    Effectiveness of enhanced diabetes care to patients of South Asian ethnicity : the United Kingdom Asian Diabetes Study (UKADS) : a cluster randomised controlled trial

    Background: Delivering high quality and evidence based healthcare to deprived sectors of the community is a major goal for society. We investigated the effectiveness of a culturally sensitive enhanced care package in UK general practice in improving cardiovascular risk factors in South Asian patients with type 2 diabetes. Methods: 21 inner city practices were randomised to intervention (enhanced practice nurse time, link worker and diabetes specialist nurse support) (n=868) or control (standard care) (n=618) groups. Prescribing algorithms with clearly defined targets were provided for all practices. Main outcome measures comprised changes in blood pressure, total cholesterol and glycaemic control (HbA1c) after 2 years. Findings: At baseline, groups were similar with respect to age, sex and cardiovascular risk factors. Comparing treatment groups, after adjustment for confounders, and clustering, differences in diastolic blood pressure (1.91mmHg, P=0.0001) and mean arterial pressure (1.36mmHg, P=0.0180) were significant. There were no significant differences between groups for total cholesterol or HbA1c. Economic analysis indicates the nurse-led intervention was not cost-effective. Across the whole study population systolic blood pressure, diastolic blood pressure and cholesterol decreased significantly by 4.9mmHg, 3.8mmHg and 0.45mmol/L respectively, but there was no change in HbA1c. Interpretation: Additional, although limited, benefits were observed from our culturally enhanced care package over and above the secular changes achieved in the UK in recent years. Stricter targets in general practice and further measures to motivate patients are needed to maximise healthcare outcomes in South Asian patients with diabetes

    Biodegradation of wool by bacteria and fungi and enhancement of wool quality by biosurfactant washing

    Biodegradation of Ryeland and Shetland wool by Bacillus subtilis W3 and Streptomyces albidoflavus were investigated. The effect of treating raw wool with Rhamnolipid was also studied. It is shown that the wool surface morphology is improved with effective displacement of surface contaminants revealing a smooth outer cuticle layer after just 2 days. These results have important practical implications for the establishment of a quick and easy biodegradable process for wool scouring finishing in textile industry or for the pre-treatment of keratinous waste materials before degradation by bacteria or fungi. This methodology provides an environmentally friendly alternative to conventional chemical pre-treatments

    Search Behaviour On Photo Sharing Platforms

    The behaviour, goals, and intentions of users while searching for images in large scale online collections are not well understood, with image search log analysis providing limited insights, in part because they tend only to have access to user search and result click information. In this paper we study user search behaviour in a large photo-sharing platform, analyzing all user actions during search sessions (i.e. including post result-click pageviews). Search accounts for a significant part of user interactions with such platforms, and we show differences between the queries issued on such platforms and those on general image search. We show that search behaviour is influenced by the query type, and also depends on the user. Finally, we analyse how users behave when they reformulate their queries, and develop URL class prediction models for image search, showing that query-specific models significantly outperform query-agnostic models. The insights provided in this paper are intended as a launching point for the design of better interfaces and ranking models for image search. © 2013 IEEE.published_or_final_versio

    Eliminating the LIGO bounds on primordial black hole dark matter

    Primordial black holes (PBHs) in the mass range (30(30--100) M⊙100)~M_{\odot} are interesting candidates for dark matter, as they sit in a narrow window between microlensing and cosmic microwave background constraints. There are however tight constraints from the binary merger rate observed by the LIGO and Virgo experiments. In deriving these constraints, PBHs were treated as point Schwarzschild masses, while the more careful analysis in an expanding universe we present here, leads to a time-dependent mass. This implies a stricter set of conditions for a black hole binary to form and means that black holes coalesce much more quickly than was previously calculated, namely well before the LIGO/Virgo's observed mergers. The observed binaries are those coalescing within galactic halos, with a merger rate consistent with data.     Rapid measurement of lactate in exhaled breath condensate: biosensor optimisation and in-human proof-of-concept

    Lactate concentration is of increasing interest as a diagnostic for sepsis, septic shock, and trauma. Compared with the traditional blood sample media, the exhaled breath condensate (EBC) has the advantages of non-invasiveness and higher user acceptance. An amperometric biosensor was developed and its application in EBC lactate detection was investigated in this paper. The sensor was modified with PEDOT:PSS-PB, and two different lactate oxidases (LODs). A rotating disk electrode and Koutecky–Levich analysis were applied for the kinetics analysis and gel optimization. The optimized gel formulation was then tested on disposable screen-printed sensors. The disposable sensors exhibited good performance and presented a high stability for both LOD modifications. Finally, human EBC analysis was conducted from a healthy subject at rest and after 30 min of intense aerobic cycling exercise. The sensor coulometric measurements showed good agreement with fluorometric and triple quadrupole liquid chromatography mass spectrometry reference methods. The EBC lactate concentration increased from 22.5 ÎŒM (at rest) to 28.0 ÎŒM (after 30 min of cycling) and dropped back to 5.3 ÎŒM after 60 min of rest

    Management of imatinib-resistant CML patients

    Imatinib has had marked impact on outcomes in chronic myelogenous leukemia (CML) patients for all stages of the disease and is endorsed by international treatment guidelines as the first line option. Although imatinib is highly effective and well tolerated, the development of resistance represents a clinical challenge. Since the most frequently identified mechanism of acquired imatinib resistance is bcr-abl kinase domain point mutations, periodic hematologic, cytogenetic, and molecular monitoring is critical throughout imatinib therapy. Once cytogenetic remission is achieved, residual disease can be monitored by bcr-abl transcript levels as assayed by reverse transcription polymerase chain reaction (RT-PCR). Detection of bcr-abl mutants prior to and during imatinib therapy can aid in risk stratification as well as in determining therapeutic strategies. Thus, mutation screening is indicated in patients lacking or losing hematologic response. Moreover, search for mutations should also be performed when a 3-log reduction of bcr-abl transcripts is not achieved or there is a reproducible increase of transcript levels. In patients harboring mutations which confer imatinib resistance, novel second line tyrosine kinase inhibitors have demonstrated encouraging efficacy with low toxicity. Only the T315I bcr-abl mutant has proved totally resistant to all clinically available bcr-abl inhibitors. Strategies to further increase the rates of complete molecular remissions represent the next frontier in the targeted therapy of CML patients

    Calpains mediate p53 activation and neuronal death evoked by DNA damage.

    DNA damage is an initiator of neuronal death implicated in neuropathological conditions such as stroke. Previous evidence has shown that apoptotic death of embryonic cortical neurons treated with the DNA damaging agent camptothecin is dependent upon the tumor suppressor p53, an upstream death mediator, and more distal death effectors such as caspases. We show here that the calcium-regulated cysteine proteases, calpains, are activated during DNA damage induced by camptothecin treatment. Moreover, calpain deficiency, calpastatin expression, or pharmacological calpain inhibitors prevent the death of embryonic cortical neurons, indicating the important role of calpain in DNA damage-induced death. Calpain inhibition also significantly reduced and delayed the induction of p53. Consistent with the actions of calpains upstream of p53 and the proximal nature of p53 death signaling, calpain inhibition inhibited cytochrome c release and DEVD-AFC cleavage activity. Taken together, our results indicate that calpains are a key mediator of p53 induction and consequent caspase-dependent neuronal death due to DNA damage
