    Design of a telescopic tower for wind energy production with reduced environmental impact

    A prototype of a telescopic pole for wind energy production with low environmental impact and its lifting system for a 60 to 250 kW turbine and a height of 30 m have been designed and manufactured. A telescopic tower, which is raised and lowered by automation or by remote control, allows to differentiate the presence of the generator within the landscape over time. The technology currently available for lifting and lowering wind turbines is made up of telescopic poles of heights of less than 10 meters and with tilting posts of height below 30 m. Without a state of the art to refer to, the telescopic pole and its lifting system have been designed starting from scratch and solving with innovative ideas the various criticalities that have arisen. The design of the telescopic coupling, the design for maintaining the preload and for the rotational decoupling, the optimization the design of the pairs of sleeves by numerical simulations, the design of the pegs and the bushes of the jack-up lifting system have been presented.     Active and Passive Brain-Computer Interfaces Integrated with Extended Reality for Applications in Health 4.0

    This paper presents the integration of extended reality (XR) with brain-computer interfaces (BCI) to open up new possibilities in the health 4.0 framework. Such integrated systems are here investigated with respect to an active and a passive BCI paradigm. Regarding the active BCI, the XR part consists of providing visual and vibrotactile feedbacks to help the user during motor imagery tasks. Therefore, XR aims to enhance the neurofeedback by enhancing the user engagement. Meanwhile, in the passive BCI, user engagement monitoring allows the adaptivity of a XR-based rehabilitation game for children. Preliminary results suggest that the XR neurofeedback helps the BCI users to carry on motor imagery tasks with up to 84% classification accuracy, and that the level of emotional and cognitive engagement can be detected with an accuracy greater than 75%

    A finite element model of abdominal human tissue for improving the accuracy in insulin absorption assessment: A feasibility study

    A Finite Element Model of the human abdomen biomechanics for patients undergoing diabetes therapies was developed. In particular, FEM was used to improve a previous insulin absorption measurement method based on bioimpedance spectroscopy (BIS). As a matter of facts, the noise introduced during the insulin injection phase significantly affects the BIS measurements. The noise, due to the pressure exerted on the abdomen tissue, arises sensibility issues on the signal correlated to the drug presence under the skin. In this study, the abdomen is modeled with three layers (skin, fat and muscle). A feasibility study about the decoupling of the mechanical deformation and the electrical dynamics is presented in order to model the effect of mechanical uncertainty sources (e.g., pressure exerted during the injection phase and/or breathing) on the impedance measurements. The proposed simplified model is realised by referring to the average values of skin, fat and muscle thickness, along with mechanical abdomen parameters al-ready presented and validated in scientific literature. The obtained results confirm the possibility to decouple me-chanical and electrical analyses when the excitation voltage is characterized by a frequency higher than 1 kHz. The results will be used to improve the accuracy of an exhaustive approach, already developed by the authors, for real-time insulin absorption measurement

    Effect of Dewaxed Coffee on Gastroesophageal Symptoms in Patients with {GERD}: A Randomized Pilot Study

    Gastroesophageal Reflux Disease (GERD) is multifactorial pathogenesis characterized by the abnormal reflux of stomach contents into the esophagus. Symptoms are worse after the ingestion of certain foods, such as coffee. Hence, a randomized pilot study conducted on 40 Italian subjects was assessed to verify the effect of standard (SC) and dewaxed coffee (DC) consumption on gastroesophageal reflux symptoms and quality of life in patients with gastrointestinal diseases. The assessment of patient diaries highlighted a significant percentage reduction of symptoms frequency when consuming DC and a significant increase in both heartburn-free and regurgitation-free days. Consequentially, patients had a significant increase of antacid-free days during the DC assumption. Moreover, the polyphenolic profile of coffee pods was ascertained through UHPLC-Q-Orbitrap HRMS analysis. Chlorogenic acids (CGAs) were the most abundant investigated compounds with a concentration level ranging between 7.316 (DC) and 6.721 mg/g (SC). Apart from CGAs, caffeine was quantified at a concentration level of 5.691 mg/g and 11.091 for DC and SC, respectively. While still preliminary, data obtained from the present pilot study provide promising evidence for the efficacy of DC consumption in patients with GERD. Therefore, this treatment might represent a feasible way to make coffee more digestible and better tolerated

    Italian intersociety consensus on management of long covid in children

    Background: Two sequelae of pediatric COVID-19 have been identified, the multisystem inflammatory syndrome in children (MIS-C) and the long COVID. Long COVID is much less precisely defined and includes all the persistent or new clinical manifestations evidenced in subjects previously infected by SARS-CoV-2 beyond the period of the acute infection and that cannot be explained by an alternative diagnosis. In this Intersociety Consensus, present knowledge on pediatric long COVID as well as how to identify and manage children with long COVID are discussed. Main findings: Although the true prevalence of long COVID in pediatrics is not exactly determined, it seems appropriate to recommend evaluating the presence of symptoms suggestive of long COVID near the end of the acute phase of the disease, between 4 and 12 weeks from this. Long COVID in children and adolescents should be suspected in presence of persistent headache and fatigue, sleep disturbance, difficulty in concentrating, abdominal pain, myalgia or arthralgia. Persistent chest pain, stomach pain, diarrhea, heart palpitations, and skin lesions should be considered as possible symptoms of long COVID. It is recommended that the primary care pediatrician visits all subjects with a suspected or a proven diagnosis of SARS-CoV-2 infection after 4 weeks to check for the presence of symptoms of previously unknown disease. In any case, a further check-up by the primary care pediatrician should be scheduled 3 months after the diagnosis of SARS-CoV-2 infection to confirm normality or to address emerging problems. The subjects who present symptoms of any organic problem must undergo a thorough evaluation of the same, with a possible request for clinical, laboratory and / or radiological in-depth analysis in case of need. Children and adolescents with clear symptoms of mental stress will need to be followed up by existing local services for problems of this type. Conclusions: Pediatric long COVID is a relevant problem that involve a considerable proportion of children and adolescents. Prognosis of these cases is generally good as in most of them symptoms disappear spontaneously. The few children with significant medical problems should be early identified after the acute phase of the infection and adequately managed to assure complete resolution. A relevant psychological support for all the children during COVID-19 pandemic must be organized by health authorities and government that have to treat this as a public health issue

    Dasatinib-Blinatumomab for Ph-Positive Acute Lymphoblastic Leukemia in Adults

    BACKGROUND: Outcomes in patients with Philadelphia chromosome (Ph)-positive acute lymphoblastic leukemia (ALL) have improved with the use of tyrosine kinase inhibitors. Molecular remission is a primary goal of treatment.METHODS: We conducted a phase 2 single-group trial of first-line therapy in adults with newly diagnosed Ph-positive ALL (with no upper age limit). Dasatinib plus glucocorticoids were administered, followed by two cycles of blinatumomab. The primary end point was a sustained molecular response in the bone marrow after this treatment.RESULTS: Of the 63 patients (median age, 54 years; range, 24 to 82) who were enrolled, a complete remission was observed in 98%. At the end of dasatinib induction therapy (day 85), 29% of the patients had a molecular response, and this percentage increased to 60% after two cycles of blinatumomab; the percentage of patients with a molecular response increased further after additional blinatumomab cycles. At a median follow-up of 18 months, overall survival was 95% and disease-free survival was 88%; disease-free survival was lower among patients who had an IKZF1 deletion plus additional genetic aberrations (CDKN2A or CDKN2B, PAX5, or both [i.e., IKZF1 plus]). ABL1 mutations were detected in 6 patients who had increased minimal residual disease during induction therapy, and all these mutations were cleared by blinatumomab. Six relapses occurred. Overall, 21 adverse events of grade 3 or higher were recorded. A total of 24 patients received a stem-cell allograft, and 1 death was related to transplantation (4%).CONCLUSIONS: A chemotherapy-free induction and consolidation first-line treatment with dasatinib and blinatumomab that was based on a targeted and immunotherapeutic strategy was associated with high incidences of molecular response and survival and few toxic effects of grade 3 or higher in adults with Ph-positive ALL.     The symposium “Putting the Whole Grain Puzzle Together: Health Benefits Associated with Whole Grains” sponsored by the ASN brought together researchers to review the evidence regarding the health benefits associated with whole grains. Current scientific evidence indicates that whole grains play an important role in lowering the risk of chronic diseases, such as coronary heart disease, diabetes, and cancer, and also contribute to body weight management and gastrointestinal health. The essential macro- and micronutrients, along with the phytonutrients present in whole grains, synergistically contribute to their beneficial effects. Current evidence lends credence to the recommendations to incorporate whole grain foods into a healthy diet and lifestyle program. The symposium also highlighted the need for further research to examine the role of whole grain foods in disease prevention and management to gain a better understanding of their mechanisms of action

    In the framework of bioimpedance spectroscopy-based insulin bioavailability assessment, a preliminary experimental campaign aimed to investigate the relationship between applied pressure and impedance variation was conducted. The experimental tests were carried out on a volunteer's abdomen in a controlled environment. They exploited four pre-gelled electrodes and a non- invasive instrument built with off-the-shelf components, namely DUSM, previously successfully used by the authors in different biomedical applications. Experimental results are compatible with the scientific literature, and show a progressive increase of the impedance magnitude according to an increasing external force which is comparable to the typical mechanical action of an insulin injection
