    OPTIMIZATION OF MAPPING ONTO A FLEXIBLE LOW-POWERELECTRONIC FABRIC ARCHITECTURE

    A combinatorial problem that arises from a novel electronic fabric architecture designed forlow-power devices such as cellular phones and palm computers is presented. We consider theproblem of efficiently mapping a given data flow graph onto a particular implementation ofthe fabric architecture. We formulate mixed integer linear programs (MILP) and design asliding partial MILP heuristic for this problem. We highlight the modeling and algorithmicaspects that are necessary to make the MILP formulation competitive. The sliding partialMILP heuristic is developed to generate mappings faster and to find mappings for benchmarkinstances that cannot be solved by the MILP formulation.We also present a method to tune software parameters using ideas from software testingand machine learning. The method is based on the key observation that for many classes ofinstances, the software shows improved performance if a few critical parameters have good values, although which parameters are critical depends on the class of instances. Our methodattempts to find good parameter values using a relatively small number of optimization trials

    Integral equations for the problem of wave diffraction on a flat strip: alternative representation

    This study investigates an effective method for solving one class of integral equations. It addresses various two dimensional problems related to do diffraction theory by metal screens, which are reduced to these integral equations. A novel approach for solving this class of integral equations is proposed.     A dosimetric comparison of different treatment plans of palliative spinal bone irradiation: analysis of dose coverage with respect to ICRU 50 report

    <p>Abstract</p> <p>Background</p> <p>This study aimed to analyze three-dimensional (3D) dosimetric data of conventional two-dimensional (2D) palliative spinal bone irradiation using different reference points and treatment plans with respect to the International Commission on Radiation Units and Measurements (ICRU) Report 50.</p> <p>Methods</p> <p>Forty-five simulation CT scans of 39 patients previously treated for thoraco-lumbar spinal bone metastases were used. Three different treatment plans were created: (1) single posterior field plans using the ICRU reference points (ICRUrps); (2) single posterior field plans using the International Bone Metastasis Consensus Working Party reference points (IBMCrps); (3) two opposed anterior-posterior (AP-PA) field plans using the ICRUrps. The intended dose range for planning target volume (PTV) was 90% to 110% of the prescribed dose for AP-PA field plans. Cumulative dose-volume histograms were generated for each plan, and minimum, maximum and mean doses to the PTV, medulla spinalis, esophagus and intestines were analyzed.</p> <p>Results</p> <p>The mean percentages of minimum, maximum and mean PTV doses ± standard deviation were, respectively, 91 ± 1.3%, 108.8 ± 1.3% and 99.7 ± 1.3% in AP-PA field plans; 77.3 ± 2.6%, 122.2 ± 4.3% and 99.8 ± 2.6% in ICRUrp single field plans; and 83.7 ± 3.3%, 133.9 ± 7.1% and 108.8 ± 3.3% in IBMCrp single field plans. Minimum doses of both single field plans were significantly lower (p < 0.001) while maximum doses were significantly higher (p < 0.001) than AP-PA field plans. Minimum, maximum and mean doses were higher in IBMCrp single field plans than in ICRUrp single field plans (p < 0.001). The mean medulla spinalis doses were lower in AP-PA field plans than single posterior field plans (p < 0.001). Maximum doses for medulla spinalis were higher than 120% of the prescribed dose in 22 of 45 (49%) IBMCrp single field plans. Mean esophagus and intestinal doses were higher (p < 0.001) in AP-PA field plans than single field plans, however, less than 95% of the prescribed dose.</p> <p>Conclusion</p> <p>In palliative spinal bone irradiation, 2D conventional single posterior field radiotherapy did not accomplish the ICRU Report 50 recommendations for PTV dose distribution, while the AP-PA field plans did achieve the intended dose ranges with a homogenous distribution and reasonable doses to the medulla spinalis, esophagus and intestines.</p

    Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy. Risk prediction models to support shared decision‐making by identifying adults in the UK at low risk of appendicitis were identified

    The Role of CT Imaging in a Fractured Coronary Stent with Pseudoaneurysm Formation

    We report a case of a 63-year-old male patient with multiple cardiovascular risk factors and previous myocardial infarction who was referred to the emergency department on September 2023 with symptoms and clinical and biological data consistent with an acute coronary event. A coronary angiography revealed severe ostial stenosis of the left anterior descending artery (LAD) and intrastent thrombotic occlusion in the first two segments of the LAD. Two drug-eluting stents were implanted and the patient was discharged when hemodynamically stable; however, three weeks later, he returned to the emergency department complaining of fever, anterior chest pain, dyspnea at rest, and high blood pressure values at home. High levels of troponin T, C-reactive protein, and NT-proBNP were detected and blood cultures showed methicillin-resistant Staphylococcus aureus. The computed tomography (CT) examination showed a saccular dilatation had developed between two fragments of a stent mounted at the level of the LAD, surrounded by a hematic pericardial accumulation. LAD pseudoaneurysm ablation and a double aortocoronary bypass with inverted saphenous vein autograft were performed and the patient showed a favorable postoperative evolution. In this case, surgical revascularization was proven to be the appropriate treatment strategy, demonstrating the need to choose an individualized therapeutic option depending on case-specific factors

    Rheumatoid Arthritis, Depression, and the Role of Celecoxib

    Rheumatoid arthritis (RA) is a chronic autoimmune disease, causing joint destruction and associated physical, mental, and financial distress. Depression is not uncommonly found in patients with RA as both disorders share sociodemographic, functional, and biologic factors. There is growing evidence on the role of anti-inflammatory agents in managing depression, particularly celecoxib, which has been shown to significantly alleviate depressive symptoms as an augmenting agent. Compared with traditional nonsteroidal anti-inflammatory drugs (tNSAIDs), however, celecoxib offers modest improvement in clinical symptoms, with uncertain results for pain management, physical function, and adverse effects in patients with RA.     A Machine Learning Strategy for the Quantitative Analysis of the Global Warming Impact on Marine Ecosystems

    It is generally observed that aquatic organisms have symmetric abilities to produce oxygen (O2) and fix carbon dioxide (CO2). A simulation model with time-dependent parameters was recently proposed to better understand the symmetric effects of accelerated climate change on coastal ecosystems. Changes in environmental elements and marine life are two examples of variables that are expected to change over time symmetrically. The sustainability of each equilibrium point is examined in addition to proving the existence and accuracy of the proposed model. To support the conclusions of this research compared to other studies, numerical simulations of the proposed model and a case study are investigated. This paper proposes an integrated bibliographical analysis of artificial neural networks (ANNs) using the Reverse-Propagation with Levenberg&ndash;Marquaradt Scheme (RP-LMS) to evaluate the main properties and applications of ANNs. The results obtained by RP-LMS show how to prevent global warming by improving the management of marine fish resources. The reference dataset for greenhouse gas emissions, environmental temperature, aquatic population, and fisheries population (GAPF) is obtained by varying parameters in the numerical Adam approach for different scenarios. The accuracy of the proposed RP-LMS neural network is demonstrated using mean square error (MSE), regression plots, and best-fit output. According to RP-LMS, the current scenario of rapid global warming will continue unabated over the next 50 years, damaging marine ecosystems, particularly fish stocks
