    Possibility of increase of the transformation ratio at plasma wakefield excitation by long relativistic electron bunch, which charge is shaped according to cosine, with purpose to provide the absence of oscillating (on length of bunch) decelerating wakefield, usually attainable, using bunch - precursor, is considered. Length of bunch is selected more larger than wavelength. It is shown analytically that at the choice of charge shaping of long bunch according to cosine distribution one can do without a bunch – precursor to eliminate oscillating decelerating wakefield. Thus the transformation ratio is just in one and a half times less than in the case with a precursor

    Perspectives of interregional economic unions and associations in conditions of geopolitical instability

    The growing importance of interregional unions and associations in the Russian economy in the period of the current economic transformations has determined the relevance of the research topic. The changes that have been formed in Russia have an indefinite relation to the nature of the processes currently taking place in the Russian economy and in its regions. The determined transition to market relations enabled the regions to independently decide on the directions and forms of their existing resources and put aside the obligation to coordinate all steps with the capital. This, of course, is a prerequisite for the growth of the result of the functioning of the economy of the regions. Obstacles for the movement of goods and services between the subjects of Russia were formed, the process of border and customs control became more complicated. There were difficulties in the work of the railway and automobile junction. These aspects of interregional economic associations are an example of the state of the economy of certain regions, and the aspect of influence on the economy. Due to the lack of own reserves of economic growth of a particular region, the task of inter-regional economic unions and associations is being formed. Therefore, a research on improving and developing such relations in the new economic conditions becomes a viable practical goal, and solving the problem is an important element of the obstacle to crisis situations in the Russian economy and the transition to sustainable economic development

    The Hitting Times with Taboo for a Random Walk on an Integer Lattice

    For a symmetric, homogeneous and irreducible random walk on d-dimensional integer lattice Z^d, having zero mean and a finite variance of jumps, we study the passage times (with possible infinite values) determined by the starting point x, the hitting state y and the taboo state z. We find the probability that these passages times are finite and analyze the tails of their cumulative distribution functions. In particular, it turns out that for the random walk on Z^d, except for a simple (nearest neighbor) random walk on Z, the order of the tail decrease is specified by dimension d only. In contrast, for a simple random walk on Z, the asymptotic properties of hitting times with taboo essentially depend on the mutual location of the points x, y and z.     Single-stage combined urethroplasty for extended strictures of the anterior urethra of tuberculous origin

    Background. Urethral strictures are currently one of the most complex pathologies in reconstructive urology. The most promising direction in this area is the development of single-stage surgical interventions that meets modern requirements for the quality of life of patients.The aim of the study. To evaluate the possibility and the effectiveness of surgical treatment of extended recurrent tuberculous urethral strictures in men using a combination of a skin flap and a buccal graft. In the literature, there is no description of the use of this technique in patients with urethral tuberculosis.Materials and methods. We observed 44 patients with urethral tuberculosis. Three men from this group of patients were diagnosed with tuberculous (post-tuberculous) extended recurrent stricture of the anterior urethra and they underwent combined single-stage urethroplasty with a ventral fasciocutaneous flap and a buccal graft using the inlay method in the penile region and with a buccal graft using the ventral onlay method in the bulbous urethra. Ultrasound of the urethra served as a method that determines the possibility of performing this surgery in case of maintaining a sufficient width of the urethral plate with a moderate degree of spongiofibrosis.Results. Patients were under observation for 34, 50 and 54 months and have good long-term functional results – all patients have unassisted urination and no residual urine. Post-micturition dribbling persists in 1 patient. The overall effectiveness of the treatment of these strictures, taking into account the treatment of repeated cases of the disease recurrence, is high.Conclusion. This technique demonstrates the possibility of performing a single-stage reconstruction of the anterior urethra in patients with extended recurrent tuberculous (post-tuberculous) urethral strictures and is also applicable for the surgical treatment of urethral strictures of other origins

The ambiguity and sometimes the opposite of the conclusions of different authors, makes it possible to recommend a balanced attitude to the advisability of screening for ATA in women with a reproductive dysfunction as well as the therapeutic measures proposed in some guidelines based on the results of such examination

    Assessment of the clinical status and behavioral risk factors in patients with cardiovascular and other noncommunicable diseases in Russia and neighboring countries during quarantine and self-isolation

    Aim. To carry out a comparative assessment of the clinical status and behavioral risk factors in patients with cardiovascular (CVD) and other noncommunicable diseases (NCDs) in Russia and neighboring countries during quarantine and selfisolation in the context of coronavirus disease 2019 (COVID-19) pandemic.Material and methods. The study involved patients from Russia, Azerbaijan, Kazakhstan, Lithuania, Kyrgyzstan. A total of 351 men and women aged 30- 69 years with one or more NCDs were included, including hypertension (HTN), coronary artery disease with or without myocardial infarction, type 2 diabetes, chronic obstructive pulmonary disease/ asthma, and cancer that were in quarantine self-isolation. To assess the clinical status and risk factors, patients underwent a questionnaire and examination. The changes in health status was assessed using the EQ-5D questionnaire.Results. During self-isolation and quarantine, 40% of patients noted a decrease and limitation of physical activity. An increase in food frequency and amount was registered in 35% of individuals. During quarantine, every third patient with HTN had hypertensive crises, and every fifth increased the doses of the drugs taken. In the examined cohort, exertional angina (30%) took second place among NCDs. In the group of Azerbaijan patients, every second had exertional angina, while in the Russian cohort — 33%, and in the Lithuanian group — every fourth patient. In general, health status deterioration among people with angina was noted in 6% of cases. In the examined group, type 2 diabetes was detected in 25% of cases. During quarantine, changes in hypoglycemic therapy were carried out in an average of 34% of patients. COVID-19 was registered in 22% of patients in the examined cohort. The largest number was found in the group from Kazakhstan — 57%, while the second place was taken by the Azerbaijan group — 40%. General condition deterioration was detected in 55% of cases in groups from Azerbaijan and Kazakhstan. In Russia, this was reported by 28,8% of patients, while in the group from Lithuania, every fourth patient noted a worsening of the health status. The lowest deterioration was registered in the Kyrgyzstan group (14%).Conclusion. During quarantine and self-isolation among patients with NCDs, a decrease in physical activity, an increase in food consumption and smoking was noted. There was health status deterioration in patients with various NCDs, primarily with CVDs, which required therapy changes. Taken together, this was reflected in general condition worsening in patients with chronic NCDs. It is obvious that the development of comprehensive preventive measures in conditions of selfisolation and quarantine is required

    Prognostic value of subclinical atherosclerosis in patients with a SCORE risk <5%: data from a 10-year follow-up

    Aim. To evaluate the contribution of subclinical atherosclerosis to the stratification of patients with a SCORE risk of cardiovascular events (CVEs) &lt;5% based on a 10-year follow-up.Material and methods. The study included 379 patients with SCORE risk of CVEs &lt;5% (82 men, 297 women). In 2009, all patients underwent clinical examination, carotid artery (CA) ultrasound with the detection of plaques, total CA occlusion, intima-media thickness (IMT) of the common carotid artery (CCA). The plaque number was determined as the total number of all plaques in 6 following segments: both CCAs, both CCA bifurcations and both internal carotid arteries. The total stenosis was calculated as the sum of stenoses in 6 CA segments in %. In 2019, a telephone survey of patients was conducted with a questionnaire assessing the following CVEs: all-cause death, cardiovascular death, myocardial infarction (MI), stroke, myocardial revascularization, cardiovascular hospitalizations, and composite endpoint.Results. The initial patients’ age ranged from 35 to 67 years (51,1±7,5 years). Plaques from 20% to 50% were detected in 303 participants (79,94%). Over the past 10 years, there have been 5 cardiovascular deaths (1,3%), 7 MIs (1,8%), 5 cases of unstable angina (1,3%), 12 cases of myocardial revascularization (3,2%), 15 strokes (4,0%), 51 cardiovascular hospitalizations (13,5%). The proportion of patients with registered endpoints (CVE+) was 22,4% (n=85). The groups of patients with and without CVEs differed in the level of systolic blood pressure (BP) and blood triglycerides, and did not differ in the level of diastolic BP, lipid profile, glucose, heart rate, smoking status, sex, and age. In the CVE+ group, there were higher values of CCA IMT (0,65 (0,64; 0,70) mm vs 0,62 (0,62; 0,66) mm, p&lt;0,05), total CA stenosis (102,5 (88,1; 120,8)% vs 80 (72,5; 88,1)%, p=0,01), and the CA plaque amount (4,0 (2,8; 3,9) vs 3,0 (2,6; 3,1), p=0,01), respectively. Total CA stenosis was an independent predictor of CVEs when adjusted for sex, age, systolic and diastolic BP (β=0,149; p&lt;0,05), but not for lipid profile. A ROC-analysis revealed a cut-off point for total CA stenosis of 82,5% (AUC=0,598, 95% confidence interval 0,5243-0,673, p&lt;0,05).Conclusion. The total CA stenosis has shown itself to be an independent predictor of CVEs in patients with a SCORE risk &lt;5%
