    An Analysis of the Maintenance and Supply System of the Atlantic Missile Range Communications Network

    The maintenance and supply system of the Atlantic Missile Range Communications Network (AMRCN) is modeled to determine the optimum level of men and parts which must be available at Cape Kennedy, the source of supply, in order to minimize the level of failed equipment at the four remote sites. The system is simulated utilizing a DYNAMO model. DYNAMO is a compiler for translating and running continuous models (models described by a set of differential equations). Flow diagrams of the breakdowns, men, and parts and the information flow between these segments of the model are presented. The results obtained are discussed and the optimal solution noted

    Family, Social Class, and Evangelism: Developing a Strategic Evangelistic Plan to Engage the Middle and Upper-Class Families in the Dennery Community of Saint Lucia

    Problem In the St. Lucia Mission of Seventh-day Adventist traditional evangelistic tent meetings have resulted in a steady growth in church membership. However, the majority of converts added to the church has consistently come from the lower-class individuals of the society. Not surprising therefore, there has been a small increase in new members from the middle and upper-class communities. Arguably, the traditional approach to evangelism conducted by the Seventh-day Adventist Church in St. Lucia is not as effective in reaching the middle and upper-class subgroups as it is with members of the lower social class groups. The need to pay urgent special attention to all social groups including middle and upper-class individuals is an imperative because of the inclusive nature of the Gospel commission. Method The method that was employed to address the problem was a family life education initiative. A survey instrument (SI) was administered to collect data from the Dennery community. This information was used to guide the formulation of an evangelism plan that resulted in contacting 130 persons. The participants voluntarily received a survey that comprised five sections of inquiry. These areas include information about marriage and family life, health and family, social issues, and spirituality and the family. A total of 91 persons volunteered and completed the field survey. Fourteen interviewers administered the survey to the 91 participants over a period of seven days. In addition, a number of activities were planned and executed to engage members of the middle and upper-class. These data were evaluated and the findings helped to guide the development and execution of the family life education plan administered to engage the affluent family members in the Dennery community. The results indicated that the wealthy in Saint Lucia required a more creative and inviting approach. The outcomes showed that the family life education approach was a friendly and viable option for engaging the affluent social classes in St. Lucia. Results The execution of a family life education model of evangelism has generated a response to the Gospel from the affluent family members. More than one hundred persons responded to the initiative by participating in more than three of the events, four were baptized, one couple were married and others have become the new advocates for this family life education initiative. Conclusion From the outcome of the project, the family life education approach offered to the middle and upper-class families has proven to be a viable option. This approach: (a) engaged the rich and wealthy families, (b) was a relationally viable option, (c) did effect transformation, (d) was a creative way of presenting the gospel, (e) engineered a culture of inclusion of the rich, (f) saw greater participation of the membership in the work, (g) has a natural discipling component, and (h) has helped the voice and service of the church become more audible and attractive. This plan has effectively engaged the wealthy middle and upper-class families

    The impact of co-participations on medical cooperative loss ratio projections in Rio Grande do Sul

    O mercado de Saúde Suplementar brasileiro enfrenta diversos desafios para manter o índice de sinistralidade das Operadoras de Planos de Assistência à Saúde em um patamar aceitável, diversos mecanismos são utilizados em tentativas de redução deste índice para possibilitar a continuidade de operação deste mercado, sendo um destes a coparticipação. Este estudo buscou verificar o impacto das coparticipações nas projeções de sinistralidade das Cooperativas Médicas, uma modalidade de Operadora de Saúde, no Brasil. Para realizar as projeções de cenários, foram utilizadas simulações de Monte Carlo com base nos dados da ANS e do IESS de cinco anos, de 2015 a 2019, referentes a este ramo do mercado de saúde suplementar. Para efeitos de comparação, primeiramente, foram realizadas projeções sem considerar os valores de recuperação por coparticipação e, na sequência, projeções que consideravam estas recuperações. O estudo verificou que existe um impacto significativo financeiro e estatístico na redução dos índices de sinistralidade quando utilizado o mecanismo da coparticipação nos Planos de Saúde deste setor, apresentando contribuição para o meio acadêmico, para a área atuarial e para o mercado de saúde suplementar, de forma a confirmar o papel da coparticipação na redução do índice de sinistralidade.The Brazilian Supplemental Health Insurance market faces several challenges to maintain the loss ratio of Health Care Plan Operators at an acceptable level, several mechanisms are used in attempts to reduce this rate to enable the continuity of operation in this market, being one of these the co-participation. This study pursued to verify the impact of co-participations in the loss ratio projections of Medical Cooperatives, a modality of Health Operator, in Brazil. To perform the scenario projections, Monte Carlo simulations were used based on ANS and IESS data for five years, from 2015 to 2019, referring to this branch of the supplementary health market. For comparison purposes, first, projections were made without considering the recovery values for co-participation and, subsequently, projections that considered these recoveries. The study found that there is a significant financial and statistical impact on the reduction of loss ratio when using the co-participation mechanism in the Health Plans of this sector, presenting a contribution to the academic environment, to the actuarial area and to the supplementary health market, in order to confirm the role of co-participation in reducing the loss ratio

    Kampung Empat Balai is one of the old villages in Kuok District, Kampar Regency. The kampung comprises traditional Malay houses as cultural heritages that urge to preserve. The house has a unique roof shape and carvings pattern. However, the house is endangering due to a lack of maintenance. This research has aimed to identify the construction of houses that belong to the Bawi ethnic group people. A qualitative descriptive approach has been carried out to investigate and analyse this Malay house. This research found that the Bawi's house has a curving roof type which is known as part of the Lotiok roof type. This stilt house has a structural system in the form of a series of line elements with non-rigid joints that bind each other using notches and pegs as a connection system that is anticipated for earthquakes.     Reliability and validity testing of team emergency assessment measure in a distributed team context

    Medical multi-professional teams are increasingly collaborating via telemedicine. In distributed team settings, members are geographically separated and collaborate through technology. Developing improved training strategies for distributed teams and finding appropriate instruments to assess team performance is necessary. The Team Emergency Assessment Measure (TEAM), an instrument validated in traditional collocated acute-care settings, was tested for validity and reliability in this study when used for distributed teams. Three raters assessed video recordings of simulated team training scenarios (n = 18) among teams with varying levels of proficiency working with a remotely located physician via telemedicine. Inter-rater reliability, determined by intraclass correlation, was 0.74–0.92 on the TEAM instrument’s three domains of leadership, teamwork, and task management. Internal consistency (Cronbach’s alpha) ranged between 0.89–0.97 for the various domains. Predictive validity was established by comparing scores with proficiency levels. Finally, concurrent validity was established by high correlations, >0.92, between scores in the three TEAM domains and the teams’ overall performance. Our results indicate that TEAM can be used in distributed acute-care team settings and consequently applied in future-directed learning and research on distributed healthcare teams

    Perfil epidemiológico da sífilis gestacional e congênita em uma área de abrangência da região Pinheirinho de São José do Rio Preto

    The aim was to identify the epidemiological profile of pregnant women and newborns with syphilis in a neighborhood of São José do Rio Preto/SP. Descriptive, quantitative and retrospective study, using medical records and the database of the Information System for Notifiable Diseases of 28 pregnant women with gestational syphilis and five newborns with congenital syphilis, at the Solo Sagrado UBSF in the area covered by the Pinheirinho de Pinheirinho Region. São José do Rio Preto/SP in the period 2019/2020. In 2019, 268 pregnant women underwent prenatal care and 21 of them were notified with a diagnosis of gestational syphilis, only four babies with congenital syphilis tested positive in the treponemal/non-treponemal test at delivery/curettage and two of them evolved to miscarriage. In 2020, 275 pregnant women underwent prenatal care and seven were diagnosed and notified with gestational syphilis and only one baby with congenital syphilis tested positive in the treponemal/non-treponemal test at delivery. Considering that the main difficulties encountered in reducing vertical transmission are associated with late diagnosis or non-treatment, or inadequate treatment of pregnant women, in addition, socioeconomic, demographic and care factors contribute to the increase in its incidence.Objetivou-se identificar o perfil epidemiológico das gestantes e recém-nascidos com sífilis em um bairro de São José do Rio Preto/SP. Estudo descritivo, quantitativo e retrospectivo, por meio de prontuários e banco de dados do Sistema de Informação de Agravos de Notificação de 28 gestantes com sífilis gestacional e cinco recém-nascidos com sífilis congênita, na UBSF Solo Sagrado da área de abrangência da Região Pinheirinho de São José do Rio Preto/SP no período de 2019/2020. Em 2019, 268 gestantes realizaram pré-natal e 21 delas foram notificadas com diagnóstico de sífilis gestacional, apenas quatro bebês com sífilis congênita testaram positivo no teste treponêmico/não treponêmico no parto/curetagem e dois deles evoluíram para aborto. Já em 2020, 275 gestantes realizaram o pré-natal e sete foram diagnosticadas e notificadas com sífilis gestacional e apenas um bebê com sífilis congênita testou positivo no teste treponêmico/não treponêmico no parto. Considerando que as principais dificuldades encontradas para redução da transmissão vertical estão associadas ao diagnóstico tardio ou não tratamento, ou o tratamento inadequado das gestantes, além disso, fatores socioeconômicos, demográficos e assistenciais contribuem para o aumento da sua incidência
