Estudio piloto sobre el efecto de la suplementación con ádico fólico en la mejor de los niveles de homocisteína, función cognitiva y estado depresivo en trastornos de la conducta alimentaria
Background & aims: Several authors have reported
low folate intake in patients with eating disorders (ED).
This vitamin plays an essential role in synthesis reactions
for neurotransmitters and structural elements of
neurons, and therefore its deficiency has been associated
with the presence of different disorders linked to mental
function. The aim of this study was to determine the effect
of folic acid supplementation on homocysteine levels and
the cognitive and depressive status of a group of patients
with eating disorders with low folate intake.
Subjects/methods: The study was designed as a
randomised, prospective clinical trial, which included 24
participants assigned to two treatment groups for six
months: supplemented group (SG) (10 mg/day of folic
acid [ACFOL®]) and a placebo group (PG). Both groups
maintained their medical, dietary and psychological
treatment. At baseline and end of the intervention,
anthropometric, dietary and biochemical parameters
(plasma homocysteine [Hcy], serum and red blood cell
folate) were recorded. Cognitive and depressive status
questionnaires were administered (Stroop Test, Trail
Making Test and Beck Depression Inventory).
Results: Twenty-two patients completed the study (SG:
12, PG: 10, mean age: 24.2 ± 8.8 years, BMI 18.9 ± 3.5
kg/m2). The SG significantly increased their serum and
red blood cell folate levels and lowered Hcy levels (9.4 ±
2.4 μmol/l vs. 7.5 ± 1.7 μmol/l, P < 0.01). The SG also
significantly improved most of their test scores for cognitive
and depressive status. The PG showed no significant
changes in any of the evaluated variables.
Conclusions: The results show that folic acid supplementation
may be used as another tool within the
comprehensive and multidisciplinary treatment applied
to patients with EDIntroducción y objetivo: Diferentes autores han reportado
una baja ingesta de ácido fólico en pacientes con
Trastornos de la Conducta Alimentaria (TCA). Esta vitamina
desempeña un papel esencial en las reacciones de
síntesis de neurotransmisores y elementos estructurales
de las neuronas y, por lo tanto, su deficiencia se ha asociado
con la presencia de diferentes trastornos relacionados
con la función mental. El objetivo de este estudio fue
determinar el efecto de la suplementación con ácido fólico
sobre los niveles de homocisteína y sobre marcadores de
función cognitiva y depresión en un grupo de pacientes
con TCA con baja ingesta de ácido fólico.
Sujetos y métodos: Estudio clínico randomizado y prospectivo
en el que se incluyeron 24 pacientes asignados a
dos grupos de tratamiento durante un período de 6
meses: grupo suplementado (SG) (10 mg/día de ácido
fólico [ACFOL®]) y grupo placebo (PG). Ambos grupos
mantuvieron su tratamiento médico, dietético y psicológico.
Al inicio del estudio y tras la intervención se evaluaron
parámetros antropométricos, dietéticos y bioquímicos
(homocisteína plasmática [Hcy], folato sérico y
eritrocitario). Como marcadores de función cognitiva y
depresión se administraron diferentes cuestionarios (Test
de Stroop, Trail Making Test, BDI: Cuestionario de percepción
de función cognitiva).
Resultados: Completaron el estudio 22 pacientes (SG:
12, PG: 10, edad media: 24,2 ± 8,8 años, IMC 18,9 ± 3,5
kg/m2). El grupo SG incrementó de forma significativa sus
niveles de folato sérico y eritrocitario y redujo el de homocisteína
(9,4 ± 2,4 μmol/l vs. 7,5 ± 1,7 μmol/l, P < 0,01). Además,
el grupo SG también mejoró significativamente las
puntuaciones de los test de función cognitiva y depresión.
En el grupo PG, en cambio, no se observaron cambios significativos
en ninguna de las variables evaluadas.
Conclusiones: Los resultados obtenidos demuestran
que la suplementación con ácido fólico podría emplearse
como una herramienta más dentro del complejo y multidisciplinario
tratamiento que requieren estos paciente
Efecto de una suplementación láctea con ácido linoleico conjugado sobre el control de peso y la composición corporal de personas sanas con sobrepeso
Introduction: Conjugated linoleic acids (CLAs) have
shown beneficial effects in weight control therapy however
this relation is not clear.
Objetive: The aim of the study was to examine the effects
and safety of 3 g of a 1:1 mix of c9-t11 and t10-c12 on weight
control and body composition in healthy overweight individuals.
Methods: A prospective, placebo-controlled, randomised
double-blind, parallel clinical trial lasting 24 weeks was
carried out in 38 volunteers (29w, 9m) aged 30-55 years and
BMI ≥27-<30 kg/m2 who consumed 200 ml/day of skimmed
milk with 3g of CLAs or 3g olive oil (placebo). Anthropometric,
biochemical and dual x-ray absorptiometry (DXA)
tests were measured. Diet and physical activity were
assessed.
Results: Subjects maintained their habitual dietary and
exercise patterns over the study. Only CLA group showed a
significant decrease in weight (74.43 ± 10.45 vs 73.54 ± 11.66
kg, p = 0.029) and waist circumference (91.45 ± 10.33 vs
90.65 ± 9.84 cm, p = 0.012) between baseline and end of the
study. BMI and waist height ratio decreased (28.44 ± 1.08 vs
27.81 ± 1.43 kg/m2, p = 0.030 and 0.57 ± 0.05 vs 0.56 ± 0.04 p
= 0.013 respectively) in CLA group at the end. CLA group
experienced a reduction in total fat mass after 24 weeks
(38.62 ± 5.02 vs 36.65 ± 5.64%, p = 0.035). No decrease was
observed in Control group. HOMA index had no changes.
Conclusions: The consumption of skimmed milk enriched
with 3g of a 1:1 mixture of c9-t11 and t10-c12 for 24
weeks led to a decrease in body weight and total fat mass in
healthy, overweight subjects who maintained habitual
diets and exercise patterns. No adverse effects were

mostrado unos efectos beneficiosos en el tratamiento del control
de peso; sin embargo, esta relación no está clara.
Objetivo: El propósito de este estudio fue examinar los efectos y
la seguridad de 3 g de una mezcla 1:1 de c9-t11 y t10-c12 sobre el
control de peso y la composición corporal en individuos sanos con
sobrepeso.
Métodos: Se realizó un estudio clínico prospectivo, de grupos
paralelos, de distribución aleatoria, a doble ciego y con control
placebo, de 24 semanas de duración, en 38 voluntarios (29 mujeres,
9 hombres) con edades de 30-55 años y un IMC ≥27- < 30
kg/m2 que consumieron 200 ml/día de leche desnatada con 3 g de
ALC o 3 g de aceite de oliva (placebo). Se midieron datos de
antropometría, bioquímica y absorciometría dual de rayos X
(DXA). Se evaluaron la dieta y la actividad física.
Resultados: Los sujetos mantuvieron sus patrones habituales
de dieta y ejercicio a lo largo del estudio. Sólo el grupo de ALC
mostró una reducción significativa del peso (74,43 ± 10,45 vs
73,54 ± 11,66 kg, p = 0,029) y de la circunferencia de la cintura
(91,45 ± 10,33 vs 90,65 ± 9,84 cm, p = 0,012) entre el periodo
basal y el final del estudio. El IMC y el cociente cintura/talla disminuyeron
(28,44 ± 1,08 vs 27,81 ± 1,43 kg/m2, p = 0,030 y 0,57 ±
0,05 vs 0,56 ± 0,04 p = 0,013, respectivamente) en el grupo ALC
al final del estudio. El grupo ALC experimentó una reducción
de la masa grasa total tras 24 semanas (38,62 ± 5,02 vs 36,65 ±
5,64 %, p = 0,035). No se observó reducción en el grupo control.
El índice HOMA no experimentó cambios.
Conclusiones: El consumo de leche desnatada enriquecida
con 3 g de una mezcla 1:1 de c9-t11 y t10-c12 durante 24 semanas
produjo un descenso del peso corporal y la masa grasa total en
sujetos sanos con sobrepeso que mantuvieron sus patrones habituales
de dieta y ejercicio físico. No se observaron efectos adversos.

Heterozygous and Homozygous Variants in SORL1 Gene in Alzheimer's Disease Patients: Clinical, Neuroimaging and Neuropathological Findings
In the last few years, the SORL1 gene has been strongly implicated in the development of Alzheimer’s disease (AD). We performed whole-exome sequencing on 37 patients with early-onset dementia or family history suggestive of autosomal dominant dementia. Data analysis was based on a custom panel that included 46 genes related to AD and dementia. SORL1 variants were present in a high proportion of patients with candidate variants (15%, 3/20). We expand the clinical manifestations associated with the SORL1 gene by reporting detailed clinical and neuroimaging findings of six unrelated patients with AD and SORL1 mutations. We also present for the first time a patient with the homozygous truncating variant c.364C>T (p.R122*) in SORL1, who also had severe cerebral amyloid angiopathy. Furthermore, we report neuropathological findings and immunochemistry assays from one patient with the splicing variant c.4519+5G>A in the SORL1 gene, in which AD was confirmed by neuropathological examination. Our results highlight the heterogeneity of clinical presentation and familial dementia background of SORL1-associated AD and suggest that SORL1 might be contributing to AD development as a risk factor gene rather than as a major autosomal dominant gene.This work was supported by the Instituto de Salud Carlos III (PI17/01067) and AGAUR from the Autonomous Catalan Government (2017SGR1134). Nutrición parenteral domiciliaria en España, 2019: informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
RESUMEN Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadyasenpe.com) del año 2019. Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE desde el 1 de enero al 31 de diciembre de 2019. Resultados: se registraron 283 pacientes (51,9 %, mujeres), 31 niños y 252 adultos procedentes de 47 hospitales españoles, lo que representa una tasa de prevalencia de 6,01 pacientes/millón de habitantes/año 2019. El diagnóstico más frecuente en los adultos fue “oncológico paliativo” y “otros” (21,0 %). En los niños fue la enfermedad de Hirschsprung junto a la enterocolitis necrotizante, las alteraciones de la motilidad intestinal y la pseudoobstrucción intestinal crónica, con 4 casos cada uno (12,9 %). El primer motivo de indicación fue el síndrome del intestino corto tanto en los niños (51,6 %) como en los adultos (37,3 %). El tipo de catéter más utilizado fue el tunelizado tanto en los niños (75,9 %) como en los adultos (40,8 %). Finalizaron 68 episodios, todos en adultos: la causa más frecuente fue el fallecimiento (54,4 %). Pasaron a la vía oral el 38,2 %. Conclusiones: el número de centros y profesionales colaboradores con el registro NADYA va incrementándose. Se mantienen estables las principales indicaciones y los motivos de finalización de la NPD
Evaluación de un programa de Programa de Intervención Nutricional en pacientes con enfermedad renal crónica avanzada (ERCA)
Introducción y Objetivos: La Enfermedad Renal Crónica
Avanzada (ERCA) se asocia a una elevada prevalencia de
malnutrición. La práctica habitual en estos pacientes va
dirigida a reducir la ingesta proteica, recomendación que
podría favorecer esta situación. Por ello, el objetivo de este
estudio fue evaluar el efecto de un programa de intervención
nutricional (PIN) sobre la función renal y el estado
nutricional en pacientes con ERCA.
Pacientes y Métodos: Se diseñó un estudio longitudinal
y prospectivo con 93 pacientes (53,7% hombres, 66 ±
17años) que participaron en un PIN durante 6 meses con
visitas mensuales. Al inicio y al final de la intervención se
evaluaron: estado nutricional en función de los criterios
de Chang, datos antropométricos, dietéticos y bioquímicos
(albúmina, prealbúmina, aclaramiento de creatinina,
fósforo y potasio séricos, Colesterol-total, LDL, HDL, triglicéridos
y PCR).
Resultados: Tras el PIN la ingesta calórica disminuyó
en los normonutridos (1833 ± 318 vs 1571 ± 219 kcal p <
0,001) y se mantuvo en los malnutridos. Se redujo significativamente
la ingesta proteica (69,9 ± 16,6 vs 54,9 ± 11 g
p < 0,001), potasio (2938 ± 949 vs 2377 ± 743 mg, p <
0,0001) y fósforo (1180 ± 304 vs 946,6 ± 211 mg, p <
0,0001). Un 16,5% requirió suplementación. El porcentaje
de pacientes que presentaba malnutrición según criterios
de Chan disminuyó tras el PIN (41,7 % (27,8% leve,
10,10% moderada y 3,8% grave) vs 16,8% (8% leve, 5%
moderada y 3,8% grave)). Tras el PIN incrementó significativamente
el aclaramiento de creatinina (17,8 ± 5,2 vs
19,4 ± 6,9 ml/min, p < 0,01), albúmina (3,3 ± 0,5 g/dL vs
3,5 ± 0,4 g/dL, p < 0,05) y disminuyó el potasio sérico (4,8
± 0,6 vs 4,5 ± 0,5 mmol/L, p < 0,05), Colesterol Total
(179,8 ± 44,3 vs 170,0 ± 15,1 mg/dL, p < 0,05), LDL colesterol
(113,2 ± 37,0 vs 108,3 ± 27,3 mg/dL, p < 0.01) y triglicéridos

Disease (ACKD) is associated with high prevalence of
malnutrition. The habitual continuous dietary restrictions
used in clinical practice. increased the malnutrition
risk. The aim of this study was to evaluate the effects of a
Nutritional intervention Program (NIP) on renal function
and nutritional status in patients with ACKD.
Patients and Methods: 93 patients, (53.7% men, 66 ± 17
years) were included in a prospective longitudinal study.
The patients recived a NIP during 6 months with mensual
visits. At baseline and six months the outcome assessed
were: nutritional status by Chang criteria, anthropometric,
dietetic and biochemical parameters (albumin,
prealbumin, creatinine clearance, serum phosphorus,
potassium, total-Cholesterol, LDL, HDL, triglycerides,
and PCR).
Results: After intervention, caloric intake decreased in
nourished patients (1833 ± 318 vs 1571 ± 219 kcal p =
.001). and it was constant in malnourished patients. The
intake of protein (69,9 ± 16,6 vs 54,9 ± 11 g p < 0.001),
potassium (2938 ± 949 vs 2377 ± 743 mg p < 0.001) and
phosphorus (1180 ± 304 vs 946,6 ± 211 mg p < 0.001) significantly
decreased. 16.5% patients required supplementation.
A total of 41.7% of patients were malnourished at
baseline (27.8% mild, 10.10% moderate and 3.8%
severe), and 16.8% at the end (8% mild, 5% moderate
and 3.8% severe) by Chang criteria. At the end of NIP,
patients significantly increased creatinine clearance (17,8
± 5,2 vs 19,4 ± 6,9 ml/min, p < 0,01), albumin (3,3 ± 0,5 vs.
3,5 ± 0,4 g/dL, p < 0,05), and decreased serum potassium
(4,8 ± 0,6 vs 4,5 ± 0,5 mmol/L, p < 0,05), total cholesterol
(179,8 ± 44,3 vs 170,0 ± 15,1 mg/dL, p < 0,05), LDL (113,2
± 37,0 vs 108,3 ± 27,3 mg/dL, p < 0.01) and tryglicerides
(141.9 ± 60.8 vs 129.9 ± 52.7 mg/dL, p < 0.05).
Conclusions: The study reflected a NIP usefulness in
the nutritional status and renal function improvements
within an interdisciplinary framework during ACKD
consultation
Motivación contextual desde la autodeterminación en las clases de Educación Física.
The objective proposed in the present study was to determine the type of motivation, from the perspective of selfdetermination, in high school students, towards the Physical Education class of the Antonio Varas de la Barra High School in Arica, Chile, and its relationship between level of studies and sex. 717 students from 1st to 4th grade participated. The instrument used was the Scale of the Perceived Locus of Causality in Physical Education (Spanish adaptation). Among the results, medium-high results (5.09 and 5.22) were found for the dimensions of Intrinsic Motivation and Identified Regulation, respectively; while the Introjected Regulation and External Regulation dimensions presented average values, 4.49 for Introjected Regulation and 4.72 for External Regulation. For its part, non-motivation reached a medium-low value (3.70). Generically, men appeared higher values than women, in all dimensions, except non-motivation, however, these differences were not statistically significant. It was concluded that Intrinsic Motivation, Identified Regulation characterize the studied population, non-motivation showed low results, so that high school students, especially men of higher levels, are motivated by the P.E. Class. (English) [ABSTRACT FROM AUTHOR]El objetivo propuesto en el presente estudio fue determinar el tipo de motivación, desde la perspectiva de la autodeterminación, en estudiantes de enseñanza media, hacia la clase de Educación Física del Liceo Antonio Varas de la Barra en Arica, Chile, y su relación entre nivel de estudios y sexo. Participaron 717 estudiantes de 1ro a 4to medio. El instrumento utilizado fue la Escala del Locus Percibido de Causalidad en Educación Física (adaptación española). Entre los resultados se hallaron resultados medios-altos (5.09 y 5.22) para las dimensiones de Motivación Intrínseca y Regulación Identificada, respectivamente; mientras que las dimensiones Regulación Introyectada y Regulación Externa presentaron valores medios, 4.49 para la Regulación Introyectada y 4.72 para la Regulación Externa. Por su parte, la no motivación alcanzó un valor mediobajo (3.70). Genéricamente, los varones aparentaron valores más alto que las damas, en todas las dimensiones, excepto en no motivación, sin embargo, estas diferencias no resultaron ser significativas estadísticamente. Se arribó a la conclusión de que la Motivación Intrínseca, la Regulación Identificada caracterizan a la población estudiada, la no motivación mostró resultados bajos, por lo que los alumnos de enseñanza media, sobre todo, los hombres de niveles superiores, se hallan motivados por la clase de Educación Física
Clinical, Biochemical, and Molecular Characterization of Two Families with Novel Mutations in the <i>LDHA</i> Gene (GSD XI)
Lactate dehydrogenase (LDH) catalyzes the reversible conversion of L-lactate to pyruvate. LDH-A deficiency is an autosomal recessive disorder (glycogenosis type XI, OMIM#612933) caused by mutations in the LDHA gene. We present two young adult female patients presenting with intolerance to anaerobic exercise, episodes of rhabdomyolysis, and, in one of the patients, psoriasis-like dermatitis. We identified in the LDHA gene a homozygous c.410C>A substitution that predicts a p.Ser137Ter nonsense mutation in Patient One and a compound heterozygous c.410C>A (p.Ser137Ter) and c.750G>A (p.Trp250Ter) nonsense mutation in Patient Two. The pathogenicity of the variants was demonstrated by electrophoretic separation of LDH isoenzymes. Moreover, a flat lactate curve on the forearm exercise test, along with the clinical combination of myopathy and psoriatic-like dermatitis, can also lead to the diagnosis

Regarding Cornelia de Lange syndrome (CdLS), up to date, only a few cases are known to follow this inheritance pattern. However, the high prevalence of somatic mosaicism recently reported in this syndrome (∼13%), together with the disparity observed in tissue distribution of the causal variant, suggests that its prevalence in this disorder could be underestimated. Here, we report a new case of parental gonadosomatic mosaicism in SMC1A gene that causes inherited CdLS, in which the mother of the patient carries the causative variant in very low allele frequencies in buccal swab and blood. While the affected child presents with typical CdLS phenotype, his mother does not show any clinical manifestations. As regards SMC1A, the difficulty of clinical identification of carrier females has been already recognized, as well as the gender differences observed in CdLS expressivity when the causal variant is found in this gene. Currently, the use of DNA deep-sequencing techniques is highly recommended when it comes to molecular diagnosis of patients, as well as in co-segregation studies. These enable us to uncover gonadosomatic mosaic events in asymptomatic or oligosymptomatic parents that had been overlooked so far, which might have great implications regarding genetic counseling for recurrence risk.</p

Impact of SARS-CoV-2 infection in patients with cystic fibrosis in Spain: Incidence and results of the national CF-COVID19-Spain survey
CF-COVID19-Spain Registry Group.[Background] Given the high incidence of confirmed infection by SARS-CoV-2 and mortality by COVID-19 in the Spanish population, its impact was analysed among persons with Cystic Fibrosis (CF) as a group at risk of a worse evolution. The possible causes of the incidence observed in them are explained and how CF Units have faced this health challenge is detailed.[Methods] Retrospective descriptive observational study, for which a Spanish CF Patients with Confirmed COVID-19 Registry is created, requesting information on number of people affected between 8 March–16 May 2020 and their clinical-demographic characteristics from the CF Units participating in the European Cystic Fibrosis Society Patient Registry (ECFSPR). The accumulated incidence is calculated, compared with that of the general population. Additionally, a survey (CF-COVID19-Spain) is carried out on prevention of SARS-CoV-2 infection, workings of CF Units and possible reasons for the incidence observed.[Results] COVID-19 was diagnosed in eight CF patients, one of whom had received a lung transplant. The accumulated incidence was 32/10000 in CF patients and 49/10000 in the general population. General death rate was 5.85/10000 while no CF patients included in the ECFSPR died. The characteristics of those affected and the results of the survey are described.[Conclusions] Despite being considered a disease at high risk of severe COVID-19, the low incidence and mortality in CF patients in Spain contrasts with the figures for the general population. The possible factors that would explain such findings are discussed, with the help of the results of the CF-COVID19-Spain survey
