    A Long-term Survivor after Congenital Acute Myeloid Leukemia with t(8 ; 16)(p11 ; p13)

    The treatment of patients with congenital leukemia is difficult and often results in a poor prognosis. We present here the case of a female child with congenital acute myeloid leukemia (AML) with t(8 ; 16) (p11 ; p13) who received chemotherapy and survived for more than 10 years without relapse. A novel MOZ-CBP chimera was found in her diagnostic sample. Although adult AML patients with MOZ-CBP have mainly been reported as having therapy-related AML and showed poor prognoses, the present case supports the idea that AML with MOZ-CBP in the pediatric population might show better prognoses

    Relationship between family support and nutrient intake status in type 2 diabetes mellitus patients

    【目的】通院中の2型糖尿病外来患者を対象に、情動的サポート、行動的サポート、また情報的サポートに着目し、家族による支援の有無別に栄養素等摂取状況の検討を行った。【方法】2016年3月~8月、新潟市B病院の2型糖尿病外来患者42名(年齢中央値68.0歳、男性比率66.7%)を対象に実施した、自記式質問紙調査及び食物摂取頻度調査データを用いた(有効回答率70.0%)。質問紙調査より得た、家族による支援の有無によって対象者を2群に区分した。食物摂取頻度調査より得たデータを用いて、2群のエネルギー及び栄養素等摂取量、食品群別摂取量(密度法によって調整)についてMann-Whitney のU 検定を用いて検討した。【結果】「食事療法を守っていることをほめる」の問いでは、支援あり7名(16.7%)、支援なし35名(83.3%)であった。2群の1000kcal あたりの栄養素等・食品群別摂取量で差がみられた項目はなかった。「同じ時間に食事をする」の問いでは、支援あり32名(76.2%)、支援なし10名(23.8%)であった。支援ありの者で相対的に、たんぱく質(p=0.045)、豆類(p=0.022)摂取量が多く、菓子類(p=0.020)、油脂類(p=0.039)の摂取量が少なかった。「家族の糖尿病の知識の有無」では、知識ありの者で油脂類のみ摂取量が相対的に多かった(p=0.013)。また、「家族の食事療法の知識の有無」では、知識ありの者で嗜好飲料のみ摂取量が相対的に多かった(p=0.028)。【結論】2 型糖尿病外来患者が受ける家族による支援として、「ほめる」という情動的サポートや疾患や食事療法の知識という情報的サポートに比べ、「同じ時間に食事をする」という家族の存在、家族の行動的サポートが栄養素等摂取状況に関連することが示唆された。Objective: To investigate the relationship between family support, specifically emotional, practical, and informational support, and nutrient intake status in type 2 diabetes mellitus (DM) outpatients. Methods: We distributed a self-report questionnaire and food frequency questionnaire to 42 outpatients with type 2 DM (median age: 68.0 years; men: 66.7%) from a community hospital-based registry in Niigata City between March to August 2016. From the self-report questionnaire, the patients were divided into two groups based on whether they received family support or not. Energy and nutrient intake and intake by food groups were compared using Mann-Whitney U test between the two groups.Results: Regarding the item “Praise for complying with medical nutritional therapy,” 7 patients (16.7%) were praised and 35 patients (83.3%) were not. There was no significant difference in nutrient intake or intake by food group between these two groups. For the item “family eating at the same time,” 32 patients (76.2%) responded that they had family meals together versus 10 patients (23.8%) who did not. Regarding energy and nutrient intake and intake by food group, families who ate together had meals with significantly higher protein (p= 0.045) and beans (p= 0.022) content with less confectionary (p= 0.010) and oil and fat (p= 0.039) content compared with families who did not eat together. Fat and oil intake was significantly higher among families with knowledge of DM and intake of beverage was higher among families with knowledge of dietary treatment than among families without such knowledge. Conclusion: For family support, our findings suggested that practical support such as eating at same time was more strongly related to nutrient intake among type 2 DM patients in comparison with emotional support such as praising patients

    Relationship between Protection against Cold and the Physiological Index during a Cold Environment

    A snow cave is a bivouac shelter used in mountain climbing that is widely used as a shelter against the cold during winter. In the outdoors, wind velocity and air temperature have an influence on temperature change. It could stabilize body temperature if it can control the convection of ambient air. This paper could develop a theory focusing on the relation between physiological indexes and the protection against the cold while staying in a snow cave. For example, protection against the cold could be thermal insulation underwear, thermal insulation gloves, thermal insulation socks, a steam warmed temperature sheet and a rescue sheet. Measurement items were heart rate, blood pressure, rectal temperature, score of a subjective thermal sensation and the activity of the parasympathetic nervous system. It was clarified that the protection against the cold could be effective for the decrease of the physiological index. These field studies suggest that they would enable the adaptation in the adjustment range of the autonomic nervous system given these protections against the cold

    A Cluster of Respiratory Syncytial Virus Infections in a Hospital Ward for Adult Immunocompromised Patients

    Four male patients admitted to the same ward in the first half of September 201Y were identified to have respiratory syncytial virus(RSV)infection. Their ages ranged from 49 to 85 years(median 72.5). One patient was infected with human immunodeficiency virus and three patients had hematological malignancies. Following immuno-chromatological testing with a nasal cavity swab, RSV infection was diagnosed. Although blood and sputum cultures were performed in three patients, no significant bacteria were detected. Two cases responded to supportive therapy. However, one patient died secondary to multiple myeloma, and another patient developed pneumonia and died with an exacerbation of leukemia. RSV infections in immunocompromised hosts are associated with a poor prognosis. Early diagnosis will facilitate isolation of infected individuals to prevent hospital outbreaks

    エリンギ(Pleurotus eryngii)のトレハラーゼ : 酵素の分布、精製および性質について

    Trehalase activity in a homogenized preparation from the fruit body of king oyster mushroom (eryngii), Pleurotus eryngii was studied in detail. Distribution of trehalose and trehalase activity was higher in the cap (pileus) fraction than in the other positions such as stalk (stipe). The tretalase peak 1 was purified by the Sephacryl S-200 and Toyopearl HW-55 column chromatography. Electorophoretic analysis of trehalase gave values of molecular weight of 36~37kDa, and oligomeric forms and blycoprotein forms of trehalase were suggested by the band patterns of resulting gels. Optimum conditons of pH and temperature were determined to be pH5.0 and 30℃, respectively. Trehalase peak 1 showed higher reactivity to α-, α-trehalose, whereas peak 2 enzyme showed higher activity to β-,β-trehalose, methyl-β-glucoside, and cellobiose, indicating that this enzyme might have a substrate specificity such asβ-glucosidase

    Synthesis and characterization of polyurethanes crosslinked by polyrotaxanes consisting of half-methylated cyclodextrins and PEGs with different chain lengths

    Three polyurethanes (PRX1500Me-PU, PRX4000Me-PU, and PRX6000Me-PU) crosslinked by polyrotaxanes (PRXs), which consist of half-methylated α-cyclodextrins (CyDs) and poly(oxyethylene)glycols with different chain lengths (PEG1500, PEG4000, and PEG6000), were synthesized. The filling ratios of CyD in PRX1500, PRX4000 and PRX6000 are 75, 63 and 37%, respectively. A polyurethane crosslinked by half-methylated CyD (CDMe-PU) was also prepared for comparison of their structure and properties. ATR-FT-IR spectra of the PUs showed that the formation ratio of hydrogen bond between the PU chains around PRXs increased with increase in the filling ratio. DSC and dynamic viscoelastic measurements and tensile tests for the PUs revealed that (i) reorganized-crystallization of the soft segment chains of PRX1500Me-PU easily occurred because of formation of a pure phase for them; (ii) the thermal and physical behaviors of PRX6000Me-PU are similar to those of CDMe-PU because CyDs as the crosslink points disperse in a similar fashion in the PUs; (iii) the PRX4000 with the moderate filling ratio of CyD in PRX4000Me-PU makes slow reorganized-crystallization of the soft segment chains in the PU as well as improves the tensile performance among the PUs
