    How Can eHealth Meet the Hearing and Communication Needs of Adults With Hearing Impairment and their Significant Others? A Group Concept Mapping Study

    Objectives: To seek the perspectives of key stakeholders regarding: (1) how eHealth could help meet the hearing and communication needs of adults with hearing impairment and their significant others; and (2) how helpful each aspect of eHealth would be to key stakeholders personally. / Design: Group concept mapping, a mixed-methods participatory research method, was used to seek the perspectives of key stakeholders: adults with hearing impairment (n = 39), significant others (n = 28), and hearing care professionals (n = 56). All participants completed a short online survey before completing one or more of the following activities: brainstorming, sorting, and rating. Brainstorming required participants to generate ideas in response to the focus prompt, “One way I would like to use information and communication technologies to address the hearing and communication needs of adults with hearing loss and their family and friends is to
.” The sorting task required participants to sort all statements into groups that made sense to them. Finally, the rating task required participants to rate each of the statements according to “How helpful would this idea be to you?” using a 5-point Likert scale. Hierarchical cluster analysis was applied to the “sorting” data to develop a cluster map using the Concept Systems software. The “rating” data were subsequently analyzed at a cluster level and an individual-item level using descriptive statistics. Differences in cluster ratings between stakeholder groups were examined using Kruskal-Wallis tests. / Results: Overall, 123 statements were generated by participants in response to the focus prompt and were included in subsequent analyses. Based on the “sorting” data and hierarchical cluster analysis, a seven-cluster map was deemed to be the best representation of the data. Three key themes emerged from the data, including using eHealth to (1) Educate and Involve Others; (2) Support Aural Rehabilitation; and (3) Educate About and Demonstrate the Impacts of Hearing Impairment and Benefits of Hearing Rehabilitation. Overall median rating scores for each cluster ranged from 3.97 (educate and involve significant others) to 3.44 (empower adults with hearing impairment to manage their hearing impairment from home). / Conclusions: These research findings demonstrate the broad range of clinical applications of eHealth that have the capacity to support the implementation of patient- and family-centered hearing care, with self-directed educational tools and resources typically being rated as most helpful. Therefore, eHealth appears to be a viable option for enabling a more biopsychosocial approach to hearing healthcare and educating and involving significant others in the hearing rehabilitation process without adding more pressure on clinical time. More research is needed to inform the subsequent development of eHealth interventions, and it is recommended that health behavior change theory be adhered to for such interventions

    Evaluation of Skylab (EREP) data for forest and rangeland surveys

    The author has identified the following significant results. Four widely separated sites (near Augusta, Georgia; Lead, South Dakota; Manitou, Colorado; and Redding, California) were selected as typical sites for forest inventory, forest stress, rangeland inventory, and atmospheric and solar measurements, respectively. Results indicated that Skylab S190B color photography is good for classification of Level 1 forest and nonforest land (90 to 95 percent correct) and could be used as a data base for sampling by small and medium scale photography using regression techniques. The accuracy of Level 2 forest and nonforest classes, however, varied from fair to poor. Results of plant community classification tests indicate that both visual and microdensitometric techniques can separate deciduous, conifirous, and grassland classes to the region level in the Ecoclass hierarchical classification system. There was no consistency in classifying tree categories at the series level by visual photointerpretation. The relationship between ground measurements and large scale photo measurements of foliar cover had a correlation coefficient of greater than 0.75.     Barley crops are at risk of being attacked by several leaf diseases. Net blotch, brown rust, Rhynchosporium and Ramularia leaf spot are among the most widespread and can cause severe attack and yield losses. Two trial protocols targeting Ramularia and net blotch, respectively, have been tested in several countries in 2021 and 2022. Ramularia trials were situated in Germany, Ireland, Scotland, and Denmark. The net blotch trials were placed in Denmark, Belgium, the UK, Germany, Finland, and France. In the two protocols, 12–13 different fungicide solutions including co-formulations of DMIs, SDHIs, QoIs, and multi-site inhibitors have been tested to compare efficacy and yield responses. Against Ramularia leaf spot, the fungicides were applied at GS 47–51 and against net blotch at GS 37–45. In six trials, the efficacy against Ramularia leaf spot was scored. The results showed a superior control from the co-formulation fluxapyroxad + metyltetraprole (78–100% control), but also solo mefentrifluconazole and the mixtures fluxapyroxad + mefentrifluconazole performed well (average 74–76% control). The mixture fluxapyroxad + metyltetraprole provided the best yield increase followed by Ascra Xpro. Folpet as a solo solution was inferior. Following the net blotch protocol, only three trials developed enough disease to rank the different fungicides; however, in five trials ranking against brown rust was also possible. Most treatments gave very good control of net blotch, and brown rust (&gt; 80% control). The mixture fluxapyroxad + metyltetraprole delivered the best control against all diseases overall. Average yield responses from eight trials showed very similar increases from the tested fungicides.</p

    Amplified ambivalence: having a sibling with juvenile idiopathic arthritis

    Despite increased awareness of family responses to chronic illness and disability, there is still a need to understand experiences of well siblings. We begin to address this by asking “What is it like to have a sibling with juvenile idiopathic arthritis?” (JIA).Eight families with an adolescent diagnosed with JIA participated. Four members of each family, including one healthy sibling, were interviewed and transcripts analyzed using grounded theory. Analysis suggests healthy siblings see their family as different to ‘normal’ families, forfeit time with peers, share vicariously adverse experiences of their ill sibling, and feel inadequately informed. Such experiences amplify the ambivalent nature of sibling relationships and are possibly felt most strongly during late childhood and early adolescence. Support from extended family can reduce these negative experiences and facilitate social and emotional adjustment which also occurs over time as the children mature. These findings have implications for healthcare professionals and voluntary organizations

    Magnetic fields and differential rotation on the pre-main sequence I: The early-G star HD 141943 - brightness and magnetic topologies

    Spectroscopic and spectropolarimetric observations of the pre-main sequence early-G star HD 141943 were obtained at four observing epochs (in 2006, 2007, 2009 and 2010). The observations were undertaken at the 3.9-m Anglo-Australian Telescope using the UCLES echelle spectrograph and the SEMPOL spectropolarimeter visitor instrument. Brightness and surface magnetic field topologies were reconstructed for the star using the technique of least-squares deconvolution to increase the signal-to-noise of the data. The reconstructed brightness maps show that HD 141943 had a weak polar spot and a significant amount of low latitude features, with little change in the latitude distribution of the spots over the 4 years of observations. The surface magnetic field was reconstructed at three of the epochs from a high order (l <= 30) spherical harmonic expansion of the spectropolarimetric observations. The reconstructed magnetic topologies show that in 2007 and 2010 the surface magnetic field was reasonably balanced between poloidal and toroidal components. However we find tentative evidence of a change in the poloidal/toroidal ratio in 2009 with the poloidal component becoming more dominant. At all epochs the radial magnetic field is predominantly non-axisymmetric while the azimuthal field is predominantly axisymmetric with a ring of positive azimuthal field around the pole similar to that seen on other active stars.Comment: 18 pages, 17 figures, accepted by MNRA

    ‘I think I'm more free with them'—Conflict, Negotiation and Change in Intergenerational Relations in African Families Living in Britain

    While the family is increasingly being recognised as pivotal to migration, there remain too few studies examining how migration impacts on intergenerational relationships. Although traditional intergenerational gaps are intensified by migration, arguably there has been an over-emphasis on the divisions between ‘traditional’ parents and ‘modern’ children at the expense of examining the ways in which both generations adapt. As Foner and Dreby [2011. “Relations Between the Generations in Immigrant Families.” Annual Review of Sociology 37: 545–564] stress, the reality of post-migration intergenerational relations is inevitably more complex, requiring the examination of both conflict and cooperation. This article contributes to this growing literature by discussing British data from comparative projects on intergenerational relations in African families (in Britain, France and South Africa). It argues that particular understandings can be gained from examining the adaptation of parents and parenting strategies post-migration and how the reconfiguration of family relations can contribute to settlement. By focusing on how both parent and child generations engage in conflict and negotiation to redefine their relationships and expectations, it offers insight into how families navigate and integrate the values of two cultures. In doing so, it argues that the reconfiguration of gender roles as a result of migration offers families the space to renegotiate their relationships and make choices about what they transmit to the next generation

    Enabling and constraining migration: the multiscalar management of temporary, skilled, international migration of English professional cricketers

    This article progresses debates about how the process of international migration is operationalized. With a focus on the geographical concept of scale this article explores how individuals, organizations and policies interact to determine the characteristics of a migration flow. Using a case study of the temporary migration of English professional cricketers moving seasonally to Australia, it is revealed that there is a complex nexus of actors and institutions at the micro-, meso- and macro-scales that influence migration and can have contradictory impacts on migratory activity. Drawing on interviews with current and former English professional cricketers and a wide range of intermediaries it is shown how migration can both be enabled and constrained by different individuals and institutions in the home and destination context. The article contends that a multi-scalar approach is vital to more fully understand how migration flows are operationalized. The findings are pertinent to wider academic debates on the enablement, constraint and growing regulation of sports labour migration and skilled migration more broadly

    Trust and contextual engagement with the PEPPER system: The qualitative findings of a clinical feasibility study

    Background and aims. PEPPER (Patient Empowerment through Predictive PERsonalised decision support) is an EU-funded research project which aims to improve self-management of type 1 diabetes (T1D). The system comprises an AI insulin bolus recommender, coupled with a safety system. The aim of the qualitative arm of this clinical feasibility study was to examine the context of participants’ interaction with the PEPPER system and identify incidents where bolus recommendations were trusted and accepted. Methods. This was a multicentre (UK and Spain) non-randomised open-labelled 6-week pilot study. Thirteen adults with T1D participated in weekly telephone interviews to explore the context of their interactions and responses to PEPPER. Data was thematically analysed through conceptual frameworks for engagement with healthcare digital behaviour change interventions. Results. Participants reported their key interactions as responding to PEPPER bolus recommendations, inputting carbohydrate values, interpreting continuous glucose monitoring (CGM) values through visualization of personal data and dealing with safety alarms. Two themes were associated with trust and engagement with the system; ‘feeling monitored’ and ‘feeling in control’. The incidents where participants trusted PEPPER also enhanced personal expertise of T1D through insights provided by the safety system such as low glucose basal insulin for pump users. Benefits were balanced against technical challenges of the system, which were used to improve the PEPPER application and enhance user experience. Conclusion. Some participants suggested that even access to PEPPER for a temporary period could positively influence self-management strategies. Contextual interviewing is a valuable tool in mobile application development for diabetes decision support systems

    Predicting protein decomposition: the case of aspartic-acid racemization kinetics

    The increase in proportion of the non-biological (D-) isomer of aspartic acid (Asp) relative to the L- isomer has been widely used in archaeology and geochemistry as a tool for dating. The method has proved controversial, particularly when used for bones. The non-linear kinetics of Asp racemization have prompted a number of suggestions as to the underlying mechanism(s) and have led to the use of mathe- matical transformations which linearize the increase in D-Asp with respect to time. Using one example, a suggestion that the initial rapid phase of Asp racemization is due to a contribution from asparagine (Asn), we demonstrate how a simple model of the degradation and racemization of Asn can be used to predict the observed kinetics. A more complex model of peptide bound Asx (Asn+Asp) racemization, which occurs via the formation of a cyclic succinimide (Asu), can be used to correctly predict Asx racemi- zation kinetics in proteins at high temperatures (95-140 °C). The model fails to predict racemization kinetics in dentine collagen at 37 °C. The reason for this is that Asu formation is highly conformation dependent and is predicted to occur extremely slowly in triple helical collagen. As conformation strongly in£uences the rate of Asu formation and hence Asx racemization, the use of extrapolation from high temperatures to estimate racemization kinetics of Asx in proteins below their denaturation temperature is called into question. In the case of archaeological bone, we argue that the D:L ratio of Asx re£ects the proportion of non- helical to helical collagen, overlain by the e¥ects of leaching of more soluble (and conformationally unconstrained) peptides. Thus, racemization kinetics in bone are potentially unpredictable, and the proposed use of Asx racemization to estimate the extent of DNA depurination in archaeological bones is challenged
