Isometric versus isotonic exercise for greater trochanteric pain syndrome: a randomised controlled pilot study
Objectives: Greater trochanteric pain syndrome (GTPS) is a common cause of lateral hip pain. Limited evidence exists for the effectiveness of exercise for GTPS. This study aimed to compare the effectiveness of isometric and isotonic exercise for individuals with GTPS.
Methods: This randomised controlled pilot trial recruited 30 participants with GTPS. Both programmes consisted of daily, progressive home exercise for 12 weeks with 8 individual physiotherapy sessions over the trial period. The primary outcome measure was the Victorian Institute of Sport Assessment-Gluteal (VISA-G) and secondary outcome measures included the Numeric Pain Rating Scale (0–10) and an 11-point Global Rating of Change Scale. Outcome measures were assessed at baseline, 4 and 12 weeks.
Results: Twenty-three participants completed the trial. After 12 weeks, mean VISA-G scores improved in both groups; 55–65 in the isometric group and 62–72 in the isotonic group. 55% of the isometric group and 58% of the isotonic group achieved a reduction in pain of at least 2 points (minimally clinically important difference (MCID)) on the Numeric Pain Rating Scale. 64% of the isometric group and 75% of the isotonic group had improved by at least 2 points (MCID) on the Global Rating of Change Scale.
Conclusion: Isometric and isotonic exercise programmes appear to be effective for individuals with GTPS and should be considered in the loading management of patients with this condition
Integrating peer-to-peer functionalities and routing in mobile ad-hoc networks
Mobile Ad-hoc Networks (MANETs) impose strict requirements in terms of battery life, communication overhead and network latency, therefore optimization should be made to applications and services such as domain name service (DNS), dynamic host configuration protocol (DHCP) and session initiation protocol (SIP) if they are to be considered for use on MANETs. Due to the decentralized and self-organizing nature of MANETs, such applications could utilize a distributed name resolution/data storage service. Distributed Hash Tables (DHTs) enable these features by virtually organizing the network topology in a peer-to-peer (P2P) overlay. P2P overlays have been designed to operate on the application layer without knowledge of the underlying network thus causing poor performance. To address this problem, we propose and evaluate two different DHTs integrated with MANET routing in order to optimize the overall performance of MANET communications when P2P applications and services are used. Both architectures share the same functionality such as decentralization, self-reorganization, and self-healing but differ in MANET routing protocol. Performance evaluation using the NS2 simulator shows that these architectures are suited to different scenarios namely increasing network size and peer velocity. Comparisons with other well-known solutions have proven their efficiency with regard to the above requirements
Effectiveness of isometric exercise in the management of tendinopathy: a systematic review and meta-analysis of randomised trials
Objective: To systematically review and critically appraise the literature on the effectiveness of isometric exercise in comparison with other treatment strategies or no treatment in tendinopathy.
Design: A systematic review and meta-analysis of randomised controlled trials.
Data sources: Electronic searches of Medline, Cumulative Index to Nursing and Allied Health Literature, EMBASE and Cochrane were undertaken from inception to May 2020.
Methods: Overall quality of each study was determined based on a combined assessment of internal validity, external validity and precision. For each outcome measure, level of evidence was rated based on the system by van Tulder et al.
Results: Ten studies were identified and included in the review, including participants with patellar (n=4), rotator cuff (n=2), lateral elbow (n=2), Achilles (n=1) and gluteal (n=1) tendinopathies. Three were of good and seven were of poor overall quality. Based on limited evidence (level 3), isometric exercise was not superior to isotonic exercise for chronic tendinopathy either immediately following treatment or in the short term (≤12 weeks) for any of the investigated outcome measures. Additionally, for acute rotator cuff tendinopathy, isometric exercise appears to be no more effective than ice therapy in the short term (limited evidence; level 3).
Summary: Isometric exercise does not appear to be superior to isotonic exercise in the management of chronic tendinopathy. The response to isometric exercise is variable both within and across tendinopathy populations. Isometric exercise can be used as part of a progressive loading programme as it may be beneficial for selected individuals
Finding the mechanism of wave energy flux damping in solar pores using numerical simulations
Context. Solar magnetic pores are, due to their concentrated magnetic fields,
suitable guides for magnetoacoustic waves. Recent observations have shown that
propagating energy flux in pores is subject to strong damping with height;
however, the reason is still unclear. Aims. We investigate possible damping
mechanisms numerically to explain the observations. Methods. We performed 2D
numerical magnetohydrodynamic (MHD) simulations, starting from an equilibrium
model of a single pore inspired by the observed properties. Energy was inserted
into the bottom of the domain via different vertical drivers with a period of
30s. Simulations were performed with both ideal MHD and non-ideal effects.
Results. While the analysis of the energy flux for ideal and non-ideal MHD
simulations with a plane driver cannot reproduce the observed damping, the
numerically predicted damping for a localized driver closely corresponds with
the observations. The strong damping in simulations with localized driver was
caused by two geometric effects, geometric spreading due to diverging field

The Propagation of Coherent Waves Across Multiple Solar Magnetic Pores
Solar pores are efficient magnetic conduits for propagating magnetohydrodynamic wave energy into the outer regions of the solar atmosphere. Pore observations often contain isolated and/or unconnected structures, preventing the statistical examination of wave activity as a function of the atmospheric height. Here, using high-resolution observations acquired by the Dunn Solar Telescope, we examine photospheric and chromospheric wave signatures from a unique collection of magnetic pores originating from the same decaying sunspot. Wavelet analysis of high-cadence photospheric imaging reveals the ubiquitous presence of slow sausage-mode oscillations, coherent across all photospheric pores through comparisons of intensity and area fluctuations, producing statistically significant in-phase relationships. The universal nature of these waves allowed an investigation of whether the wave activity remained coherent as they propagate. Utilizing bisector Doppler velocity analysis of the Ca ii 8542 Å line, alongside comparisons of the modeled spectral response function, we find fine-scale 5 mHz power amplification as the waves propagate into the chromosphere. Phase angles approaching zero degrees between co-spatial line depths spanning different line depths indicate standing sausage modes following reflection against the transition region boundary. Fourier analysis of chromospheric velocities between neighboring pores reveals the annihilation of the wave coherency observed in the photosphere, with examination of the intensity and velocity signals from individual pores indicating they behave as fractured waveguides, rather than monolithic structures. Importantly, this work highlights that wave morphology with atmospheric height is highly complex, with vast differences observed at chromospheric layers, despite equivalent wave modes being introduced into similar pores in the photosphere
Immunotherapy of lung cancer: An update
In Germany lung cancer is the leading cause of cancer-associated death in men. Surgery, chemotherapy and radiation may enhance survival of patients suffering from lung cancer but the enhancement is typically transient and mostly absent with advanced disease; eventually more than 90% of lung cancer patients will die of disease. New approaches to the treatment of lung cancer are urgently needed. Immunotherapy may represent one new approach with low toxicity and high specificity but implementation has been a challenge because of the poor antigenic characterization of these tumors and their ability to escape immune responses. Several different immunotherapeutic treatment strategies have been developed. This review examines the current state of development and recent advances with respect to non-specific immune stimulation, cellular immunotherapy ( specific and non-specific), therapeutic cancer vaccines and gene therapy for lung cancer. The focus is primarily placed on immunotherapeutic cancer treatments that are already in clinical trial or well progressed in preclinical studies. Although there seems to be a promising future for immunotherapy in lung cancer, presently there is not standard immunotherapy available for clinical routine
Polarization aberrations in next-generation giant segmented mirror telescopes (GSMTs) I. Effect on the coronagraphic performance
Next-generation large segmented mirror telescopes are expected to perform
direct imaging and characterization of Earth-like rocky planets, which requires
contrast limits of to at wavelengths from I to J band. One
critical aspect affecting the raw on-sky contrast are polarization aberrations
arising from the reflection from the telescope's mirror surfaces and instrument
optics. We simulate the polarization aberrations and estimate their effect on
the achievable contrast for three next-generation ground-based large segmented
mirror telescopes. We performed ray-tracing in Zemax and computed the
polarization aberrations and Jones pupil maps using the polarization
ray-tracing algorithm. The impact of these aberrations on the contrast is
estimated by propagating the Jones pupil maps through a set of idealized
coronagraphs using hcipy, a physical optics-based simulation framework. The
optical modeling of the giant segmented mirror telescopes (GSMTs) shows that
polarization aberrations create significant leakage through a coronagraphic
system. The dominant aberration is retardance defocus, which originates from
the steep angles on the primary and secondary mirrors. The retardance defocus
limits the contrast to to at 1 at visible
wavelengths, and to at infrared wavelengths. The
simulations also show that the coating plays a major role in determining the
strength of the aberrations. Polarization aberrations will need to be
considered during the design of high-contrast imaging instruments for the next
generation of extremely large telescopes. This can be achieved either through
compensation optics, robust coronagraphs, specialized coatings, calibration,
and data analysis approaches or by incorporating polarimetry with high-contrast

The impact of the Netherlandish landscape tradition on poetry and painting in early modern England
Copyright © 2013 The University of Chicago Press.The relationship between poetry and painting has been one of the most debated issues in the history of criticism. The present article explores this problematic relationship in the context of sixteenth- and seventeenth-century England, taking into account theories of rhetoric, visual perception, and art. It analyzes a rare case in which a specific school of painting directly inspired poetry: in particular, the ways in which the Netherlandish landscape tradition influenced natural descriptions in the poem Poly-Olbion (1612, 1622) by Michael Drayton (1563–1631). Drayton — under the influence of the artistic principles of landscape depiction as explained in Henry Peacham’s art manuals, as well as of direct observation of Dutch and Flemish landscape prints and paintings — successfully managed to render pictorial landscapes into poetry. Through practical examples, this essay will thoroughly demonstrate that rhetoric is capable of emulating pictorial styles in a way that presupposes specialized art-historical knowledge, and that pictorialism can be the complex product as much of poetry and rhetoric as of painting and art-theoretical vocabulary
Different molecular bases underlie the mitochondrial respiratory activity in the homoeothermic spadices of Symplocarpus renifolius and the transiently thermogenic appendices of Arum maculatum
Symplocarpus renifolius and Arum maculatum are known to produce significant heat during the course of their floral development, but they use different regulatory mechanisms, i.e. homoeothermic compared with transient thermogenesis. To further clarify the molecular basis of species-specific thermogenesis in plants, in the present study we have analysed the native structures and expression patterns of the mitochondrial respiratory components in S. renifolius and A. maculatum. Our comparative analysis using Blue native PAGE combined with nano LC (liquid chromatography)-MS/MS (tandem MS) has revealed that the constituents of the respiratory complexes in both plants were basically similar, but that several mitochondrial components appeared to be differently expressed in their thermogenic organs. Namely, complex II in S. renifolius was detected as a 340 kDa product, suggesting an oligomeric or supramolecular structure in vivo. Moreover, the expression of an external NAD(P)H dehydrogenase was found to be higher in A. maculatum than in S. renifolius, whereas an internal NAD(P)H dehydrogenase was expressed at a similar level in both species. Alternative oxidase was detected as smear-like signals that were elongated on the first dimension with a peak at around 200 kDa in both species.
