    Low-density Lipoprotein Cholesterol And Radiotherapy-induced Carotid Atherosclerosis In Subjects With Head And Neck Cancer

    Background: Radiotherapy (RT) is a risk factor for accelerated carotid artery atherosclerotic disease in subjects with head and neck cancer. However, the risk factors of RT-induced carotid artery remodeling are not established. This study aimed to investigate the effects of RT on carotid and popliteal arteries in subjects with head and neck cancer and to evaluate the relationship between baseline clinical and laboratory features and the progression of RT-induced atherosclerosis.Findings: Eleven men (age = 57.9 ± 6.2years) with head and neck cancer who underwent cervical bilateral irradiation were prospectively examined by clinical and laboratory analysis and by carotid and popliteal ultrasound before and after treatment (mean interval between the end of RT and the post-RT assessment = 181 ± 47 days).     Degradable and injectable poly(aldehyde guluronate) hydrogels for bone tissue engineering

    Degradable and injectable hydrogels may be ideal for bone-tissue engineering, especially in the craniofacial region because of the ease of access for injection. Alginate hydrogels potentially could be used as injectable cell delivery vehicles, but they exhibit a limited range of mechanical properties and uncontrollable disintegration time. Therefore we synthesized new hydrogels, composed of poly(aldehyde guluronate) (PAG) and adipic acid dihydrazide, that have a wide range of mechanical stiffness and controllable degradation rate. MC3T3-E1 cells adhered and multiplied on PAG hydrogels in vitro . When primary rat calvarial osteoblasts were mixed with PAG hydrogels and subcutaneously injected into the backs of mice, mineralized bone tissues were formed 9 weeks following implantation. These hydrogels may find wide utility as an injectable delivery system for bone precursor cells as well as for other applications in tissue engineering. © 2001 John Wiley & Sons, Inc.     Desempenho psicométrico da versão brasileira do Mini-cuestionario de calidad de vida en la hipertensión arterial (MINICHAL)

    This study aimed to evaluate the feasibility, acceptability, ceiling and floor effects, reliability, and convergent construct validity of the Brazilian version of the Mini Cuestionario de Calidad de Vida en la Hipertensión Arterial (MINICHAL). The study included 200 hypertensive outpatients in a university hospital and a primary healthcare unit. The MINICHAL was applied in 3.0 (± 1.0) minutes with 100% of the items answered. A "ceiling effect" was observed in both dimensions and in the total score, as well as evidence of measurement stability (ICC=0.74). The convergent validity was confirmed by significant positive correlations between similar dimensions of the MINICHAL and the SF-36, and significant negative correlations with the Minnesota Living with Heart Failure Questionnaire - MLHFQ, however, correlations between dissimilar constructs were also observed.     Development and reliability of an instrument to measure psychosocial determinants of salt consumption among hypertensive patients

    Este estudio tuvo como objetivo presentar el análisis de validez de contenido y de confiabilidad de un instrumento para estudiar los factores determinantes del consumo de sodio entre hipertensos, basado en la extensión de la Teoría del Comportamiento Planificado. El instrumento fue sometido a validez de contenido por 3 jueces y a una prueba piloto con 5 sujetos. El instrumento final, compuesto por 3 comportamientos relacionados al consumo de la sal y por las variables psicosociales correspondientes, fue aplicado a 32 sujetos para evaluación de la consistencia interna y de la estabilidad temporal (intervalo de 15 días). Fueron observados coeficientes alfa de Cronbach>;0,70 para la mayoría de las variables y coeficientes de correlación entra clases significativas, que apuntaron la estabilidad temporal de los conceptos mensurados. El instrumento desarrollado mostró evidencias de validez de contenido y de confiabilidad.This study aimed to present the content validity and reliability analyses of an instrument to study the determinant factors of salt consumption among hypertensive subjects, based on an extension of the Theory of Planned Behavior. Content validity was assessed by 3 experts and a pre-test was carried out with 5 subjects. The final tool, comprising 3 different behaviors related to salt consumption and corresponding psychosocial variables, was applied to 32 subjects for internal consistency and temporal stability (15-day interval) analysis. Cronbach's alpha coefficients >; 0.70 and significant intra-class correlation coefficients were observed for most variables, indicating the temporal stability of the measured concepts.     Intensive treatment of hyperglycemia in the acute phase of myocardial infarction: the tenuous balance between effectiveness and safety: a systematic review and meta-analysis of randomized clinical trials

    In acute myocardial infarction (AMI), each 18 mg/dl (1 mmol/L) increment is associated with a 3% increase in mortality rates. All strategies applied for reducing blood glucose to this date, however, have not presented encouraging results. We searched the Medline (PubMed) and Cochrane Library databases for randomized clinical trials (RCTs) from 1995 to 2017 that used the intensive strategy or GIK therapy for blood glucose control during the acute stage of the AMI. We included eight studies. In order to identify the effects of GIK or insulin therapy, we calculated a overall risk ratio (RR) with meta-analysis of fixed and random effects models. A two-tail p-value of 36 mg/dL) in relation to the estimated average blood glucose (estimated by HbA1c) were associated with higher mortality, whereas lower reductions in blood glucose ( 36 mg/dL) em relação à glicemia estimada média se associaram à maior mortalidade, enquanto reduções menores não se associaram com seu incremento ou redução. A redução glicêmica na fase aguda em relação à glicemia estimada média foi mais efetiva e segura na faixa em torno de 18 mg/dL. Esta meta-análise levanta a hipótese de haver um limite tênue entre efetividade e segurança para a redução glicêmica na fase aguda, sendo que os alvos não devem exceder uma redução maior do que 36 mg/dL de glicemia

    Mapeamento da vegetação na floresta atlântica usando o classificador de árvore de decisão para integrar dados de sensoriamento remoto e modelo digital de terreno

    O manejo e o monitoramento ecológico de parques nacionais e outras áreas protegidas requerem uma detalhada descrição do padrão de distribuição da vegetação. Esse artigo possui como objetivo produzir um mapa de vegetação para o Parque Nacional da Serra dos Órgãos (PARNASO). Essa unidade de conservação está localizada na Floresta Atlântica dentro de uma variação topográfica desde o nível do mar até 2.263 metros. A classificação da vegetação baseou-se em dados do satélite ASTER, fotografias aéreas de alta resolução e modelo digital de elevação (MDE). O MDE indica estruturas de vegetação em ambiente com alta variabilidade espacial porque se correlaciona com fatores ambientais, tais como o microclima, umidade, solo e processos geomorfológicos. O classificador de árvore de decisão foi usado para extrair informações dos dados de MDE e sensoriamento remoto. Sete fisionomias foram identificadas: Agropecuária (1,29% da área do Parque), Campos de Altitude (24,27%), Floresta Ombrófila Densa Alto-Montana (37,47%), Floresta Ombrófila Densa Montana (21,54%), Floresta Ombrófila Densa Sub-Montana (5,22%), Floresta Secundária (4,13%) e áreas sem vegetação (6,08%). As três maiores fisionomias estão associadas a altitudes superiores a 1.000 metros e representaram 55,5% da área total. A construção da árvore de decisão combinando informações do MDE e sensoriamento remoto pode melhorar o resultado sobre a distribuiçãoda floresta tropical.The management and ecological monitoring of national parks and other protected areas requires a detailed description of the vegetation distribution patterns. This paper aims to produce a vegetation map for the Serra dos Órgãos National Park (PARNASO). This conservation unit is localized in Atlantic Forest within a topographic variation from sea level to 2,263 meters. The vegetation classification based on the ASTER satellite data, high-resolution aerial photographs and Digital Elevation Model (DEM). The DEM indicates vegetation structures in landscape with high spatial variability because it correlates with environmental factors, such as microclimate, moisture, soil and geomorphological processes. Decision tree classifier was used to extract information of DEM and remote sensing data. Seven classes were identified: Agropecuária (1.29% of total Park area), Campos de Altitude (24.27%), Floresta Ombrófila Densa Alto-Montana (37.47%), Floresta Ombrófila Densa Montana (21.54%), Floresta Ombrófila Densa Sub-Montana (5.22%), Floresta Secundária (4.13%), and no vegetation area (6.08%). The three highest physiognomies were associated with altitude higher than 1,000 m and represented 55.5% of the total area. The construction of decision trees combining the DEM and remote sensing information can improve the result on the forest tropical distribution

    Statin short-term inhibition of insulin sensitivity and secretion during acute phase of ST-elevation myocardial infarction

    Hyperglycemia during myocardial infarction (MI) has a strong and direct association with mortality. In stable patients and experimental models, statins favor the elevation of glycaemia. The present study investigated whether short-course treatment with statins during MI can influence glucose homeostasis and thus the clinical outcome. In this prospective study, euglycemic hyperinsulinemic clamp (EHC) was performed at second (D2) and sixth (D6) day after MI in patients randomized to simvastatin (S)10 or 80 mg/day during hospitalization (n = 27). In addition, patients (n = 550) were treated without (WS) or with simvastatin (S) at 20, 40 or 80 mg/day had HOMA2S on admission (D1) and fifth (D5) day after MI. According to EHC, insulin sensitivity increased by 20 +/- 60% in S10 and decreased by -6 +/- 28% in S80 (p = 0.025). Consistently, the changes in HOMA2S between D1 and D5 were 40 +/- 145% (WS), 22 +/- 117% (S20), 16 +/- 61% (S40) and -2% +/- 88% (S80) (p = 0.001). Theearly brain lesion of CP is reconceptualised as a neurobiological constraint that exists in therelations between the neural, cognitive and social levels. Through a multi-method study of twochildren with CP, it is analysed how neurobiological constraints arise, evolve and sometimes areresolved through local matches between the child and its social practices.     Omega-3 intake is associated with attenuated inflammatory response and cardiac remodeling after myocardial infarction

    Myocardial infarction (MI) elicits an intense acute inflammatory response that is essential for cardiac repair. However, an excessive inflammatory response also favors myocardial apoptosis, cardiac remodeling, and cardiovascular mortality. Omega-3 polyunsaturated fatty acids (-3) bear anti-inflammatory effects, which may mitigate the inflammatory response during MI. This study investigated whether -3 intake is associated with attenuation of the MI-related inflammatory response and cardiac remodeling. ST-elevation MI (STEMI) patients (n=421) underwent clinical, biochemical, nutritional, 3D echocardiogram, Cardiac Magnetic Resonance imaging (CMRi) at 30 days and 3D echocardiogram imaging at six months after the MI. Blood tests were performed at day one (D1) and day five (D5) of hospitalization. Changes in inflammatory markers (D5-D1) were calculated. A validated food frequency questionnaire estimated the nutritional consumption and -3 intake in the last 3months before admission. The intake of -3 below the median (<1.7g/day) was associated with a short-term increase in hs-C-reactive protein [OR:1.96(1.24-3.10); p=0.004], Interleukin-2 [OR:2.46(1.20-5.04); p=0.014], brain-type natriuretic peptide [OR:2.66(1.30-5.44); p=0.007], left-ventricle end-diastolic volume [OR:5.12(1.11-23.52)]; p=0.036] and decreases in left-ventricle ejection fraction [OR:2.86(1.47-6.88); p=0.017] after adjustment for covariates. No differences were observed in the extension of infarcted mass obtained by CMRi.
