    GONADOTROPHIN RESPONSES TO GnRH PULSES IN HYPOGONADOTROPHIC HYPOGONADISM: LH RESPONSIVENESS IS MAINTAINED IN THE PRESENCE OF LUTEAL PHASE CONCENTRATIONS OF OESTROGEN AND PROGESTERONE

    LH pulse secretion changes during the menstrual cycle from a rapid regular pattern in the follicular phase to a slower and irregular pattern in the luteal phase. To determine whether the irregular LH pulse pattern in the luteal phase reflects altered GnRH secretion or altered pituitary responsiveness to GnRH, we gave low dose GnRH pulses (25 ng/kg i.v.) every 2 h or every hour for 10 or 12 d to three women with isolated GnRH deficiency. After 4 d of GnRH alone, oestradiol (E 2 ) was given and after 6 d progesterone (P) was added to mimic the hormonal milieu of the luteal phase. LH and FSH were measured every 4 h throughout and also every 20 min for 6 or 12 h, before and after GnRH alone (day 0 and day 4), after E 2 (day 6), and after E 2 + P (day 10 and day 12). Both GnRH pulse frequencies resulted in a rapid increase in plasma FSH to peaks on day 4 (every 2 h) and day 2 and 3 (every hour). FSH concentrations then declined as plasma E 2 rose to 50–80 pg/ml reflecting the selective inhibitory effect of E 2 on FSH release. Plasma LH was also increased after the hourly GnRH injections and this regimen was associated with a more rapid rise in E 2 reflecting follicular maturation. In contrast to the differences in mean hormone concentrations, administration of GnRH at both frequencies resulted in sustained one-on-one responsiveness of LH that was maintained in the presence of both oestrogen and progesterone at mid-luteal phase concentrations.     EFFECT OF CORTISOL TREATMENT ON HORMONAL RELATIONSHIPS IN CONGENITAL ADRENAL HYPERPLASIA

    The temporal relationship between administration of cortisol and serum 17Α-hydroxyprogesterone was investigated in five patients aged 9-19 years with congenital adrenal hyperplasia due to 21-hydroxylase deficiency. There was marked variability in the 17Α-hydroxyprogesterone response (determined hourly for 24 h) of individual patients to administration of cortisol. Mean concentration was less than 0.030 Μmol/l in one patient but 0.519Μ mol/l in another. Levels were higher in all patients while off treatment, and were greatest in those with salt-losing adrenal hyperplasia. Growth hormone secretion was not suppressed by treatment with cortisol. Withdrawal of cortisol for 3 days resulted in a significant decrease in the mean serum FSH/LH ratio and a rise in serum testosterone in all subjects.     SERUM LUTEINIZING HORMONE AND FOLLICLE STIMULATING HORMONE IN NORMAL CHILDREN AND PATIENTS WITH VARIOUS CLINICAL DISORDERS

    Serum concentrations of luteinizing hormone (LH) and follicle stimulating hormone (FSH) were determined in 329 normal children and 185 individuals with endocrinological abnormalities or variations of development. A significant increase of gonadotrophins is noted at the onset of puberty among the boys and at menarche for girls. The values are compared with serum concentrations of LH and FSH in children with abnormalities of sexual development, pituitary malfunction as well as other clinical abnormalities. Comparable levels for age and stage of development were found for premature thelarche, premature adrenarche, cryptorchidism, male pseudohermaphroditism and pubertal gynaecomastia. Hypogonadal individuals (Klinefelter's and Turner's syndrome, pure ovarian dysgenesis and testicular dysgenesis) have markedly elevated values while those with pituitary hypofunction had low values.     Hepatitis A outbreak disproportionately affecting men who have sex with men (MSM) in the European Union and European Economic Area, June 2016 to May 2017

    Free PMC Article: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6205254/Between 1 June 2016 and 31 May 2017, 17 European Union (EU) and European Economic Area countries reported 4,096 cases associated with a multi-country hepatitis A (HA) outbreak. Molecular analysis identified three co-circulating hepatitis A virus (HAV) strains of genotype IA: VRD_521_2016, V16-25801 and RIVM-HAV16-090. We categorised cases as confirmed, probable or possible, according to the EU outbreak case definitions. Confirmed cases were infected with one of the three outbreak strains. We investigated case characteristics and strain-specific risk factors for transmission. A total of 1,400 (34%) cases were confirmed; VRD_521_2016 and RIVM-HAV16-090 accounted for 92% of these. Among confirmed cases with available epidemiological data, 92% (361/393) were unvaccinated, 43% (83/195) travelled to Spain during the incubation period and 84% (565/676) identified as men who have sex with men (MSM). Results depict an HA outbreak of multiple HAV strains, within a cross-European population, that was particularly driven by transmission between non-immune MSM engaging in high-risk sexual behaviour. The most effective preventive measure to curb this outbreak is HAV vaccination of MSM, supplemented by primary prevention campaigns that target the MSM population and promote protective sexual behaviour.info:eu-repo/semantics/publishedVersio

    Radioimmunoassay of human follicle-stimulating hormone with antisera to the ovine hormone

    Since anti-human FSH sera suitable for specific measurement of human FSH (HFSH) in homologous radioimmunoassays (i.e. assays in which the hormone used as antigen and the labeled hormone are from the same species) have been located infrequently, we investigated the possibility of using heterologous systems (i.e. assays in which antisera to a hormone of one species are used in combination with labeled hormone of another species). In the present study, anti-ovine FSH (OFSH) sera from 33 rabbits have been tested for their ability to bind labeled HFSH and to be used subsequently to measure HFSH. Most of the anti-OFSH sera capable of binding appreciable amounts of labeled HFSH could be considered for development of HFSH radioimmunoassays, since the resulting heterologous systems discriminated to a high degree between FSH and HCG. One of these systems has been validated for specific measurement of HFSH. Inhibition curves for sera and pituitary preparations were parallel to each other and to both standards: the 2nd IRP-HMG, of urinary origin, and LER-907, of pituitary origin. The agreement between bioassay and radioimmunoassay estimates of the FSH activity contained in pituitary preparations with widely varying LH to FSH ratios was similar to that obtained with a previously described homologous assay (J Clin Endocr 27: 295, 1967). When a pituitary extract was submitted to polyacrylamide gel electrophoresis, the radioimmunoassayable FSH activity was completely separated from LH and from TSH. There was a nearly perfect correlation between FSH estimates of 27 sera determined simultaneously with this heterologous assay and with the aforementioned homologous assay.
