enhanced recovery after surgery guidelines recommend against their routine use owing to a lack of clear clinical benefit. This study aimed to describe international variation in intraperitoneal drain placement and the safety of this practice. Methods COMPASS (COMPlicAted intra-abdominal collectionS after colorectal Surgery) was a prospective, international, cohort study which enrolled consecutive adults undergoing elective colorectal surgery (February to March 2020). The primary outcome was the rate of intraperitoneal drain placement. Secondary outcomes included: rate and time to diagnosis of postoperative intraperitoneal collections; rate of surgical site infections (SSIs); time to discharge; and 30-day major postoperative complications (Clavien-Dindo grade at least III). After propensity score matching, multivariable logistic regression and Cox proportional hazards regression were used to estimate the independent association of the secondary outcomes with drain placement. Results Overall, 1805 patients from 22 countries were included (798 women, 44.2 per cent; median age 67.0 years). The drain insertion rate was 51.9 per cent (937 patients). After matching, drains were not associated with reduced rates (odds ratio (OR) 1.33, 95 per cent c.i. 0.79 to 2.23; P = 0.287) or earlier detection (hazard ratio (HR) 0.87, 0.33 to 2.31; P = 0.780) of collections. Although not associated with worse major postoperative complications (OR 1.09, 0.68 to 1.75; P = 0.709), drains were associated with delayed hospital discharge (HR 0.58, 0.52 to 0.66; P < 0.001) and an increased risk of SSIs (OR 2.47, 1.50 to 4.05; P < 0.001). Conclusion Intraperitoneal drain placement after elective colorectal surgery is not associated with earlier detection of postoperative collections, but prolongs hospital stay and increases SSI risk

    The problem of cutting difficult-to-machine materials used in the aerospace industry, aircraft industry and automobile industry, led to the development and application of today the most attractive technology for contour cutting - Abrasive Water Jet Cutting. Besides high accuracy, flexibility, and low material loss, abrasive water jet cutting can be described as 'ecological' or 'clean' technology over inert and abundant main processing substances (water, sand) and absence of airborne dust particles, smoke and fumes. The recycling of the abrasives makes the process more economical (savings of 50% in operating costs), effective and environmentally friendly (reduction in waste material). It is widely understood that modern, intensive agricultural production is based on the exploitation of natural, primarily biological, chemical and physical resources. A model indicator of environmental sustainability in rural areas and agricultural production in the form of the index is given. It contains 10 main indicators, where each indicator contains 10 variables. The indicators on which the index is calculated are: soil quality; water quality; air quality; external effects (geographic, climatic, edaphic, and others); the need for growing plant and animal products; biological efficiency; dependence on external sources of energy (biofuel); human participation component (inclination to work, training for work in rural areas); social aspects and economic impacts.     Sagittal osteotomy of the patella after Morscher

    The aim of this study was to present long-term results of sagittal osteotomy of the patella after Morscher. The study included 70 patients treated for patellar dysplasia with a postoperative follow-up from nine to 20 years. There were 59 females and 11 males with an average age of 21 years at the time of operation. Results were evaluated for the entire group of 70 patients and then presented separately for the 30 patients in whom sagittal osteotomy of the patella was performed as a single procedure and 40 patients in whom, in addition to sagittal osteotomy of the patella, medialisation and ventralisation or distalisation of the tibial tuberosity were also performed. The mean value of the Wiberg patellar angle was 112 degrees preoperatively and 140 degrees postoperatively. In the whole group of patients excellent results were obtained in 24 (35%), good results in 30 (42%), fair results in 13 (19%) and poor results in three (4%) patients. In 30 patients with sagittal osteotomy as a single operation excellent results were obtained in 13 (43%), good results in 14 (47%) and fair results in three (10%) patients, while in the 40 patients with sagittal osteotomy and additional surgical procedures, 11 (27%) were excellent, 16 (40%) good, ten (25%) fair and three (8%) had poor results. Sagittal osteotomy of the patella after Morscher is an enrichment of the spectrum of the secondary-causal and preventive procedures for the treatment of patello-femoral disorders

    Short term priming effect of brain-actuated muscle stimulation using bimanual movements in stroke

    Objective: Brain-computer interface triggered-functional electrical stimulation (BCI-FES) is an emerging neurorehabilitation therapy post stroke, mostly for the affected hand. We explored the feasibility of a bimanual BCI-FES and its short-term priming effects, i.e. stimuli-induced behaviour change. We compared EEG parameters between unimanual and bimanual movements and differentiated the effect of age from the effect of stroke. Methods: Ten participants with subacute stroke, ten age-matched older healthy adults, and ten younger healthy adults underwent unimanual and bimanual BCI-FES sessions. Delta alpha ratio (DAR) and brain symmetry index (BSI) were derived from the pre- and post- resting-state EEG. Event-related desynchronization (ERD) and laterality index were derived from movement- EEG. Results: Participants were able to control bimanual BCI-FES. ERD was predominantly contralateral for unimanual movements and bilateral for bimanual movements. DAR and BSI only changed in healthy controls. Baseline values indicated that DAR was affected by stroke while BSI was affected by both age and stroke. Conclusions: Bimanual BCI control offers a larger repertoire of movements, while causing the same short-term changes as unimanual BCI-FES. Prolonged practice may be required to achieve a measurable effect on DAR and BSI for stroke. Significance: Bimanual BCI-FES is feasible in people affected by stroke
