Molecular characterization of multidrug resistant Enterobacterales strains isolated from liver and kidney transplant recipients in Spain
The objective of this study was to analyse the mechanisms of resistance to carbapenems and other extended-spectrum-?-lactams and to determine the genetic relatedness of multidrug-resistant Enterobacterales (MDR-E) causing colonization or infection in solid-organ transplantation (SOT) recipients. Prospective cohort study in kidney (n= 142), liver (n= 98) or kidney/pancreas (n= 7) transplant recipients between 2014 and 2018 in seven Spanish hospitals. We included 531 MDR-E isolates from rectal swabs obtained before transplantation and weekly for 4?6 weeks after the procedure and 10 MDR-E from clinical samples related to an infection. Overall, 46.2% Escherichia coli, 35.3% Klebsiella pneumoniae, 6.5% Enterobacter cloacae, 6.3% Citrobacter freundii and 5.7% other species were isolated. The number of patients with MDR-E colonization post-transplantation (176; 71.3%) was 2.5-fold the number of patients colonized pre-transplantation (71; 28.7%). Extended spectrum ?-lactamases (ESBLs) and carbapenemases were detected in 78.0% and 21.1% of MDR-E isolates respectively. In nine of the 247 (3.6%) transplant patients, the microorganism causing an infection was the same strain previously cultured from surveillance rectal swabs. In our study we have observed a low rate of MDR-E infection in colonized patients 4?6 weeks post-transplantation. E. coli producing blaCTX-M-G1 and K. pneumoniae harbouring blaOXA-48 alone or with blaCTX-M-G1 were the most prevalent MDR-E colonization strains in SOT recipients.Acknowledgements The authors thank Mª Jesús Lecea and Laura Álvarez for technical assistance. Transmission Clusters, Predominantly Associated With Men Who Have Sex With Men, Play a Main Role in the Propagation of HIV-1 in Northern Spain (2013-2018)
Viruses of HIV-1-infected individuals whose transmission is related group phylogenetically in transmission clusters (TCs). The study of the phylogenetic relations of these viruses and the factors associated with these individuals is essential to analyze the HIV-1 epidemic. In this study, we examine the role of TCs in the epidemiology of HIV-1 infection in Galicia and the Basque County, two regions of northern Spain. A total of 1,158 HIV-1-infected patients from both regions with new diagnoses (NDs) in 2013-2018 were included in the study. Partial HIV-1 pol sequences were analyzed phylogenetically by approximately maximum-likelihood with FastTree 2. In this analysis, 10,687 additional sequences from samples from HIV-1-infected individuals collected in Spain in 1999-2019 were also included to assign TC membership and to determine TCs' sizes. TCs were defined as those which included viruses from ≥4 individuals, at least 50% of them Spaniards, and with ≥0.95 Shimodaira-Hasegawa-like node support in the phylogenetic tree. Factors associated to TCs were evaluated using odds ratios (OR) and their 95% CI. Fifty-one percent of NDs grouped in 162 TCs. Male patients (OR: 2.6; 95% CI: 1.5-4.7) and men having sex with men (MSM; OR: 2.1; 95% CI: 1.4-3.2) had higher odds of belonging to a TC compared to female and heterosexual patients, respectively. Individuals from Latin America (OR: 0.3; 95% CI: 0.2-0.4), North Africa (OR: 0.4; 95% CI: 0.2-1.0), and especially Sub-Saharan Africa (OR: 0.02; 95% CI: 0.003-0.2) were inversely associated to belonging to TCs compared to native Spaniards. Our results show that TCs are important components of the HIV-1 epidemics in the two Spanish regions studied, where transmission between MSM is predominant. The majority of migrants were infected with viruses not belonging to TCs that expand in Spain. Molecular epidemiology is essential to identify local peculiarities of HIV-1 propagation. Calibrachoa: Plagas y manejo
Calibrachoa es un género exclusivamente americano, afín a Petunia, de distribución predominan temente sub-tropical atlántica entre los paralelos 18 y 37 ºS. En la Argentina existen 13 especies nativas presentes principalmente en la Mesopotamia. Se las encuentra creciendo en ambientes abiertos con suelo arenoso o pedregoso de buen drenaje de Entre Ríos y Corrientes, o en suelos rojizos ácidos de Misiones o norte de Corrientes.
Desde su creación, el Instituto de Floricultura de INTA ha trabajado en el desarrollo de variedades
nacionales de calibrachoa como fruto del mejoramiento genético de germoplasma nativo. Tanto
es así, que el mercado florícola argentino cuenta con tres variedades nacionales: Overá fucsia INTA
y Pampa salmón INTA (inscriptas en el Registro Nacional de Cultivares del Instituto Nacional de
Semillas) e INTA 06575 (patentada en la Patent and Trademark Office de Estados Unidos).
Las variedades de calibrachoa desarrolladas por INTA son plan tas herbáceas perennes, de pleno sol y comportamiento anual.
Florecen constantemente desde la primavera hasta el otoño en forma muy abundante, con un pico entre el fin de la primavera y el inicio del verano. Ofrecen néctar a visitantes florales, son vigo rosas y con amplia cobertura del espacio, recomendables para ser plantadas en macetas colgantes o macetas grandes (Figura 1) o
utilizadas en espacios abiertos a pleno sol, en canteros y borduras (Figura 2). Son de fácil mantenimiento.
La variedad Overá fucsia INTA (Figura 3) es de porte erecto, compacta, con floración abundante.
Sus flores son de color fucsia y aparecen en forma temprana, independientemente del fotoperio do. Prefiere suelos o sustratos levemente ácidos (pH 5,5-6,5). Es ideal para cultivo en macetas,
pero también presenta un buen comportamiento en canteros o borduras.
La variedad Pampa salmón INTA (Figura 4) es de porte semierecto. Sus flores son de color salmón.
La floración es más tardía que en otras variedades, requiere de fotoperiodo mayor a 12 horas y alta
iluminación para florecer plenamente. Es ideal para espacios abiertos como canteros o borduras
a pleno sol. A diferencia de las otras variedades, no es tan exigente de suelos o sustratos ácidos.
La variedad INTA 06575 (Figura 5) es la de crecimiento más vigoroso. Sus flores son de color púr pura. Al igual que las de Overá fucsia INTA, aparecen en forma temprana, independientemente
del fotoperiodo. Prefiere suelos o sustratos mediana a ligeramente ácidos (pH 5,5-6,5) y tolera las

Evolution of Quality of Life and Treatment Adherence after One Year of Intermittent Bladder Catheterisation in Functional Urology Unit Patients
Objective: To determine patient difficulties and concerns when performing IBC (Intermittent Bladder Catheterisation), as well as the evolution of adherence, quality of life, and emotional state of patients one year after starting IBC. Method: A prospective, observational, multicentre study conducted in 20 Spanish hospitals with a one-year follow-up. Data sources were patient records and the King's Health Questionnaire on quality of life, the Mini-Mental State Examination (MMSE), and the Hospital Anxiety and Depression Scale (HADS). Perceived adherence was measured using the ICAS (Intermittent Catheterization Adherence Scale) and perceived difficulties with IBC were assessed using the ICDQ (Intermittent Catheterization Difficulty Questionnaire). For data analysis, descriptive and bivariate statistics were performed for paired data at three points in time (T1: one month, T2: three months, T3: one year). Results: A total of 134 subjects initially participated in the study (T0), becoming 104 subjects at T1, 91 at T2, and 88 at T3, with a mean age of 39 years (standard deviation = 22.16 years). Actual IBC adherence ranged from 84.8% at T1 to 84.1% at T3. After one year of follow-up, a statistically significant improvement in quality of life (p <= 0.05) was observed in all dimensions with the exception of personal relationships. However, there were no changes in the levels of anxiety (p = 0.190) or depression (p = 0.682) at T3 compared to T0. Conclusions: Patients requiring IBC exhibit good treatment adherence, with a significant proportion of them performing self-catheterisation. After one year of IBC, a significant improvement in quality of life was noted, albeit with a significant impact on their daily lives and their personal and social relationships. Patient support programmes could be implemented to improve their ability to cope with difficulties and thus enhance both their quality of life and the maintenance of their adherence
Long daytime napping is associated with increased adiposity and type 2 diabetes in an elderly population with metabolic syndrome
Research examining associations between objectively-measured napping time and type
2 diabetes (T2D) is lacking. This study aimed to evaluate daytime napping in relation to T2D and
adiposity measures in elderly individuals from the Mediterranean region. A cross-sectional analysis of
baseline data from 2190 elderly participants with overweight/obesity and metabolic syndrome, in the
PREDIMED-Plus trial, was carried out. Accelerometer-derived napping was measured. Prevalence
ratios (PR) and 95% confidence intervals (CI) for T2D were obtained using multivariable-adjusted
Cox regression with constant time. Linear regression models were fitted to examine associations of
napping with body mass index (BMI) and waist circumference (WC). Participants napping ≥90 min
had a higher prevalence of T2D (PR 1.37 (1.06, 1.78)) compared with those napping 5 to <30 min per
day. Significant positive associations with BMI and WC were found in those participants napping
≥30 min as compared to those napping 5 to <30 min per day. The findings of this study suggest that
longer daytime napping is associated with higher T2D prevalence and greater adiposity measures in
an elderly Spanish population at high cardiovascular risk
Waist to height ratio and cardiovascular risk factors in elderly individuals at high cardiovascular risk
Introduction: Several anthropometric measurements have been associated with cardiovascular disease, type-2 diabetes
mellitus and other cardiovascular risk conditions, such as hypertension or metabolic syndrome. Waist-to-height-ratio has
been proposed as a useful tool for assessing abdominal obesity, correcting other measurements for the height of the
individual. We compared the ability of several anthropometric measurements to predict the presence of type-2 diabetes,
hyperglycemia, hypertension, atherogenic dyslipidemia or metabolic syndrome.
Materials and Methods: In our cross-sectional analyses we included 7447 Spanish individuals at high cardiovascular risk,
men aged 55–80 years and women aged 60–80 years, from the PREDIMED study. Logistic regression models were fitted to
evaluate the odds ratio of presenting each cardiovascular risk factor according to various anthropometric measures. The
areas under the receiver-operating characteristic curve (AUC) were used to compare the predictive ability of these
measurements.
Results: In this relatively homogeneous cohort with 48.6% of type-2 diabetic individuals, the great majority of the studied
anthropometric parameters were significantly and positively associated with the cardiovascular risk factors. No association
was found between BMI and body weight and diabetes mellitus. The AUCs for the waist-to-height ratio and waist
circumference were significantly higher than the AUCs for BMI or weight for type-2 diabetes, hyperglycemia, atherogenic
dyslipidemia and metabolic syndrome. Conversely, BMI was the strongest predictor of hypertension.
Conclusions: We concluded that measures of abdominal obesity showed higher discriminative ability for diabetes mellitus,
high fasting plasma glucose, atherogenic dyslipidemia and metabolic syndrome than BMI or weight in a large cohort of
elderly Mediterranean individuals at high cardiovascular risk. No significant differences were found between the predictive
abilities of waist-to-height ratio and waist circumference on the metabolic disease
Long-term follow-up of IPEX syndrome patients after different therapeutic strategies : an international multicenter retrospective study
Background: Immunodysregulation polyendocrinopathy enteropathy x-linked(IPEX) syndrome is a monogenic autoimmune disease caused by FOXP3 mutations. Because it is a rare disease, the natural history and response to treatments, including allogeneic hematopoietic stem cell transplantation (HSCT) and immunosuppression (IS), have not been thoroughly examined.
Objective: This analysis sought to evaluate disease onset, progression, and long-term outcome of the 2 main treatments in long-term IPEX survivors.
Methods: Clinical histories of 96 patients with a genetically proven IPEX syndrome were collected from 38 institutions worldwide and retrospectively analyzed. To investigate possible factors suitable to predict the outcome, an organ involvement (OI) scoring system was developed.
Results: We confirm neonatal onset with enteropathy, type 1 diabetes, and eczema. In addition, we found less common manifestations in delayed onset patients or during disease evolution. There is no correlation between the site of mutation and the disease course or outcome, and the same genotype can present with variable phenotypes. HSCT patients (n = 58) had a median follow-up of 2.7 years (range, 1 week-15 years). Patients receiving chronic IS (n 5 34) had a median follow-up of 4 years (range, 2 months-25 years). The overall survival after HSCT was 73.2% (95% CI, 59.4-83.0) and after IS was 65.1% (95% CI, 62.8-95.8). The pretreatment OI score was the only significant predictor of overall survival after transplant (P = .035) but not under IS.
Conclusions: Patients receiving chronic IS were hampered by disease recurrence or complications, impacting long-term.disease-free survival. When performed in patients with a low OI score, HSCT resulted in disease resolution with better quality of life, independent of age, donor source, or conditioning regimen
Development of the PSYCHS: Positive SYmptoms and Diagnostic Criteria for the CAARMS Harmonized with the SIPS
Aim: To harmonize two ascertainment and severity rating instruments commonly used for the clinical high risk syndrome for psychosis (CHR-P): the Structured Interview for Psychosis-risk Syndromes (SIPS) and the Comprehensive Assessment of At-Risk Mental States (CAARMS). Methods: The initial workshop is described in the companion report from Addington et al. After the workshop, lead experts for each instrument continued harmonizing attenuated positive symptoms and criteria for psychosis and CHR-P through an intensive series of joint videoconferences. Results: Full harmonization was achieved for attenuated positive symptom ratings and psychosis criteria, and modest harmonization for CHR-P criteria. The semi-structured interview, named Positive SYmptoms and Diagnostic Criteria for the CAARMS Harmonized with the SIPS (PSYCHS), generates CHR-P criteria and severity scores for both CAARMS and SIPS. Conclusions: Using the PSYCHS for CHR-P ascertainment, conversion determination, and attenuated positive symptom severity rating will help in comparing findings across studies and in meta-analyses
