The impact of four common lumbar spine diagnoses upon overall health status
Background Context: The SF-36 health survey has been shown to be a valid instrument when used to measure the self-reported physical and mental health of patients. The impact of lumbar spinal disorders can be assessed as the difference between the SF-36 scale scores and age-and-gender specific population norms.
Purpose: To establish the impact upon the self-reported health status of patients with one of four common lumbar spinal diagnoses.
Study Design: A cross-sectional, observational assessment of the health status of spine patients.
Methods: Data from patients presenting to the participating centers of the National Spine Network with low back pain and/or leg pain, was collected prospectively using the Health Status Questionnaire 2.0. A database search identified patients with either herniated nucleus pulposus with radicular pain (HNP), lumbar spinal stenosis without deformity (SPS), degenerative spondylolisthesis (DS), and painful disc degeneration/spondylosis (DDD). The mean SF-36 scale scores were generated for each of the diagnostic groups. The impact of these diagnoses on health status was determined as the calculated difference from the age-and-gender specific population norms for each of the eight health scale scores. These scores, usually negative in this population, represent how far below normal these patients are. The analysis was stratified according to the age of the patients (\u3c40 yrs, 40-60 yrs, \u3e60 yrs). Analysis of variance and pair-wise comparison with Bonferroni correction were used to assess the significance of differences across diagnosis and age groups.
Results: Data from a total of 4,442 patients was available for this study. All four diagnostic groups had large, negative impact scores for the eight general health scales with the greatest impact upon the three scales that best measure physical health. The greatest impact on these physical health scales (physical functioning, role-physical, and bodily pain) was seen in the HNP diagnostic group. The younger age groups (\u3c40 yrs and 40-60 yrs) had the greatest physical impairment when compared to the age-and-gender specific population norms. Analysis of variance showed a significant relationship between diagnosis and SF-36 scores, and between age groups and SF-36 scores.
Conclusions: All four lumbar spine disorders have a significant negative impact on all eight of the SF-36 scales. The greatest negative impact was seen in those scales that measure physical health (RP, PF, and BP). The HNP diagnostic group experienced a significantly greater impact upon these three scales. This diagnostic group had the youngest patients, whose baseline physical functional status would be expected to be the most optimal. When we stratified by age in all the diagnostic groups, the greatest negative impact scores for physical health were seen in the \u3c 40 yrs and 40-60 yrs age groups. These patients were also more likely to perceive their health as poor, experience decreased energy, and have more social impairment when compared to their age/gender norms
Associations between schizophrenia genetic risk, anxiety disorders and manic/hypomanic episode in a longitudinal population cohort study.
BACKGROUND: Studies involving clinically recruited samples show that genetic liability to schizophrenia overlaps with that for several psychiatric disorders including bipolar disorder, major depression and, in a population study, anxiety disorder and negative symptoms in adolescence.AimsWe examined whether, at a population level, association between schizophrenia liability and anxiety disorders continues into adulthood, for specific anxiety disorders and as a group. We explored in an epidemiologically based cohort the nature of adult psychopathology sharing liability to schizophrenia. METHOD: Schizophrenia polygenic risk scores (PRSs) were calculated for 590 European-descent individuals from the Christchurch Health and Development Study. Logistic regression was used to examine associations between schizophrenia PRS and four anxiety disorders (social phobia, specific phobia, panic disorder and generalised anxiety disorder), schizophrenia/schizophreniform disorder, manic/hypomanic episode, alcohol dependence, major depression, and - using linear regression - total number of anxiety disorders. A novel population-level association with hypomania was tested in a UK birth cohort (Avon Longitudinal Study of Parents and Children). RESULTS: Schizophrenia PRS was associated with total number of anxiety disorders and with generalised anxiety disorder and panic disorder. We show a novel population-level association between schizophrenia PRS and manic/hypomanic episode. CONCLUSIONS: The relationship between schizophrenia liability and anxiety disorders is not restricted to psychopathology in adolescence but is present in adulthood and specifically linked to generalised anxiety disorder and panic disorder. We suggest that the association between schizophrenia liability and hypomanic/manic episodes found in clinical samples may not be due to bias.Declarations of interestNone
Verification of tolerance to infection by Ceratocystis manginecans in clones of Acacia mangium
Ceratocystis canker and wilt disease has had a devastating impact on plantations of Acacia mangium in Sabah,
Malaysia effectively resulting in its discontinuation in the region. The immediate future of industrial
tree plantations in Malaysia relies on alternative species, such as Eucalyptus pellita, which are suited to the
environment and market opportunities. However, identifying A. mangium planting stock with high levels
of tolerance to Ceratocystis manginecans provides substantial opportunities for its large scale planting and
sustainability in the future. The aim of this study was to verify tolerance to C. manginecans in over 100
putatively tolerant A. mangium clones selected from a family screening trial consisting of 100 wild families.
Selections from the family screening trial were based on either short-lesion length measured six weeks after
inoculation or survival 12 months post inoculation. Six clonal trials were established under field conditions
over two years with more than five ramets of most clones tested in at least two separate trials. The trees were
inoculated with C. manginecans 12 months after trial establishment, and assessments of crown health and
survival as well as the presence or absence of sunken bark, gummosis or stem borer infestation were carried
out 12 months post inoculation. Narrow-sense heritability estimates were moderate for external variables
and for crown health (0.14–0.24) and survival (0.14–0.22). Genetic correlation estimates between trials were
generally high, indicating that assessments were repeatable across trials. Correlations between traits used to
assess damage following inoculation indicated that different traits may be used to identify clones that tolerate
infection. The accuracy of the screening showed that resistant clones can be identified and used to produce

The Search for Young Planetary Systems And the Evolution of Young Stars
The Space Interferometer Mission (SIM) will provide a census of planetary systems by con- ducting a broad survey of 2,000 stars that will be sensitive to the presence of planets with masses as small as approx. 15 Earth masses (1 Uranus mass) and a deep survey of approx. 250 of the nearest, stars with a mass limit of approx.3 Earth masses. The broad survey will include stars spanning a wide range of ages, spectral types, metallicity, and other important parameters. Within this larger context, the Young Stars and Planets Key Project will study approx. 200 stars with ages from 1 Myr to 100 Myr to understand the formation and dynamical evolution of gas giant planets. The SIM Young Stars and Planets Project will investigate both the frequency of giant planet formation and the early dynamical history of planetary systems. We will gain insight into how common the basic architecture of our solar system is compared with recently discovered systems with close-in giant planets by examining 200 of the nearest (less than 150 pc) and youngest (1-100 Myr) solar-type stars for planets. The sensitivity of the survey for stars located 140 pc away is shown in the planet mass-separation plane. We expect to find anywhere from 10 (assuming that only the presently known fraction of stars. 5-7%, has planets) to 200 (all young stars have planets) planetary systems. W-e have set our sensitivity threshold to ensure the detection of Jupiter-mass planets in the critical orbital range of 1 to 5 AU. These observations, when combined with the results of planetary searches of mature stars, will allow us to test theories of planetary formation and early solar system evolution. By searching for planets around pre-main sequence stars carefully selected to span an age range from 1 to 100 Myr, we will learn a t what epoch and with what frequency giant planets are found at the water-ice snowline where they are expected to form. This will provide insight into the physical mechanisms by which planets form and migrate from their place of birth, and about their survival rate. With these data in hand, we will provide data, for the first time, on such important questions as: What processes affect the formation and dynamical evolution of planets? When and where do planets form? What is initial mass distribution of planetary systems around young stars? How might planets be destroyed? What is the origin of the eccentricity of planetary orbits? What is the origin of the apparent dearth of companion objects between planets and brown dwarfs seen in mature stars? The observational strategy is a compromise between the desire to extend the planetary mass function as low as possible and the essential need to build up sufficient statistics on planetary occurrence. About half of the sample will be used to address the "where" and "when" of planet formation. We will study classical T Tauri stars (cTTs) which have massive accretion disks and post- accretion, weak-lined T Tauri stars (wTTs). Preliminary estimates suggest the sample will consist of approx. 30% cTTs and approx. 70% wTTs, driven in part by the difficulty of making accurate astrometric measurements toward objects with strong variability or prominent disks
Multiperspectival designs and processes in interpretative phenomenological analysis research
Researchers using interpretative phenomenological analysis (IPA) within applied research typically use homogenous samples exploring shared perspectives on a single phenomenon of interest. This article explores the challenges and opportunities involved with developing rigorous and epistemologically coherent research designs for capturing more complex and systemic experiential phenomena, through the use of multiple perspectives to explore the same phenomenon. We outline a series of multiple perspective designs and analytic procedures that can be adapted and used across many diverse settings and populations. Whilst building upon existing approaches within qualitative methods and IPA, these designs and procedures are intended to scaffold clear routes to practical application, psychological intervention, the design of behaviour change interventions, and other recommendations for policy and practice. We discuss a variety of conceptual antecedents which situate these designs within phenomenology, pluralistic idiography, qualitative psychology, and wider debates within psychology and other social and behavioural sciences
The experiential impact of hospitalisation:parents' accounts of caring for young people with early psychosis
This research examines the experiential impact of hospitalisation on the parents of young people with early psychosis. In-depth interviews were conducted with a small sample of parents, and the resulting transcripts were analysed using Interpretative Phenomenological Analysis. Five themes emerged from the data: Accepting and blaming, Feeling out of control, Hospitalisation as temporary containment, Feeling let down by services and Stigma. Aspects of the hospitalisation process were characterised by parents as generally negative, but a number of positive affirmations were also offered regarding the containing, supportive and crucial role of services. Parents' perceptions of hospitalisation as a difficult, and sometimes distressing, experience are exacerbated by the complexity of being the carer of a young person. Negotiating services and boundaries within the context of this relationship contributes to feelings of exclusion and disregard by professionals and services. The implications of this study resonate with the current government mental health strategy with regard to how services can engage and include carers in the mental health system, and equip and enable them to support their relatives with early psychosis
Potential of global croplands and bioenergy crops for climate change mitigation through deployment for enhanced weathering.
Conventional row crop agriculture for both food and fuel is a source of carbon dioxide (CO2) and nitrous oxide (N2O) to the atmosphere, and intensifying production on agricultural land increases the potential for soil C loss and soil acidification due to fertilizer use. Enhanced weathering (EW) in agricultural soils-applying crushed silicate rock as a soil amendment-is a method for combating global climate change while increasing nutrient availability to plants. EW uses land that is already producing food and fuel to sequester carbon (C), and reduces N2O loss through pH buffering. As biofuel use increases, EW in bioenergy crops offers the opportunity to sequester CO2 while reducing fossil fuel combustion. Uncertainties remain in the long-term effects and global implications of large-scale efforts to directly manipulate Earth's atmospheric CO2 composition, but EW in agricultural lands is an opportunity to employ these soils to sequester atmospheric C while benefitting crop production and the global climate
A robust system for RNA interference in the chicken using a modified microRNA operon
AbstractRNA interference (RNAi) provides an effective method to silence gene expression and investigate gene function. However, RNAi tools for the chicken embryo have largely been adapted from vectors designed for mammalian cells. Here we present plasmid and retroviral RNAi vectors specifically designed for optimal gene silencing in chicken cells. The vectors use a chicken U6 promoter to express RNAs modelled on microRNA30, which are embedded within chicken microRNA operon sequences to ensure optimal Drosha and Dicer processing of transcripts. The chicken U6 promoter works significantly better than promoters of mammalian origin and in combination with a microRNA operon expression cassette (MOEC), achieves up to 90% silencing of target genes. By using a MOEC, we show that it is also possible to simultaneously silence two genes with a single vector. The vectors express either RFP or GFP markers, allowing simple in vivo tracking of vector delivery. Using these plasmids, we demonstrate effective silencing of Pax3, Pax6, Nkx2.1, Nkx2.2, Notch1 and Shh in discrete regions of the chicken embryonic nervous system. The efficiency and ease of use of this RNAi system paves the way for large-scale genetic screens in the chicken embryo
