‘Savage times come again’ : Morel, Wells, and the African Soldier, c.1885-1920

The African soldier trained in western combat was a figure of fear and revulsion in the late nineteenth and early twentieth centuries. My article examines representations of African soldiers in nonfictional writings by E.D. Morel about the Congo Free State (1885-1908), the same author’s reportage on African troops in post-First World War Germany, and H.G. Wells’s speculative fiction When the Sleeper Wakes (1899, 1910). In each text racist and anti-colonialist discourses converge in representing the African soldier as the henchman of corrupt imperialism. His alleged propensity for taboo crimes of cannibalism and rape are conceived as threats to white safety and indeed supremacy. By tracing Wells’s connections to the Congo reform campaign and situating his novel between two phases of Morel’s writing career, I interpret When the Sleeper Wakes as neither simply a reflection of past events in Africa or as a prediction of future ones in Europe. It is rather a transcultural text which reveals the impact of European culture upon the ‘Congo atrocities’, and the inscription of this controversy upon European popular cultural forms and social debates

Use of Fluid-Ventilated, Gas-Permeable Scleral Lens for Management of Severe Keratoconjunctivitis Sicca Secondary to Chronic Graft-versus-Host Disease

AbstractKeratoconjunctivitis sicca (KCS) occurs in 40%-60% of patients with chronic graft-versus-host-disease (cGVHD) after allogeneic hematopoietic cell transplantation. Although immunosuppressive therapy is the primary treatment of chronic GVHD, ocular symptoms require measures to improve ocular lubrication, decrease inflammation, and maintain mucosal integrity. The liquid corneal bandage provided by a fluid-ventilated, gas-permeable scleral lens (SL) has been effective in mitigating symptoms and resurfacing corneal erosions in patients with KCS related to causes other than cGVHD. We report outcomes in 9 consecutive patients referred for SL fitting for cGVHD-related severe KCS that was refractory to standard treatments. All patients reported improvement of ocular symptoms and reduced the use of topical lubricants after SL fitting resulting from decreased evaporation. No serious adverse events or infections attributable to the SL occurred. The median Ocular Surface Disease Index improved from 81 (75-100) to 21 (6-52) within 2 weeks after SL fitting, and was 12 (2-53) at the time of last contact, 1-23 months (median, 8.0) after SL fitting. Disability related to KCS resolved in 7 patients after SL fitting. The use of SL appears to be safe and effective in patients with severe cGVHD-related KCS refractory to conventional therapies

A multicenter, longitudinal, interventional, double blind randomized clinical trial in hematopoietic cell transplant recipients residing in remote areas: Lessons learned from the late cytomegalovirus prevention trial

AbstractPurposeThe logistics of conducting double-blinded phase III clinical trials with participants residing in remote locations are complex. Here we describe the implementation of an interventional trial for the prevention of late cytomegalovirus (CMV) disease in hematopoietic cell transplantation (HCT) subjects in a long-term follow-up environment.MethodsA total of 184 subjects at risk for late CMV disease surviving 80 days following allogeneic HCT were randomized to receive six months of valganciclovir or placebo. Subjects were followed through day 270 post-transplant at their local physician's office within the United States. Anti-viral treatment interventions were based on CMV DNAemia as measured by polymerase chain reaction (PCR) (>1000 copies/mL) and granulocyte colony stimulating factor (G-CSF) was prescribed for neutropenia (absolute neutrophil count (ANC < 1.0 × 109 cells/L). Blood samples for viral testing and safety monitoring were shipped to a central laboratory by overnight carrier. Real-time communication was established between the coordinating center and study sites, primary care physicians, and study participants to facilitate starting, stopping and dose adjustments of antiviral drugs and G-CSF. The time required to make these interventions was analyzed.ResultsOf the 4169 scheduled blood specimens, 3832 (92%) were received and analyzed; the majority (97%) arriving at the central site within 2 days. Among subjects with positive CMV DNAemia (N = 46), over 50% received open label antiviral medication within one day. The median time to start G-CSF for neutropenia was <1 day after posting of laboratory results (range 0–6; N = 38). Study drug dose adjustments for abnormal renal function were implemented 203 times; within one day for 48% of cases and within 2 days for 80% of cases.ConclusionComplex randomized, double-blind, multicenter interventional trials with treatment decisions made at a central coordinating site can be conducted safely and effectively according to Good Clinical Practice (GCP) guidelines over a large geographic area

    Assessing Big-Bang Nucleosynthesis

Systematic uncertainties in the light-element abundances and their evolution make a rigorous statistical assessment difficult. However, using Bayesian methods we show that the following statement is robust: the predicted and measured abundances are consistent with 95\% credibility only if the baryon-to-photon ratio is between 2×10102\times 10^{-10} and 6.5×10106.5\times 10^{-10} and the number of light neutrino species is less than 3.9. Our analysis suggests that the 4^4He abundance may have been systematically underestimated.Comment: 7 pages, LaTeX(2.09), 6 postscript figures (attached).
