    Ultrasound Assisted Pleural Biopsy with Abram Needle is a Valuable Tool in diagnosing Exudative Lymphocytic Pleural Effusions

    Objectives: To evaluate the efficacy and safety of US assisted pleural biopsy using Abram Needle. Study Design: A non-randomized prospective study. The study was approved by Ethical Board of Gulab Devi Chest hospital Lahore. Methodology: The study was conducted at the Department of Respiratory Medicine, Gulab Devi Chest Hospital Lahore. 141 male patients with undiagnosed, unilateral, exudative, lymphocytic pleural effusion were included in the study during Jan. 2014 to Nov. 2015. Pleural biopsy was performed with Abram needle after an informed consent, having localized the biopsy point by chest radiographs & sonography.  Tissue samples underwent histopathological evaluation. Diagnosis was made on histopathology reports. Statistics was applied. Results: Six samples were inadequate while 135 were adequate.  31.11% cases were malignant while 68.88% were nonmalignant. Out of 93 nonmalignant cases, 7.52 % were normal pleurae, 46.23% were caseous granulomatous inflammation, 25.80% were chronic nonspecific pleuritis, 10.75 % cases were chronic pleuritis with fibrous thickening, 9.67 % cases were acute pleuritis. Biopsy success rate was 95.74%. All patients tolerated the procedure well. No serious complication occurred. Conclusions:  Abram needle pleural biopsy using ultrasound assistance is very safe and excellent diagnostic tool in diagnosing exudative lymphocytic pleural effusions.  &nbsp

    Outcome of Topical Epidural Methylprednisolone Versus Control in Lumbar Disc Surgery Patients

    Objective:  The use of topical intraoperative methylprednisolone in lumbar disc surgery leads to significant improvement in pain relief and early resumption of daily life activities. The study determined the outcome of topical epidural methylprednisolone. Materials & Methods:  60 patients included with diagnosed cases of the herniated lumbar disc with ASA Grade 2 and below. Group A was for patients who had to receive topical methylprednisolone while group B was the control group where no topical methylprednisolone had been used. The outcome was measured from OLBI and VAS scores. Results:  In Group A, the mean hospital stay was 2 days, mean OLBI score was 35%, mean VAS score was 3, mean time to resumption of ADL was 7 days, mean dose of paracetamol (per day) was 3 mg, mean dose of Ketorolac (per day) was 90 mg. Whereas Group B, mean hospital stay was 3 days, mean OLBI score was 45%, mean VAS score was 5, mean time to resumption of ADL was 10 days, mean dose of paracetamol (per day) was 4 mg, mean dose of Ketorolac (per day) was 100 mg. In Group A, 91% of patients had a favorable outcome and 9% of patients had an unfavorable outcome. Whereas in Group B, 83% of patients had a favorable outcome and 15%of patients had an unfavorable outcome. There existed insignificant results in outcomes concerning diabetic/non-diabetic, and hypertension/non-hypertension. Conclusion:  The use of topical intraoperative methylprednisolone in lumbar disc surgery leads to significant improvement in pain relief and early resumption of daily life activities as compared to without the use of topical methylprednisolone.     The Efficacy of Surgery for Supra-Tentorial Gliomas in Preventing Seizures

    Objectives:  The study examined the surgical outcome of supratentorial gliomas in terms of improvement in seizures in patients who presented to a tertiary care institution. Material and Methods:  A descriptive case series was conducted in Neurosurgery Department at Northwest General Hospital & Research, Peshawar. Patients (n = 95) with supratentorial gliomas with seizures between 18 – 70 years were included. Supratentorial gliomas were diagnosed by neuroimaging as MRI brain with contrast, diffusion-weighted, Fluid-attenuated inversion recovery, and magnetic resonance spectroscopy. The patient was observed for seizures postoperatively. Data was stratified for age and gender. Results:  The majority of patients (36.8%) were in 41 – 50 years. 55.78% of patients were males whereas 44.21% of patients were females. 42 (44.21%) involved the frontal lobe, 16 (16.84%) involved the parietal lobe, 26 (27.36%) involved the temporal lobe, and 11 (11.57%) patients involved the occipital lobe. According to Engel’s classification, 53 patients were in class I, 16 in class II, 10 in class III, and 5 in class IV. 84 (88.42%) experienced post-op seizure reduction. An insignificant association was found with the seizure improvement (yes/no) with different age groups and gender. Conclusion:  The frontal lobe was the most prevalent location for supratentorial gliomas. After surgery, a large proportion of patients improved in terms of seizure management.     Parametric Study and Optimization of Ceiling Fan Blades for Improved Aerodynamic Performance

    This paper includes parametric study and optimization of non-linear ceiling fan blades by combining the techniques of Design of Experiments (DOE), Response Surface Methods (RSM) and Computational Fluid Dynamics (CFD). Specifically, the nonlinear (elliptical) planform shape of ceiling fan blade is investigated in conjunction with blade tip width, root and tip angle of attack. Sixteen cases are designed for three blade ceiling fan using two level full factorial model. The flow field is modeled using Reynolds-Averaged-Navier-Stokes approach. The performance variables used to formulate a multi-objective optimization problem are volumetric flow rate, torque and energy efficiency. Response Surface Method is used to generate the optimized design for non-linear ceiling fan blade profile. The results reveal that the interactions between the design variables play a significant role in determining the performance. It is concluded that the nonlinear forward sweep has a moderate effect on response parameters

    Interactive effects of vanadium and phosphorus on their uptake, growth and heat shock proteins in chickpea genotypes under hydroponic conditions

    The present study was carried out to examine the interaction of vanadium and phosphorus and changes in heat shock genes to optimize the growth of chickpea genotypes. Two sets of hydroponic experiments were carried out using vanadium and phosphorus with five-level central composite design. Five levels of vanadium (0-1180 mu M) and phosphorus (0-100011 mu M) were used to evaluate their interactive effects. Plants fresh biomass and uptake of vanadium and phosphorus were influenced by vanadium and phosphorus application. Enhanced fresh biomass was most likely a result of increased phosphorus uptake by chickpea genotypes. Addition of vanadium induced toxic effects while, higher concentration of phosphorus alleviated its toxic effects. The obtained results also indicated that lower vanadium concentration promoted phosphorus absorption however; higher concentration of vanadium inhibited the phosphorus uptake. The morphological changes in leaves indicated that the cells were deformed and reduced in size when treated with higher vanadium levels with fixed phosphorus while, there was little deformation and reduction in cells size were observed when plants were treated with higher levels of phosphorus with fixed vanadium. Whereas, the proportion of deformation of cells were higher in Balkasar as compared to C-44 genotype. The results also showed that at elevated vanadium with fixed phosphorus, Hsp70 was expressed only in C-44 while, not in Balkasar however, Hsp90 and GAPDH showed non-significant results.     Application of copper and aluminium electrode in electro coagulation process for municipal wastewater treatment: A case study at Karachi

    The reuse of treated domestic wastewater is an imperative source of water for numerous purposes. The treatment of municipal wastewater can be process by utilizing the technique for electrocoagulation. Electrocoagulation (EC) is an eco-friendly technique that combines the functions and advantages of conventional coagulation, flotation, and electrochemistry in water and wastewater treatment. The aim of present study was to assess the potential of electrocoagulation process in removing COD, BOD, TSS, turbidity, sulphate, nitrate, chloride and TDS from municipal wastewater. This experimental study was carried out at a batch system by using copper and aluminium electrodes aiming to treat the municipal wastewater at (0, 7, 14, and 21 volts, 50mamp for 60minutes) from Sample Baloch Colony (SBC), Sample Rind Goth (SRG), Sample Cattle Colony (SCC), Sample Pakistan Machine Tool Factory (SPMTF) and Sample Mehran Highway (SMH). The results revealed that the maximum removal efficiency of COD 96% for SPMTF, BOD 38.5% for SPMTF, TSS 98.14% for SMH, Turbidity 95.7% for SPMTF, Sulphate 95.9% for SRG, Nitrate 95.23% for SMH, Chloride 97.92% for SMH and TDS 96.9% for SRG at 21 volt. The present study suggested that the treated wastewater could be safely reuse for lawns, parks, tree plantation and recreation purpose
