    Utility of irritable bowel syndrome subtypes and most troublesome symptom in predicting disease impact and burden

    Background Little is known about the characteristics of individuals with irritable bowel syndrome (IBS) according to stool subtype or the most troublesome symptom reported by the individual, or whether these are useful in predicting the impact of IBS. Methods We collected demographic, gastrointestinal, and psychological symptoms, healthcare usage and direct healthcare costs, impact on work and activities of daily living, and quality of life data from individuals with Rome IV-defined IBS. Key Results We recruited 752 people with Rome IV IBS. Individuals with IBS-D reported a poorer disease-specific quality of life than those with IBS-C or IBS-M (mean (SD) IBS-QOL 45.3 (23.0) for IBS-D, vs. 52.3 (19.9) for IBS-C, vs. 49.4 (22.0) for IBS-M, p = 0.005). Mean (SD) IBS-QOL scores were also lower amongst those who reported diarrhea (44.8 (22.3)) or urgency (44.6 (22.3)) as their most troublesome symptom, compared with those reporting abdominal pain (52.2 (22.9)), constipation (49.5 (21.8)), or abdominal bloating or distension (50.4 (21.3)). However, there were no differences in mean EQ-5D scores, IBS severity, levels of anxiety, depression, somatoform symptom-reporting, or gastrointestinal symptom-specific anxiety. Direct healthcare costs of IBS were similar across all subtypes and all most troublesome symptom groups, although some differences in work productivity and social leisure activities were detected. Conclusions and Inferences There appears to be limited variation in the characteristics of individuals with Rome IV IBS based on both stool subtypes and most troublesome symptom reported, suggesting that gastrointestinal symptoms alone have limited ability to predict disease impact and burden

    Association between hourly wages and dietary intake after the first phase of implementation of the Minneapolis minimum wage ordinance

    Objective: In 2018, Minneapolis began phased implementation of an ordinance to increase the local minimum wage to 15/h. We sought to determine whether the first phase of implementation was associated with changes in frequency of consumption of fruits and vegetables (F&V), whole-grain-rich foods, and foods high in added sugars among low-wage workers. Design: Natural experiment. Setting: The Wages Study is a prospective cohort study of 974 low-wage workers followed throughout the phased implementation of the ordinance (2018-2022). We used difference-in-difference analysis to compare outcomes among workers in Minneapolis, Minnesota, to those in a comparison city (Raleigh, North Carolina). We assessed wages using participants' pay stubs and dietary intake using the National Cancer Institute Dietary Screener Questionnaire. Participants: Analyses use the first two waves of Wages data (2018 (baseline), 2019) and includes 267 and 336 low-wage workers in Minneapolis and Raleigh, respectively. Results: After the first phase of implementation, wages increased in both cities, but the increase was 0·84 greater in Minneapolis (P = 0·02). However, the first phase of the policy's implementation was not associated with changes in daily frequency of consumption of F&V (IRR = 1·03, 95 % CI: 0·86, 1·24, P = 0·73), whole-grain-rich foods (IRR = 1·23, 95 % CI: 0·89, 1·70, P = 0·20), or foods high in added sugars (IRR = 1·13, 95 % CI: 0·86, 1·47, P = 0·38) among workers in Minneapolis compared to Raleigh. Conclusions: The first phase of implementation of the Minneapolis minimum wage policy was associated with increased wages, but not with changes in dietary intake. Future research should examine whether full implementation is associated dietary changes

    The eye of the storm: COVID-19 vaccination and the eye

    The COVID-19 pandemic has galvanized the global response towards the development of new vaccines based on novel technologies at an unprecedented pace. Since the widespread implementation of vaccination campaigns, case reports on vaccines' systemic side effects, including ocular manifestations, have emerged. Since administered vaccines are generally not able to cause the disease in the recipient, or induce an immune response against the pathogen, we hypothesize that the development of ocular phenomena post-COVID-19 vaccination may occur via an immune response elicited by the vaccine. Of many, the most common ocular adverse events include facial nerve palsy, central venous sinus thrombosis and acute anterior uveitis. These COVID-19 vaccine-induced ocular (CVIO) adverse events could resemble the ocular findings in some of the COVID-19 patients. This review will provide a comprehensive overview of published ocular side effects potentially associated with COVID-19 vaccination and serve as a springboard for further research into CVIO adverse events.Ophthalmic researc

    Evaluation of hydrochars from lignin hydrous pyrolysis to produce biocokes after carbonization

    Hydrochars were obtained after hydrous pyrolysis of a pine Kraft lignin using different reaction conditions (temperature, water content and residence time) and the residues were characterized through a wide range of analytical techniques including high-temperature rheometry, solid-state 13C nuclear magnetic resonance (NMR), thermal gravimetric analysis (TGA), diffuse reflectance infrared Fourier transform spectroscopy (DRIFTS) and field emission scanning electron microscopy (FE-SEM). The results indicated that an increase in reaction temperature, an increase in residence time or a decrease in water content reduces the amount of fluid material in the residue. The hydrous pyrolysis conditions studied were not able to increase the maturation of lignin, which would result in an increase in the resolidification temperature, but reduced the amount of mineral matter in the hydrochar produced. On the other hand, the hydrochars obtained from pristine lignin, torrefied lignin (300 °C, 1 h) and their 50:50 wt.%/wt.% blend at temperatures of 350 °C after 6 h using 30 ml of water had lower ash contents (45%) is excessively high compared to that of the good coking coal (10%) and the micro-strength of the biocokes (R139%) and high microporous surface areas ( > 400 m2/g) of the biocokes and high alkalinity index of the lignins (>27%) compared to those of the coke (27% and 145 m2/g) and coal (0.6%), respectively. Furthermore, the biocoke derived from the hydrous pyrolysed torrefied lignin did not agglomerate, which could not be explained by changes in the chemical properties of the material and requires further investigation

    Demystifying an unidentified EGRET source by VHE gamma-ray observations

    In a novel approach in observational high-energy gamma-ray astronomy, observations carried out by imaging atmospheric Cherenkov telescopes provide necessary templates to pinpoint the nature of intriguing, yet unidentified EGRET gamma-ray sources. Using GeV-photons detected by CGRO EGRET and taking advantage of high spatial resolution images from H.E.S.S. observations, we were able to shed new light on the EGRET observed gamma-ray emission in the Kookaburra complex, whose previous coverage in the literature is somewhat contradictory. 3EGJ1420-6038 very likely accounts for two GeV gamma-ray sources (E>1 GeV), both in positional coincidence with the recently reported pulsar wind nebulae (PWN) by HESS in the Kookaburra/Rabbit complex. PWN associations at VHE energies, supported by accumulating evidence from observations in the radio and X-ray band, are indicative for the PSR/plerionic origin of spatially coincident, but still unidentified Galactic gamma-ray sources from EGRET. This not only supports the already suggested connection between variable, but unidentified low-latitude gamma-ray sources with pulsar wind nebulae (3EGJ1420-6038 has been suggested as PWN candidate previoulsy), it also documents the ability of resolving apparently confused EGRET sources by connecting the GeV emission as measured from a large-aperture space-based gamma-ray instrument with narrow field-of-view but superior spatial resolution observations by ground-based atmospheric Cherenkov telescopes, a very promising identification technique for achieving convincing individual source identifications in the era of GLAST-LAT.Comment: 4 pages, 5 figures, Accepted for publication in Astrophysics and Space Science, Proc. of "The Multi-Messenger Approach to High-Energy Gamma-ray Sources (Third Workshop on the Nature of Unidentified High-Energy Sources)", Barcelona, July 4-7, 2006, one typo correcte

    Ocular adverse events after COVID-19 vaccination

    Purpose The COVID-19 pandemic has galvanized the development of new vaccines at an unprecedented pace. Since the widespread implementation of vaccination campaigns, reports of ocular adverse effects after COVID-19 vaccinations have emerged. This review summarizes ocular adverse effects possibly associated with COVID-19 vaccination, and discusses their clinical characteristics and management. Methods Narrative Literature Review. Results Ocular adverse effects of COVID-19 vaccinations include facial nerve palsy, abducens nerve palsy, acute macular neuroretinopathy, central serous retinopathy, thrombosis, uveitis, multiple evanescent white dot syndrome, Vogt-Koyanagi-Harada disease reactivation, and new-onset Graves' Disease. Studies in current literature are primarily retrospective case series or isolated case reports - these are inherently weak in establishing association or causality. Nevertheless, the described presentations resemble the reported ocular manifestations of the COVID-19 disease itself. Hence, we hypothesize that the human body's immune response to COVID-19 vaccinations may be involved in the pathogenesis of the ocular adverse effects post-COVID-19 vaccination. Conclusion Ophthalmologists and generalists should be aware of the possible, albeit rare, ocular adverse effects after COVID-19 vaccination.Ophthalmic researc

    Lorentz and CPT Violation in Neutrinos

    A general formalism is presented for violations of Lorentz and CPT symmetry in the neutrino sector. The effective hamiltonian for neutrino propagation in the presence of Lorentz and CPT violation is derived, and its properties are studied. Possible definitive signals in existing and future neutrino-oscillation experiments are discussed. Among the predictions are direction-dependent effects, including neutrino-antineutrino mixing, sidereal and annual variations, and compass asymmetries. Other consequences of Lorentz and CPT violation involve unconventional energy dependences in oscillation lengths and mixing angles. A variety of simple models both with and without neutrino masses are developed to illustrate key physical effects. The attainable sensitivities to coefficients for Lorentz violation in the Standard-Model Extension are estimated for various types of experiments. Many experiments have potential sensitivity to Planck-suppressed effects, comparable to the best tests in other sectors. The lack of existing experimental constraints, the wide range of available coefficient space, and the variety of novel effects imply that some or perhaps even all of the existing data on neutrino oscillations might be due to Lorentz and CPT violation.Comment: 25 pages REVTe
