    O módulo aborda temas relevantes para a prática odontológica na Atenção Básica, tais como as dores de origem endodôntica e de origem periodontal, bem como traumas dental e de face. Na unidade sobre dor de origem endodôntica são abordados subsídios para as práticas diagnósticas e terapêuticas no cuidado a pessoas portadoras deste tipo de dor. A unidade sobre dor de origem periodontal e de mucosa enfoca este evento comum do cotidiano do profissional dentista, ressaltando a importância de diagnosticar e promover a conduta necessária para sua prevenção e tratamento. A unidade sobre trauma dental inicia destacando que as situações de traumatismo dental são eventos agudos cada vez mais frequentes na clínica odontológica da Atenção Básica, e aponta as principais intervenções e ações preventivas, que podem ser realizadas tanto no âmbito individual quanto no coletivo. A unidade sobre trauma de face destaca a importância do treinamento de cirurgiões-dentistas para o atendimento inicial desse trauma, orienta sobre tipos de trauma de face, fraturas do complexo naso-orbitoetmoidal, do complexo zigomático-maxilar e do arco zigomático, além de abordar procedimentos terapêuticos pertinentes.1.0UNA-SUSMinistério da Saúd

    OBJECTIVE: Primary pigmented nodular adrenocortical disease (PPNAD) is the main endocrine manifestation of Carney complex, a multiple neoplasia syndrome caused by PRKAR1A gene mutations. The presence of PRKAR1A loss of heterozygosity (LOH) in adrenocortical tumorigenesis remains controversial. The aim of the present study is to investigate the presence of PRKAR1A LOH in adrenocortical cells in a patient with Carney complex. METHODS: The LOH was investigated using a PRKAR1A informative intragenic marker by GeneScan software analysis in DNA obtained from laser-captured microdissected cells of several adrenal nodules. Patients: A young adult male patient with Carney complex and his family were studied. RESULTS: A novel heterozygous mutation (p. Y21X) was identified at PRKAR1A in blood DNA of the male proband and his relatives. No PRKAR1A LOH was evidenced in the laser-captured microdissected cells from PPNAD tissue by different methodologies. CONCLUSION: We identified a new PRKAR1A nonsense mutation and in addition we did not evidence PRKAR1A LOH in laser-captured nodules cells, suggesting that adrenocortical tumorigenesis in PPNAD may occurs apart from the second hit.OBJETIVO: A doença adrenocortical nodular pigmentosa primária (PPNAD) é uma das manifestações do complexo de Carney, uma neoplasia endócrina múltipla causada por mutações no PRKAR1A. A perda de heterozigose (LOH) do PRKAR1A na tumorigenese adrenal permanece controversa dada à possibilidade de contaminação com o tecido normal. Nosso objetivo foi investigar a presença de LOH no PRKAR1A a partir de células do nódulo adrenal de um paciente com complexo de Carney. MÉTODOS: A pesquisa da LOH do PRKAR1A foi realizada através do estudo de um marcador intragênico em DNA de células do nódulo adrenal microdissecadas a laser, evitando contaminação com o tecido normal. Pacientes: Um paciente com PPNAD e cinco familiares foram estudados. RESULTADOS: A nova mutação (p. Y21X) foi identificada no PRKAR1A sem evidência de LOH no tecido adrenal. CONCLUSÃO: Identificamos uma nova mutação no PRKAR1A e não evidenciamos LOH nas células dos nódulos adrenocorticais, sugerindo que a PPNAD possa ocorrer na ausência de um segundo evento molecular

    Quality of peach palm heart minimally processed

    The peach palm heart is a hypocaloric gourmet vegetable, rich in fiber and minerals such as potassium, calcium, and magnesium, and it can be incorporated into calorie restriction diets. Minimum processing consists of sanitizing and cutting the products before reaching the consumer, providing convenience to consumption without losing its quality, thus resulting in sufficient shelf life for distribution and marketing. Sanitation, appropriate packaging, and refrigeration are essential practices to extend the shelf life of minimally processed products. Therefore, this study evaluated the quality of peach palm heart produced in Pariquera-Açu – SP, minimally processed in the form of chopped cut and stick, packed in different plastic packages and kept under refrigeration (10 ± 0.5 °C and 85 ± 5% RH) for 9 days. Hearts-of-palm, in chopped cut and stick, packed in PET and PP trays with lids, had their fresh masses and luminosity preserved for 9 days, but there was a failure in the sealing of the lid, which allowed gas exchange with the environment, compromising its quality. The peach palm heart cut in stick, packed in a stretchable PVC coated PE tray, provided protection and efficiency in maintaining quality. The packaging of chopped hearts-of-palm in LDPE packages (0.04 and 0.07 mm), over the storage time, resulted in a high CO2 concentration inside the package, thus, these packages are not recommended

    Synthesis and evaluation of new modified diglycolamides with different stereochemistry for extraction of tri- and tetravalent metal ions

    Americium separation from high level nuclear waste can provide certain benefits such as reducing the long-term heat generation or the possibility of re-use as fuel in innovative reactors. The separation is carried out through solvent extraction. Among commonly used ligands, diglycolamide (DGA) derivatives have shown high distribution ratios for trivalent lanthanides (Ln(iii)) and actinides (An(iii)). Recently, different diastereomers of DGAs with substitutions in the backbone have shown an inverse selectivity for Am over Cm, in comparison with commonly used DGAs. Hence, we synthesised, characterized, and tested novel syn- and anti-diastereomers of tetra-n-octyl and tetra-n-decyl DGAs with two propyl and ethyl-methyl substituents at the backbone methylene carbon atoms. The distribution ratios for An(iii) and Ln(iii) were found to be generally lower than for the unsubstituted parent molecules due to steric hindrance. On the other hand, 239Pu was much better extracted. All tested ligands showed the inverse selectivity for Am with an Am/Cm separation factor (SFAm/Cm) around 1.5. Finally, slope analysis confirmed a 1 : 3 metal : ligand ratio for the DGA complexes.</p

    Surgical site infection after gastrointestinal surgery in high-income, middle-income, and low-income countries: a prospective, international, multicentre cohort study

    Background: Surgical site infection (SSI) is one of the most common infections associated with health care, but its importance as a global health priority is not fully understood. We quantified the burden of SSI after gastrointestinal surgery in countries in all parts of the world. Methods: This international, prospective, multicentre cohort study included consecutive patients undergoing elective or emergency gastrointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating centres were stratified into high-income, middle-income, and low-income groups according to the UN's Human Development Index (HDI). Data variables from the GlobalSurg 1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using Bayesian multilevel logistic regression models. This trial is registered with ClinicalTrials.gov, number NCT02662231. Findings: Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58·5%) patient were from high-HDI countries (193 hospitals in 30 countries), 3918 (31·2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (10·2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12·3%) patients had SSI within 30 days of surgery. The incidence of SSI varied between countries with high (691 [9·4%] of 7339 patients), middle (549 [14·0%] of 3918 patients), and low (298 [23·2%] of 1282) HDI (p < 0·001). The highest SSI incidence in each HDI group was after dirty surgery (102 [17·8%] of 574 patients in high-HDI countries; 74 [31·4%] of 236 patients in middle-HDI countries; 72 [39·8%] of 181 patients in low-HDI countries). Following risk factor adjustment, patients in low-HDI countries were at greatest risk of SSI (adjusted odds ratio 1·60, 95% credible interval 1·05–2·37; p=0·030). 132 (21·6%) of 610 patients with an SSI and a microbiology culture result had an infection that was resistant to the prophylactic antibiotic used. Resistant infections were detected in 49 (16·6%) of 295 patients in high-HDI countries, in 37 (19·8%) of 187 patients in middle-HDI countries, and in 46 (35·9%) of 128 patients in low-HDI countries (p < 0·001). Interpretation: Countries with a low HDI carry a disproportionately greater burden of SSI than countries with a middle or high HDI and might have higher rates of antibiotic resistance. In view of WHO recommendations on SSI prevention that highlight the absence of high-quality interventional research, urgent, pragmatic, randomised trials based in LMICs are needed to assess measures aiming to reduce this preventable complication
