    Dueling Electrospray Implemented on a Traveling-Wave Ion Mobility/Time-Of-Flight Mass Spectrometer: Towards a Gas-Phase Workbench for Structural Biology

    • Covalent chemistry was performed in an ion mobility/mass spectrometer (IM/MS). • The effects of various parameters on the extent of reactions was observed. • Angiotensin I and ubiquitin were covalently labeled in the gas-phase. • Ion/ion reactions modify ions with solution-like conformations.The traveling wave trap cell of a commercial ion mobility mass spectrometer (IM/MS) was used as a gas-phase reactor for covalent chemistry by making a simple modification to a standard nanoelectrospray source. Reagents and analytes were generated from pulsed opposite polarity nanoelectrospray sources and isolated by their m/z prior to reaction. Covalent bond formation was first observed with the model peptide angiotensin I. The modification site was identified as the N-terminus of the peptide by collision induced dissociation (CID). The IM cell separated the covalent reaction product from the proton transfer product by their respective ion mobilities. Next, the effects of several trapping parameters, including the trap traveling wave height, the trap RF voltage, and the trap pressure, were evaluated. Decreasing traveling wave height and increasing RF voltage and pressure increased the number of proton transfer events from apomyoglobin to reagent anions. The 6+ charge state of ubiquitin generated from nanospray under native-like conditions was covalently modified in the gas phase through ion/ion reactions. Probing the reacted protein with CID led to the assignment of lysine 29 and arginine 54 as reactive nucleophiles accessible to the reagent. IM analysis of the unmodified native-like 6+ charge state revealed that the gas-phase structure of the protein in the trap was in its compact form.     A novel cognitive cueing approach to gait retraining in Parkinson’s disease: A pilot study

    Background: Parkinson’s disease (PD) impairs gait performance, which can lead to falls and decreased quality of life. This study examined the feasibility of implementing a novel home-based intervention designed to elicit gait improvement in individuals with PD. Methods: Five participants with PD completed a two-week home-based gait retraining intervention designed around guided video feedback. Semi-structured interviews were conducted postintervention and two months postintervention to acquire feedback from the participants about their experience with the intervention. Spatiotemporal parameters of gait and functional mobility were assessed pre and postintervention and at two months postintervention. Results: Participants reported high levels of usability and expressed they believed that the intervention improved their gait and led to a fortified sense of ability and revived sense of empowerment. Comparisons of spatiotemporal and mobility parameters of gait identified that improvements occurred between preintervention and postintervention—step length (x̄ = 10.7%), gait velocity (x̄ = 15.1%), and TUG scores (x̄ = 9.8%)—and between preintervention and two months postintervention—step length (x̄ = 3.9%), gait velocity (x̄ = 9.9%), and TUG scores (x̄ = 4.2%). Conclusions: Guided home-based video training has potential to be an effective treatment strategy for improving gait impairment among individuals with PD

    Performance of formulae based estimates of glomerular filtration rate for carboplatin dosing in stage 1 seminoma

    <b>Background:</b> Single cycle carboplatin, dosed by glomerular filtration rate (GFR), is standard adjuvant therapy for stage 1 seminoma. Accurate measurement of GFR is essential for correct dosing. Isotopic methods remain the gold standard for the determination of GFR. Formulae to estimate GFR have improved the assessment of renal function in non-oncological settings. We assessed the utility of these formulae for carboplatin dosing.<p></p> <b>Methods:</b> We studied consecutive subjects receiving adjuvant carboplatin for stage 1 seminoma at our institution between 2007 and 2012. Subjects underwent 51Cr-ethylene diamine tetra-acetic acid (EDTA) measurement of GFR with carboplatin dose calculated using the Calvert formula. Theoretical carboplatin doses were calculated from estimated GFR using Chronic Kidney Disease-Epidemiology (CKD-EPI), Management of Diet in Renal Disease (MDRD) and Cockcroft–Gault (CG) formulae with additional correction for actual body surface area (BSA). Carboplatin doses calculated by formulae were compared with dose calculated by isotopic GFR; a difference <10% was considered acceptable.<p></p> <b>Results:</b> 115 patients were identified. Mean isotopic GFR was 96.9 ml/min/1.73 m2. CG and CKD-EPI tended to overestimate GFR whereas MDRD tended to underestimate GFR. The CKD-EPI formula had greatest accuracy. The CKD-EPI formula, corrected for actual BSA, performed best; 45.9% of patients received within 10% of correct carboplatin dose. Patients predicted as underdosed (13.5%) by CKD-EPI were more likely to be obese (p = 0.013); there were no predictors of the 40.5% receiving an excess dose.<p></p> <b>Conclusions:</b> Our data support further evaluation of the CKD-EPI formula in this patient population but clinically significant variances in carboplatin dosing occur using non-isotopic methods of GFR estimation. Isotopic determination of GFR should remain the recommended standard for carboplatin dosing when accuracy is essential.<p></p&gt

    Star Clusters in M31: I. A Catalog and a Study of the Young Clusters

    We present an updated catalog of 1300 objects in the field of M31, including 670 likely star clusters of various types. Archival images were inspected to confirm cluster classifications where possible, but most of the classifications were based on spectra taken of ~1000 objects with the Hectospec fiber positioner and spectrograph on the 6.5m MMT. The spectra and images of young clusters are analyzed in detail in this paper. Ages, reddenings and masses of 140 young clusters are derived by comparing the observed spectra and photometry with model spectra. We find these to have masses as great as 10^5 with a median of 10^4 M_sun, and a median age of 0.25 Gyr. Thus these clusters are similar in mass to the LMC young massive clusters, and are in between Milky Way open clusters and the globulars of M31 and the Milky Way. Most of the M31 young clusters have the low concentration typical of Milky Way open clusters, and we expect that most of these will be disrupted in the next Gyr, but a few have high concentrations and will likely survive longer. The spatial distribution of the young clusters is well correlated with the star-forming regions as mapped out by mid-IR emission.     Medical nutrition therapy for gestational diabetes mellitus in Australia : what has changed in 10 years and how does current practice compare with best practice?

    Background: The present study aimed to report Australian dietetic practice regarding management of gestational diabetes mellitus (GDM) and to make comparisons with the findings from a 2009 survey of dietitians and with the Academy of Nutrition and Dietetics Evidence-Based Nutrition Practice Guidelines (NPG). Methods: Cross-sectional surveys were conducted in 2019 and 2009 of dietitians providing medical nutrition therapy (MNT) to women with GDM in Australia. The present study compares responses on demographics, dietetic assessment and interventions, and guideline use in 2019 vs. 2009. Results: In total, 149 dietitians (2019) and 220 (2009) met survey inclusion criteria. In both surveys >60% of respondents reported dietary interventions aiming for >45% energy from carbohydrate, 15%–25% energy from protein and 15%–30% energy from fat. Many variations in MNT found in 2009 continued to be evident in 2019, including the percentage of energy from carbohydrate aimed for (30%–65% in 2019 vs. 20%–75% in 2009) and the wide range in the recommended minimum daily carbohydrate intake (40–220 and 60–300 g). Few dietitians reported aiming for the NPG minimum of 175 g of carbohydrate daily in both surveys (32% in 2019 vs. 26% in 2009). There were, however, some significant increases in MNT consistent with NPG recommendations in 2019 vs. 2009, including the minimum frequency of visits provided (49%, n = 61 vs. 33%, n = 69; p < 0.001) and provision of gestational weight gain advice (59%, n = 95 vs. 40%, n = 195; p < 0.05). Conclusions: Although many dietitians continue to provide MNT consistent with existing NPG, there is a need to support greater uptake, especially for recommendations regarding carbohydrate intake
